Evolution and treatment of vitamin B12 deficiency as a risk factor for (cognitive and functional) neurodegenerative diseases in institutionalized elderly = Evolución y tratamiento de la deficiencia de vitamina B12 como factor de riesgo de enfermedades neurodegenerativas (cognitivas y funcionales) en las personas mayores institucionalizadas.
Prevalence of vitamin B12 deficiency is very common in elderly people and can reach values as high as 40.5% of the population. It can be the result of the interaction among several factors. Vitamin B12 deficiencies have been associated with neurological, cognitive deterioration, haematological abnormalities and cardiovascular diseases that have an important influence on the health of the elderly and their quality of life. It is necessary to approach the problems arisen from the lack of data relative to them. The main objective of this thesis was to analyse the evolution of vitamin B12 status and related parameters, lipid and haematological profiles and their relationship to health risk factors, and to functional and cognitive status over one year and to determine the effect of an oral supplementation of 500 μg of cyanocobalamin for a short period of 28 days. An additional objective was to analyze the possible effects of medicine intakes on vitamin B status. Three studies were performed: a) a one year longitudinal follow-up with four measure points; b) an intervention study providing an oral liquid supplement of 500 μg of cyanocobalamin for a 28 days period; and c) analysis of the possible effect of medication intake on vitamin B status using the ATC classification of medicines. The participants for these studies were recruited from nursing homes for the elderly in the Region of Madrid. Sixty elders (mean age 84 _ 7y, 19 men and 41 women) were recruited for Study I and 64 elders (mean age 82 _ 7y, 24 men and 40 women) for Study II. For Study III, baseline data from the initially recruited participants of the first two studies were used. An informed consent was obtained from all participants or their mentors. The studies were approved by the Ethical Committee of the University of Granada. Blood samples were obtained at each examination date and were analyzed for serum cobalamin, holoTC, serum and RBC folate and total homocysteine according to laboratory standard procedures. The haematological parameters analyzed were haematocrit, haemoglobin and MCV. For the lipid profile TG, total cholesterol, LDL- and HDLcholesterol were analyzed. Anthropometric measures (BMI, skinfolds [triceps and subscapular], waist girth and waist to hip ratio), functional tests (hand grip, arm and leg strength tests, static balance) and MMSE were obtained or administered by trained personal. The vitamin B12 supplement of Study II was administered with breakfast and the medication intake was taken from the residents’ anamnesis. Data were analyzed by parametric and non-parametric statistics depending on the obtained data. Comparisons were done using the appropriate ANOVAs or non-parametric tests. Pearsons’ partial correlations with the variable “time” as control were used to define the association of the analyzed parameters. XIII The results showed that: A) Over one year, in relationship to vitamin B status, serum cobalamin decreased, serum folate and mean corpuscular volumen increased significantly and total homocysteine concentrations were stable. Regarding blood lipid profile, triglycerides increased and HDL-cholesterol decreased significantly. Regarding selected anthropometric measurements, waist circumference increased significantly. No significant changes were observed for the rest of parameters. B) Prevalence of hyperhomocysteinemia was high in the elderly studied, ranging from 60% to 90 % over the year depending on the cut-off used for the classification. LDL-cholesterol values were high, especially among women, and showed a tendency to increase over the year. Results of the balance test showed a deficiency and a tendency to decrease; this indicates that the population studied is at high risk for falls. Lower extremity muscular function was deficient and showed a tendency to decrease. A highly significant relationship was observed between the skinfold of the triceps and blood lipid profile. C) Low cobalamin concentrations correlated significantly with low MMSE scores in the elderly studied. No correlations were observed between vitamin B12 status and functional parameters. D) Regarding vitamin B12 status, holo-transcobalamin seems to be more sensitive for diagnosis; 5-10% of the elderly had a deficiency using serum cobalamin as a criterion, and 45-52% had a deficiency when using serum holotranscobalamin as a criterion. E) 500 μg of cyanocobalamin administered orally during 28 days significantly improved vitamin B12 status and significantly decreased total homocysteine concentrations in institutionalized elderly. No effect of the intervention was observed on functional and cognitive parameters. F) The relative change (%) of improvement of vitamin B12 status was higher when using serum holo-transcobalamin as a criterion than serum cobalamin. G) Antiaenemic drug intake normalized cobalamin, urologic drugs and corticosteroids serum folate, and psychoanaleptics holo-transcobalamin levels. Drugs treating pulmonary obstruction increased total homocysteine concentration significantly. H) The daily mean drug intake was 5.1. Fiftynine percent of the elderly took medication belonging to 5 or more different ATC groups. The most prevalent were psycholeptic (53%), antiacid (53%) and antithrombotic (47%) drugs
A review of Tai Chi Chuan and parameters related to balance
Introduction: Previous systematic reviews of the literature on the effects of Tai Chi Chuan(TCC)on balance have focussed either on determining the quality of the research design or have provided just a general description of the studies.To the best of our knowledge none have approached
this topic by conducting an analysis from the point of view of the factors which affect balance.It is important to present this perspective as it will help to guide future research in this field.
