    Spontaneous fracture of the mandibular genial tubercles : a case report

    Fracture of the mandibular genial tubercles is an uncommon pathology affecting edentulous patients with severe maxillary atrophy. Usually occurs spontaneously which complicates the diagnosis. Their importance lies in the functional alterations, which occur as a consequence of the disinsertion of the genihyoid and genioglossus muscles. The treatment of fracture of the genial tubercles is controversial, including no surgical intervention, excision of the avulsed bone fragments, and muscular repositioning. There have been only 11 cases reported in the literature of this fracture, most of them spontaneous. We present a difficult diagnosis situation of spontaneous fracture of the genial tubercles in an 86-year-old edentulous female with a painful sublingual and submental hematoma and anterior cervical echimosis. Computerized Tomography should be made to confirm the diagnosis. Surgical treatment was not necessary, and follow-up at 6 months revealed complete symptomatic recovery, and full return of function

    Development of a stress-induced martensitic transformation criterion for a Cu-Al-Be polycrystalline shape memory alloy undergoing uniaxial tension

    This study presents a criterion for predicting the martensitic variants (MVs) that appear during the stress-induced martensitic transformation (SIMT) in a polycrystalline sample of Cu 11.5% wt. Al 0.5% wt. Be under simple tension. Our criterion is based on crystallographic parameters, such as the crystal orientation and Schmid factor (SF). The displacement vector fields (DVFs) were obtained in the observation system by a mathematical model and were used to distort the boundary of a set of grains. From the DVF, the strain tensor for each grain was obtained, and the strain ratio (SR) in the observation system was calculated. Electron backscattering diffraction (EBSD) measurements were performed to determine the crystal orientation of the grains. The inverse SF was used to determine the in-plane stress transformation diagrams (STDs) for each studied grain. The combination of a balance criterion (BC) and STD provided a criterion that allowed us to predict the possible order of stress-induced MVs formed as a function of the crystal orientation and thermomechanical parameters of the shape memory alloy (SMA) with higher accuracy than when using the criteria separately. To validate our criteria, we tested other researchers published results.     Specific gene correction of the AGXT gene and direct cell reprogramming for the treatment of Primary Hyperoxaluria Type 1

    P428 Primary Hyperoxaluria Type 1 (PH1) is an inherited rare metabolic liver disease caused by the deficiency in the alanine: glyoxylate aminotransferase enzyme (AGXT), involved in the glyoxylate metabolism. The only potentially curative treatment is organ transplantation. Thus, the development of new therapeutic approaches for the treatment of these patients appears as a priority.We propose the combination of site-specific gene correction and direct cell reprogramming for the generation of autologous phenotypically healthy induced hepatocytes (iHeps) from skin-derived fibroblast of PH1 patients. For the correction of AGXT mutations, we have designed specific gene editing tools to address gene correction by two different strategies, assisted by CRISPR/Cas9 system. Accurate specific point mutation correction (c.853T-C) has been achieved by homologydirected repair (HDR) with ssODN harbouring wild-type sequence. In the second strategy, an enhanced version ofAGXTcDNAhas been inserted near the transcription start codon of the endogenous gene, constituting an almost universal correction strategy for PH1 mutations. Direct reprogramming of fibroblasts has been conducted by overexpression of hepatic transcription factors and in vitro culture in defined media.     Efeito do expurgo sobre a preservação do alho armazenado

