    The development of psychiatric disorders and adverse behaviors : from context to prediction

    Psychiatric disorders by definition cause significant impairment in an individual’s daily functioning. Certain disorders, such as borderline personality disorder (BPD) and eating disorders, have worse prognosis and high mortality rates compared to other psychiatric disorders. Similarly, adverse behaviors such as self-harm, suicide, and crime are often present in individuals with psychiatric disorders. It is of interest to further understand the etiology and associations of BPD and eating disorders to uncover potential avenues and opportunities for intervention. Moreover, prediction modeling has recently come of interest to psychiatric epidemiologists with the rise of large data sets. Prediction modeling may provide valuable information about the nature of risk factors and eventually aid clinical diagnostics and prognostics. Thus, the studies included in this thesis seek to examine the etiology, associations, and prediction approaches of psychiatric disorders and adverse behaviors. Study I examined the individual and familial association between type 1 diabetes (T1D) and eating disorder diagnoses. We used national health care records from Denmark (n = 1,825,920) and Sweden (n = 2,517,277) to calculate the association within individuals, full siblings, half siblings, full cousins, and half cousins. Individuals with T1D had twice the hazard rate ratio of being diagnosed with an eating disorder compared to the general population. There was conflicting evidence for the risk of an eating disorder in full siblings of T1D patients. However, there was no evidence to support a further familial relationship between the two conditions. Study II aimed to illuminate the nature of the correlates for BPD across time, sex, and for their full siblings. We examined 87 variables across psychiatric disorders, somatic illnesses, trauma, and adverse behaviors (such as self-harm). In a sample of 1,969,839 Swedes with 12,175 individuals diagnosed with BPD, we found that BPD was associated with nearly all of the examined variables. The associations were largely consistent across time and between the sexes. Finally, we found that having a sibling diagnosed with BPD was associated with psychiatric disorders, trauma, and adverse behaviors but not somatic illnesses. Study III created a prediction model that could predict who would have high or low psychiatric symptoms at age 15 based on data from parental reports and national health care registers collected at age 9 or 12. Additionally, we compared multiple types of machine learning algorithms to assess predictive performance. The sample included 7,638 twins from the Child and Adolescent Twin Study in Sweden (CATSS). Our model was able to predict the outcome with reasonable performance but is not suitable for use in clinics. Each model performed similarly indicating that researchers with similar data and research questions do not need to forgo standard logistic regression. Study IV aimed to determine if an individual will exhibit suicidal behaviour (self-harm or suicidal thoughts), aggressive behaviour, both, or neither before adulthood with prediction modeling. Through variable importance scores we examined the usefulness of genetic variables within the model. A total of 5,974 participants from CATSS and 2,702 participants from the Netherlands Twin Register (NTR) were included in the study. The model had adequate performance in both the CATSS and NTR datasets for all classes except for the suicidal behaviors class in the NTR, which did not perform better than chance. The included genetic data had higher variable importance scores than questionnaire data completed at age 9 or 12, indicating that genetic biomarkers can be useful when combined with other data types. In conclusion, the development of psychiatric disorders and symptoms are associated with many factors across somatic illnesses, other psychiatric disorders, trauma, and harmful behaviors. The results of this thesis demonstrates the limitations of prediction modeling in psychiatric clinics but highlights their use in research and on the path forward towards personalized medicine

    Social media sites are becoming increasingly popular go-to sources of information for learning how to cope with personal difficulties. The purpose of our study was to gain insights into how to design social media messages and strategies to help young adults cope with issues related to parental divorce. Our research question focused on how the use of social media (Twitter, Facebook, etc.) was used by young adults to cope with the divorce of their parents. The participants of this study were 18-25 year olds with divorced parents. This sample was collected because we felt people in this age group had a clear understanding of why their parents may have divorced. We asked questions about three major topics: 1) What types of social media are they currently using? 2) What were the most difficult issues they faced in regards to their parents divorce and how did they cope? 3) Did they find any helpful information to cope on the Internet, and if so, what was it? From our analyses, we expect to be able to identify social media strategies to help get science-based information to young adults who have divorced parents.Ope

    Millions of youth have divorced or separated parents, and many youth experience behavior problems or cope with this family transition in unhealthy ways. At the same time, evidence-based programs to help youth cope with their parents divorce are limited. Moreover, research with contemporary youth that was designed to inform such programming is further scarce. The purpose of this study was to explore emerging adults (N = 141) perceptions of the challenges, as well as their coping methods throughout their own parents divorce-related transitions. Themes surrounding feeling caught in the middle between parents and attempting to maintain contact with both parents emerged from the data as two of the most commonly coded categories. Participants also frequently reported struggles regarding parents new romantic relationships or the remarriage of their parents and feeling sad, depressed, or experiencing other negative emotions. The most frequently mentioned coping aid was friends, particularly someone with a similar experience dealing with parental divorce. These data provide insight into the types of topics and coping strategies that could form the basis for developing outreach efforts for helping youth who are coping with their parents divorce.Ope

    Study Protocol for a Cluster-Randomized Trial of a Bundle of Implementation Support Strategies to Improve the Fidelity of Implementation of Schoolwide Positive Behavioral Interventions and Supports in Rural Schools

