Miller Early Childhood Sustained Home-visiting (MECSH) trial: design, method and sample description
<p>Abstract</p> <p>Background</p> <p>Home visiting programs comprising intensive and sustained visits by professionals (usually nurses) over the first two years of life show promise in promoting child health and family functioning, and ameliorating disadvantage. Australian evidence of the effectiveness of sustained nurse home visiting in early childhood is limited. This paper describes the method and cohort characteristics of the first Australian study of sustained home visiting commencing antenatally and continuing to child-age two years for at-risk mothers in a disadvantaged community (the Miller Early Childhood Sustained Home-visiting trial).</p> <p>Methods and design</p> <p>Mothers reporting risks for poorer parenting outcomes residing in an area of socioeconomic disadvantage were recruited between February 2003 and March 2005. Mothers randomised to the intervention group received a standardised program of nurse home visiting. Interviews and observations covering child, maternal, family and environmental issues were undertaken with mothers antenatally and at 1, 12 and 24 months postpartum. Standardised tests of child development and maternal-child interaction were undertaken at 18 and 30 months postpartum. Information from hospital and community heath records was also obtained.</p> <p>Discussion</p> <p>A total of 338 women were identified and invited to participate, and 208 were recruited to the study. Rates of active follow-up were 86% at 12 months, 74% at 24 months and 63% at 30 months postpartum. Participation in particular data points ranged from 66% at 1 month to 51% at 24 months postpartum. Rates of active follow-up and data point participation were not significantly different for the intervention or comparison group at any data point. Mothers who presented for antenatal care prior to 20 weeks pregnant, those with household income from full-time employment and those who reported being abused themselves as a child were more likely to be retained in the study. The Miller Early Childhood Sustained Home-visiting trial will provide Australian evidence of the effectiveness of sustained nurse home visiting for children at risk of poorer health and developmental outcomes.</p> <p>Trial registration</p> <p>ACTRN12608000473369</p
2020 roadmap on solid-state batteries
Li-ion batteries have revolutionized the portable electronics industry and empowered the electric vehicle (EV) revolution. Unfortunately, traditional Li-ion chemistry is approaching its physicochemical limit. The demand for higher density (longer range), high power (fast charging), and safer EVs has recently created a resurgence of interest in solid state batteries (SSB). Historically, research has focused on improving the ionic conductivity of solid electrolytes, yet ceramic solids now deliver sufficient ionic conductivity. The barriers lie within the interfaces between the electrolyte and the two electrodes, in the mechanical properties throughout the device, and in processing scalability. In 2017 the Faraday Institution, the UK's independent institute for electrochemical energy storage research, launched the SOLBAT (solid-state lithium metal anode battery) project, aimed at understanding the fundamental science underpinning the problems of SSBs, and recognising that the paucity of such understanding is the major barrier to progress. The purpose of this Roadmap is to present an overview of the fundamental challenges impeding the development of SSBs, the advances in science and technology necessary to understand the underlying science, and the multidisciplinary approach being taken by SOLBAT researchers in facing these challenges. It is our hope that this Roadmap will guide academia, industry, and funding agencies towards the further development of these batteries in the future
Sustained Economic Hardship and Cognitive Function: The Coronary Artery Risk Development in Young Adults Study
INTRODUCTION: The relationship between low income and worse health outcomes is evident, yet its association with cognitive outcomes is less explored. Most studies have measured income at one time and none have examined how sustained exposure to low income influences cognition in a relatively young cohort. This study examined the effect of sustained poverty and perceived financial difficulty on cognitive function in midlife. METHODS: Income data were collected six times between 1985 and 2010 for 3,383 adults of the Coronary Artery Risk Development in Young Adults prospective cohort study. Sustained poverty was defined by the percentage of time participants’ household income was <200% of the federal poverty level—“never” in poverty, “0< to <1/3,” “≥1/3 to <100%” or “all-time.” In 2010, at a mean age of 50 years, participants underwent a cognitive battery. Data were analyzed in 2015. RESULTS: In demographic-adjusted linear regression models, individuals with all-time poverty performed significantly worse than individuals never in poverty: 0.92 points less on verbal memory (z-score, −0.28; 95% CI= −0.43, −0.13), 11.60 points less on processing speed (z-score, −0.72; 95% CI= −0.85, −0.58), and 3.50 more points on executive function (z-score, −0.32; 95% CI= −0.47, −0.17). Similar results were observed with perceived financial difficulty. Findings were robust when restricted to highly educated participants, suggesting little evidence for reverse causation. CONCLUSIONS: Cumulative exposure to low income over 2 decades was strongly associated with worse cognitive function of a relatively young cohort. Poverty and perceived hardship may be important contributors to premature aging among disadvantaged populations
Relationship between the neighbourhood built environment and early child development
© 2017 Elsevier Ltd The relationship between features of the neighbourhood built environment and early child development was investigated using area-level data from the Australian Early Development Census. Overall 9.0% of children were developmentally vulnerable on the Physical Health and Well-being domain, 8.1% on the Social Competence domain and 8.1% on the Emotional Maturity domain. After adjustment for socio-demographic factors, Local Communities with the highest quintile of home yard space had significantly lower odds of developmental vulnerability on the Emotional Maturity domain. Residing in a Local Community with fewer main roads was associated with a decrease in the proportion of children developmentally vulnerable on the Social Competence domain. Overall, sociodemographic factors were more important than aspects of the neighbourhood physical environment for explaining variation between Local Communities in the developmental vulnerability of children

Socioeconomic Status and Psychological Function in Children with Chromosome 22q11.2 Deletion Syndrome: Implications for Genetic Counseling
The purpose of this study is to examine the association between parental socio-economic status (SES) and childhood neurocognition and behavior in children with chromosome 22q11.2 deletion syndrome (22q11DS). Although undoubtedly, the deletion of genes in the 22q11.2 interval is primarily responsible for the psychological manifestations, little is known about the role of the environment in either mitigating or contributing to these problems. We examined the association of parental socio-economic status (SES) with cognition and behavior in children with 22q11DS (n=65) and matched healthy control subjects (n=52), since SES is a component of family resources. We found that in children with 22q11DS, higher SES correlated with better overall functioning (p<.01) and social skills (p<.01), and less frequent oppositional defiant behavior (p<.001). These findings were in contrast to the control subjects in whom SES correlated with cognition and achievement, but not behavior. Our results indicate that environmental factors influence the behavioral phenotype in children with 22q11DS, providing a framework for developing appropriate interventions. As such, genetic counseling for families with 22q11DS may include consideration of family resources and inclusion of other health professionals, such as social workers, to explore with the family available social supports and resources
