    Efectividad del ácido hialurónico en el tratamiento de enfermedades degenerativas de la articulación temporomandibular. Revisión de Literatura.

    Background: The degenerative diseases of the tempo-romandibular joint (TMJ) are characterized by a progressive destruction of the articular tissues of the condyle and the glenoid fossa. The main aim of this review is to describe the effectiveness of the hyaluronic acid (HA) in the treatment of degenerative diseases of the TMJ in accordance with the available scientific evidence. Material and Methods: A literature search was made in the following databases EBSCO, Pubmed, Cochrane and Trip Database, using the keywords hyaluronic, hyaluronan, NaH, hyaluronate, tmj, tmd, cmd, craniomandibular, orofacial pain and temporomandibular. There were no date or language restrictions applied. Results: After applying inclusion and exclusion criteria, 14 studies were included in this review (11 randomized controlled clinical trials and 3 non-randomized clinical trials). Conclusion: The studies reported a decrease in pain and improvement in functional parameters after treatment of TMJ osteoarthritis with HA. The use of arthrocentesis associated with the administration of HA provides effects synergistic, reaching a superiority the protocols with multiple injections with respect to those of a single session. The adverse effects related to the injection of HA with or without associated arthrocentesis were minor and transitory.Resumen: Objetivo: Las enfermedades degenerativas de la articulación temporomandibular (ATM) se caracterizan por una destrucción progresiva de tejidos articulares en el cóndilo y la fosa glenoidea. El objetivo principal de esta revisión es describir la efectividad del uso de ácido hialurónico en el tratamiento de enfermedades degenerativas de la articulación temporomandibular de acuerdo con la evidencia científica disponible. Material y Métodos: Se realizó una búsqueda de la literatura en las bases de datos electrónicas EBSCO, PubMed, Cochrane y Trip Database, utilizando las palabras claves hyaluronic, hyaluronan, NaH, hyaluronate, tmj, tmd, cmd, craniomandibular, orofacial pain y temporomandibular, sin límite de fecha ni de idioma hasta Mayo del año 2020, complementada con una búsqueda retrógrada. Resultados: Con base en los criterios de inclusión y exclusión, 14 estudios fueron incluidos en esta revisión (11 ensayos clínicos controlados aleatorizados y 3 ensayos clínicos controlados no aleatorizados). Conclusión: Los estudios reportaron una disminución del dolor y mejora en los parámetros funcionales luego del tratamiento de osteoartritis de la ATM con AH. El uso de artrocentesis asociada a la administración del AH provee efectos sinérgicos, alcanzando una superioridad los protocolos con múltiples inyecciones con respecto a aquellos de una sola sesión. Los efectos adversos relacionados con la inyección de AH con o sin artrocentesis asociada fueron menores y transitorios

Además de darnos la oportunidad de entrevistar a una de sus educadoras de párvulos, para poder extraer información necesaria con respecto a las competencias que las mismas poseen en relación al desarrollo de la clase efectiva de educación física, y como ellas promueven la adquisición y desarrollo de las habilidades motrices de sus estudiantes

    El problema de la investigación fue que el restaurante Marcial Tacos y Enchiladas debe difundir y contar con mayor presencia en las plataformas virtuales en tendencia en el momento dentro de grupo de interés, a través de estrategias digitales, buscando siempre estar a la vanguardia de la tecnología para así poder estar en constante innovación del contenido a generar para el público objetivo. El objetivo de la investigación fue determinar la relación del marketing digital y el posicionamiento de la empresa Marcial Tacos & Enchiladas, sede Jardines Oeste, San Juan de Lurigancho - 2020. Estudio de diseño no experimental de corte transversal de un nivel descriptivo correlacional. La población fue de 6000 clientes y la muestra fue probabilístico sistemático, conformado por 173 clientes de la empresa. Los instrumentos utilizados fueron dos; uno para la variable marketing digital conformada por 30 ítems y otro para la variable posicionamiento conformada por 24 ítems, ambos con una escala de medición tipo Likert. Con los resultados obtenidos se encontró que existe relación significativa entre marketing digital y posicionamiento con una significancia de p<0.05(0.000) y una correlación de tipo directa y de grado positiva media de r=0.849. Por otro lado, se encontró que existe relación entre web y posicionamiento con una significancia de p<0.05 (0.000) y una correlación de tipo directa y de grado positiva media de r=0.788. Además, se encontró que existe relación entre redes sociales y posicionamiento con una significancia de p<0.05 (0.000) y una correlación de tipo directa y de grado positiva media de r=0.822

