    Distribution and salinity tolerance of the invasive isopod Synidotea laticauda in the Guadalquivir estuary (SW Spain): Field and laboratory observations

In Europe, the exotic species Synidotea laticauda has been introduced in several estuaries but there was not any previous biological study about these non-native populations. The spatio-temporal field distribution of S. laticauda in the last 30 km of the Guadalquivir estuary (salinities 0 to 30) was assessed during 7 years (August 1997-June 2004) by sampling at each new moon with a mesh size net of 1 mm. Survival and osmoregulatory patterns of the species under different experimental salinity conditions were also estimated. This exotic species is a permanent resident of the estuary, with presence of juveniles and adults during most of the year, but showing maximum densities in the warmest period (summer to early autumn). Spatial patterns were closely related to the salinity gradient: the highest densities were observed between 10 and 30 of salinity, with a maximum at 20 (≈ isosmotic point). In fact, the isopod is a weak osmoregulator, which maintains the osmolality of the hemolymph partially independent of the medium osmolality (in a salinity range of 5 to 25). Moreover, survival experiments showed a high tolerance (mortality < 30%) to sudden salinity changes between 2 and 35 and virtually no mortality in salinities (15-25) close to the isosmotic point (20). Osmoregulatory and survival patterns were not dependent on sex but they seemed to be specific-dependent on salinity acclimation. Although it is a euryhaline species, its weak osmoregulatory capacity explains its salinity-dependent distribution pattern. Our results provide a framework to predict the distribution of this invasive species under sceneries of climate change and consequent freshwater scarcity.N

    Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

    Freshwater scarcity effects on the aquatic macrofauna of a European Mediterranean-climate estuary

    In the Mediterranean-climate zone, recurrent drought events and increasing water demand generally lead to a decrease in freshwater input to estuaries. This water scarcity may alter the proper function of estuaries as nursery areas for marine species and as permanent habitat for estuarine species. A 12-year data set of the aquatic macrofauna (fish, decapod and mysid crustaceans) in a Mediterranean estuary (Guadalquivir estuary, South Spain) was analysed,to test if water scarcity favours the nursery function of regional estuaries to the detriment of permanent estuarine inhabitants. Target species typically displayed a salinity-related distribution and estuarine salinisation in dry years resulted in a general upstream community displacement. However, annual densities of marine species were neither consistently higher in dry years nor estuarine species during wet years, Exceptions included the estuarine mysid Neomysis integer and the marine shrimp Crangon crangon, which were more abundant in wet and dry years, respectively. High and persistent turbidity, a collateral effect of water scarcity, altered both the structural (salinity-related pattern) and functional (key prey species and predator density) community characteristics, chiefly after the second drought period of the analysis. The observed high inter-year environmental variability, as well as species-specific effects of water scarcity, suggests that exhaustive and long-term sampling programmes will be required for rigorously monitoring the estuarine communities of the Mediterranean-climate region.Versión del editor3,258

    Gulf of Cadiz zooplankton: community structure, zonation and temporal variation

    This study provides the first comprehensive description of the Gulf of Cadiz (SW Spain) marine zooplankton community. To do this we compiled information from a number of surveys and monitoring programs carried out in the area over the last 14 years. Using this time-series of quarterly (3 times per year) resolution, we describe their general composition, spatio-temporal variability and relationship with environmental factors. Zooplankton is dominated by copepods (mainly Clausocalanus, Paracalanus, Oncaea and Oithona), except in warm periods, when outbreaks of cladocerans (mainly Penilia avirostris but also Podon intermedius and Pleopis polyphemoides) outnumber copepods, accounting for more than 80% of total abundance on occasion. Zooplankton diversity was found to be higher in deeper waters while the density of most taxa (but not their abundance) decreased towards the ocean. On top of this general coast-ocean pattern a north-south gradient could also be distinguished resulting in three distinct zones. In relation to environmental structuring, mesoscale processes, sea surface temperature, winds, river runoff and primary productivity were identified as main factors shaping this zonation. Overall, zooplankton abundance showed a decreasing trend during the study period (2001-2015), caused by a drop of summer abundances, that was mainly driven by cladocerans.Versión del editor3,26

    30-Day morbidity and mortality of bariatric metabolic surgery in adolescence during the COVID-19 pandemic – The GENEVA study

    Background: Metabolic and bariatric surgery (MBS) is an effective treatment for adolescents with severe obesity. Objectives: This study examined the safety of MBS in adolescents during the coronavirus disease 2019 (COVID-19) pandemic. Methods: This was a global, multicentre and observational cohort study of MBS performed between May 01, 2020, and October 10,2020, in 68 centres from 24 countries. Data collection included in-hospital and 30-day COVID-19 and surgery-specific morbidity/mortality. Results: One hundred and seventy adolescent patients (mean age: 17.75 ± 1.30 years), mostly females (n&nbsp;=&nbsp;122, 71.8%), underwent MBS during the study period. The mean pre-operative weight and body mass index were 122.16 ± 15.92 kg and 43.7&nbsp;± 7.11 kg/m2, respectively. Although majority of patients had pre-operative testing for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) (n&nbsp;=&nbsp;146; 85.9%), only 42.4% (n&nbsp;=&nbsp;72) of the patients were asked to self-isolate pre-operatively. Two patients developed symptomatic SARS-CoV-2 infection post-operatively (1.2%). The overall complication rate was 5.3% (n&nbsp;=&nbsp;9). There was no mortality in this cohort. Conclusions: MBS in adolescents with obesity is safe during the COVID-19 pandemic when performed within the context of local precautionary procedures (such as pre-operative testing). The 30-day morbidity rates were similar to those reported pre-pandemic. These data will help facilitate the safe re-introduction of MBS services for this group of patients
