Crawler travel gear is a type of heavy vehicle propulsion that is commonly found in tanks, excavators, and specialized off-road vehicles. They have an advantage over wheels when it comes to robust vehicle weight distribution over soft terrain, and some disadvantages as well. They can damage paved roads and have complex design so, considering the enormous weight they must carry, their reliability must be determined and verified. The main parts of the assembly are the drive wheels, which move the crawler, and the supporting structure that holds four-wheel bogies and two-wheel bogies. A case study on the impact of Artificial Intelligence supported spirometry in primary care
Background:
Spirometry is a key test to identify respiratory diseases. However, long waiting lists are present throughout
England, with an estimated backlog of 200 – 250 patients per 500.000. Moreover, poor quality of spirometry
data and a lack of confidence when interpreting spirometry in primary care further drive suboptimal use.
This study aims to investigate the impact of providing ArtiQ.Spiro, i.e. AI-based software to support primary
care practitioners in performing and interpreting spirometry.
Methods:
Two ARTP-accredited healthcare professionals, a general practitioner and a nurse, used ArtiQ.Spiro over 5
months. They evaluated spirometry quality and made a diagnostic interpretation first without the ArtiQ.Spiro
software and then with AI support. For each, they recorded (i) the time it took to interpret the test, (ii) how
confident they were in their interpretation.
Results:
51 spirometry sessions were collected. The average time to evaluate the spirometry results decreased by
using ArtiQ.Spiro from 10.6 ± 4.1 min. to 5.6 ± 5.6 min (p<0.001). The confidence level in the interpretation
did not change, with a median of 4 on a 5-point Likert scale without and with AI support. The AI matched
the quality assessment in 94% of the cases and matched the diagnosis in 86% of the cases (4% missing
data). The final diagnosis needed further clinical consideration.
Conclusion:
This study shows that AI has the potential to reduce the time for interpretation of spirometry traces and
support healthcare professionals in the execution and interpretation of spirometry. This could improve
access to spirometry service
Epigenetic regulation of human SOX3 gene expression during early phases of neural differentiation of NT2/D1 cells
Sox3/SOX3 is one of the earliest neural markers in vertebrates. Together with the Sox1/ SOX1 and Sox2/SOX2 genes it is implicated in the regulation of stem cell identity. In the present study, we performed the first analysis of epigenetic mechanisms (DNA methylation and histone marks) involved in the regulation of the human SOX3 gene expression during RA-induced neural differentiation of NT2/D1 cells. We show that the promoter of the human SOX3 gene is extremely hypomethylated both in undifferentiated NT2/D1 cells and during the early phases of RA-induced neural differentiation. By employing chromatin immunoprecipitation, we analyze several histone modifications across different regions of the SOX3 gene and their dynamics following initiation of differentiation. In the same timeframe we investigate profiles of selected histone marks on the promoters of human SOX1 and SOX2 genes. We demonstrate differences in histone signatures of SOX1, SOX2 and SOX3 genes. Considering the importance of SOXB1 genes in the process of neural differentiation, the present study contributes to a better understanding of epigenetic mechanisms implicated in the regulation of pluripotency maintenance and commitment towards the neural lineage
Challenges and opportunities in resuming spirometry services in England post-pandemic with potential to adopt Artificial Intelligence decision support software: a qualitative study
Background: Spirometry services to diagnose and monitor lung disease in primary care are restarting post-pandemic in England, identified as a priority in the NHS Long Term Plan, however evidence regarding best practice is limited.Aims: To explore perspectives on spirometry provision in primary care, and the potential for Artificial Intelligence (AI) decision support software to aid quality and interpretation.Design and Setting: Semi-structured interviews with stakeholders in spirometry services in primary care.Methods: Semi-structured interviews were conducted with key stakeholders in spirometry services across England. Participants were recruited by snowball sampling. Interviews explored the pre-pandemic delivery of spirometry, restarting of services and perceptions of the role of AI. Transcripts were analysed thematically.Results: 28 participants (mean [SD], 21.6 [9.4, range 3-40] years’ clinical experience) were interviewed between April and June 2022. Participants included clinicians (n=25) and commissioners (n=3); eight held regional and/or national respiratory network advisory roles. Four themes were identified: 1) Historical challenges in spirometry provision; 2) Inequity in post-pandemic spirometry provision and challenges to restarting spirometry in primary care; 3) Future delivery closer to patients’ homes by appropriately trained staff; 4) The potential for AI to have supportive roles in spirometry.Conclusion: Stakeholders highlighted historic challenges and the damaging effects of the pandemic contributing to inequity in provision of spirometry, which must be addressed. Overall stakeholders were positive about the potential of AI to support clinicians in quality assessment and interpretation of spirometry. However, it was evident that validation of the software must be sufficiently robust for clinicians and healthcare commissioners to have trust in the process
Comparing performance of primary care clinicians in the interpretation of SPIROmetry with or without Artificial Intelligence Decision support software (SPIRO-AID): a protocol for a randomised controlled trial.
