    A survey to investigate the association of pain, foot disability and quality of life with corns

    Background Corns are a common foot problem affecting a large proportion of the population. This study describes the characteristics of corns experienced by 201 participants taking part in a randomised controlled trial to investigate associations between demographic and corn parameters on pain, foot related disability and quality of life (QoL). Methods Pain from the main (index) corn was measured using a visual analogue scale (VAS); foot related disability was assessed with the Foot Disability Questionnaire (now known as the Manchester Foot Pain and Disability Index) and quality of life was recorded with the EQ-5D questionnaire. The effect of demographic and corn parameters on the pain and quality of life outcomes was assessed with analysis of variance (ANOVA) methods. The effect of the same factors on a linear combination of the foot-related disability outcome measures was assessed using multivariate ANOVA methods. Pain was also tested for its mediating properties on the causal pathway between the independent variables and quality of life. Results The mean pain score was 5.29 points on a 10 cm VAS, with females reporting substantively higher pain levels than males. Age affected foot-related disability, with lower levels on all domains of the MFPDI reported in older participants; each year of advancing age was associated with falls of: 0.009 points on the Concern about Appearance (CA) domain; 0.047 points on the Functional Limitation (FL) domain and 0.048 points on the Pain Intensity (PI) domain. Sex and corn type also affected disability, with higher scores reported by females and participants with plantar corns. Conclusions The effect of pain was shown to mediate the relationship between sex and foot-related disability. The presence of plantar corns has a more detrimental effect on QoL than dorsal/inter-digital corns

    Precise Switching of Flagellar Gene Expression in Escherichia Coli by the FlgM–FliA Regulatory Network

    A remarkable feature of flagellar synthesis in Escherichia coli is that gene expression is sequential and coupled to the assembly process. The interaction of two key proteins, the flagellar sigma factor FliA and its anti-sigma factor FlgM serves as a major checkpoint in the assembly process that temporally separates middle and late gene expression. While the sequential nature within each gene class has been studied using large-scale transcriptional data, much less is known about the timing controlled by the checkpoint mechanism. In this article, we analyze timing, sensitivity and robustness of the FlgM–FliA core regulatory mechanism based on quantitative molecule data and a detailed stochastic as well as reduced deterministic reaction kinetics model. We find that the pool of free anti-sigma factor FlgM, accumulated during middle gene expression, acts as a molecular timer that determines the delay between successful completion of the hook basal body subunit and the start of expression of flagellar filament proteins. Furthermore, we find that the number of free FliA molecules needs to be tightly controlled for a precise switch from middle to late gene expression. A sensitivity analysis based on the reduced reaction kinetics model reveals that the checkpoint mechanism is very sensitive to changes in levels of competing sigma factors, allowing the bacterium to rapidly adapt to a changing environment. In addition, we find that the reduced model also shows a high sensitivity to the effective synthesis rates of FliA and FlgM. However, this high sensitivity does not generally carry over to the original parameters of transcriptional and translational processes in the detailed model. As a consequence, care has to be taken whenever interpreting results from the robustness analysis of reaction kinetic models comprising lumped or effective parameters

    Age and gender differences in disabling foot pain using different definitions of the manchester foot pain and disability index

    Extent: 9p.Background: The Manchester Foot Pain and Disability Index (MFPDI) has been used to determine the prevalence of disabling foot pain in several studies, however there is some debate as to which case definition is most appropriate. The objective of this study was to explore age and gender differences in the proportion of people with disabling foot pain using three different case definitions of the MFPDI and for each individual MFPDI item. Methods: A random sample of 223 participants aged 27 to 90 years (88 males and 135 females) from the North West Adelaide Health Study, who reported having pain, aching or stiffness in either of their feet on most days in the last month, completed the MFPDI by telephone interview. The proportion of people with disabling foot pain was determined using three definitions: (i) Definition A-at least one of the 17 items documented on at least some days in the last month; (ii) Definition B-at least one of the 17 items documented on most/every day(s) in the last month, and; (iii) Definition C-at least one of the ten functional limitation items documented on most/every day(s) in the last month. Cross-tabulations and chi-squared statistics were used to explore differences in responses to the MFPDI items according to age and gender. Results: The proportion of people with disabling foot pain according to each definition was as follows: Definition A (100%), Definition B (95.1%) and Definition C (77.6%). Definition C was most sensitive to age and gender differences. Exploration of individual MFPDI items indicated that age significantly affected both the pain intensity and functional limitation items, with younger people more likely to report their foot pain being worse in the morning, and older people more likely to report functional limitations. Although gender did not influence responses to the personal appearance items, women were more likely report functional limitations than men. Conclusions: Definition C of the MFPDI is more sensitive to age and gender differences in the proportion of people with disabling foot pain, and would therefore seem to be the most appropriate case definition to use in epidemiological studies involving a broad age range of participants.Hylton B Menz, Tiffany K Gill, Anne W Taylor and Catherine L Hil