    Post-COVID-19 condition: a sex-based analysis of clinical and laboratory trends

    Background and aimPost-COVID-19 condition (PCC) encompasses long-lasting symptoms in individuals with COVID-19 and is estimated to affect between 31–67% of patients, with women being more commonly affected. No definitive biomarkers have emerged in the acute stage that can help predict the onset of PCC, therefore we aimed at describing sex-disaggregated data of PCC patients from a local cohort and explore potential acute predictors of PCC and neurologic PCC.MethodsA local cohort of consecutive patients admitted with COVID-19 diagnosis between June 2020 and July 2021 were registered, and clinical and laboratory data were recorded. Only those &lt;65 years, discharged alive and followed up at 6 and 12 months after admission were considered in these analyses. Multivariable logistic regression analysis was performed to explore variables associated with PCC (STATA v 18.0).ResultsFrom 130 patients in the cohort, 104 were contacted: 30% were women, median age of 42 years. At 6 months, 71 (68%) reported PCC symptoms. Women exhibited a higher prevalence of any PCC symptom (87 vs. 60%, p = 0.007), lower ferritin (p = 0.001) and procalcitonin (p = 0.021) and higher TNF levels (p = 0.042) in the acute phase compared to men. Being women was independently associated to 7.60 (95% CI 1.27–45.18, p = 0.026) higher risk for PCC. Moreover, women had lower return to normal activities 6 and 12 months.ConclusionOur findings highlight the lasting impact of COVID-19, particularly in young women, emphasising the need for tailored post-COVID care. The lower ferritin levels in women are an intriguing observation, warranting further research. The study argues for comprehensive strategies that address sex-specific challenges in recovery from COVID-19

    Considerations for Prenatal Counselling of Patients with Cardiac Rhabdomyomas based on their Cardiac and Neurologic Outcomes

    Cardiac rhabdomyomas are benign cardiac tumours with few cardiac complications, but with a known association to tuberous sclerosis that affects the neurologic outcome of the patients. We have analysed the long-term cardiac and neurological outcomes of patients with cardiac rhabdomyomas in order to allow comprehensive prenatal counselling, basing our findings on the records of all patients seen prenatally and postnatally with an echocardiographic diagnosis of cardiac rhabdomyoma encountered from August, 1982, to September, 2007. We analysed factors such as the number and the location of the tumours to establish their association with a diagnosis of tuberous sclerosis, predicting the cardiac and neurologic outcomes for the patients. Cardiac complications include arrhythmias, obstruction of the ventricular outflow tracts, and secondary cardiogenic shock. Arrhythmias were encountered most often during the neonatal period, with supraventricular tachycardia being the commonest rhythm disturbance identified. No specific dimension or location of the cardiac rhabdomyomas predicted the disturbances of rhythm. The importance of the diagnosis of tuberous sclerosis is exemplified by the neurodevelopmental complications, with four-fifths of the patients showing epilepsy, and two-thirds having delayed development. The presence of multiple cardiac tumours suggested a higher risk of being affected by tuberous sclerosis. The tumours generally regress after birth, and cardiac-related problems are rare after the perinatal period. Tuberous sclerosis and the associated neurodevelopmental complications dominate the clinical picture, and should form an important aspect of the prenatal counselling of parent