Methodology: Seven electronic data bases were searched for publications dated between 1996 and 2012.The inclusion criteria were;randomized controlled trials(RCT)written in English.
Results: From a total of 397 articles identified, 27 randomized controlled trials were eligible for the analysis.
Conclusions: Studies reviewed appear to confirm that TCC improves static and dynamic balance and in the functional factors which affect balance in persons of over 55 years of age.Only one study was identified on people affected with problems with the vestibular system. No studies on the
influence of TCC on improvement in balance in individuals suffering from deteriorated brain function were identified
Evaluation of features for the automatic recognition of OFDM signals in monitoring or cognitive receivers, Journal of Telecommunications and Information Technology, 2008, nr 2
The automatic recognition of signal types is an important task of monitoring receivers and also cognitive receivers. Several modulation recognition or classification procedures exist for single channel signal types while a simple robust procedure for automatic recognition of OFDM signals is lacking because of its numerous frequency channelslying close together. The task considered in this paper is the discrimination between OFDM (or multi-channel) signals and other signal types. The number of frequency channels of the OFDM signals is assumed to be unknown a priori. So, together with the automatic OFDM detection the estimation of the number of frequency channels is treated. Several discrimination features have been examined and the most promising ones are described: measures of the variation, of the skewness, of the kurtosis, and of the specific picket-fence shape of the spectrum which is typical for many OFDM signals. For a number of real-world OFDM samples, recorded from the high frequency range, results are presented. An automatic discrimination from single channel or noise like signals is achieved and the number of system channels can be estimated
El ejercicio agudo aumenta las concentraciones de homocisteína en varones físicamente activos
Introducción: Niveles altos de Homocisteína (Hcy) se
han identificado como un factor de riesgo cardiovascular.
En relación con la práctica de ejercicio físico, los resultados son contradictorios.
Objetivos: El objetivo del presente estudio fue determinar
la influencia de ejercicios agudos máximo y submáximo
sobre las concentraciones de homocisteína total (tHcy) y otros parámetros sanguíneos relacionados.
Material y métodos: Diez varones (23,5 ± 1,8 años) físicamente activos realizaron una prueba incremental
máxima y otra submáxima a una intensidad del 65% del
consumo máximo de oxígeno (VO2max) en tapiz rodante. Se
analizaron antes y después las concentraciones de tHcy,
folato, vitamina B12 y creatinina séricas.
Resultados: Las concentraciones de tHcy séricas
aumentaron significativamente tras las pruebas de intensidad
máxima (p < 0,05) y submáxima (p < 0,01). El folato
y la vitamina B12 también aumentaron significativamente
tras ambas pruebas (p < 0,05). Las concentraciones de
creatinina aumentaron significativamente únicamente en
la prueba máxima (p < 0,001). Se encontró una relación
inversa entre los niveles de folato y de tHcy en todos los
puntos (p < 0,05).
