Albumin Administration in Acute Ischemic Stroke: Safety Analysis of the ALIAS Part 2 Multicenter Trial

BACKGROUND: Albumin treatment of ischemic stroke was associated with cardiopulmonary adverse events in previous studies and a low incidence of intracranial hemorrhage. We sought to describe the neurological and cardiopulmonary adverse events in the ALIAS Part 2 Multicenter Trial. METHODS: Ischemic stroke patients, aged 18-83 and a baseline NIHSS ≥ 6, were randomized to treatment with ALB or saline control within 5 hours of stroke onset. Neurological adverse events included symptomatic intracranial hemorrhage, hemicraniectomy, neurological deterioration and neurological death. Cardiopulmonary adverse events included pulmonary edema/congestive heart failure, acute coronary syndromes, atrial fibrillation, pneumonia and pulmonary thromboembolism. RESULTS: Among 830 patients, neurological and cardiopulmonary adverse events were not differentially associated with poor outcome between ALB and saline control subjects. The rate of symptomatic intracranial hemorrhage in the first 24h was low overall (2.9%, 24/830) but more common in the ALB treated subjects (RR = 2.4, CI95 1.01-5.8). The rate of pulmonary edema/CHF in the first 48h was 7.9% (59/830) and was more common among ALB treated subjects (RR = 10.7, CI95 4.3-26.6); this complication was expected and was satisfactorily managed with mandated diuretic administration and intravenous fluid guidelines. Troponin elevations in the first 48h were common, occurring without ECG change or cardiac symptoms in 52 subjects (12.5%). CONCLUSIONS: ALB therapy was associated with an increase in symptomatic ICH and pulmonary edema/congestive heart failure but this did not affect final outcomes. Troponin elevation occurs routinely in the first 48 hours after acute ischemic stroke. TRIAL REGISTRATION: ClincalTrials.gov NCT00235495

The emergence of word-internal repetition through iterated learning:Explaining the mismatch between learning biases and language design

    Get PDF
The idea that natural language is shaped by biases in learning plays a key role in our understanding of how human language is structured, but its corollary that there should be a correspondence between typological generalisations and ease of acquisition is not always supported. For example, natural languages tend to avoid close repetitions of consonants within a word, but developmental evidence suggests that, if anything, words containing sound repetitions are more, not less, likely to be acquired than those without. In this study, we use word-internal repetition as a test case to provide a cultural evolutionary explanation of when and how learning biases impact on language design. Two artificial language experiments showed that adult speakers possess a bias for both consonant and vowel repetitions when learning novel words, but the effects of this bias were observable in language transmission only when there was a relatively high learning pressure on the lexicon. Based on these results, we argue that whether the design of a language reflects biases in learning depends on the relative strength of pressures from learnability and communication efficiency exerted on the linguistic system during cultural transmission

This paper analyzes the infractions against Honor typified in the old Penal Code in force until 2014, and how they are incorporated into the current Organic Code of Comprehensive Criminal Law (COIP). It also analyzes the procedure used in both cases, starting from a study of the concept "right to Honor", to later observe how the Ecuadorian legislation since the first Penal Code dealt with infractions against Honor. The procedure and resources that could be used before and after the issuance of the COIP are compared, especially when this norm treats the offense as a contravention. Finally, the manner in which the right to Honor with Freedom of Expression and several resolutions of the Inter-American Court of Human Rights, which set a precedent of greater tolerance against criticism and censorship towards public figures, is studied; it is clear that the supposed legislative advances introduced in the COIP actually served as a political tool used by the previous government against its opponents.Magíster en Derecho PenalCuenc

Successful adaptation of a research methods course in South America

Background: South America has low research productivity. The lack of a structured research curriculum is one of the barriers to conducting research. Objective: To report our experience adapting an active learning-based research methods curriculum to improve research productivity at a university in Ecuador. Design: We used a mixed-method approach to test the adaptation of the research curriculum at Universidad Catolica Santiago de Guayaquil. The curriculum uses a flipped classroom and active learning approach to teach research methods. When adapted, it was longitudinal and had 16-hour programme of in-person teaching and a six-month follow-up online component. Learners were organized in theme groups according to interest, and each group had a faculty leader. Our primary outcome was research productivity, which was measured by the succesful presentation of the research project at a national meeting, or publication in a peer-review journal. Our secondary outcomes were knowledge and perceived competence before and after course completion. We conducted qualitative interviews of faculty members and students to evaluate themes related to participation in research. Results: Fifty university students and 10 faculty members attended the course. We had a total of 15 groups. Both knowledge and perceived competence increased by 17 and 18 percentage points, respectively. The presentation or publication rate for the entire group was 50%. The qualitative analysis showed that a lack of research culture and curriculum were common barriers to research. Conclusions: A US-based curriculum can be successfully adapted in low-middle income countries. A research curriculum aids in achieving pre-determined milestones.
