    One-Stage Reconstruction for Midfacial Defect after Radical Tumor Resection

    A serious midface defect involving resection of squamous cell carcinoma originating from the hard palate was treated by an unusual reconstructive strategy. After tumor resection, surgical reconstruction was accomplished in one stage using one free flap with one distant and local flap: a radial forearm flap to reconstruct the upper lip, a forehead flap to reconstruct the external nose, a cantilever calvarial bone graft to replace the nasal skeleton and a nasolabial flap and split thickness skin graft to cover the internal nasal lining. The rationale for this one-stage reconstruction and the problems associated with midfacial reconstruction after wide tumor excision are discussed

    Free jejunal flap transfer containing multiple vascular pedicles for pharyngoesophageal reconstruction in conjunction with anterior mediastinal tracheostomy

    BackgroundWhen pharyngoesophagectomy is performed in conjunction with anterior mediastinal tracheostomy, reconstructing both the trachea and alimentary tract is extremely difficult. We developed a novel 1‐stage reconstructive procedure using a single free jejunal flap containing multiple vascular pedicles to decrease postoperative morbidity and mortality. Free jejunal flap transfer with multiple vascular pedicles could offer a viable option for reducing associated life‐threatening complications.MethodsWe performed a retrospective review of 34 patients who underwent free jejunal flap transfer with multiple vascular pedicles in anterior mediastinal tracheostomy and pharyngoesophagectomy due to lesions involving both the airway and esophagus. In all cases, 1‐stage reconstruction of the digestive tract and trachea was performed. Technical details and outcomes were analyzed.ResultsAll 34 jejunal flaps (100%) survived. Major morbidity classified as Clavien–Dindo grades III and IV occurred in 10 (29.4%) and 0 (0%) patients, respectively during hospitalization. With regard to common complications, anastomotic leakage from transferred jejunal flaps and surgical site infections occurred in 0 (0%) and 7 (20.6%) patients, respectively. Five (14.7%) patients experienced tracheal stoma dehiscence. Donor site morbidity was observed in 2 (5.9%) patients. The overall in‐hospital mortality rate was 2.9%.ConclusionsOur 1‐stage reconstruction procedure achieved low morbidity and low mortality rates following anterior mediastinal tracheostomy and pharyngoesophagectomy.     The main objective of this paper is not to analyse the problem of International Law as a legal tool but it seeks to raise the question of International Law as a science, as well as focus specially on the Sociology of International Law of which one of its most integral components, as a special discipline is status nascendi. Other elements of the Science of International Law are: International Law Theory, International Law Dogma and International Law History. In addition, International Relations Sociology also has its own set of components such as theory, dogma, methodology and history and it is based primarily on Sociology, Sociology of Law and the International Relations Theories. Political Rules are one of the most important elements of International Law; on the other hand, however, one of the key functions of the Sociology of International Law is the defence of the Science of International Law with respect to its intentions to discard the theories of political scientists from the United States (International Relations Theory). In addition, it tackles certain tendencies related to rejected judicial positivism. The sociology of International Relations is an adequate scientific response to certain destructive nihilistic positions that have taken root in certain important theories of International Relations Theory with respect to the role of International Relations

    Databáze akustických nahrávek je aplikace přístupná přes klasický webový prohlížeč. Využity byly technologie, běžně ve webových aplikacíh používané. Použitím těchto technlogíí je zajištěna přístupnost databáze pro široký okruh uživatelů. Vytvořená aplikace umožňuje na server nahrávat zvukové soubory typu WAV a přidat k těmto souborům popisky různého charakteru. Kromě správy řečových nahrávek umožňuje aplikace porovnávání účinnosti metod zvýraznění řečového signálu ze šumu, kterými byly nahrávky zpracovány.The databsae of accoustical recordings is a web-based application, which is accessible with an usual web browser. There were used technologies, that are ussually used in web applications. This ensures, that the application is open for using by wide range of users. The application enables uploading WAWE files to the server and allows the user to add various description of the recordings. The application allows also comparing the quality of recordings, which were processed with some method for highlighting the accoustical signal from noise. This function is established by listening tests, which are open for every user, who wants to join the tests.
