C Wright Mills, power and the power elites ? a reappraisal
This paper revisits and presents a critical appraisal of Mills's analysis of power and the power elite. There are signs of a revival of interest in Mills, but recent commentators have shown little interest in the intellectual, social or political context of his analysis. Setting Mills's thesis in its historical context, we consider an element of his project that has been particularly neglected in recent discussion: Mills's search for possible ways of redistributing power and his attempt to forge an ethico-political stance. Reflecting on recent discussion of contemporary elite formations, we comment on what critics might take from Mills in our own time in relation to the analysis of elites and the politics of critical management studies
Selective aortic arch perfusion: a first-in-human observational cadaveric study.
BACKGROUND: Selective aortic arch perfusion (SAAP) is a novel endovascular technique that combines thoracic aortic occlusion with extracorporeal perfusion of the brain and heart. SAAP may have a role in both haemorrhagic shock and in cardiac arrest due to coronary ischaemia. Despite promising animal studies, no data is available that describes SAAP in humans. The primary aim of this study was to assess the feasibility of selective aortic arch perfusion in humans. The secondary aim of the study was to assess the feasibility of achieving direct coronary artery access via the SAAP catheter as a potential conduit for salvage percutaneous coronary intervention. METHODS: Using perfused human cadavers, a prototype SAAP catheter was inserted into the descending aorta under fluoroscopic guidance via a standard femoral percutaneous access device. The catheter balloon was inflated and the aortic arch perfused with radio-opaque contrast. The coronary arteries were cannulated through the SAAP catheter. RESULTS: The procedure was conducted four times. During the first two trials the SAAP catheter was passed rapidly and without incident to the intended descending aortic landing zone and aortic arch perfusion was successfully delivered via the device. The SAAP catheter balloon failed on the third trial. On the fourth trial the left coronary system was cannulated using a 5Fr coronary guiding catheter through the central SAAP catheter lumen. CONCLUSIONS: For the first time using a perfused cadaveric model we have demonstrated that a SAAP catheter can be easily and safely inserted and SAAP can be achieved using conventional endovascular techniques. The SAAP catheter allowed successful access to the proximal aorta and permitted retrograde perfusion of the coronary and cerebral circulation
Rayleigh-B\'{e}nard convection in a homeotropically aligned nematic liquid crystal
We report experimental results for convection near onset in a thin layer of a
homeotropically aligned nematic liquid crystal heated from below as a function
of the temperature difference and the applied vertical magnetic
field and compare them with theoretical calculations. The experiments cover
the field range 8 \alt h \equiv H/ H_{F} \alt 80 ( is the
Fr\'eedericksz field). For less than a codimension-two field the bifurcation is subcritical and oscillatory, with travelling- and
standing-wave transients. Beyond the bifurcation is stationary and
subcritical until a tricritical field is reached, beyond which it
is supercritical. The bifurcation sequence as a function of found in the
experiment confirms the qualitative aspects of the theoretical predictions.
However, the value of is about 10% higher than the predicted value and
the results for are systematically below the theory by about 2% at small
and by as much as 7% near . At , is continuous within
the experimental resolution whereas the theory indicates a 7% discontinuity.
The theoretical tricritical field is somewhat below the
experimental one. The fully developed flow above for is
chaotic. For the subcritical stationary bifurcation also
leads to a chaotic state. The chaotic states persist upon reducing the Rayleigh
number below , i.e. the bifurcation is hysteretic. Above the tricritical
field , we find a bifurcation to a time independent pattern which within

Understanding global patterns of domestic cannabis cultivation
Purpose – Unlike other plant-based drugs, cannabis is increasingly grown within the country of consumption, requires minimal processing before consumption, and can be easily grown almost anywhere using indoor or outdoor cultivation techniques. Developments in agronomic technologies have led to global growth in domestic cultivation, both by cannabis users for self- and social-supply, and by more commercially-oriented growers. Cross-national research is needed to better understand who is involved in domestic cultivation, the diversity in cultivation practices and motivations, and cultivators’ interaction with the criminal justice system and cannabis control policies. Design/methodology/approach – The article introduces the Global Cannabis Cultivation Research Consortium (GCCRC), describes its evolution and aims, and outlines the methodology of its ongoing cross-national online survey of cannabis cultivation. Findings – Despite differing national contexts, the GCCRC successfully developed a core questionnaire to be used in different countries. It accommodates varying research interests through the addition of optional survey sections. The benefits to forming an international consortium to conduct web-based survey research include the sharing of expertise, recruitment efforts and problem-solving. Research limitations/implications – The article discusses the limitations of using non-representative online sampling and the strategies used to increase validity. Originality/value – The GCCRC is conducting the largest cross-national study of domestic cannabis cultivation to date. The aim is not only to better understand patterns of cannabis cultivation and how they differ between countries but also to build upon online engagement methodology with hidden populations
