Lifetime corneal edema load model

Purpose: To highlight the potential benefits for long-term use of silicone hydrogels daily disposable (DD) contact lenses, particularly with patients who are noncompliant, sleeping or napping while wearing their lenses, or those who have higher oxygen demands and wear this modality for decades. Methods: Published data for corneal swelling with lenses and no lens wear were used to develop a nonlinear least squares model. The edema load experienced with a range of oxygen transmissibilities (Dk/t) and wear compliance (sleep and napping) was determined. A mixed-effects linear regression model was used to compare the edema load for high and average corneal swellers. Results: The edema load generated demonstrates that a high Dk/t silicone hydrogel lens results in edema levels close to that with no lens wear. In comparison, hydrogels with a Dk/t of 27 (×10−9 [cm mL{O2}][s mLmmHg]), worn on a daily wear schedule will result in 1.5 times more edema and up to two times more if the patient is noncompliant over each decade of wear. High swellers after four decades of wear will have an edema load 10 to 17 times greater than average swellers depending on Dk/t and their degree of noncompliance with the daily wear modality. Conclusions: Prescribing silicone hydrogelDDlenses, particularly with higher DK/t,may help to maintain the long-term ocular health of patients, when they wear their lenses fulltime for many decades. Translational Relevance: Illustrates the importance of Dk/t for any CL wear modality where patients nap or sleep in lenses or have high oxygen needs

The forgotten '45 : Donald Dubh's rebellion in an archipelagic context

The final rebellion of Donald Dubh, heir to the forfeited MacDonald lordship of the Isles, is usually examined within the context of Highland rebellions that occurred in the half century after forfeiture. However, the factors that motivated the Islesmen to rise in rebellion in 1545 are multi-faceted and can only be fully understood by placing the rising in a wider context, which considers national and archipelagic events. The discussion that follows explores the reasons why the Islesmen, almost unanimously, entered into agreement with Henry VIII to attack Scotland from the west and why this endeavour failed. At the same time, the article highlights Henry’s recognition of the strategic importance of the west which led him into alliance with Donald Dubh and his supporters

The Care Certificate Evaluation: an overview

BACKGROUND: Front line care workers play a key role in health and social care provision for older people and this has been reflected in training innovations aiming to improve the care provided by these workers. One recent training initiative has been the Care Certificate, which was fully launched in England in April 2015 and which aims to provide a consistent and transferable approach to this training. However, little is currently known about the patterns of implementation adopted by care organisations or about the perceptions of the Care Certificate held by care providers and by those in receipt of this care. AIMS: In order to address these issues and to optimise its impact, the implementation of the Care Certificate is being evaluated through an 18 month national study funded by the Department of Health Policy Research Programme. Project Reference Number PR-R14-0915-12004. METHODS: This study includes a telephone survey of care organisations in England as well as qualitative explorations of the views and experiences of the Care Certificate held by care providers and care receivers. This poster provides an overview of this national evaluation, highlighting why it is needed, how it is being carried out as well as summarising some of its preliminary findings