    Prevalence and correlates of foot pain in a population-based study: the North West Adelaide health study

    Background: Few population-based studies have examined the prevalence of foot pain in thegeneral community. The aims of this study were therefore to determine the prevalence, correlatesand impact of foot pain in a population-based sample of people aged 18 years and over living in the northwest region of Adelaide, South Australia.Methods: The North West Adelaide Health Study is a representative longitudinal cohort study ofn = 4,060 people randomly selected and recruited by telephone interview. The second stage of datacollection on this cohort was undertaken between mid 2004 and early 2006. In this phase,information regarding the prevalence of musculoskeletal conditions was included. Overall, n = 3,206 participants returned to the clinic during the second visit, and as part of the assessment were asked to report whether they had pain, aching or stiffness on most days in either of their feet. Data were also collected on body mass index (BMI); major medical conditions; other joint symptoms and health-related quality of life (the Medical Outcomes Study Short Form 36 [SF-36]).Results: Overall, 17.4% (95% confidence interval 16.2 – 18.8) of participants indicated that theyhad foot pain, aching or stiffness in either of their feet. Females, those aged 50 years and over,classified as obese and who reported knee, hip and back pain were all significantly more likely to report foot pain. Respondents with foot pain scored lower on all domains of the SF-36 afteradjustment for age, sex and BMI.Conclusion: Foot pain affects nearly one in five of people in the community, is associated withincreased age, female sex, obesity and pain in other body regions, and has a significant detrimental impact on health-related quality of life.Catherine L Hill, Tiffany K Gill, Hylton B Menz and Anne W Taylo

    Projected climate change and its impacts on glaciers and water resources in the headwaters of the Tarim River, NW China/Kyrgyzstan

Open Access funding enabled and organized by Projekt DEAL.Glacierised river catchments are highly sensitive to climate change, while large populations may depend on their water resources. The irrigation agriculture and the communities along the Tarim River, NW China, strongly depend on the discharge from the glacierised catchments surrounding the Taklamakan Desert. While recent increasing discharge has been beneficial for the agricultural sector, future runoff under climate change is uncertain. We assess three climate change scenarios by forcing two glacio-hydrological models with output of eight general circulation models. The models have different glaciological modelling approaches but were both calibrated to discharge and glacier mass balance observations. Projected changes in climate, glacier cover and river discharge are examined over the twenty-first century and generally point to warmer and wetter conditions. The model ensemble projects median temperature and precipitation increases of + 1.9–5.3 °C and + 9–24%, respectively, until the end of the century compared to the 1971–2000 reference period. Glacier area is projected to shrink by 15–73% (model medians, range over scenarios), depending on the catchment. River discharge is projected to first increase by about 20% in the Aksu River catchments with subsequent decreases of up to 20%. In contrast, discharge in the drier Hotan and Yarkant catchments is projected to increase by 15–60% towards the end of the century. The large uncertainties mainly relate to the climate model ensemble and the limited observations to constrain the glacio-hydrological models. Sustainable water resource management will be key to avert the risks associated with the projected changes and their uncertainties.Publisher PDFPeer reviewe

    Effects of age on strength and morphology of toe flexor muscles

    Study Design: Cross-sectional. 27 Objective: To compare the strength and size of the toe flexor muscles of older adults relative 28 to their younger counterparts. 29 Background: Age related muscle atrophy is common in lower limb muscles and we therefore 30 speculated that foot muscles also diminish with age. However, there is a paucity of literature 31 characterizing foot muscle strength and morphology, and any relationship between these two, 32 in older people. 33 Methods: Seventeen young adults with a normal foot type were matched by gender and BMI 34 to 17 older adults with a normal foot type, from an available sample of 41 young (18-50 35 years) and 44 older (60+ years) adults. Among the matched groups (n=34), muscle thickness 36 and cross-sectional area (CSA) for five intrinsic and two extrinsic toe flexor muscles were 37 obtained using ultrasound. Toe strength was assessed using a pressure platform. Differences 38 in toe flexor strength and muscle size between the young and older matched groups were 39 determined using ANCOVA (controlling for height). Correlations between strength and size 40 of the toe flexor muscles of the pooled group (n=34) were also calculated. 41 Results: Toe strength and the thickness and CSA of most foot muscles and were significantly 42 reduced in the older adults (P<0.05). Hallux and toe flexor strength were strongly correlated 43 with the size of the intrinsic muscles toe flexor muscles. 44 Conclusion: The smaller foot muscles appear to be affected by sarcopenia in older adults. 45 This could contribute to reduced toe flexion force production and affect the ability of older 46 people to walk safely. Interventions aimed at reversing foot muscle atrophy in older people 47 require further investigation

