Management of skin-sparing mastectomy: Results of a survey of German Hospitals
Background: The aim of this study was to evaluate the current management of skin-sparing mastectomy in German hospitals and to determine its oncologic safety. For this purpose, 100 surgeons were surveyed regarding their use of skin-sparing mastectomy. Results: Almost all surveyed hospitals performed skin-sparing mastectomy. Most of them believe that the recurrence rate is equal to that of conventional mastectomy. 95% regard inflammatory cancer as a contraindication to skin-sparing surgery. Most of the hospitals thin out the skin without leaving any macroscopic glandular tissue behind, and 73% leave the nipple-areola complex (NAC) on the basis of frozen sections. Volume replacement is most commonly done with latissimus dorsi muscle flaps and pedicled TRAM flaps. In 76% of the surveyed hospitals, reconstruction after mastectomy is performed by the gynecological department. Conclusion: Skin-sparing mastectomy is considered to be the best cosmetic option for breast reconstruction in selected breast cancer patients. At present, statistical proof of its oncologic safety is lacking. The surgical techniques used for skin-sparing mastectomy have not yet been standardized. Trends in Metal Oxide Stability for Nanorods, Nanotubes, and Surfaces
The formation energies of nanostructures play an important role in
determining their properties, including the catalytic activity. For the case of
15 different rutile and 8 different perovskite metal oxides, we find that the
density functional theory (DFT) calculated formation energies of (2,2)
nanorods, (3,3) nanotubes, and the (110) and (100) surfaces may be described
semi-quantitatively by the fraction of metal--oxygen bonds broken and the
bonding band centers in the bulk metal oxide
Action planning with two-handed tools
In tool use, the intended external goals have to be transformed into bodily movements by taking into account the target-to-movement mapping implemented by the tool. In bimanual tool use, this mapping may depend on the part of the tool that is operated and the effector used (e.g. the left and right hand at the handle bar moving in opposite directions in order to generate the same bicycle movement). In our study, we investigated whether participants represent the behaviour of the tool or only the effector-specific mapping when using two-handed tools. In three experiments, participants touched target locations with a two-jointed lever, using either the left or the right hand. In one condition, the joint of the lever was constant and switching between hands was associated with switching the target-to-movement-mapping, whereas in another condition, switching between hands was associated with switching the joint, but the target-to-movement-mapping remained constant. Results indicate pronounced costs of switching hands in the condition with constant joint, whereas they were smaller with constant target-to-movement mapping. These results suggest that participants have tool-independent representations of the effector-specific mappings
Quasiparticle interfacial level alignment of highly hybridized frontier levels: HO on TiO(110)
Knowledge of the frontier levels' alignment prior to photo-irradiation is
necessary to achieve a complete quantitative description of HO
photocatalysis on TiO(110). Although HO on rutile TiO(110) has been
thoroughly studied both experimentally and theoretically, a quantitative value
for the energy of the highest HO occupied levels is still lacking. For
experiment, this is due to the HO levels being obscured by hybridization
with TiO(110) levels in the difference spectra obtained via ultraviolet
photoemission spectroscopy (UPS). For theory, this is due to inherent
difficulties in properly describing many-body effects at the
HO-TiO(110) interface. Using the projected density of states (DOS) from
state-of-the-art quasiparticle (QP) , we disentangle the adsorbate and
surface contributions to the complex UPS spectra of HO on TiO(110). We
perform this separation as a function of HO coverage and dissociation on
stoichiometric and reduced surfaces. Due to hybridization with the TiO(110)
surface, the HO 3a and 1b levels are broadened into several peaks
between 5 and 1 eV below the TiO(110) valence band maximum (VBM). These
peaks have both intermolecular and interfacial bonding and antibonding
character. We find the highest occupied levels of HO adsorbed intact and
dissociated on stoichiometric TiO(110) are 1.1 and 0.9 eV below the VBM. We
also find a similar energy of 1.1 eV for the highest occupied levels of HO
when adsorbed dissociatively on a bridging O vacancy of the reduced surface. In
both cases, these energies are significantly higher (by 0.6 to 2.6 eV) than
those estimated from UPS difference spectra, which are inconclusive in this
energy region. Finally, we apply self-consistent QP (scQP1) to obtain

Global similarity with local differences in linkage disequilibrium between the Dutch and HapMap–CEU populations
The HapMap project has facilitated the selection of tagging single nucleotide polymorphisms (tagSNPs) for genome-wide association studies (GWAS) under the assumption that linkage disequilibrium (LD) in the HapMap populations is similar to the populations under investigation. Earlier reports support this assumption, although in most of these studies only a few loci were evaluated. We compared pair-wise LD and LD block structure across autosomes between the Dutch population and the CEU-HapMap reference panel. Nash Equilibria in Multi-Agent Motor Interactions
Social interactions in classic cognitive games like the ultimatum game or the
prisoner's dilemma typically lead to Nash equilibria when multiple
competitive decision makers with perfect knowledge select optimal strategies.
