    Udar mózgu jest drugą lub trzecią pod względem częstości przyczyną zgonów na świecie oraz wiodącą przyczyną trwałego inwalidztwa, szczególnie w krajach wysokorozwiniętych. We wstępie pracy, dotyczącym ogólnych aspektów rozpoznawania udaru, autor przedstawił aktualne wytyczne postępowania w udarze mózgu, przede wszystkim na podstawie standardów opracowanych przez European Stroke Initiative (EUSI), uzupełniając je o przegląd licznych doniesień z ostatnich lat. Autor zaprezentował współczesny stan wiedzy na temat monitorowania stanu chorego w ostrym okresie udaru mózgu, z uwzględnieniem aspektów praktycznych, między innymi wpływu poszczególnych parametrów na rokowanie i śmiertelność oraz warunków funkcjonowania oddziału udarowego. Podstawowe zadania terapeutyczne w ostrej fazie udaru to: stabilizacja ogólnego stanu chorego; ukierunkowane leczenie, na które składają się ewentualna rewaskularyzacja i neuroprotekcja; profilaktyka i leczenie powikłań, takich jak: wtórne ukrwotocznienie udaru, obrzęk mózgu, napady padaczkowe, infekcje, odleżyny, zakrzepowe zapalenie żył głębokich powikłane zatorowością płucną; wczesna rehabilitacja. W dalszej części pracy omówiono techniki diagnostyczne przydatne w ostrej fazie udaru mózgu, a następnie szczegółowo przedstawiono zasady monitorowania podstawowych funkcji życiowych, objawów neurologicznych ze szczególnym uwzględnieniem stanu świadomości, układu oddechowego, równowagi wodno-elektrolitowej, ciśnienia śródczaszkowego, funkcji układu krążenia, ciśnienia tętniczego, metabolizmu glukozy oraz termoregulacji.Acute stroke is one of the leading causes of morbidity and mortality worldwide. At the same time stroke has increasingly been recognised as a medical emergency like myocardial infarction or severe trauma. Acute care of stroke patients in specialised stroke units has been proven to be effective in acute ischaemic stroke. Accordingly, several neurological societies have established guidelines for the management of acute stroke. A comprehensive set of recommendations, together with an overview of established and widely used therapeutic strategies, has been published by the European Stroke Initiative (EUSI). While revascularisation therapies such as IV and IA thrombolysis emerged as the only specific treatments in acute stroke to have been demonstrated as being effective in multicentre randomised controlled trials, the general supportive care and treatment of acute complications are of equal importance, even if these procedures have not been investigated in randomised controlled trials.     Cigarette smoking and cerebral microvasculature in patients with type 1 diabetes: a pilot study

    Introduction. A decrease in vasomotor reactivity reserve (VMRr) or an increase in pulsatility index (PI) are the early signs of cerebral microangiopathy in type 1 diabetes. Cigarette smoking is a risk factor for microvascular complications of type 1 diabetes, but cigarette smokers are routinely excluded from studies on VMRr or PI in type 1 diabetes (T1DM) and there is no evidence of any significant impact of smoking on these variables in T1DM. Therefore, we aimed to assess the impact of cigarette smoking on VMRr and PI in these patients. Methods. VMRr and PI of the middle cerebral artery were measured with Transcranial Doppler in 79 patients with T1DM (median age 33.0 years, range 20–51, 44% males) without a history of cerebrovascular events, coronary heart disease or carotid stenosis. The relationship between cigarette smoking (n = 20, mean pack-years 9.4 ± 6.1) and VMRr, PI, concomitant risk factors, medications and the presence of systemic microvascular complications were analysed. Results. Smokers and non-smokers did not differ in terms of their clinical characteristics, with an exception of higher circadian insulin demand in smokers (60 ± 12.9 v. 49.2 ± 14.2 units; p = 0.004). A correlation between pack-years and PI (r = 0.6, p = 0.004), but not with VMRr, was found in smokers. However, no significant differences between smokers and non-smokers were found regarding either VMRr (mean 85.9 ± 20% v. 84.1 ± 20.1%; p = 0.74) or PI (median 0.85, range 0.61–1.09 v. 0.88, range 0.48–1.52; p = 0.2). Conclusions. We did not prove any significant impact of smoking on VMRr in T1DM patients, but the association between pack-years and PI may indicate the negative impact of intensive cigarette smoking on the cerebral microvasculature in type 1 diabetes.

