Gli aneurismi delle arterie viscerali: esperienza personale e revisione della letteratura
Introduzione: Gli aneurismi delle arterie viscerali costituiscono un'entit\ue0 patologica
rara. La loro incidenza si attesta tra lo 0,01 e il2%. Nella maggior parte dei casi sono asintomatici,
rivelando la propria presenza solo al momento della rottura e risultando associati
ad una mortalit\ue0 elevata.
Scopo: In questo lavoro abbiamo voluto confrontare la nostra esperienza con i dati
presenti in letteratura, con lo scopo di individuare alcune linee di comportamento per
quanto riguarda le indicazioni e il tipo di trattamento da attuare per questi aneurismi.
Materiali e Metodi: Dal 1985 al 2010 sono giunti alla nostra osservazione 42 pazienti
per un totale di 46 lesioni. Le arterie colpite erano: l'a. splenica in 25 casi, l'a. epatica in 6
casi, l'a. renale in 6 casi, il tronco celiaco in 3 casi, l'a. gastroduodenale in 3 casi, l'a. pancreaticoduodenale
in 1 caso, i vasi gastrici in 2 casi. Di questi 26 casi sono stati trattati
chirurgicamente, 19 per via percutanea, 1 non \ue8 stato trattato, ma viene seguito con il follow-
up.
Risultati: La nostra esperienza, in accordo con i dati forniti dalla letteratura, ci suggerisce
un atteggiamento aggressivo per tutti gli aneurismi delle arterie viscerali con diametro
superiore a 2 cm, sintomatici e non. Il trattamento endovascolare \ue8 meno invasivo,
riduce i tempi di degenza e permette di raggiungere lesioni difficilmente aggredibili chirurgicamente.
La chirurgia tradizionale \ue8 indicata in condizioni di emergenza, in lesioni
morfologicamente inadatte all'approccio percutaneo e in condizioni in cui l'occlusione
del vaso espone ad elevato rischio ischemico distale
L\u2019infiltrazione vascolare della vena porta e/o mesenterica superiore \ue8 ancora segno di non resecabilit\ue0 chirurgica nelle neoplasie del pancreas ?
Premessa. Il carcinoma pancreatico infiltra frequentemente la vena porta e/o mesenterica
superiore. La resezione pancreatica combinata a resezione vascolare di vena porta e/
o mesenterica superiore rappresenta in questi casi un trattamento potenzialmente curativo,
ma allo stesso tempo ancora molto controverso.
Metodi. Tramite ricerca computerizzata, sono stati selezionati 12 articoli, pubblicati
nel periodo compreso fra Gennaio 2000 e Marzo 2008. La popolazione risultante era costituita
da 399 pazienti sottoposti a resezione pancreatica associata a resezione vascolare
per adenocarcinoma duttale della testa del pancreas. Sono stati esaminati i dati riguardanti
la selezione preoperatoria, l'intervento, le terapie adiuvanti, l'istopatologia degli specimen
resecati, i risultati perioperatori e la sopravvivenza a distanza. Inoltre, \ue8 stata esaminata
anche la Letteratura antecedente il periodo preso in considerazione.
Risultati. I tassi di mortalit\ue0 operatoria e morbidit\ue0 postoperatoria si variano nei diversi
articoli rispettivamente da 0 a 7,7% e da 16,7% a 54%. La sopravvivenza media \ue8
compresa da 13 a 22 mesi. La percentuale di sopravvivenza a 5 anni va dal 9% al 18 %.
Conclusioni. La Letteratura corrente suggerisce che la resezione pancreatica combinata
ad una resezione vascolare rappresenta un'operazione valida e sicura, che aumenta
il numero di pazienti sottoposti a intervento curativo e quindi fornisce importanti benefici,
in termini di sopravvivenza, a selezionati gruppi di pazienti. Questa procedura, in assenza
di altre controindicazioni alla resezione, dovrebbe sempre essere presa in considerazione,
nel caso in cui si sospetti un coinvolgimento da parte del tumore di vena porta e/o mesenterica
superiore, al fine di ottenere margini di resezione negativi
Should current criteria for detecting and repairing arteriovenous fistula stenosis be reconsidered? Interim analysis of a randomized controlled trial
BACKGROUND:The vascular access guidelines recommend that arteriovenous fistulas (AVFs) with access dysfunction and an access blood flow (Qa) 50%) stenosis, however, may be detected in a well-functioning AVF with a Qa > 500 mL/min, too, but whether it is worth correcting or not remains to be seen.METHODS:In October 2006, we began an open randomized controlled trial enrolling patients with an AVF with subclinical stenosis and Qa > 500 mL/min, to see how elective stenosis repair [treatment group (TX)] influenced access failure (thrombosis or impending thrombosis requiring access revision), or loss and the related cost compared with stenosis correction according to the guidelines, i.e. after the onset of access dysfunction or a Qa 500 mL/min cost-effectively reduces the risk of thrombosis and access loss in comparison with the approach of the Kidney Disease Outcomes Quality Initiative (KDOQI) guidelines, raising the question of whether the currently recommended criteria for assessing and treating stenosis should be reconsidered
Il trattamento del carcinoma mammario invasivo maschile: 40 anni di esperienza in un singolo centro
Purpose: We conducted a retrospective analysis to evaluate the management and outcome of invasive male breast cancer treated in a single-institution over a period of 40 years. Materials and methods: We reviewed the clinical and pathological features of 60 male patients affected by breast carcinoma treated at our Radiotherapy Unit between 1971 and 2011. Tumours were classified according to histological type and the updated 2010 TNM classification of malignant tumours. Results: At a median follow-up of 8.9 [range, 0.6-20; standard deviation (SD), 4.98] years, 32 patients (53.3%) were alive and 16 patients died (26.7%) due to disease progression and 12 (20%) due to other causes. At univariate analysis for overall survival, pathological tumour size (p=0.031), histological subtype (p=0.013) and nodal status (p=0.006) emerged as significant predictors of death. At multivariate analysis, independent death predictors were advanced pathological tumour size (p=0.016), positive nodal status (p=0.003) and invasive cribriform histological type (p=0.0003). Conclusions: In consideration of the rarity of the disease, many issues are still being debated, and future collaborative studies are required. However, our experience confirms the prognostic role of greater pathological tumour size and positive nodal status as unfavourable features for survival in male breast cancer. © 2012 Springer-Verlag Italia
Current advances in solid free-form techniques for osteochondral tissue engineering
Osteochondral (OC) lesions are characterized by defects in two different zones, the
cartilage region and subchondral bone region. These lesions are frequently associated
with mechanical instability, as well as osteoarthritic degenerative changes in the knee.
The lack of spontaneous healing and the drawbacks of the current treatments has
increased the attention from the scientific community to this issue. Different tissue
engineering approaches have been attempted using different polymers and different
scaffolds' processing. However, the current conventional techniques do not allow the
full control over scaffold fabrication, and in this type of approaches, the tuning ability is
the key to success in tissue regeneration. In this sense, the researchers have placed
their efforts in the development of solid free-form (SFF) techniques. These techniques
allow tuning different properties at the micro-macro scale, creating scaffolds with
appropriate features for OC tissue engineering. In this review, it is discussed the
current SFF techniques used in OC tissue engineering and presented their promising
