    The purpose of the article is to describe the main barriers and challenges for the development of telehealth in Peru. This article discusses the role of the digital ecosystem in Latin America, the digital transformation of the health sector and the access to telehealth services in Peru. The barriers and challenges are grouped into components such as: infrastructure, infostructure, regulatory framework, trained health personnel, data management and information security, and the administrative processes for telehealth in Peru. Telemedicine represents an efficient strategy to ensure the continuity of health services to the population in situations such as the current COVID-19 pandemic. It is important to promote telehealth in primary health-care centers, due to their important role in the patient flow management to be referred to a more complex center. In addition, to strengthening the training of human resources in telehealth and digital health is essential to promote the development of telehealth in Peru.El objetivo del artículo es describir las principales barreras y desafíos para el desarrollo de la telesalud en el Perú. Se discute el rol del ecosistema digital en América Latina, la transformación digital en el sector salud, el acceso a la telesalud en el Perú. Las barreras y desafíos se agrupan en componentes tales como: infraestructura, infoestructura, marco normativo, personal de salud capacitado, tratamiento de datos y seguridad informática, y los procesos administrativos para la telesalud en el Perú. La telemedicina representa una estrategia eficiente para garantizar la continuidad de la prestación de servicios de salud a la población en situaciones como la actual pandemia por el COVID-19. Resulta importante impulsar la implementación de la telesalud en los establecimientos de salud que conforman el primer nivel de atención ya que juegan el rol importante en la gestión de pacientes para su referencia a establecimientos de mayor complejidad. Asimismo, fortalecer la formación de recursos humanos en telesalud y la salud digital resulta fundamental para impulsar el desarrollo de la telesalud en el Perú

    The He abundance in the metal-deficient blue compact dwarf galaxies Tol 1214-277 and Tol 65

    We present high-quality Keck telescope spectroscopic observations of the two metal-deficient blue compact dwarf (BCD) galaxies Tol 1214-277 and Tol 65. These data are used to derive the heavy-element and helium abundances. We find that the oxygen abundances in Tol 1214-277 and Tol 65 are the same, 12+logO/H=7.54+/-0.01, or Zsun/24, despite the different ionization conditions in these galaxies. The nitrogen-to-oxygen abundance ratio in both galaxies is logN/O=-1.64+/-0.02 and lies in the narrow range found for the other most metal-deficient BCDs. We use the five strongest HeI emission lines 3889, 4471, 5876, 6678 and 7065, to correct self-consistently their intensities for collisional and fluorescent enhancement mechanisms and to derive the He abundance. Underlying stellar absorption is found to be important for the HeI 4471 emission line in both galaxies, being larger in Tol 65. The weighted He mass fractions in Tol 1214-277 and Tol 65 are respectively Y=0.2458+/-0.0039 and 0.2410+/-0.0050 when the three HeI emission lines, 4471, 5876 and 6678, are used, and are, respectively, 0.2466+/-0.0043 and 0.2463+/-0.0057 when the HeI 4471 emission line is excluded. These values are in very good agreement with recent measurements of the He mass fraction in others of the most metal-deficient BCDs by Izotov and coworkers. We find that the combined effect of the systematic uncertainties due to the underlying HeI stellar absorption lines, ionization and temperature structure of the HII region and collisional excitation of the hydrogen emission lines is likely small, not exceeding ~2% (the error is 2sigma).     Carbon Stars and other Luminous Stellar Populations in M33

    The M33 galaxy is a nearby, relatively metal-poor, late-type spiral. Its proximity and almost face-on inclination means that it projects over a large area on the sky, making it an ideal candidate for wide-field CCD mosaic imaging. Photometry was obtained for more than 10^6 stars covering a 74' x 56' field centered on M33. Main sequence (MS), supergiant branch (SGB), red giant branch (RGB) and asymptotic giant branch (AGB) populations are identified and classified based on broad-band V and I photometry. Narrow-band filters are used to measure spectral features allowing the AGB population to be further divided into C and M-star types. The galactic structure of M33 is examined using star counts, colour-colour and colour-magnitude selected stellar populations. We use the C to M-star ratio to investigate the metallicity gradient in the disk of M33. The C/M-star ratio is found to increase and then flatten with increasing galactocentric radius in agreement with viscous disk formation models. The C-star luminosity function is found to be similar to M31 and the SMC, suggesting that C-stars should be useful distance indicators. The ``spectacular arcs of carbon stars'' in M33 postulated recently by Block et al. (2004) are found in our work to be simply an extension of M33's disk.Comment: 20 pages, 20 figures.     The deployment of zero-emission vehicles has the potential to drastically reduce air pollution and greenhouse gas emissions from road transport. The purpose of this study is to provide evidence on, and quantify the factors that influence, the European market for electric and fuel cell car technologies. The paper reports the results of a stated preference survey among 1,248 car owners in France, Germany, Italy, Poland, Spain and the United Kingdom. The variables that influence powertrain choice are quantified in a nested multinomial logit model. We find that the electric car purchase price continues to be a major deterrent to sales in the surveyed countries. The majority of the respondents considered government incentives as fundamental or important for considering an electric car purchase. Because of the differences in the socio-economic characteristics of consumers in each country, the effectiveness of government incentives may vary across Europe

    LOFAR discovery of a 700-kpc remnant radio galaxy at low redshift

    Remnant radio galaxies represent the final dying phase of radio galaxy evolution, in which the jets are no longer active. Due to their rarity in flux limited samples and the difficulty of identification, this dying phase remains poorly understood and the luminosity evolution largely unconstrained. Here we present the discovery and detailed analysis of a large (700 kpc) remnant radio galaxy with a low surface brightness that has been identified in LOFAR images at 150 MHz. By combining LOFAR data with new follow-up Westerbork observations and archival data at higher frequencies, we investigated the source morphology and spectral properties from 116 to 4850 MHz. By modelling the radio spectrum we probed characteristic timescales of the radio activity. The source has a relatively smooth, diffuse, amorphous appearance together with a very weak central compact core which is associated with the host galaxy located at z=0.051. From our ageing and morphological analysis it is clear that the nuclear engine is currently switched off or, at most, active at a very low power state. The host galaxy is currently interacting with another galaxy located at a projected separation of 15 kpc and a radial velocity offset of 300 km/s. This interaction may have played a role in the triggering and/or shut down of the radio jets. The spectral shape of this remnant radio galaxy differs from the majority of the previously identified remnant sources, which show steep or curved spectra at low to intermediate frequencies. In light of this finding and in preparation for new-generation deep low-frequency surveys, we discuss the selection criteria to be used to select representative samples of these sources.Comment: 10 pages, 6 figures, 5 tables, A&A accepte

All patients received surgical treatment, and most antibiotic regimens (43.2%) included β-lactam. Thirty-four patients (87.17%) were cured; five showed relapse. The final outcome (cure vs. relapse) showed a nonsignificant trend toward higher failure frequency among patients with previous prosthesis (OR: 6.89; 95% CI: 0.80–58.90) or prior surgery and infection (OR: 10.67; 95% IC: 1.08–105.28) in the same joint. Patients treated with clindamycin alone had a higher recurrence rate (40.0% vs. 8.8%). Rifampin treatment did not decrease recurrence in patients treated with β-lactams. Prior prosthesis, surgery, or infection in the same joint might be related to recurrence, and rifampin-based combinations do not seem to improve prognosis. Debridement and implant retention appear a safe option for surgical treatment of early PJI.This work received no specific funding from the public, private, or not-for-profit sectors