    Cervical Artery Dissection in Young Adults in the Stroke in Young Fabry Patients (sifap1) Study

    Background: Patients with carotid artery dissection (CAD) have been reported to have different vascular risk factor profiles and clinical outcomes to those with vertebral artery dissection (VAD). However, there are limited data from recent, large international studies comparing risk factors and clinical features in patients with cervical artery dissection (CeAD) with other TIA or ischemic stroke (IS) patients of similar age and sex. Methods: We analysed demographic, clinical and risk factor profiles in TIA and IS patients ≤55 years of age with and without CeAD in the large European, multi-centre, Stroke In young FAbry Patients 1 (sifap1) study. Patients were further categorised according to age (younger: 18-44 years; middle-aged: 45-55 years), sex, and site of dissection. Results: Data on the presence of dissection were available in 4,208 TIA and IS patients of whom 439 (10.4%) had CeAD: 196 (50.1%) had CAD, 195 (49.9%) had VAD, and 48 had multiple artery dissections or no information regarding the dissected artery. The prevalence of CAD was higher in women than in men (5.9 vs. 3.8%, p < 0.01), whereas the prevalence of VAD was similar in women and men (4.6 vs. 4.7%, n.s.). Patients with VAD were younger than patients with CAD (median = 41 years (IQR = 35-47 years) versus median = 45 years (IQR = 39-49 years); p < 0.01). At stroke onset, about twice as many patients with either CAD (54.0 vs. 23.1%, p < 0.001) or VAD (63.4 vs. 36.6%, p < 0.001) had headache than patients without CeAD and stroke in the anterior or posterior circulation, respectively. Compared to patients without CeAD, hypertension, concomitant cardiovascular diseases and a patent foramen ovale were significantly less prevalent in both CAD and VAD patients, whereas tobacco smoking, physical inactivity, obesity and a family history of cerebrovascular diseases were found less frequently in CAD patients, but not in VAD patients. A history of migraine was observed at a similar frequency in patients with CAD (31%), VAD (27.8%) and in those without CeAD (25.8%). Conclusions: We identified clinical features and risk factor profiles that are specific to young patients with CeAD, and to subgroups with either CAD or VAD compared to patients without CeAD. Therefore, our data support the concept that certain vascular risk factors differentially affect the risk of CAD and VAD

    Lack of association between proton pump inhibitor use and brain aging: a cross-sectional study

    PURPOSE Due to conflicting scientific evidence for an increased risk of dementia by intake of proton pump inhibitors (PPIs), this study investigates associations between PPI use and brain volumes, estimated brain age, and cognitive function in the general population. METHODS Two surveys of the population-based Study of Health in Pomerania (SHIP) conducted in Northeast Germany were used. In total, 2653 participants underwent brain magnetic resonance imaging (MRI) and were included in the primary analysis. They were divided into two groups according to their PPI intake and compared with regard to their brain volumes (gray matter, white matter, total brain, and hippocampus) and estimated brain age. Multiple regression was used to adjust for confounding factors. Cognitive function was evaluated by the Verbal Learning and Memory Test (VLMT) and the Nuremberg Age Inventory (NAI) and put in relation to PPI use. RESULTS No association was found between PPI use and brain volumes or the estimated brain age. The VLMT score was 1.11 lower (95% confidence interval: - 2.06 to - 0.16) in immediate recall, and 0.72 lower (95% CI: - 1.22 to - 0.22) in delayed recall in PPI users than in non-users. PPI use was unrelated to the NAI score. CONCLUSIONS The present study does not support a relationship between PPI use and brain aging

    Meta-analysis of genome-wide association studies from the CHARGE consortium identifies common variants associated with carotid intima media thickness and plaque

