    Life cycle assessment of cost-optimized buttress earth-retaining walls: A parametric study

    In this paper life cycle assessments are carried out on 30 optimized earth-retaining walls of various heights (4e13 m) and involving different permissible soil stresses (0.2, 0.3 and 0.4 MPa) in Spain. Firstly, the environmental impacts considered in the assessment method developed by the Leiden University (CML 2001) are analyzed for each case, demonstrating the influence of the wall height and permissible soil stress. Secondly, this paper evaluates the contribution range of each element to each impact. The elements considered are: concrete, landfill, machinery, formwork, steel, and transport. Moreover, the influence of the wall height on the contribution of each element over the total impact is studied. This paper then provides the impact factors per unit of concrete, steel, and formwork. These values enable designers to quickly evaluate impacts from available measurements. Finally, the influence of steel recycling on the environmental impacts is highlighted. Findings indicate that concrete is the biggest contributor to all impact categories, especially the global warming potential. However, the steel doubles its contribution when the wall heights increase from 4 m to 13 m.     A cost utility analysis alongside a cluster-randomised trial evaluating a minor ailment service compared to usual care in community pharmacy.

    BACKGROUND: Minor ailments are "self-limiting conditions which may be diagnosed and managed without a medical intervention". A cluster randomised controlled trial (cRCT) was designed to evaluate the clinical, humanistic and economic outcomes of a Minor Ailment Service (MAS) in community pharmacy (CP) compared with usual care (UC). METHODS: The cRCT was conducted for 6 months from December 2017. The pharmacist-patient intervention consisted of a standardised face-to-face consultation on a web-based program using co-developed protocols, pharmacists' training, practice change facilitators and patients' educational material. Patients requesting a non-prescription medication (direct product request) or presenting minor ailments received MAS or UC and were followed-up by telephone 10-days after the consultation. The primary economic outcomes were incremental cost-utility ratio (ICUR) of the service and health related quality of life (HRQoL). Total costs included health system, CPs and patient direct costs: health professionals' consultation time, medication costs, pharmacists' training costs, investment of the pharmacy and consultation costs within the 10 days following the initial consultation. The HRQoL was obtained using the EuroQoL 5D-5L at the time of the consultation and at 10-days follow up. A sensitivity analysis was carried out using bootstrapping. There were two sub-group analyses undertaken, for symptom presentation and direct product requests, to evaluate possible differences. RESULTS: A total of 808 patients (323 MAS and 485 UC) were recruited in 27 CPs with 42 pharmacists (20 MAS and 22 UC). 64.7% (n = 523) of patients responded to follow-up after their consultation in CP. MAS patients gained an additional 0.0003 QALYs (p = 0.053). When considering only MAS patients presenting with symptoms, the ICUR was 24,733€/QALY with a 47.4% probability of cost-effectiveness (willingness to pay of 25,000€/QALY). Although when considering patients presenting for a direct product request, MAS was the dominant strategy with a 93.69% probability of cost-effectiveness. CONCLUSIONS: Expanding community pharmacists' scope through MAS may benefit health systems. To be fully cost effective, MAS should not only include consultations arising from symptom presentation but also include an oversight of self-selected products by patients. MAS increase patient safety through the appropriate use of non-prescription medication and through the direct referral of patients to GP. TRIAL REGISTRATION: ISRCTN, ISRCTN17235323 .     Numerical resolution of the hyperbolic heat equation using smoothed mathematical functions instead of Heaviside and Dirac delta distributions

    The hyperbolic bioheat equation (HBE) has been used to model heating applications involving very short power pulses. This equation includes two mathematical distributions (Heaviside and Delta) which have to be necessarily substituted for smoothed mathematical functions when the HBE is solved by numerical methods. This study focuses on which type of smoothed functions would be suitable for this purpose, i.e. those which would provide solutions similar to those obtained analytically from the original Heaviside and Delta distributions. The logistic function was considered as a substitute for the Heaviside function, while its derivative and the probabilistic Gaussian function were considered as substitutes for the Delta distribution. We also considered polynomial interpolation functions, in particular, the families of smoothed functions with continuous second derivative without overshoot used by COMSOL Multiphysics. All the smoothed functions were used to solve the HBE by the Finite Element Method (COMSOL Multiphysics), and the solutions were compared to those obtained analytically from the original Heaviside and Delta distributions. The results showed that only the COMSOL smoothed functions provide a numerical solution almost identical to the analytical one. Finally, we demonstrated mathematically that in order to find a suitable smoothed function (f) that must adequately substitute any mathematical distribution (D) in the HBE, the difference D - f must have compact support.     Optical spectroscopy of Yb3+ centers in BaMgF4 ferroelectric crystal