    Stored garlic (Allium sativum L.) is damaged by she insects Plodia interpunctella (Huber), Ephestia cautela (Walker), Ephestia alutella (Hubner) (Lepidoptera, Phycitidae), and Araecerus fasciculatus (De Geer) (Coleoptera, Anthribidae), and the mite Eriophyes tulipae (Keifer 1938) (Acariformes, Eriophyidae). Phosphin and methyl bromide fumigants were tested at four dosage levels and three applications for protection of stored garlic. One sample treated with 4% malathion powder after fumigation and one untreated sample were included as checks. Fumigation plus malathion application showed to be efficient to protect stored garlic from moths and mites. Phosphin was as efficient as methyl bromide for the control of both insects and mites. Damage was less in the malathion treated sample (34.9% damaged bulbils by moths and 45% by mites). Both fumigants were effective in protecting stored garlic. However, methyl bromide at the dosage level of 15 cm3/m3 decreased bulbing of garlic.O alho (Allium sativum L.) armazenado é praguejado pelos insetos Plodia interpunctella (Hubner), Ephestia cautella (Walker), Ephestia elutella (Hubner), (Lepidoptera, Phycitidae), Araecerus fasciculatus (De Geer) (Coleoptera, Anthribidae), e pelo ácaro Eriophyes tulipae (Keifer 1938) (Acariformes, Eriophyidae). Testou-se o expurgo com brometo de metila e fosfina em quatro dosagens diferentes e três aplicações. Todas as parcelas expurgadas foram tratadas com malation 4% após o expurgo. Usaram-se duas testemunhas não expurgadas: uma, tratada com malation, pó, 4%, e outra, sem defensivo. O expurgo + malation demonstrou ser uma prática eficaz para proteger o alho armazenado dos ataques das traças e do ácaro. A fosfina e o brometo de meta igualaram-se no controle destas pragas; as dosagens de 5 cm3/m3 para o brometo e de uma pastilha/m3 para a fosfina foram suficientes numa única aplicação, no primeiro caso, e em três aplicações (outubro, dezembro e fevereiro) no segundo caso. Estas dosagens não prejudicaram a brotação, mas quando a dosagem de brometo de metila foi elevada para 15 cm3/m3, observou-se prejuízo na brotação. O malation, isoladamente, se bem que dando um controle superior à testemunha sem tratamento, não foi eficaz no controle das traças e do ácaro. A presença destas pragas nos bulbos não influenciou o número de brotos resultantes nem o seu peso

    Controlled trial of desmopressin in liver cirrhosis and other conditions associated with a prolonged bleeding time

    The synthetic vasopressin derivative desmopressin (DDAVP) shortens a prolonged bleeding time (BT) in patients with uremia, congenital platelet dysfunction, and von Willebrand disease. To establish the limits of the clinical usefulness of DDAVP, a controlled randomized study was carried out in 53 patients and ten volunteers with different conditions that have in common a prolonged BT. DDAVP significantly shortened the BT in 21 cirrhotics (P less than .01), in eight patients with unclassified prolonged BT (P less than .05) and in ten volunteers taking the antiplatelet drugs aspirin (P less than .05) and ticlopidine. The BT changes were not statistically significant in 15 patients with severe thrombocytopenia nor in nine with congenital platelet dysfunction, even though a few patients with storage pool deficiency responded with a marked BT shortening. Our findings indicate that DDAVP might be given when biopsies or other surgical procedures must be carried out in patients with prolonged BT. However, the compound is often ineffective in patients with thrombocytopenia or congenital platelet dysfunction

    Prevalence and Incidence of Atrial Fibrillation in Heart Failure with Mildly Reduced or Preserved Ejection Fraction:(Additive) Value of Implantable Loop Recorders

    BACKGROUND: Atrial fibrillation (AF) is common in heart failure with mildly reduced or preserved ejection fraction (HFmrEF/HFpEF) and has a negative impact on outcome. Reliable data on prevalence, incidence, and detection of AF from contemporary, prospective HFmrEF/HFpEF studies are scarce.METHODS: This was a prespecified sub-analysis from a prospective, multicenter study. Patients with HFmrEF/HFpEF underwent 12-lead electrocardiography (ECG), 24 h Holter monitoring, and received an implantable loop recorder (ILR) at the study start. During the 2 year follow-up, rhythm monitoring was performed via ILR, yearly ECG, and two yearly 24 h Holter monitors.RESULTS: A total of 113 patients were included (mean age 73 ± 8 years, 75% HFpEF). At baseline, 70 patients (62%) had a diagnosis of AF: 21 paroxysmal, 18 persistent, and 31 permanent AF. At study start, 45 patients were in AF. Of the 43 patients without a history of AF, 19 developed incident AF during a median follow-up of 23 [15-25] months (44%; incidence rate 27.1 (95% confidence interval 16.3-42.4) per 100 person-years). Thus, after the 2-year follow-up, 89 patients (79%) had a diagnosis of AF. In 11/19 incident AF cases (i.e., 58%), AF was solely detected on the ILR. Yearly 12-lead ECG detected six incident AF cases and four of these cases were also detected on two yearly 24 h Holter monitors. Two incident AF cases were detected on an unplanned ECG/Holter.CONCLUSIONS: Atrial fibrillation is extremely common in heart failure with HFmrEF/HFpEF and may inform on symptom evaluation and treatment options. AF screening with an ILR had a much higher diagnostic yield than conventional modalities.</p