However, in evolutionary game theory it has been shown that Nash equilibria can
also arise as attractors in dynamical systems that can describe, for example,
the population dynamics of microorganisms. Similar to such evolutionary
dynamics, we find that Nash equilibria arise naturally in motor interactions in
which players vie for control and try to minimize effort. When confronted with
sensorimotor interaction tasks that correspond to the classical
prisoner's dilemma and the rope-pulling game, two-player motor
interactions led predominantly to Nash solutions. In contrast, when a single
player took both roles, playing the sensorimotor game bimanually, cooperative
solutions were found. Our methodology opens up a new avenue for the study of
human motor interactions within a game theoretic framework, suggesting that the
coupling of motor systems can lead to game theoretic solutions
Interpretation time of serial chest CT examinations with stacked-metaphor workstation versus film alternator
PURPOSE: Interpretation time of serial staging chest CT cases, which each contained current and previous examinations, with a simple prototype workstation called filmstack was experimentally compared with interpretation time with a film alternator. MATERIALS AND METHODS: The filmstack displayed a "stack" of sections for each examination; user controls allowed rapid selection of preset attenuation windows and both synchronized and unsynchronized scrolling. Eight radiologists were timed as they used the filmstack and the film alternator to interpret four ergonomically complex serial CT cases. RESULTS: All reports dictated on the basis of findings with filmstack and film were of acceptable clinical accuracy. The time to examine a case with filmstack was significantly faster than the time with film, including the time to load and unload the alternator (99% confidence [P = .01]). There was no statistically significant difference in interpretation time between filmstack and prehung film. CONCLUSION: Use of a low-cost stacked CT workstation with a single 1,024 x 1,024 monitor is an effective means of interpreting cases that require comparison of multiple CT examinations
Association Analysis of 94 Candidate Genes and Schizophrenia-Related Endophenotypes
While it is clear that schizophrenia is highly heritable, the genetic basis of this heritability is complex. Human genetic, brain imaging, and model organism studies have met with only modest gains. A complementary research tactic is to evaluate the genetic substrates of quantitative endophenotypes with demonstrated deficits in schizophrenia patients. We used an Illumina custom 1,536-SNP array to interrogate 94 functionally relevant candidate genes for schizophrenia and evaluate association with both the qualitative diagnosis of schizophrenia and quantitative endophenotypes for schizophrenia. Subjects included 219 schizophrenia patients and normal comparison subjects of European ancestry and 76 schizophrenia patients and normal comparison subjects of African ancestry, all ascertained by the UCSD Schizophrenia Research Program. Six neurophysiological and neurocognitive endophenotype test paradigms were assessed: prepulse inhibition (PPI), P50 suppression, the antisaccade oculomotor task, the Letter-Number Span Test, the California Verbal Learning Test-II, and the Wisconsin Card Sorting Test-64 Card Version. These endophenotype test paradigms yielded six primary endophenotypes with prior evidence of heritability and demonstrated schizophrenia-related impairments, as well as eight secondary measures investigated as candidate endophenotypes. Schizophrenia patients showed significant deficits on ten of the endophenotypic measures, replicating prior studies and facilitating genetic analyses of these phenotypes. A total of 38 genes were found to be associated with at least one endophenotypic measure or schizophrenia with an empirical p-value<0.01. Many of these genes have been shown to interact on a molecular level, and eleven genes displayed evidence for pleiotropy, revealing associations with three or more endophenotypic measures. Among these genes were ERBB4 and NRG1, providing further support for a role of these genes in schizophrenia susceptibility. The observation of extensive pleiotropy for some genes and singular associations for others in our data may suggest both converging and independent genetic (and neural) pathways mediating schizophrenia risk and pathogenesis