Conclusión: Se observaron niveles altos de homocisteína
después del ejercicio agudo tanto máximo como submáximo
A pilot case study on the impact of a self-prescribed ketogenic diet on biochemical parameters and running performance in healthy and physically active individuals
Background: Ketogenic diets (KDs) have gained some popularity not only as effective weight-loss diets and treatment options for several diseases, but also among healthy and physically active individuals for various reasons. However, data on the effects of ketosis in the latter group of individuals are scarce. We therefore collected pilot data on the physiological response to a self-prescribed ketogenic diet lasting 5-7 weeks in a small cohort of healthy and physically active individuals. Methods: Twelve subjects (7 males, 5 females, age 24-60 years) who followed moderate to intensive exercise routines underwent blood testing, bioelectrical impedance analysis (BIA) and spiroergometry during an incremental treadmill test. On the next day, they went on a self-prescribed KD for a median of 38 days (range 35-50 days), after which the same tests were performed again. Ketosis was self-monitored by urinary ketone strips. Subjective feeling during the diet was assessed by a questionnaire after the intervention. Due to the small and heterogenous sample, the results are interpreted in the context of the already existing literature. Results: The KDs were tolerated well by the majority of individuals. Impaired recovery from exercise remained the most frequently reported side effect until the end of the study. Most blood parameters remained stable during the intervention. However, there were significant elevations of total and LDL cholesterol concentrations (p<0.01) and a trend towards increased HDL-cholesterol (p=0.05). The drastic reduction of carbohydrates had no statistically significant influence on running performance judged by the time to exhaustion, VO2max and respiratory compensation points. BIA measurements showed significant increases in phase angle (p=0.01) indicating improvements of body composition with an estimated decrease of 3.4 kg of fat mass (p=0.002) and gain of 1.3 kg of fat free mass. We discuss the validity of these estimates taking into account a possibly altered hydration status due to the KD. Conclusions: Active healthy individuals will probably experience no major problems during a short term KD lasting several weeks. The drastically reduced carbohydrate content of the diet seems to be no limiting factor for running performance. In addition, improvements in body composition can be expected. While most biochemical parameters are not influenced by the diet, there seems to be an impact on the blood lipid profile that could be considered problematic with respect to cardiovascular disease risk. However, the predictive role of cholesterol levels alone in individuals undergoing regular physical activity remains to be elucidated

Harmonization Process and Reliability Assessment of Anthropometric Measurements in the Elderly EXERNET Multi-Centre Study
Background: The elderly EXERNET multi-centre study aims to collect normative anthropometric data for old functionally
independent adults living in Spain.Purpose: To describe the standardization process and reliability of the anthropometric measurements carried out in the
pilot study and during the final workshop, examining both intra- and inter-rater errors for measurements.Materials and Methods: A total of 98 elderly from five different regions participated in the intra-rater error assessment, and
10 different seniors living in the city of Toledo (Spain) participated in the inter-rater assessment. We examined both intra-
and inter-rater errors for heights and circumferences.Results: For height, intra-rater technical errors of measurement (TEMs) were smaller than 0.25 cm. For circumferences and
knee height, TEMs were smaller than 1 cm, except for waist circumference in the city of Ca´ceres. Reliability for heights and
circumferences was greater than 98% in all cases. Inter-rater TEMs were 0.61 cm for height, 0.75 cm for knee-height and
ranged between 2.70 and 3.09 cm for the circumferences measured. Inter-rater reliabilities for anthropometric
measurements were always higher than 90%.Conclusion: The harmonization process, including the workshop and pilot study, guarantee the quality of the
anthropometric measurements in the elderly EXERNET multi-centre study. High reliability and low TEM may be expected

Pamidronic acid and cabergoline as effective long-term therapy in a 12-year-old girl with extended facial polyostotic fibrous dysplasia, prolactinoma and acromegaly in McCune-Albright syndrome: a case report
<p>Abstract</p> <p>Introduction</p> <p>McCune-Albright syndrome is a complex inborn disorder due to early embryonal postzygotic somatic activating mutations in the <it>GNAS</it>1 gene. The phenotype is very heterogeneous and includes polyostotic fibrous dysplasia, typically involving the facial skull, numerous café-au-lait spots and autonomous hyperfunctions of several endocrine systems, leading to hyperthyroidism, hypercortisolism, precocious puberty and acromegaly.</p> <p>Case presentation</p> <p>Here, we describe a 12-year-old Caucasian girl with severe facial involvement of fibrous dysplasia, along with massive acromegaly due to growth hormone excess and precocious puberty, with a prolactinoma. Our patient was treated with a bisphosphonate and the prolactin antagonist, cabergoline, resulting in the inhibition of fibrous dysplasia and involution of both the prolactinoma and growth hormone excess. During a follow-up of more than two years, no severe side effects were noted.</p> <p>Conclusion</p> <p>Treatment with bisphosphonates in combination with cabergoline is a suitable option in patients with McCune-Albright syndrome, especially in order to circumvent surgical interventions in patients suffering from polyostotic fibrous dysplasia involving the skull base.</p
Digital single-operator pancreatoscopy for the treatment of symptomatic pancreatic duct stones: a prospective multicenter cohort trial
BACKGROUND
Digital single-operator pancreatoscopy (DSOP)-guided lithotripsy is a novel treatment modality for pancreatic endotherapy, with demonstrated technical success in retrospective series of between 88 % and 100 %. The aim of this prospective multicenter trial was to systematically evaluate DSOP in patients with chronic pancreatitis and symptomatic pancreatic duct stones.
