Clinical-evolutional particularities of the cryoglobulinemic vasculitis in the case of a patient diagnosed with hepatitis C virus in the predialitic phase
Hepatitis C virus (HCV) represents a fundamental issue for public health, with long term evolution and the gradual appearance of several complications and associated pathologies. One of these pathologies is represented by cryoglobulinemic vasculitis, a disorder characterized by the appearance in the patientâs serum of the cryoglobulins, which typically precipitate at temperatures below normal body temperature (37°C) and dissolve again if the serum is heated. Here, we describe the case of a patient diagnosed with HCV that, during the evolution of the hepatic disease, developed a form of cryoglobulinemic vasculitis. The connection between the vasculitis and the hepatic disorder was revealed following treatment with interferon, with the temporary remission of both pathologies and subsequent relapse at the end of the 12 months of treatment, the patient becoming a non-responder. The particularity of the case is represented by both the severity of the vasculitic disease from its onset and the deterioration of renal function up to the predialitic phase, a situation not typical of the evolution of cryoglobulinemia. Taking into account the hepatic disorder, the inevitable evolution towards cirrhosis, and the risk of developing the hepatocellular carcinoma, close monitoring is necessary
Towards Landslide Predictions: Two Case Studies
In a previous work [Helmstetter, 2003], we have proposed a simple physical
model to explain the accelerating displacements preceding some catastrophic
landslides, based on a slider-block model with a state and velocity dependent
friction law. This model predicts two regimes of sliding, stable and unstable
leading to a critical finite-time singularity. This model was calibrated
quantitatively to the displacement and velocity data preceding two landslides,
Vaiont (Italian Alps) and La Clapi\`ere (French Alps), showing that the former
(resp. later) landslide is in the unstable (resp. stable) sliding regime. Here,
we test the predictive skills of the state-and-velocity-dependent model on
these two landslides, using a variety of techniques. For the Vaiont landslide,
our model provides good predictions of the critical time of failure up to 20
days before the collapse. Tests are also presented on the predictability of the

Non-parametric kernel estimation for symmetric Hawkes processes. Application to high frequency financial data
We define a numerical method that provides a non-parametric estimation of the
kernel shape in symmetric multivariate Hawkes processes. This method relies on
second order statistical properties of Hawkes processes that relate the
covariance matrix of the process to the kernel matrix. The square root of the
correlation function is computed using a minimal phase recovering method. We
illustrate our method on some examples and provide an empirical study of the
estimation errors. Within this framework, we analyze high frequency financial
price data modeled as 1D or 2D Hawkes processes. We find slowly decaying
(power-law) kernel shapes suggesting a long memory nature of self-excitation
phenomena at the microstructure level of price dynamics.Comment: 6 figure
The critical earthquake concept applied to mine rockbursts with time-to-failure analysis
We report new tests of the critical earthquake concepts performed on
rockbursts in deep South African mines. We extend the concept of an optimal
time and space correlation region and test it on the eight main shocks of our
catalog provided by ISSI. In a first test, we use the simplest signature of
criticality in terms of a power law time-to-failure formula. Notwithstanding
the fact that the search for the optimal correlation size is performed with
this simple power law, we find evidence both for accelerated seismicity and for
the presence of logperiodic behavior with a prefered scaling factor close to 2.
We then propose a new algorithm based on a space and time smoothing procedure,
which is also intended to account for the finite range and time mechanical
interactions between events. This new algorithm provides a much more robust and
efficient construction of the optimal correlation region, which allows us the
use of the logperiodic formula directly in the search process. In this
preliminary work, we have only tested the new algorithm on the largest event on
the catalog. The result is of remarkable good quality with a dramatic
improvement in accuracy and robustness. This confirms the potential importance
of logperiodic signals. Our study opens the road for an efficient implemention
of a systematic testing procedure of real-time predictions.Comment: 22 pages, 32 figure
Dragon-kings: mechanisms, statistical methods and empirical evidence
This introductory article presents the special Discussion and Debate volume
"From black swans to dragon-kings, is there life beyond power laws?" published
in Eur. Phys. J. Special Topics in May 2012. We summarize and put in
perspective the contributions into three main themes: (i) mechanisms for
dragon-kings, (ii) detection of dragon-kings and statistical tests and (iii)
empirical evidence in a large variety of natural and social systems. Overall,
we are pleased to witness significant advances both in the introduction and
clarification of underlying mechanisms and in the development of novel
efficient tests that demonstrate clear evidence for the presence of
dragon-kings in many systems. However, this positive view should be balanced by
the fact that this remains a very delicate and difficult field, if only due to
the scarcity of data as well as the extraordinary important implications with
respect to hazard assessment, risk control and predictability.Comment: 20 page
Molecular Dynamics Studies of Dislocations in CdTe Crystals from a New Bond Order Potential
Cd1-xZnxTe (CZT) crystals are the leading semiconductors for radiation
detection, but their application is limited by the high cost of detector-grade
materials. High crystal costs primarily result from property non-uniformity
that causes low manufacturing yield. While tremendous efforts have been made in
the past to reduce Te inclusions / precipitates in CZT, this has not resulted
in an anticipated improvement in material property uniformity. Moreover, it is
recognized that in addition to Te particles, dislocation cells can also cause
electric field perturbation and the associated property non-uniformity. Further
improvement of the material, therefore, requires that dislocations in CZT
crystals be understood and controlled. Here we use a recently developed CZT
bond order potential to perform representative molecular dynamics simulations
to study configurations, energies, and mobilities of 29 different types of
possible dislocations in CdTe (i.e., x = 1) crystals. An efficient method to
derive activation free energies and activation volumes of thermally activated
dislocation motion will be explored. Our focus gives insight into understanding
important dislocations in the material, and gives guidance toward experimental
efforts for improving dislocation network structures in CZT crystals
Escape Rates and Physically Relevant Measures for Billiards with Small Holes
We study the billiard map corresponding to a periodic Lorentz gas in
2-dimensions in the presence of small holes in the table. We allow holes in the
form of open sets away from the scatterers as well as segments on the
boundaries of the scatterers. For a large class of smooth initial
distributions, we establish the existence of a common escape rate and
normalized limiting distribution. This limiting distribution is conditionally
invariant and is the natural analogue of the SRB measure of a closed system.