    Individual tracking reveals long-distance flight-path control in a nocturnally migrating moth

    Each year, trillions of insects make long-range seasonal migrations. These movements are relatively well understood at a population level, but how individual insects achieve them remains elusive. Behavioral responses to conditions en route are little studied, primarily owing to the challenges of tracking individual insects. Using a light aircraft and individual radio tracking, we show that nocturnally migrating death's-head hawkmoths maintain control of their flight trajectories over long distances. The moths did not just fly with favorable tailwinds; during a given night, they also adjusted for head and crosswinds to precisely hold course. This behavior indicates that the moths use a sophisticated internal compass to maintain seasonally beneficial migratory trajectories independent of wind conditions, illuminating how insects traverse long distances to take advantage of seasonal resources

    The imaging properties of the Gas Pixel Detector as a focal plane polarimeter

    X-rays are particularly suited to probe the physics of extreme objects. However, despite the enormous improvements of X-ray Astronomy in imaging, spectroscopy and timing, polarimetry remains largely unexplored. We propose the photoelectric polarimeter Gas Pixel Detector (GPD) as an instrument candidate to fill the gap of more than thirty years of lack of measurements. The GPD, in the focus of a telescope, will increase the sensitivity of orders of magnitude. Moreover, since it can measure the energy, the position, the arrival time and the polarization angle of every single photon, allows to perform polarimetry of subsets of data singled out from the spectrum, the light curve or the image of source. The GPD has an intrinsic very fine imaging capability and in this work we report on the calibration campaign carried out in 2012 at the PANTER X-ray test facility of the Max-Planck-Institut f\"ur extraterrestrische Physik of Garching (Germany) in which, for the first time, we coupled it to a JET-X optics module with a focal length of 3.5 m and an angular resolution of 18 arcsec at 4.5 keV. This configuration was proposed in 2012 aboard the X-ray Imaging Polarimetry Explorer (XIPE) in response to the ESA call for a small mission. We derived the imaging and polarimetric performance for extended sources like Pulsar Wind Nebulae and Supernova Remnants as case studies for the XIPE configuration, discussing also possible improvements by coupling the detector with advanced optics, having finer angular resolution and larger effective area, to study with more details extended objects.Comment: Accepted for publication in The Astrophysical Journal Supplemen

    Adaptive strategies of high-flying migratory hoverflies in response to wind currents: Flight behaviour of migrant hoverflies

    Large migrating insects, flying at high altitude, often exhibit complex behaviour. They frequently elect to fly on winds with directions quite different from the prevailing direction, and they show a degree of common orientation, both of which facilitate transport in seasonally beneficial directions. Much less is known about the migration behaviour of smaller (10-70 mg) insects. To address this issue, we used radar to examine the high-altitude flight of hoverflies (Diptera: Syrphidae), a group of day-active, medium-sized insects commonly migrating over the UK. We found that autumn migrants, which must move south, did indeed show migration timings and orientation responses that would take them in this direction, despite the unfavourability of the prevailing winds. Evidently, these hoverfly migrants must have a compass (probably a time-compensated solar mechanism), and a means of sensing the wind direction (which may be determined with sufficient accuracy at ground level, before take-off). By contrast, hoverflies arriving in the UK in spring showed weaker orientation tendencies, and did not correct for wind drift away from their seasonally adaptive direction (northwards). However, the spring migrants necessarily come from the south (on warm southerly winds), so we surmise that complex orientation behaviour may not be so crucial for the spring movements

    An investigation to assess ankle mobility in healthy individuals from the application of multi-component compression bandages and compression hosiery

    Background An investigation was undertaken to compare the effect of multi-component compression bandages and compression hosiery kits on individuals’ range of ankle motion whilst wearing typical and medical footwear, and barefoot. Methods A convenience sample of 30 healthy individuals recruited from the staff and student population at the University of Huddersfield, UK. Plantarflexion/dorsiflexion range of ankle motion (ROAM) was measured in participants over 6 steps in every combination of typical, medical and no footwear; and multi-component bandages, compression hosiery and no garments. Results Controlling for age, gender and garments, the use of typical footwear was associated with a mean increase in ROAM of 2.54° at best estimate compared with barefoot; the use of medical footwear was associated with a mean decrease in ROAM of 1.12° at best estimate compared with barefoot. Controlling for age, gender and footwear, the use of bandaging was associated with a mean decrease in ROAM of 2.51° at best estimate compared with no garments. Controlling for age, gender and footwear, the use of hosiery was not associated with a significant change in ROAM compared with no garments. Conclusions Bandages appear to restrict ROAM more than hosiery when used in conjunction with a variety of footwear types