METHODS
Patients with symptomatic chronic pancreatitis and three or fewer stones ≥ 5mm in the main pancreatic duct (MPD) of the pancreatic head or body were included. The primary end point was complete stone clearance (CSC) in three or fewer treatment sessions with DSOP. Current guidelines recommend extracorporeal shock wave lithotripsy (ESWL) for MPD stones > 5 mm. A performance goal was developed to show that the CSC rate of MPD stones using DSOP was above what has been previously reported for ESWL. Secondary end points were pain relief measured with the Izbicki pain score (IPS), number of interventions, and serious adverse events (SAEs).
RESULTS
40 chronic pancreatitis patients were included. CSC was achieved in 90 % of patients (36/40) on intention-to-treat analysis, after a mean (SD) of 1.36 (0.64) interventions (53 procedures in total). The mean (SD) baseline IPS decreased from 55.3 (46.2) to 10.9 (18.3). Overall pain relief was achieved in 82.4 % (28/34) after 6 months of follow-up, with complete pain relief in 61.8 % (21/34) and partial pain relief in 20.6 % (7/34). SAEs occurred in 12.5 % of patients (5/40), with all treated conservatively.
CONCLUSION
DSOP-guided endotherapy is effective and safe for the treatment of symptomatic MPD stones in highly selected patients with chronic pancreatitis. It significantly reduces pain and could be considered as an alternative to standard ERCP techniques for MPD stone treatment in these patients
Valores de referencia para leptina, cortisol, insulina y glucosa entre los adolescentes europeos y su asociación con adiposidad: Estudio Helena
Background and Objective: Adequate concentrations
of leptin, cortisol, and insulin are important for a suitable
metabolism and development during adolescence. These
hormones jointly with glucose play a major role in fat
metabolism and development of childhood obesity. Our
main objective was to quantify biomarkers as leptin, cortisol,
insulin and glucose status in European adolescents
to contribute to establish reference ranges.
Methods: A representative sample of 927 adolescents
(45% males, 14.9±1.2 years for the overall population)
from ten European cities of the HELENA study was used
to obtain fasting blood samples for these biomarkers. The
percentile distributions were computed by sex and age
and percentiles were associated with BMI classification.
Results: Serum leptin concentration in adolescents
varied significantly according to BMI, sex and age (all
p<0.001). Cortisol presented a tendency to increase with
age, both for females and males, while insulin and glucose
were stable with age. Leptin and insulin were highest
in obese adolescents (p<0.001), whilst cortisol and glucose
did not vary with BMI. Percentiles 5, 25, 50, 75 and 95, 26.70 and 65.33 ng/ml for leptin; 5.00, 8.11, 11.14, 15.00
and 24.51 μg/dl for cortisol and 3.65, 6.15, 8.52, 11.90 and
20.53 μlU/ml for insulin.
Conclusions: In adolescents, leptin, cortisol, insulin
and glucose concentrations are differently affected by
age, sex and BMI. Establishment of reference ranges
(percentiles) of these biomarkers would be of great interest
when pediatricians have to assess the trend of an
adolescent to develop obesity years after.Objetivo: Concentraciones adecuadas de leptina, cortisol
e insulina son importantes para un metabolismo normal
durante la adolescencia, puesto que valores alterados
de estas hormonas, junto con la glucosa, se asocian con el
desarrollo de la obesidad infantil. Nuestro principal objetivo
fue cuantificar estos marcadores en adolescentes
europeos con el fin de establecer rangos de referencia.
Métodos: Muestras de sangre procedentes de 927 adolescentes
en ayunas (14,9 ± 1,2 años, 45% varones, estudio
HELENA), fueron analizadas para cuantificar la
leptina, cortisol, insulina y glucosa. Las distribuciones de
percentiles se determinaron teniendo en cuenta el sexo y
la edad. También se estudió la asociación entre percentiles
y la clasificación del IMC.
Resultados: La concentración de leptina en suero variaba
significativamente con el IMC, el sexo y la edad
(todos p<0,001). El cortisol presentó una tendencia a aumentar
con la edad, tanto para varones como mujeres,
mientras que la insulina y la glucosa eran estables con
la edad. La leptina y la insulina fueron más altas en los
adolescentes obesos (p <0,001), mientras que el cortisol y
glucosa no variaron con el IMC. Los percentiles 5, 25, 50,
75 y 95, para los valores de hormonas fueron, respectivamente:
1.27, 4.06, 11.54, 26.70 y 65.33 ng/ml para la leptina;
5.00, 8.11, 11.14, 15.00 y 24.51 μg/dl para el cortisol y