INTRODUCTION: Spirometry is a point-of-care lung function test that helps support the diagnosis and monitoring of chronic lung disease. The quality and interpretation accuracy of spirometry is variable in primary care. This study aims to evaluate whether artificial intelligence (AI) decision support software improves the performance of primary care clinicians in the interpretation of spirometry, against reference standard (expert interpretation). METHODS AND ANALYSIS: A parallel, two-group, statistician-blinded, randomised controlled trial of primary care clinicians in the UK, who refer for, or interpret, spirometry. People with specialist training in respiratory medicine to consultant level were excluded. A minimum target of 228 primary care clinician participants will be randomised with a 1:1 allocation to assess fifty de-identified, real-world patient spirometry sessions through an online platform either with (intervention group) or without (control group) AI decision support software report. Outcomes will cover primary care clinicians' spirometry interpretation performance including measures of technical quality assessment, spirometry pattern recognition and diagnostic prediction, compared with reference standard. Clinicians' self-rated confidence in spirometry interpretation will also be evaluated. The primary outcome is the proportion of the 50 spirometry sessions where the participant's preferred diagnosis matches the reference diagnosis. Unpaired t-tests and analysis of covariance will be used to estimate the difference in primary outcome between intervention and control groups. ETHICS AND DISSEMINATION: This study has been reviewed and given favourable opinion by Health Research Authority Wales (reference: 22/HRA/5023). Results will be submitted for publication in peer-reviewed journals, presented at relevant national and international conferences, disseminated through social media, patient and public routes and directly shared with stakeholders. A comparison between padding and bath exhaustion to apply microcapsules onto cotton

They have been applied as a possible means of introducing new products to textiles, such as insect repellents, antibiotics, skin moisturizers, etc. Microencapsulation technology has improved the fragrance durability on fabrics. Historically, the durability of the fragrance was poor, especially once the fabric had been washed. Microcapsules have been used in textiles for many years, however their previous characterization, adhesion behaviour and permanence on the fabrics are not well known. Nowadays the majority of textile industries are not able to characterize commercial products, or to study the process of adhering the microcapsule to the fibre's surface nor their functionality. Thus, the characterization of microencapsulated fabrics with different active core and the knowledge of the various application processes becomes a major challenge in the field of microcapsules use. There are various industrial processes to apply microcapsules, but determining optimal amounts of products, temperature, conditions and other process variables are an important challenge for the textile sector in order to achieve the highest depositions and retention of microcapsules. This work is focused on determining and quantifying presence fragrance microcapsules when applied onto fabrics by padding and by bath exhaustion and determining which method is the most effective. Consequently, diverse analysis techniques such as microscopy (SEM), spectroscopy FTIR and XPS have been used. We concluded that proposed techniques seem to be useful to compare fabrics treated with microcapsules. A recurrent p.Arg92Trp variant in steroidogenic factor-1 (NR5A1) can act as a molecular switch in human sex development
Cell lineages of the early human gonad commit to one of the two mutually antagonistic organogenetic fates, the testis or the ovary. Some individuals with a 46,XX karyotype develop testes or ovotestes (testicular or ovotesticular disorder of sex development; TDSD/OTDSD), due to the presence of the testis-determining gene, SRY Other rare complex syndromic forms of TDSD/OTDSD are associated with mutations in pro-ovarian genes that repress testis development (e.g. WNT4); however, the genetic cause of the more common non-syndromic forms is unknown. Steroidogenic factor-1 (known as NR5A1) is a key regulator of reproductive development and function. Loss-of-function changes in NR5A1 in 46,XY individuals are associated with a spectrum of