Low temperature absorption spectroscopy reveals that Yb3+ incorporates into the matrix at four well differentiated centers. High resolution site selective experiments have been performed to determine the energy level schemes associated with the major Yb3+ centers detected in the system. The fluorescence decay times recorded at 10 K under selective excitation are analyzed for each Yb3+ center. The spectroscopic behavior of the codoped Yb3+:Na+:BaMgF4+ system has been also investigated. Codoping with Na+ eliminates two Yb3+ centers present in the singly doped Yb3+:BaMgF 4+ crystal. The GenoFlow array test showed good sensitivity and specificity compared to the phenotypic Bactec 460TB. Both obesity and SDB have been separately associated with poorer brain health, yet whether SDB severity affects brain health in children with obesity remains unanswered. This study aimed to examine associations of SDB severity with academic performance and brain structure (i.e., total brain and gray and white matter volumes and gray matter volume in the hippocampus) in children with overweight/obesity. One hundred nine children aged 8–12 years with overweight/obesity were included. SDB severity and its subscales (i.e., snoring, daytime sleepiness, and inattention/hyperactivity) were evaluated via the Pediatric Sleep Questionnaire (PSQ), and academic performance was evaluated with the Woodcock-Muñoz standardized test and school grades. Brain structure was assessed by magnetic resonance imaging. SDB severity was not associated with academic performance measured by the standardized test (all |β|> 0.160, P > 0.076), yet it was associated with the school grade point average (β = -0.226, P = 0.007) and natural and social science grades (β = -0.269, P = 0.024). Intention/hyperactivity seemed to drive these associations. No associations were found between SDB severity and the remaining school grades (all β 0.065) or brain volumes (all P > 0.05). Conclusion: Our study shows that SDB severity was associated with lower school grades, yet it was not associated with the standardized measurement of academic performance or with brain volumes in children with overweight/obesity.     Brote nosocomial de COVID-19 en una planta de medicina interna: probable transmisión aérea

    Transmissió aèria; Brot; COVID-19Transmisión aérea; Brote; COVID-19Airborne transmission; Outbreak; COVID-19Background and objectives: Despite the increasing evidence supporting the importance of airborne transmission in SARS-CoV-2 infection, it has not been considered relevant in the vast majority of reported nosocomial outbreaks of COVID-19. The aim of this study is to describe a nosocomial outbreak of SARS-CoV-2 infection whose features suggest that aerosol transmission had an important role. Methods: This is a descriptive analysis of a nosocomial outbreak of SARS-CoV-2 infection in an internal medicine ward that occurred in December 2020. All cases were confirmed by a positive PCR test for SARS-CoV-2. Results: From December 5 to December 17, 21 patients and 44 healthcare workers (HCWs) developed a nosocomial SARS-CoV-2 infection. Fifty-one of the 65 cases (78.5%) were diagnosed between December 6 and 9. The attack rate in patients was 80.8%. Among HCWs, the attack rate was higher in those who had worked at least one full working day in the ward (56.3%) than in those who had occasionally been in the ward (25.8%; p = 0.005). Three days before the first positive case was detected, two extractor fans were found to be defective, affecting the ventilation of three rooms. Sixteen cases were asymptomatic, 48 cases had non-severe symptoms, and 2 cases required admission to the intensive care unit. All patients eventually recovered. Conclusion: The high attack rate, the explosive nature of the outbreak, and the coincidence in time with the breakdown in air extractors in some rooms of the ward suggest that airborne transmission played a key role in the development of the outbreak.Antecedentes y objetivos A pesar de los datos cada vez mayores que respaldan la importancia de la transmisión aérea en la infección por el SARS-CoV-2, en la inmensa mayoría de los brotes nosocomiales descritos de COVID-19 no se ha considerado relevante. El objetivo de este estudio consiste en describir un brote nosocomial de infección por el SARS-CoV-2 cuyas características indican que la transmisión por aerosoles desempeñó un papel importante. Métodos Se trata de un análisis descriptivo de un brote nosocomial de infección por el SARS-CoV-2 en una planta de Medicina Interna que tuvo lugar en diciembre de 2020. Todos los casos se confirmaron mediante una PCR positiva para SARS-CoV-2. Resultados Entre el 5 y el 17 de diciembre, 21 pacientes y 44 profesionales sanitarios contrajeron una infección nosocomial por el SARS-CoV-2. De los 65 casos, 51 (78,5%) se diagnosticaron entre el 6 y el 9 de diciembre. La tasa de ataque en los pacientes fue del 80,8%. Entre los profesionales sanitarios, la tasa de ataque fue mayor en los que habían trabajado al menos una jornada laboral completa en la planta (56,3%) que en los que habían estado ocasionalmente en ella (25,8%; p = 0,005). Tres días antes de detectar el primer caso positivo se identificó una avería en dos extractores de aire, que afectó a la ventilación de tres habitaciones. Dieciséis casos cursaron de forma asintomática, 48 manifestaron síntomas leves y 2 precisaron ingreso en la unidad de cuidados intensivos. Todos los casos se recuperaron finalmente. Conclusiones La elevada tasa de ataque, la naturaleza explosiva del brote y la coincidencia en el tiempo con la avería de los extractores de aire en algunas habitaciones de la planta indican que la transmisión aérea desempeñó un papel fundamental en el desarrollo del brote