    En la actualidad, todo el mundo se ha visto afectado por un virus inesperado llamado COVID-19. Esto genera que grandes empresas quebraran e incluso cerraran, logrando así despedir a trabajadores sin razón alguna. El emprendimiento realizado por alumnos de la Universidad Peruana de Ciencias Aplicadas (UPC) que se evidenciará a continuación, consiste básicamente en ayudar a los emprendedores mediante una aplicación para que tengan conocimientos de los pasos a realizar en un nuevo emprendimiento de manera eficiente , rápida y legal mediante asesorías con diferentes profesionales. Se generó esta idea de negocio, debido a que en Perú se obtuvieron resultados desfavorables de empresas que despidieron a sus trabajadores sin brindarles la liquidación o incluso sin pagarles el sueldo correspondiente por meses. Los resultados relevantes dentro del proyecto se ven reflejados en los 4 experimentos realizados para validar la intención de compra de los usuarios. Dentro de este se validan los intereses de compra mediante redes sociales tales como: Instagram, WhatsApp y correos electrónicos. Al igual que se evidencian los diferentes resultados de compras de los planes ofrecidos. Proporcionando así el plan financiero del emprendimiento que contará con una inversión inicial del 70% proporcionado por los integrantes del grupo y un 30% de financiamiento por terceros. En conclusión, dentro del proyecto se podrá evidenciar los pasos que hemos desarrollado para poder finalizar con una aplicación eficiente la cual , tiene como segmento a clientes de 18 a 35 años que tengan un estilo de vida progresista.Today, everyone has been affected by an unexpected virus called COVID-19. The entrepreneurship carried out by students at the Peruvian University of Applied Sciences (UPC) that will be evidenced below, is basically to help entrepreneurs by means of an application so that they have knowledge of the steps to be carried out in a new undertaking in an efficient way, fast and legal through advising with different professionals. This business idea was generated, because in Peru there were unfavorable results of companies that dismissed their workers without providing the settlement or even without paying them the corresponding salary for months. The relevant results within the project are reflected in the 4 experiments carried out to validate the purchase intention of the users. Within this are validated buying interests through social networks such as: Instagram, WhatsApp, and emails. As evidenced by the different purchasing results of the offered plans. Thus, providing the financial plan of the venture that will have an initial investment of 70% provided by the members of the group and 30% of financing by third parties. In conclusion, within the project we will be able to show the steps that we have developed to be able to finish with an efficient application which has as a segment client from 18 to 35 years old who have a progressive lifestyle.Trabajo de investigació

    Consequences of gestational diabetes mellitus on neonatal cardiovascular health: MySweetHeart Cohort study

    Hyperglycaemic disorders of pregnancy are associated with offspring cardiovascular alterations. MySweetHeart cohort study aimed to assess the effect of maternal gestational diabetes (GDM) on offsprings' cardiovascular health. Newborns underwent clinical and echocardiographic examinations between 2016 and 2020. Compared to mothers without GDM (n = 141), mothers with GDM (n = 123) were more likely to have had GDM in previous pregnancies and had higher weight, BMI, blood glucose, and HbA1c. Newborns of both groups showed similar clinical characteristics. Echocardiography was performed on the 3rd (interquartile range, IQR, 2nd-4th) day of life in 101 offsprings of mothers without and 116 offsprings of mothers with GDM. Left ventricular (LV) mass was similar. Children born to mothers with GDM had a thicker posterior LV wall (z-score +0.15, IQR -0.38/0.62, versus +0.47, IQR -0.11/+1.1, p = 0.004), a smaller end-systolic (1.3 mL, IQR 1.0-1.5 mL, versus 1.4 mL, IQR 1.2-1.8 mL, p = 0.044) but a similar end-diastolic LV volume. They also had shorter tricuspid valve flow duration and aortic valve ejection time, lower tricuspid E-wave and pulmonary valve velocities. Newborns of mothers with or without GDM had similar clinical characteristics and LV mass. However, some echocardiographic differences were detected, suggesting an altered myocardial physiology among infants of mothers with GDM. ClinicalTrials.gov (NCT02872974). Hyperglycaemic disorders of pregnancy are known to be associated with offspring cardiovascular alterations. Clinical characteristics and estimated left ventricular (LV) mass were similar in children issued from mothers with and without gestational diabetes (GDM). Children born to mothers with GDM had a thicker posterior LV wall and a smaller end-systolic LV volume. Although LV mass is not different, myocardial physiology may be altered in these infants. Further studies should investigate the endothelial function of this population and the cardiovascular evolution of these children over time

    The homotrimeric P2X7 purinergic receptor has sparked interest because of its capacity to sense adenosine triphosphate (ATP) and nicotinamide adenine dinucleotide (NAD) released from cells and to induce calcium signaling and cell death. Here, we examine the response of arginine mutants of P2X7 to soluble and covalently bound ligands. High concentrations of ecto-ATP gate P2X7 by acting as a soluble ligand and low concentrations of ecto-NAD gate P2X7 following ADP-ribosylation at R125 catalyzed by toxin-related ecto-ADP-ribosyltransferase ART2.2. R125 lies on a prominent cysteine-rich finger at the interface of adjacent receptor subunits, and ADP-ribosylation at this site likely places the common adenine nucleotide moiety into the ligand-binding pocket of P2X7