    Carotid intima media thickness (cIMT) and plaque determined by ultrasonography are established measures of subclinical atherosclerosis that each predicts future cardiovascular disease events. We conducted a meta-analysis of genome-wide association data in 31,211 participants of European ancestry from nine large studies in the setting of the Cohorts for Heart and Aging Research in Genomic Epidemiology (CHARGE) Consortium. We then sought additional evidence to support our findings among 11,273 individuals using data from seven additional studies. In the combined meta-analysis, we identified three genomic regions associated with common carotid intima media thickness and two different regions associated with the presence of carotid plaque (P < 5 × 10 -8). The associated SNPs mapped in or near genes related to cellular signaling, lipid metabolism and blood pressure homeostasis, and two of the regions were associated with coronary artery disease (P < 0.006) in the Coronary Artery Disease Genome-Wide Replication and Meta-Analysis (CARDIoGRAM) consortium. Our findings may provide new insight into pathways leading to subclinical atherosclerosis and subsequent cardiovascular events

Это обуславливает высокую актуальность применения геттерирования в современной технологии микроэлектроники. В статье рассмотрены существующие методы геттерирования кремниевых пластин и механизмы их протекания.Збільшення ступеня інтеграції елементної бази пред'являє все більш жорсткі вимоги до зменшення концентрації забруднюючих домішок та окислювальних дефектів упаковки у вихідних кремнієвих пластинах за її збереження у технологічному циклі виготовлення ІМС. Це обумовлює високу актуальність застосування гетерування в сучасній технології мікроелектроніки.     Identification of additional risk loci for stroke and small vessel disease: a meta-analysis of genome-wide association studies

    BACKGROUND: Genetic determinants of stroke, the leading neurological cause of death and disability, are poorly understood and have seldom been explored in the general population. Our aim was to identify additional loci for stroke by doing a meta-analysis of genome-wide association studies. METHODS: For the discovery sample, we did a genome-wide analysis of common genetic variants associated with incident stroke risk in 18 population-based cohorts comprising 84 961 participants, of whom 4348 had stroke. Stroke diagnosis was ascertained and validated by the study investigators. Mean age at stroke ranged from 45·8 years to 76·4 years, and data collection in the studies took place between 1948 and 2013. We did validation analyses for variants yielding a significant association (at p<5 × 10(-6)) with all-stroke, ischaemic stroke, cardioembolic ischaemic stroke, or non-cardioembolic ischaemic stroke in the largest available cross-sectional studies (70 804 participants, of whom 19 816 had stroke). Summary-level results of discovery and follow-up stages were combined using inverse-variance weighted fixed-effects meta-analysis, and in-silico lookups were done in stroke subtypes. For genome-wide significant findings (at p<5 × 10(-8)), we explored associations with additional cerebrovascular phenotypes and did functional experiments using conditional (inducible) deletion of the probable causal gene in mice. We also studied the expression of orthologs of this probable causal gene and its effects on cerebral vasculature in zebrafish mutants. FINDINGS: We replicated seven of eight known loci associated with risk for ischaemic stroke, and identified a novel locus at chromosome 6p25 (rs12204590, near FOXF2) associated with risk of all-stroke (odds ratio [OR] 1·08, 95% CI 1·05-1·12, p=1·48 × 10(-8); minor allele frequency 21%). The rs12204590 stroke risk allele was also associated with increased MRI-defined burden of white matter hyperintensity-a marker of cerebral small vessel disease-in stroke-free adults (n=21 079; p=0·0025). Consistently, young patients (aged 2-32 years) with segmental deletions of FOXF2 showed an extensive burden of white matter hyperintensity. Deletion of Foxf2 in adult mice resulted in cerebral infarction, reactive gliosis, and microhaemorrhage. The orthologs of FOXF2 in zebrafish (foxf2b and foxf2a) are expressed in brain pericytes and mutant foxf2b(-/-) cerebral vessels show decreased smooth muscle cell and pericyte coverage. INTERPRETATION: We identified common variants near FOXF2 that are associated with increased stroke susceptibility. Epidemiological and experimental data suggest that FOXF2 mediates this association, potentially via differentiation defects of cerebral vascular mural cells. Further expression studies in appropriate human tissues, and further functional experiments with long follow-up periods are needed to fully understand the underlying mechanisms