phenotypes in humans ranging from a lack of testis formation to male infertility. Mutations in NR5A1 in 46,XX women are associated with primary ovarian insufficiency, which includes a lack of ovary formation, primary and secondary amenorrhoea as well as early menopause. Here, we show that a specific recurrent heterozygous missense mutation (p.Arg92Trp) in the accessory DNA-binding region of NR5A1 is associated with variable degree of testis development in 46,XX children and adults from four unrelated families. Remarkably, in one family a sibling raised as a girl and carrying this NR5A1 mutation was found to have a 46,XY karyotype with partial testicular dysgenesis. These unique findings highlight how a specific variant in a developmental transcription factor can switch organ fate from the ovary to testis in mammals and represents the first missense mutation causing isolated, non-syndromic 46,XX testicular/ovotesticular DSD in humans
Mutations involving the SRY-related gene SOX8 are associated with a spectrum of human reproductive anomalies
SOX8 is an HMG-box transcription factor closely related to SRY and SOX9. Deletion of the gene encoding Sox8 in mice causes reproductive dysfunction but the role of SOX8 in humans is unknown. Here, we show that SOX8 is expressed in the somatic cells of the early developing gonad in the human and influences human sex-determination. We identified two individuals with 46,XY disorders/differences in sex development (DSD) and chromosomal rearrangements encompassing the SOX8 locus and a third individual with 46,XY DSD and a missense mutation in the HMG-box of SOX8. In-vitro functional assays indicate that this mutation alters the biological activity of the protein. As an emerging body of evidence suggests that DSDs and infertility can have common etiologies, we also analyzed SOX8 in a cohort of infertile men (n = 274) and two independent cohorts of women with primary ovarian insufficiency (POI; n = 153 and n = 104). SOX8 mutations were found at increased frequency in oligozoospermic men (3.5%; p<0.05) and POI (5.06%; p=4.5x10-5) as compared to fertile/normospermic control populations (0.74%). The mutant proteins identified altered SOX8 biological activity as compared to the wild-type protein. These data demonstrate that SOX8 plays an important role in human reproduction and SOX8 mutations contribute to a spectrum of phenotypes including 46,XY DSD, male infertility and 46,XX POI
Longitudinal Evaluation of the Hypothalamic-Pituitary-Testicular Function in 8 Boys with Adrenal Hypoplasia Congenita (AHC) Due to NR0B1 Mutations
BACKGROUND:Boys carrying mutations in the NR0B1 gene develop adrenal hypoplasia congenita (AHC) and impaired sexual development due to the combination of hypogonadotropic hypogonadism (HH) and primary defects in spermatogenesis. METHODS:We analysed the evolution of hypothalamic-pituitary-testicular function of 8 boys with AHC due to NR0B1 mutations. Our objective was to characterize and monitor the progressive deterioration of this function. RESULTS:The first symptoms appeared in the neonatal period (n = 5) or between 6 months and 8.7 years (n = 3). Basal plasma adrenocorticotrophic hormone (ACTH) concentrations increased in all boys, whilst cortisol levels decreased in one case. The natremia was equal or below 134 mmol/L and kaliemia was over 5 mmol/L. All had increased plasma renin. In 3 of 4 patients diagnosed in the neonatal period and evaluated during the first year, the basal plasma gonadotropins concentrations, and their response to gonadotropin releasing hormone (GnRH) test (n = 2), and those of testosterone were normal. The plasma inhibin B levels were normal in the first year of life. With the exception of two cases these concentrations decreased to below the normal for age. Anti-Müllerian hormone concentrations were normal for age in all except one case, which had low concentrations before the initiation of testosterone treatment. In 3 of the 8 cases the gene was deleted and the remaining 5 cases carried frameshift mutations that are predicted to introduce a downstream nonsense mutation resulting in a truncated protein. CONCLUSIONS:The decreases in testosterone and inhibin B levels indicated a progressive loss of testicular function in boys carrying NR0B1 mutations. These non-invasive examinations can help to estimate the age of the testicular degradation and cryopreservation of semen may be considered in these cases as investigational procedure with the aim of restoring fertility