Finally, we prove that as the size of the hole tends to zero, the limiting
distribution converges to the smooth invariant measure of the billiard map.Comment: 39 pages, 4 figure
Beliefs and perceptions about the causes of breast cancer: a case-control study
Background: Attributions of causality are common for many diseases, including breast cancer. The risk of developing breast cancer can be reduced by modifications to lifestyle and behaviours to minimise exposure to specific risk factors, such as obesity. However, these modifications will only occur if women believe that certain behaviours/lifestyle factors have an impact on the development of breast cancer. Method: The Breast Cancer, Environment and Employment Study is a case-control study of breast cancer conducted in Western Australia between 2009 and 2011. As part of the study 1109 women with breast cancer and 1633 women without the disease completed a Risk Perception questionnaire in which they were asked in an open-ended question for specific cause/s to the development of breast cancer in themselves or in others. The study identified specific causal beliefs, and assessed differences in the beliefs between women with and without breast cancer. Results: The most common attributions in women without breast cancer were to familial or inherited factors (77.6%), followed by lifestyle factors, such as poor diet and smoking (47.1%), and environmental factors, such as food additives (45.4%). The most common attributions in women with breast cancer were to mental or emotional factors (46.3%), especially stress, followed by lifestyle factors (38.6%) and physiological factors (37.5%), particularly relating to hormonal history.Conclusions: While the majority of participants in this study provided one or more causal attributions for breast cancer, many of the reported risk factors do not correspond to those generally accepted by the scientific community. These misperceptions could be having a significant impact on the success of prevention and early detection programs that seek to minimise the pain and suffering caused by this disease. In particular, women who have no family history of the disease may not work to minimise their exposure to the modifiable risk factors
The National Adult Inpatient Survey conducted in the English National Health Service from 2002 to 2009: how have the data been used and what do we know as a result?
<p>Abstract</p> <p>Background</p> <p>When it was initiated in 2001, England's national patient survey programme was one of the first in the world and has now been widely emulated in other healthcare systems. The aim of the survey programme was to make the National Health Service (NHS) more "patient centred" and more responsive to patient feedback. The national inpatient survey has now been running in England annually since 2002 gathering data from over 600,000 patients. The aim of this study is to investigate how the data have been used and to summarise what has been learned about patients' evaluation of care as a result.</p> <p>Methods</p> <p>Two independent researchers systematically gathered all research that included analyses of the English national adult inpatient survey data. Journals, databases and relevant websites were searched. Publications prior to 2002 were excluded. Articles were also identified following consultation with experts. All documents were then critically appraised by two co-authors both of whom have a background in statistical analysis.</p> <p>Results</p> <p>We found that the majority of the studies identified were reports produced by organisations contracted to gather the data or co-ordinate the data collection and used mainly descriptive statistics. A few articles used the survey data for evidence based reporting or linked the survey to other healthcare data. The patient's socio-demographic characteristics appeared to influence their evaluation of their care but characteristics of the workforce and the. At a national level, the results of the survey have been remarkably stable over time. Only in those areas where there have been co-ordinated government-led campaigns, targets and incentives, have improvements been shown. The main findings of the review are that while the survey data have been used for different purposes they seem to have incited little academic interest.</p> <p>Conclusions</p> <p>The national inpatient survey has been a useful resource for many authors and organisations but the full potential inherent in this large, longitudinal publicly available dataset about patients' experiences has not as yet been fully exploited.</p> <p>This review suggests that the presence of survey results alone is not enough to improve patients' experiences and further research is required to understand whether and how the survey can be best used to improve standards of care in the NHS.</p
A global agenda for advancing freshwater biodiversity research
Global freshwater biodiversity is declining dramatically, and meeting the challenges of this crisis requires bold goals and the mobilisation of substantial resources. While the reasons are varied, investments in both research and conservation of freshwater biodiversity lag far behind those in the terrestrial and marine realms. Inspired by a global consultation, we identify 15 pressing priority needs, grouped into five research areas, in an effort to support informed stewardship of freshwater biodiversity. The proposed agenda aims to advance freshwater biodiversity research globally as a critical step in improving coordinated actions towards its sustainable management and conservation.Peer reviewe