Este texto caracteriza os gastos públicos com as ações de Reforma Agrária no Brasil desenvolvidas entre 2000 e 2005, contextualizando-as entre as políticas macroeconômicas realizadas. Além disto, desenvolve e aplica uma nova metodologia para estimar as transferências diretas específi cas da União para assentamento de uma família. Esta estimativa é defi nida a partir da execução orçamentária e de informações gerenciais e cotejada com indicadores de geração de empregos em diversos setores da economia.     Protein levels for Landrace and Duroc boars

    Foi estudado o efeito de quatro sequências de níveis protéicos sobre suínos Duroc e Landrace, submetidos à dois sistemas de alimentação. Os resultados de ganho em peso, dos 25 aos 100 kg de peso vivo, mostraram que a sequência de níveis de 16% de proteína para o crescimento e 14% na terminação, sustentou ganhos significativamente superiores (P <0,05) às sequências de 14-12% não diferindo porém das sequências de 18 - 16 e 20 -18. A melhor conversão alimentar e o menor consumo de alimento foram observados quando se ministrou a sequência protéica de 18 - 16% respectivamente para crescimento - terminação com diferença significativa (P <0,05) em relação a sequência 14 -12% não diferindo porém de 16 - 14 ou 20 - 18%. As características de carcaça estudadas: espessura de toucinho, área do olho de lombo, percentagem de pernil e relação carne/gordura apresentaram linearidade quando feita a análise de regressão; mostrando que a medida que a percentagem de proteína aumentou na ração, houve melhoria das mesmas. O comprimento de carcaça não foi afetado pelos diferentes níveis protéicos utilizados nas rações. Os sistemas de alimentação, controlada ou à vontade, não tiveram nenhum efeito sobre o desempenho dos animais ou nas características de carcaça. A raça Landrace apresentou as melhores características de carcaças (P <0,05) quando comparada com a raça Duroc.The main objective was to study the effects of several protein levels sequences of Landrace and Duroc boars reared under two feeding systems. The results of weight gain from 25 up to 100 kg live weight, showed that the best daily gain was achieved on the sequence levels of 16% protein during growing phase and 14% protein for finishing phase. This daily gain was statistically different (P <0.05) from 18-16% or 20-18% protein seguences. The best feed efficiency and the lowest feed intake were achieved on the sequence levels of 18-16% protein for growing finishing phases, respectively. These data were statistically different (P <0.05) from seguence 14-12% but were not different from sequences 16-14 or 20-18% protein. The carcass traits studied: fat thickness, loin eye area, percent of ham and relation meat/fat - showed a linear response when analyzed through regression. This linearity means that increasing protein levels in the diet, the carcass traits were improved. Carcass lenght was unaffected by protein levels. The feeding systems controlled or ad libitum did not affect performance or carcass traits of the animals. The Landrace showed the best carcass traits (P <0.05) when compared to Duroc breed

A new generation of studies is emerging in this literature that recommends the inclusion of mediating variables between HPWS and organizational performance. The increasing rate of dynamism in competitive environments suggests that measures of employee adaptability should be included as a mechanism that may explain the relevance of HPWS to firm competitiveness.