The prevalence of Angiostrongylus cantonensis/mackerrasae complex in molluscs from the Sydney region

Angiostrongylus cantonensis and Angiostrongylus mackerrasae are metastrongyloid nematodes that infect various rat species. Terrestrial and aquatic molluscs are intermediate hosts of these worms while humans and dogs are accidental hosts. Angiostrongylus cantonensis is the major cause of angiostrongyliasis, a disease characterised by eosinophilic meningitis. Although both A. cantonensis and A. mackerrasae are found in Australia, A. cantonensis appears to account for most infections in humans and animals. Due to the occurrence of several severe clinical cases in Sydney and Brisbane, the need for epidemiological studies on angiostrongyliasis in this region has become apparent. In the present study, a conventional PCR and a TaqMan assay were compared for their ability to amplify Angiostrongylus DNA from DNA extracted from molluscs. The TaqMan assay was more sensitive, capable of detecting the DNA equivalent to one hundredth of a nematode larva. Therefore, the TaqMan assay was used to screen molluscs (n=500) of 14 species collected from the Sydney region. Angiostrongylus DNA was detected in 2 of the 14 mollusc species; Cornu aspersum [14/ 312 (4.5%)], and Bradybaenia similaris [1/10 (10%)], which are non-native terrestrial snails commonly found in urban habitats. The prevalence of Angiostrongylus spp. was 3.0% ±0.8% (CI 95%). Additionally, experimentally infected Austropeplea lessoni snails shed A. cantonensis larvae in their mucus, implicating mucus as a source of infection. This is the first Australian study to survey molluscs using real-time PCR and confirms that the garden snail, C. aspersum, is a common intermediate host for Angiostrongylus spp. in Sydney
Can Waterlow score predict 30-day mortality and length of stay in acutely admitted medical patients (aged ≥65 years)? Evidence from a single centre prospective cohort study
OBJECTIVE: This study aimed to explore the potential for the Waterlow score (WS) to be used as a predictor of 30-day mortality and length of hospital stay (LHS) in acutely admitted medical patients aged 65 years and older.
DESIGN: Prospective observational cohort study.
SETTING: UK District General Hospital.
SUBJECTS: 834 consecutive patients aged 65 years and older admitted acutely to medical specialties between 30 May and 22 July 2014.
METHODS ADMISSION: WS (range 4–64) assessment paired with the patient’s status at 30 days in terms of mortality and their LHS.
PRIMARY OUTCOMES: 30-day mortality and length of inpatient stay.
RESULTS: 834 consecutive acute medical admissions had their WS recorded. 30-day mortality was 13.1% (109 deaths). A significant difference in the distribution of WS (p<0.001) was seen between those who survived (median 12) and those who died (median 16) within 30 days, particularly within respiratory (p<0.001), stroke (p<0.001), cardiology (p<0.016), non-respiratory infections (p<0.018) and trauma (p<0.044) subgroups. Odds of dying within 30 days increased threefold for every 10-unit increase in the WS (p<0.001, 95% CI 2.1 to 4.3). LHS was also positively linearly associated with the WS in those who survived 30 days (median=5, IQR=10; r=0.32, p<0.01). A five-unit increase in WS was associated with approximately 5 days increase in LHS. On the other hand, quadratic regression showed this relationship was curvilinear and negative (concave) for those who died within 30 days where a five-unit increase in WS was associated with an approximately 10 days decrease in LHS.
CONCLUSION: This study demonstrates an association between a high WS and both 30-day mortality and LHS. This is particularly significant for mortality in patients in the respiratory, stroke and cardiac subcategories. The WS, a nursing-led screening tool that is carried out on virtually all admissions to UK hospitals, could have additional use at the time of patient admission as a risk assessment tool for 30-day mortality as well as a predictor of LHS
The challenges and benefits of analyzing feedback comments in surveys: Lessons from a cross-national online survey of small-scale cannabis growers
It is common practice in survey questionnaires to include a general open and non-directive feedback question at the end, but the analysis of this type of data is rarely discussed in the methodological literature. While these open-ended comments can be useful, most researchers fail to report on this issue. The aim of this article is to illustrate and reflect upon the benefits and challenges of analyzing responses to open-ended feedback questions. The article describes the experiences of coding and analyzing data generated through a feedback question at the end of an international online survey with small-scale cannabis cultivators carried out by the Global Cannabis Cultivation Research Consortium. After describing the design and dataset of the web survey, the analytical approach and coding frame are presented. The analytical strategies chosen in this study illustrate the diversity and complexity of feedback comments which pose methodological challenges to researchers wishing to use them for data analyses. In this article, three types of feedback comments (political/policy comments, general comments of positive and negative appreciation, and methodological comments) are used to illustrate the difficulties and advantages of analyzing this type of data. The advantages of analyzing feedback comments are well known, but they seem to be rarely exploited. General feedback questions at the end of surveys are typically non-directive. If researchers want to use these data for research and analyses, they need a clear strategy. They ought to give enough thought to why they are including this type of question, and develop an analytical strategy at the design stage of the study
