“You Need to Set a Daily Schedule”: Understanding Successful Aging via Three-Sided Viewpoints
This study aimed to identify the constructs related to successful aging in the context of engagement in social and productive activities. A qualitative design was used to explore three viewpoints on the aging period with 9 focus groups (3 each for adults 65 years or older, older adults' family members, and health professionals) and 18 interviews with older adults (older adults Mage = 71.7 years, Standard Deviation = 4.62). The transcripts were analyzed using constructivist grounded theory principles. Three categories were identified for successful aging: (a) engagement with life, including social participation, fixed/flexible schedule, time, and meaningful occupation; (b) self-management abilities, including producing daily schedules, independence, and initiation/striving toward goals; and (c) diversity among older adults, including their views on retirement, being active or not, and their dreams/values/goals. Considering the diverse nature of older adults, recognizing the importance of life engagement and self-management abilities emphasizes the necessity for an occupation-based, self-tailored approach to enable successful aging
Centralized Management of the Covid-19 Pandemic in Long-Term Care Facilities in Israel
Covid-19 was first diagnosed in Israel in late February 2020. During the first month following the outbreak, it became clear that the most severe outcomes were occurring in the Long-Term Care Facilities (LTCFs) and that the fragmented supervision of these facilities was a key weakness in the fight against the pandemic. It was therefore decided to establish the Fathers and Mothers Shield taskforce, which included representatives from all of the relevant government ministries and which would facilitate communication and consultation with the representatives of the LTCFs and with the relevant public sector organizations. We analyse three main policy measures implemented by the taskforce in the effort to mitigate the effects of the pandemic in the LTCFs: an increase in SARS-CoV-2 testing of LTCF residents and employees; the opening of specialized Corona wards in geriatric hospitals and LTCFs; and the regulation of family visits.
The introduction of centralized management of the pandemic in the LTCFs achieved a number of important outcomes and is considered to have been highly successful. The share of the LTCFs within total Covid-19 mortality dropped from 45% to 36%, and the load on general hospitals became more manageable. With the arrival of the SARS-CoV-2 vaccines, the taskforce viewed priority vaccination of the LTCF population as its main goal in early 2021
Obesity and Blood Pressure in 17-Year-Old Offspring of Mothers with Gestational Diabetes: Insights from the Jerusalem Perinatal Study
Objective. Gestational diabetes mellitus (GDM) influences fetal development and offspring's metabolic risk. We evaluated this association in 17-year-old offspring adjusting for birth weight (BW) and prepregnancy maternal BMI (mBMI). Study Design. The JPS birth cohort contains extensive data on 92,408 births from 1964 to 1976. Offspring's BMI and blood pressure (BP) were obtained from military records. For a subcohort born between 1974 and 1976, prepregnancy mBMI was available. Offspring were classified as born to mothers with GDM (n = 293) or born to mothers without recorded GDM (n = 59,499).
Results. GDM offspring had higher mean BMI and systolic and diastolic BP compared to no-recorded-GDM offspring. After adjusting for BW, GDM remained significantly associated with offspring BMI and diastolic BP (β = 1.169 and 1.520, resp.). In the subcohort, when prepregnancy mBMI was entered to the models, it markedly attenuated the associations with GDM. Conclusions. Maternal characteristics have long-term effects on cardiometabolic outcomes of their offspring aged 17 years

Hydrocarbon-related microbial processes in the deep sediments of the Eastern Mediterranean Levantine Basin
During the 2011 exploration season of the EV Nautilus in the Mediterranean Sea, we conducted a multidisciplinary study, aimed at exploring the microbial populations below the sediment–water interface (SWI) in the hydrocarbon-rich environments of the Levantine basin. Two c. 1000-m-deep locations were sampled: sediments fueled by methane seepage at the toe of the Palmachim disturbance and a patch of euxinic sediment with high sulfide and methane content offshore Acre, enriched by hydrocarbon from an unknown source. We describe the composition of the microbial population in the top 5 cm of the sediment with 1 cm resolution, accompanied by measurements of methane and sulfate concentrations, and the isotopic composition of this methane and sulfate (δ13CCH4, δ18OSO4, and δ34SSO4). Our geochemical and microbiological results indicate the presence of the anaerobic methane oxidation (AOM) coupled to bacterial sulfate reduction (BSR). We show that complex methane and sulfur metabolizing microbial populations are present in both locations, although their community structure and metabolic preferences differ due to potential variation in the hydrocarbon source
Repeated out-of-Africa expansions of Helicobacter pylori driven by replacement of deleterious mutations

resembling that of its host. However, H. pylori fromEurope and the Middle East
trace substantially more ancestry from modern African populations than the
humans that carry them. Here, we use a collection of Afro-Eurasian H. pylori
genomes to show that this African ancestry is due to at least three distinct
admixture events. H. pylori from East Asia, which have undergone little
admixture, have accumulated many more non-synonymous mutations than
African strains. European and Middle Eastern bacteria have elevated African
ancestry at the sites of these mutations, implying selection to remove them
during admixture. Simulations show that population fitness can be restored
after bottlenecks bymigration and subsequent admixture of small numbers of
bacteria from non-bottlenecked populations. We conclude that recent spread
of African DNA has been driven by deleterious mutations accumulated during
the original out-of-Africa bottleneck.This work

Rifaximin for small intestinal bacterial overgrowth in patients without irritable bowel syndrome
State of play and future direction with NOACs: An expert consensus.
Atrial fibrillation (AF) and venous thromboembolism (VTE) are cardiovascular conditions significant in contemporary practice. In both, the use of anticoagulation with vitamin K antagonists (VKAs) has been traditionally used to prevent adverse events. However, VKA therapy is associated with challenges relating to dose maintenance, the need to monitor anticoagulation, and bleeding risks. The non-vitamin K oral anticoagulants (NOACs) are becoming accepted as a clear alternative to VKA therapy for both AF and VTE management. The aim of this paper was to review contemporary evidence on the safety of NOACs in both conditions. A comprehensive literature review was conducted to explore key safety issues and expert consensus was achieved from eight professionals specialised in AF and VTE care. Consensus-based statements were formulated where available evidence was weak or contradictory. The expert statements in this paper form a key overview of the safety of NOACs compared with VKA therapy, and the comparative safety of different NOACs. It is apparent that a detailed patient work-up is required in order to identify and manage individual risk factors for bleeding and thrombosis prior to NOAC therapy. Additional measures, such as dose reductions, may also be used to maintain the safety of NOACs in practice