    Background: Improving the implementation of evidence-based interventions is important for population-level impacts. Positive Behavioral Interventions and Supports (PBIS) is effective for improving school climate and students’ behavioral outcomes, but rural schools often lag behind urban and suburban schools in implementing such initiatives. Methods/Design: This paper describes a Type 3 hybrid implementation-effectiveness trial of Rural School Support Strategies (RS3), a bundle of implementation support strategies selected to improve implementation outcomes in rural schools. In this two-arm parallel group trial, 40 rural public schools are randomized to receive: 1) a series of trainings about PBIS; or 2) an enhanced condition with training plus RS3. The trial was planned for two years, but due to the pandemic has been extended another year. RS3 draws from the Interactive Systems Framework, with a university-based team (support system) that works with a team at each school (school-based delivery system), increasing engagement through strategies such as: providing technical assistance, facilitating school team functioning, and educating implementers. The primary organizational-level outcome is fidelity of implementation, with additional implementation outcomes of feasibility, acceptability, appropriateness, and cost. Staff-level outcomes include perceived climate and self-reported adoption of PBIS core components. Student-level outcomes include disciplinary referrals, academic achievement, and perceived climate. Mediators being evaluated include organizational readiness, school team functioning, and psychological safety. Discussion: The study tests implementation strategies, with strengths including a theory-based design, mixed methods data collection, and consideration of mediational mechanisms. Results will yield knowledge about how to improve implementation of universal prevention initiatives in rural schools

    Attention-deficit/hyperactivity disorder symptoms and dietary habits in adulthood: A large population-based twin study in Sweden

    Associations between adult attention-deficit/hyperactivity disorder (ADHD) symptoms and dietary habits have not been well established and the underlying mechanisms remain unclear. We explored these associations using a Swedish population-based twin study with 17,999 individuals aged 20–47 years. We estimated correlations between inattention and hyperactivity/impulsivity with dietary habits and fitted twin models to determine the genetic and environmental contributions. Dietary habits were defined as (a) consumption of food groups, (b) consumption of food items rich in particular macronutrients, and (c) healthy and unhealthy dietary patterns. At the phenotypic level, inattention was positively correlated with seafood, high-fat, high-sugar, high-protein food consumptions, and unhealthy dietary pattern, with correlation coefficients ranging from 0.03 (95%CI: 0.01, 0.05) to 0.13 (95% CI: 0.11, 0.15). Inattention was negatively correlated with fruits, vegetables consumptions and healthy dietary pattern, with correlation coefficients ranging from −0.06 (95%CI: −0.08, −0.04) to −0.07 (95%CI: −0.09, −0.05). Hyperactivity/impulsivity and dietary habits showed similar but weaker patterns compared to inattention. All associations remained stable across age, sex and socioeconomic status. Nonshared environmental effects contributed substantially to the correlations of inattention (56–60%) and hyperactivity/impulsivity (63–80%) with dietary habits. The highest and lowest genetic correlations were between inattention and high-sugar food (rA = .16, 95% CI: 0.07, 0.25), and between hyperactivity/impulsivity and unhealthy dietary pattern (rA = .05, 95% CI: −0.05, 0.14), respectively. We found phenotypic and etiological overlap between ADHD and dietary habits, although these associations were weak. Our findings contribute to a better understanding of common etiological pathways between ADHD symptoms and various dietary habits.publishedVersio

    The ability for a host to recognize infection is critical for virus clearance and often begins with induction of inflammation. The PB1-F2 of pathogenic influenza A viruses (IAV) contributes to the pathophysiology of infection, although the mechanism for this is unclear. The NLRP3-inflammasome has been implicated in IAV pathogenesis, but whether IAV virulence proteins can be activators of the complex is unknown. We investigated whether PB1-F2-mediated activation of the NLRP3-inflammasome is a mechanism contributing to overt inflammatory responses to IAV infection. We show PB1-F2 induces secretion of pyrogenic cytokine IL-1β by activating the NLRP3-inflammasome, contributing to inflammation triggered by pathogenic IAV. Compared to infection with wild-type virus, mice infected with reverse engineered PB1-F2-deficient IAV resulted in decreased IL-1β secretion and cellular recruitment to the airways. Moreover, mice exposed to PB1-F2 peptide derived from pathogenic IAV had enhanced IL-1β secretion compared to mice exposed to peptide derived from seasonal IAV. Implicating the NLRP3-inflammasome complex specifically, we show PB1-F2 derived from pathogenic IAV induced IL-1β secretion was Caspase-1-dependent in human PBMCs and NLRP3-dependent in mice. Importantly, we demonstrate PB1-F2 is incorporated into the phagolysosomal compartment, and upon acidification, induces ASC speck formation. We also show that high molecular weight aggregated PB1-F2, rather than soluble PB1-F2, induces IL-1β secretion. Furthermore, NLRP3-deficient mice exposed to PB1-F2 peptide or infected with PB1-F2 expressing IAV were unable to efficiently induce the robust inflammatory response as observed in wild-type mice. In addition to viral pore forming toxins, ion channel proteins and RNA, we demonstrate inducers of NLRP3-inflammasome activation may include disordered viral proteins, as exemplified by PB1-F2, acting as host pathogen 'danger' signals. Elucidating immunostimulatory PB1-F2 mediation of NLRP3-inflammasome activation is a major step forward in our understanding of the aetiology of disease attributable to exuberant inflammatory responses to IAV infection