    Fodder, Nitrogen, and Energy Balances in Grasslands with Algarroba Trees (Prosopis juliflora (S.W.) DC.) under Dairy Cow Grazing

    The purpose of this study was to evaluate the effect of algarroba (Prosopis juliflora (S.W.) DC.) on fodder, nitrogen and energy contents in Ecuadoran dairy farm grasslands. The study was made at ESPAM bovine facility, 15 meters above sea level, in Manabí, 00º49’23’’, south latitude, and 80º11’01” west longitude, with 962.4 mm of annual precipitation, between September 2011 and December 2014. The stocking rate was 1.09 LU/ha. The areas were populated with 1-4 algarroba trees/ha by 2011, and 8-35 trees/ha, in 2014. Fodder, nitrogen, and energy balances depended on the arborization degree. As a result, 52 t of DM were estimated in 2014, in comparison to the 21 t produced in 2011. Nitrogen was higher with increased arborization between 2011 (60.9 kg/ha), greater nutrient intake from external sources, and 2014 (39.3 kg/ha), with less use of supplements and mineral fertilizers, and greater N2 contribution by arborization. The energy values were higher in 2014, with an increase in algarroba population/ha. The rise in trees/ha in 2014 favored forage yields, with improved N2 and energy efficiency, which was linked to the benefits acquired by the grassland, the contribution of nitrogen to the ecosystem, and the reduction in feed and fertilizer consumption, which led to energy savings

    Pollination Biology and Adaptive Radiation of Agavaceae, with Special Emphasis on the Genus Agave

    Agavaceae are an American family that comprises nine genera and ca. 300 species distributed in arid and semiarid environments, mainly in Mexico. The family is very successful and displays a wide array of ecological, reproductive, and morphological adaptations. Many of its members play important roles as keystone species, because they produce abundant resources during the reproductive season. In this paper we analyze the current knowledge about the pollination ecology of the different genera in the family and the role that pollination systems have played in the ecological and phylogenetic success of the group. After providing an overview of each of the genera in the family, we discuss in detail aspects of the reproductive ecology of species in the genus Agave s.l., which is composed of ca. 208 species and includes subgenera of Agave (Agave and Littaea), Manfreda, Polianthes, and Prochnyanthes. Finally, we describe the results of analyses to test the hypothesis that there has been an adaptive radiation in the genus Agave. Using chloroplast and nuclear DNA sequences we estimate the age of the Agavaceae family and the genus Agave to be 12-26 millions of years ago (MYA) and 10 MYA, respectively, and show that mean rates of diversification were higher in the genus Agave than the genus Yucca. The values we report for rates of diversification in Agave s.l. are high when compared to other radiations in plants and animals. We suggest that the desertification of North America, which started ca. 15 MY A was critical in the radiation of agaves and that the generalist pollination system of Agave has been more successful in generating new species than the extreme specialization of Yucca
