    Neoadjuvant therapy for breast cancer

    Objective: To evaluate the frequency of neoadjuvant therapy (NT) in women with stage I–III breast cancer in Italy and whether it is influenced by biological characteristics, screening history, and geographic area. Methods: Data from the High Resolution Study conducted in 7 Italian cancer registries were used; they are a representative sample of incident cancers in the study period (2009–2013). Included were 3546 women aged <85 years (groups <50, 50–69, 70–64, and 75+) with stage I–III breast cancer at diagnosis who underwent surgery. Women were classified as receiving NT if they received chemotherapy, target therapy, and/or hormone therapy before the first surgical treatment. Logistic models were built to test the association with biological and contextual variables. Results: Only 8.2% of women (290 cases) underwent NT; the treatment decreases with increasing age (14.5% in age <50 and 2.2% in age 75+), is more frequent in women with negative receptors (14.8%), HER2-positive (15.7%), and triple-negative (15.6%). The multivariable analysis showed the probability of receiving NT is higher in stage III (odds ratio [OR] 3.83; 95% confidence interval [CI] 2.83–5.18), luminal B (OR 1.87; 95% CI 1.27–2.76), triple-negatives (OR 1.88; 95% CI 1.15–3.08), and in symptomatic cancers (OR 1.98; 95% CI 1.13–3.48). Use of NT varied among geographic areas: Reggio Emilia had the highest rates (OR 2.29; 95% CI 1.37–3.82) while Palermo had the lowest (OR 0.41; 95% CI 0.24–0.68). Conclusions: The use of NT in Italy is limited and variable. There are no signs of greater use in hospitals with more advanced care

    Salmonella serotypes in wild boars (Sus scrofa) hunted in northern Italy

    BACKGROUND: Salmonella species (spp.) are zoonotic enteric bacteria able to infect humans, livestock and wildlife. However, little is known about the prevalence and the presence of the different serovars in wildlife. Considering the wide distribution of wild boars and the feeding behaviour (omnivorous scavengers), wild boars may be a good indicator for environmental presence of Salmonella spp. The aims of this study were to determine the presence of Salmonella spp. in hunted wild boars and to determine the serotype the isolated strains. FINDINGS: Over three hunting seasons, the intestinal contents of 1,313 boars hunted in northern Italy were sampled and cultured. Salmonella spp. were isolated from 326 boars (24.82%). Thirty different serovars belonging to three different S. enterica spp. were found. Twenty-one serovars of S. enterica subsp. Enterica were found including the human pathogens S. Typhimurium and S. Enteritidis. In addition, nine serovars belonging to S.enterica subsp. diarizonae and S. enterica subsp. houtenae were detected. CONCLUSIONS: Considering the widespread occurrence of wild boars in Europe, the epidemiological role of this species in relation to salmonellosis might be relevant and should be further investigated. Wild boars may act as healthy carriers of a wide range of Salmonella serotypes

    Design alternatives in air conditioned buildings may be easily compared just by summing the hourly consumption of primary energy, while quantitative approachs for bioclimatic design strategies are difficult to be assessed and compared. A actively heated and passively cooled school building is considered as an application field of a novel methodology to promote an informed choice about the retrofit strategies to be adopted for buildings, defined as the Gained Comfort Cost (GCC). A functional and significant unit (i.e. a classroom), is used to test different energy retrofit solutions and their performances were compared with a baseline, in terms of the capacity to reduce the indoor air temperature variation. The novel methodology is a visual tool allowing to understand the “distance” of indoor conditions from comfort; the retrofit strategies are promoted to reduce this distance considering however the associated costs (LCC) to deal with actual feasibility

    Simulation of a trust and reputation based mitigation protocol for a black hole style attack on VANETs

    From a security standpoint, VANETs (Vehicular ad hoc Networks) are vulnerable to attacks by malicious users, due to the decentralized and open nature of the wireless system. For many of these kinds of attacks detection is unfeasible, thus making it hard to produce security. Despite their characterization as dynamically reconfigurable networks, it is nonetheless essential to identify topology and population properties that can optimise mitigation protocols’ deployment. In this paper, we provide an algorithmic definition and simulation of a trust and mitigation based protocol to contain a Black Hole style attack on a VANET. We experimentally show its optimal working conditions: total connectivity, followed by a random network; connection to external networks; early deployment of the protocol and ranking of the message. We compare results with those of existing protocols and future work shall focus on repeated broadcasting, opportunistic message forwarding and testing on real data

    Pleiotropic antitumor effects of the pan-HDAC inhibitor ITF2357 against c-Myc-overexpressing human B-cell non-Hodgkin lymphomas,

