    Vehicular ad hoc networking based on the incorporation of geographical information in the IPv6 header

    Several approaches can be identified in the domain of vehicular ad hoc networks (VANET). Internet Protocol version 6 (IPv6) networking and non-IP geographical networking can each fulfill a subset of the application requirements. In general, a combination of both techniques is proposed to meet all of the application requirements. In this case, packets of one VANET routing protocol are encapsulated inside packets of another. This tunneling, together with the position service required for non-IP geographical unicasting, makes such a combined solution rather complex, and hence more challenging to implement, debug, and maintain. In this article, a new VANET approach is presented that relies on the key assumptions that geo-anycast functionality is not required by the applications, and that geographic unicasting is not needed when IP-based unicasting is provided. This enables the adoption of an IPv6-only VANET solution, removing the need for tunneling and position services. New techniques are required to support IPv6-based geo-broadcasting. In this article, it is described how addresses should be assigned, how geographical data can be incorporated in the IPv6 address, how the other IPv6 header fields can be used to contain additional VANET information, and how routing should be handled to guarantee that no modifications are required to the application units. The implementation of the proposed techniques is described, and the correct functionality of the solutions is experimentally demonstrated. Finally, to prove the added value compared to current state-of-the-art propositions, the presented solution is stacked up against the recently released ETSI standards TS 102 636-4-1 (geographical addressing and forwarding) and TS 102 636-6-1 (transmission of IPv6 packets over GeoNetworking protocols)

    Traffic differentiation: a basic step towards providing end-to-end QoS on the train-to-wayside wireless communication system

    We developed a network platform that is responsible for an uninterrupted and seamless connectivity from the train to the wayside through heterogeneous wireless access technologies. However, limiting the offered services to only an onboard Internet is not a feasible business case. A viable one should extend to a broad spectrum of railway communication services like: train control, diagnostics, real time passenger information, entertainment, security CCTV surveillance etc. In a highly dynamic environment (from the communication link point of view) such a fast moving train, it is neccesary to introduce prioritization among different traffic classes. This will implicitly determine under what conditions a certain flow should get suspended or dropped in order to preserve the flows of a higher priority as long as possible and to ensure that they meet their QoS demands.     A novel network architecture for train-to-wayside communication with quality of service over heterogeneous wireless networks

    In the railway industry, there are nowadays different actors who would like to send or receive data from the wayside to an onboard device or vice versa. These actors are e.g., the Train Operation Company, the Train Constructing Company, a Content Provider, etc. This requires a communication module on each train and at the wayside. These modules interact with each other over heterogeneous wireless links. This system is referred to as the Train-to-Wayside Communication System (TWCS). While there are already a lot of deployments using a TWCS, the implementation of quality of service, performance enhancing proxies (PEP) and the network mobility functions have not yet been fully integrated in TWCS systems. Therefore, we propose a novel and modular IPv6-enabled TWCS architecture in this article. It jointly tackles these functions and considers their mutual dependencies and relationships. DiffServ is used to differentiate between service classes and priorities. Virtual local area networks are used to differentiate between different service level agreements. In the PEP, we propose to use a distributed TCP accelerator to optimize bandwidth usage. Concerning network mobility, we propose to use the SCTP protocol (with Dynamic Address Reconfiguration and PR-SCTP extensions) to create a tunnel per wireless link, in order to support the reliable transmission of data between the accelerators. We have analyzed different design choices, pinpointed the main implementation challenges and identified candidate solutions for the different modules in the TWCS system. As such, we present an elaborated framework that can be used for prototyping a fully featured TWCS

    Extracorporeal Membrane Oxygenation in Patients With COVID-19:An International Multicenter Cohort Study

