Looking at the city from artistic research
Cost-effectiveness of multicomponent interventions in type 2 diabetes mellitus in a cluster randomised controlled trial: the INDICA study
Objective: To analyse the cost-effectiveness of multicomponent interventions designed to improve outcomes in type 2 diabetes mellitus (T2DM) in primary care in the Canary Islands, Spain, within the INDICA randomised clinical trial, from the public health system perspective. Design: An economic evaluation was conducted for the within-trial period (2 years) comparing the four arms of the INDICA study. Setting: Primary care in the Canary Islands, Spain. Participants: 2334 patients with T2DM without complications were included. Interventions: Interventions for patients (PTI), for primary care professionals (PFI), for both (combined intervention arm for patients and professionals, CBI) and usual care (UC) as a control group. Outcomes: The main outcome was the incremental cost per quality-adjusted life-years (QALY). Only the intervention and the healthcare costs were included. Analysis: Multilevel models were used to estimate results, and to measure the size and significance of incremental changes. Missed values were treated by means of multiple imputations procedure. Results: There were no differences between arms in terms of costs (p=0.093), while some differences were observed in terms of QALYs after 2 years of follow-up (p=0.028). PFI and CBI arms were dominated by the other two arms, PTI and UC. The differences between the PTI and the UC arms were very small in terms of QALYs, but significant in terms of healthcare costs (p=0.045). The total cost of the PTI arm (€2571, 95% CI €2317 to €2826) was lower than the cost in the UC arm (€2750, 95% CI €2506 to €2995), but this difference did not reach statistical significance. Base case estimates of the incremental cost per QALY indicate that the PTI strategy was the cost-effective option. Conclusions: The INDICA intervention designed for patients with T2DM and families is likely to be cost-effective from the public healthcare perspective. A cost-effectiveness model should explore this in the long term. This study received financial support from the Spanish Ministry of Economy, Industry and Competitiveness (Instituto de Salud Carlos III), grants: ADE10/00032 and PI16/00769, jointly funded by the European Regional Development Fund (FEDER) ‘A way to make Europe’. Women representation in soil science: gender indicators in the University Program of Interdisciplinary Soil Studies
IntroductionIn the world, 33% of soils are degraded, and 2.9 million people are affected by land degradation, with problems associated with food security, conflicts over natural resources, and migration with different impacts on men or women. To support sustainable soil management, it is necessary to include women’s contributions to soil Sciences; their achievements and academic performance still need to be represented. Women in Science represent 30% worldwide. In Mexico, only 24% of top academic positions are women. For commitment to soil Sciences for Sustainable Development Goals (SDGs), UNAM created the University Program for Interdisciplinary Soil Studies (PUEIS).MethodsThis research evaluates women’s representation through gender indicators in the PUEIS and SNI datasets and discusses their implications for the gender gap in the soil Science community from Mexico. The data was collected with an online semi-structured survey and the gender indicators selected were related to participation, gender gap, sexism, equal opportunities, exclusion, and academic performance.ResultsThe results show that in the PUEIS, 54% of members identify themselves as women and 46% as men. The gender gap shows equality in the total number of members. However, low-rank jobs, such as lecturers and lab technicians are women dominated, and the top-ranked positions as a full professor, associate professor, and research scientist are equal. One result to consider for the PUEIS members is that the younger generation, as is the older generation, is dominated by men. This could indicate a setback in intermediate generations’ progress toward achieving gender equality. In the case of SNI members, there is a gender gap problem; of members with a Ph.D. degree, only 38% are women, and the elite group of scientists with a Ph.D. at the top position is represented only by 24% of females.DiscussionThis work constitutes the first gender exercise for analyzing women’s participation in the soil Sciences in Mexico. From our perspective, it is not about competition in scientific careers between women and men; however, it is essential to recognize that gender inequalities are related to income, professional development, and science funding inequalities, and these disparities impact women more than men
PPAR-γ Gene Expression in Human Adipose Tissue Is Associated with Weight Loss After Sleeve Gastrectomy
[EN] Background
The peroxisome proliferator-activated receptor (PPAR)-γ plays a key role in adipose tissue differentiation and fat metabolism. However, it is unclear which factors may regulate its expression and whether obese patients have changes in adipose tissue expression of PPAR-γor potential regulators such as miR-27. Thus, our aims were to analyze PPAR-γ and miR-27 expression in adipose tissue of obese patients, and to correlate their levels with clinical variables.
Subjects and Methods.
We included 43 morbidly obese subjects who underwent sleeve gastrectomy (31 of them completed 1-year follow-up) and 19 non-obese subjects. mRNA expression of PPAR-γ1 and PPAR-γ2, miR-27a, and miR-27b was measured by qPCR in visceral and subcutaneous adipose tissue. Clinical variables and serum adipokine and hormone levels were correlated with PPAR-γ and miR-27 expression. In addition, a systematic review of the literature regarding PPAR-γ expression in adipose tissue of obese patients was performed.
Results
We found no differences in the expression of PPAR-γ and miR-27 in adipose tissue of obese patients vs. controls. The literature review revealed discrepant results regarding PPAR-γ expression in adipose tissue of obese patients. Of note, we described a significant negative correlation between pre-operative PPAR-γ1 expression in adipose tissue of obese patients and post-operative weight loss, potentially linked with insulin resistance markers.
Conclusion
PPAR-γ1 expression in adipose tissue is associated with weight loss after sleeve gastrectomy and may be used as a biomarker for response to surgery.Open Access funding provided thanks to the CRUE-CSIC agreement with Springer Nature. Grafting of Multiwalled Carbon Nanotubes with Chicken Feather Keratin
Keratin, obtained from chicken feathers, was grafted on the surface of commercially available carbon nanotubes. The original procedure developed allows a covalent interaction between some specific chemical groups characteristic of the keratin, with some functional groups introduced on purpose on the surface of the nanotubes, as revealed by infrared and Raman spectroscopies, which also allowed to determine structural changes introduced during the process, such as crystallinity, which lead to changes in other properties, as well