    Histone deacetylases (HDAC) extensively contribute to the c-Myc oncogenic program, pointing to their inhibition as an effective strategy against c-Myc-overexpressing cancers. We, thus, studied the therapeutic activity of the new-generation pan-HDAC inhibitor ITF2357 (Givinostat®) against c-Myc-overexpressing human B-cell non-Hodgkin lymphomas (B-NHLs). ITF2357 anti-proliferative and pro-apoptotic effects were analyzed in B-NHL cell lines with c-Myc translocations (Namalwa, Raji and DOHH-2), stabilizing mutations (Raji) or post-transcriptional alterations (SU-DHL-4) in relationship to c-Myc modulation. ITF2357 significantly delayed the in vitro growth of all B-NHL cell lines by inducing G1 cell-cycle arrest, eventually followed by cell death. These events correlated with the extent of c-Myc protein, but not mRNA, downregulation, indicating the involvement of post-transcriptional mechanisms. Accordingly, c-Myc-targeting microRNAs let-7a and miR-26a were induced in all treated lymphomas and the cap-dependent translation machinery components 4E-BP1, eIF4E and eIF4G, as well as their upstream regulators, Akt and PIM kinases, were inhibited in function of the cell sensitivity to ITF2357, and, in turn, c-Myc downregulation. In vivo, ITF2357 significantly hampered the growth of Namalwa and Raji xenografts in immunodeficient mice. Noteworthy, its combination with suboptimal cyclophosphamide, achieved complete remissions in most animals and equaled or even exceeded the activity of optimal cyclophosphamide. Collectively, our findings provide the rationale for testing the clinical advantages of adding ITF2357 to current therapies for the still very ominous c-Myc-overexpressing lymphomas. They equally provide the proof-of-concept for its clinical evaluation in rational combination with the promising inhibitors of B-cell receptor and PI3K/Akt/mTOR axis currently in the process of development

    Gravity in the 3+1-Split Formalism II: Self-Duality and the Emergence of the Gravitational Chern-Simons in the Boundary

    We study self-duality in the context of the 3+1-split formalism of gravity with non-zero cosmological constant. Lorentzian self-dual configurations are conformally flat spacetimes and have boundary data determined by classical solutions of the three-dimensional gravitational Chern-Simons. For Euclidean self-dual configurations, the relationship between their boundary initial positions and initial velocity is also determined by the three-dimensional gravitational Chern-Simons. Our results imply that bulk self-dual configurations are holographically described by the gravitational Chern-Simons theory which can either viewed as a boundary generating functional or as a boundary effective action.Comment: 25 pages; v2: minor improvements, references adde

    Volcanic ash as an oceanic iron source and sink

    Volcanic ash deposition to the ocean forms a natural source of iron (Fe) to surface water microbial communities. Inputs of lithogenic material may also facilitate Fe removal through scavenging. Combining dissolved Fe (dFe) and thorium-234 observations alongside modeling, we investigate scavenging of Fe in the North Atlantic following the Eyjafjallajökull volcanic eruption. Under typical conditions biogenic particles dominate scavenging, whereas ash particles dominate during the eruption. The size of particles is important as smaller scavenging particles can become saturated with surface-associated ions. Model simulations indicate that ash deposition associated with Eyjafjallajökull likely led to net Fe removal. Our model suggests a threefold greater stimulation of biological activity if ash deposition had occurred later in the growing season when the region was Fe limited. The implications of ash particle scavenging, eruption timing, and particle saturation need to be considered when assessing the impact of ash deposition on the ocean Fe cycle and productivity

    Mutual heavy ion dissociation in peripheral collisions at ultrarelativistic energies

    We study mutual dissociation of heavy nuclei in peripheral collisions at ultrarelativistic energies. Earlier this process was proposed for beam luminosity monitoring via simultaneous registration of forward and backward neutrons in zero degree calorimeters at Relativistic Heavy Ion Collider. Electromagnetic dissociation of heavy ions is considered in the framework of the Weizsacker-Williams method and simulated by the RELDIS code. Photoneutron cross sections measured in different experiments and calculated by the GNASH code are used as input for the calculations of dissociation cross sections. The difference in results obtained with different inputs provides a realistic estimation for the systematic uncertainty of the luminosity monitoring method. Contribution to simultaneous neutron emission due to grazing nuclear interactions is calculated within the abrasion model. Good description of CERN SPS experimental data on Au and Pb dissociation gives confidence in predictive power of the model for AuAu and PbPb collisions at RHIC and LHC.Comment: 46 pages with 7 tables and 13 figures, numerical integration accuracy improved, next-to-leading-order corrections include

    Gravity in the 3+1-Split Formalism I: Holography as an Initial Value Problem

    We present a detailed analysis of the 3+1-split formalism of gravity in the presence of a cosmological constant. The formalism helps revealing the intimate connection between holography and the initial value formulation of gravity. We show that the various methods of holographic subtraction of divergences correspond just to different transformations of the canonical variables, such that the initial value problem is properly set up at the boundary.