    BACKGROUND: To report and compare the characteristics and outcomes of COVID-19 patients on extracorporeal membrane oxygenation (ECMO) to non-COVID-19 acute respiratory distress syndrome (ARDS) patients on ECMO. METHODS: We performed an international retrospective study of COVID-19 patients on ECMO from 13 intensive care units from March 1 to April 30, 2020. Demographic data, ECMO characteristics and clinical outcomes were collected. The primary outcome was to assess the complication rate and 28-day mortality; the secondary outcome was to compare patient and ECMO characteristics between COVID-19 patients on ECMO and non-COVID-19 related ARDS patients on ECMO (non-COVID-19; January 1, 2018 until July 31, 2019). RESULTS: During the study period 71 COVID-19 patients received ECMO, mostly veno-venous, for a median duration of 13 days (IQR 7-20). ECMO was initiated at 5 days (IQR 3-10) following invasive mechanical ventilation. Median PaO(2)/FiO(2) ratio prior to initiation of ECMO was similar in COVID-19 patients (58 mmHg [IQR 46-76]) and non-COVID-19 patients (53 mmHg [IQR 44-66]), the latter consisting of 48 patients. 28-day mortality was 37% in COVID-19 patients and 27% in non-COVID-19 patients. However, Kaplan-Meier curves showed that after a 100-day follow-up this non-significant difference resolves. Non-surviving COVID-19 patients were more acidotic prior to initiation ECMO, had a shorter ECMO run and fewer received muscle paralysis compared to survivors. CONCLUSIONS: No significant differences in outcomes were found between COVID-19 patients on ECMO and non-COVID-19 ARDS patients on ECMO. This suggests that ECMO could be considered as a supportive therapy in case of refractory respiratory failure in COVID-19

    Plasma Transfusion and Procoagulant Product Administration in Extracorporeal Membrane Oxygenation:A Secondary Analysis of an International Observational Study on Current Practices

    OBJECTIVES: To achieve optimal hemostatic balance in patients on extracorporeal membrane oxygenation (ECMO), a liberal transfusion practice is currently applied despite clear evidence. We aimed to give an overview of the current use of plasma, fibrinogen concentrate, tranexamic acid (TXA), and prothrombin complex concentrate (PCC) in patients on ECMO.DESIGN: A prespecified subanalysis of a multicenter retrospective study. Venovenous (VV)-ECMO and venoarterial (VA)-ECMO are analyzed as separate populations, comparing patients with and without bleeding and with and without thrombotic complications. SETTING: Sixteen international ICUs.PATIENTS: Adult patients on VA-ECMO or VV-ECMO.INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: Of 420 VA-ECMO patients, 59% (n = 247) received plasma, 20% (n = 82) received fibrinogen concentrate, 17% (n = 70) received TXA, and 7% of patients (n = 28) received PCC. Fifty percent of patients (n = 208) suffered bleeding complications and 27% (n = 112) suffered thrombotic complications. More patients with bleeding complications than patients without bleeding complications received plasma (77% vs. 41%, p &lt; 0.001), fibrinogen concentrate (28% vs 11%, p &lt; 0.001), and TXA (23% vs 10%, p &lt; 0.001). More patients with than without thrombotic complications received TXA (24% vs 14%, p = 0.02, odds ratio 1.75) in VA-ECMO, where no difference was seen in VV-ECMO. Of 205 VV-ECMO patients, 40% (n = 81) received plasma, 6% (n = 12) fibrinogen concentrate, 7% (n = 14) TXA, and 5% (n = 10) PCC. Thirty-nine percent (n = 80) of VV-ECMO patients suffered bleeding complications and 23% (n = 48) of patients suffered thrombotic complications. More patients with than without bleeding complications received plasma (58% vs 28%, p &lt; 0.001), fibrinogen concentrate (13% vs 2%, p &lt; 0.01), and TXA (11% vs 2%, p &lt; 0.01). CONCLUSIONS: The majority of patients on ECMO receive transfusions of plasma, procoagulant products, or antifibrinolytics. In a significant part of the plasma transfused patients, this was in the absence of bleeding or prolonged international normalized ratio. This poses the question if these plasma transfusions were administered for another indication or could have been avoided.</p

    Early Palaeozoic ocean anoxia and global warming driven by the evolution of shallow burrowing

    The evolution of burrowing animals forms a defining event in the history of the Earth. It has been hypothesised that the expansion of seafloor burrowing during the Palaeozoic altered the biogeochemistry of the oceans and atmosphere. However, whilst potential impacts of bioturbation on the individual phosphorus, oxygen and sulphur cycles have been considered, combined effects have not been investigated, leading to major uncertainty over the timing and magnitude of the Earth system response to the evolution of bioturbation. Here we integrate the evolution of bioturbation into the COPSE model of global biogeochemical cycling, and compare quantitative model predictions to multiple geochemical proxies. Our results suggest that the advent of shallow burrowing in the early Cambrian contributed to a global low-oxygen state, which prevailed for ~100 million years. This impact of bioturbation on global biogeochemistry likely affected animal evolution through expanded ocean anoxia, high atmospheric CO2 levels and global warming
