    Responsabilidades legales laborales en la contratación de trabajadores contratistas del sector construcción en Colombia y cómo evitar su incumplimiento

    La modalidad de contratación de trabajadores contratistas es una práctica normal del sector construcción en la que se adquiere responsabilidades legales por lo cual, bajo el análisis jurídico, se quiere identificar cuáles son sus consecuencias de su incumplimiento y si estas afectan directamente a las empresas del sector construcción en Colombia según su contexto. De ahí que, el objetivo principal de este artículo es plantear estrategias que eviten el incumplimiento de los riesgos legales laborales en la contratación de trabajadores contratistas del sector construcción. Para la metodología del artículo se realizó una revisión de fuentes bibliográficas en bases de datos y páginas especializadas según las tres temáticas principales: generalidad de la contratación, las responsabilidades legales laborales y las generalidades del sector construcción. En los resultados se presentaron cinco estrategias de mitigación, las cuales se describen de manera detallada a partir de los hallazgos de las temáticas analizadas. Estos resultados inclusive permitieron identificar que en su mayoría las estrategias de mitigación se encuentran disponibles a través de entidades del Estado y algunas en forma gratuita para implementación por parte de las empresas en general y sus contratistas. Sin embargo, para la puesta en marcha de una de las estrategias que se relacionan si es necesario el interés del Estado por ejercerlas, esta no depende de las empresas. Se concluye de manera general que, a pesar de la gran importancia del sector construcción para el País, los esfuerzos se orientan hacia otros objetivos lo que deja en evidencia las condiciones laborales desfavorables para sus trabajadores, y a su vez, los riesgos legales laborales que las empresas involucradas en toda la cadena de contratación asumen.Universidad Libre – Facultad de Derecho -- Especialización en Derecho Laboral y Seguridad SocialThe procedure to contract worker’s, is a normal practice in the construction business, in which legal responsibilities are acquired, under the legal analysis. We want to identify the consequences of a non -compliance at work, in which they are directed to the companies in Colombia, pertaining to their sectors and their context. Hence, the main objective of this article is to avoid the breach of labor strategies in hiring of contractor workers in the construction sector in Colombia. For the methods of this article, a review of bibliographic sources in databases were carried out according to the three main themes: General Contracting, Legal Labor Responsibilities and Generalities of the Construction Sector. In the results, five mitigation strategies were presented, which are described in detail based on the findings of the topics analyzed. These inclusive results made it possible to identify that most of the mitigation strategies are available through State entities and some are free for implementation by companies in general and their contractors. However, for the implementation of one of these strategies that are related, if necessary, the interest of the State to exercise them, this does not depend on the companies. It is generally concluded that, despite the great importance of the construction sector for the County, thus efforts are directed towards other objectives, which reveals the unfavorable working conditions for its workers, and in turn, the legal occupational risks that the companies in which are directly involved in the entire procurement

    Individual voxelwise dosimetry of targeted 90Y-labelled substance P radiotherapy for malignant gliomas

    Purpose: Substance P is the main ligand of neurokinin type 1 (NK-1) receptors, which are consistently overexpressed in malignant gliomas. The peptidic vector 111In/90Y-DOTAGA-substance P binds to these receptors and can be used for local treatment of brain tumours. Dosimetry for this interstitial brachytherapy has mainly been done using geometrical models; however, they often do not faithfully reproduce the in vivo biodistribution of radiopharmaceuticals, which is indispensable to correlate the deposited energy with clinical response. The aim of this study was to establish a reproducible dosimetry protocol for intratumoural radiopeptide therapy. Methods: For test and therapeutic injections, 2MBq of 111In-substance P and 370-3,330MBq of 90Y-substance P, respectively, were applied in 12 patients with malignant gliomas. Over a period of 24h, serial SPECT scans were performed on a dual-head SPECT camera. The scans were acquired in a double-energy window technique together with 99mTc-ECD in order to co-register the dose distributions with a separately acquired, contrast-enhanced CT scan. Quantitative voxelwise dose distribution maps (in Gy/GBq) were computed from these data using a mono-exponential decay approach. Pre- and post-therapeutic values were compared. Results: Agreement between pre- and post-therapeutic dosimetry was very good and delivered absolute dose values in Gy per injected GBq. In all patients, the pretherapeutic test injection together with the CT overlay technique could predict the precise localisation of dose deposition in an anatomical context. Conclusion: This protocol allows a precise pretherapeutic computation of the expected three-dimensional dose distribution and is clearly superior to the previously used dosimetry based on planar scintigraphic images.     Safety evaluation in healthy Colombian volunteers of P2Et extract obtained from caesalpinia spinosa: Design 3+3 phase I clinical trial

    Q3Q2The polyphenol-enriched extract called P2Et derived from Caesalpinia spinosa (C. spinosa) had antitumor and immunomodulatory activities reported in breast cancer, leukemia, and melanoma. The aim of this study was to evaluate the safety and maximum tolerated dose of P2Et extract in Colombian healthy volunteers in a phase 1 clinical trial, open labelled, single-arm, dose-escalation design 3 + 3. Seven healthy volunteers were included; P2Et was administrated in capsules of 600 mg/d for 28 days. Analysis by intention to treat was performed. 4 volunteers showed adverse events and discontinued the intervention. 94.6% of AE were grade 1, and most of AE had a reasonable possibility of a relationship with the P2Et (83.8%). We found that the oral administration of P2Et is safe in healthy humans with a maximum tolerated dose of 600 mg/d.     Non-coding Class Switch Recombination-Related Transcription in Human Normal and Pathological Immune Responses

    Antibody class switch recombination (CSR) to IgG, IgA, or IgE is a hallmark of adaptive immunity, allowing antibody function diversification beyond IgM. CSR involves a deletion of the IgM/IgD constant region genes placing a new acceptor Constant gene, downstream of the VDJH exon. CSR depends on non-coding (CSRnc) transcription of donor Iμ and acceptor IH exons, located 5′ upstream of each CH coding gene. Although, our knowledge of the role of CSRnc transcription has advanced greatly, its extension and importance in healthy and diseased humans is scarce. We analyzed CSRnc transcription in 70,603 publicly available RNA-seq samples, including GTEx, TCGA, and the Sequence Read Archive using recount2, an online resource consisting of normalized RNA-seq gene and exon counts, as well as, coverage BigWig files that can be programmatically accessed through R. CSRnc transcription was validated with a qRT-PCR assay for Iμ, Iγ3, and Iγ1 in humans in response to vaccination. We mapped IH transcription for the human IGH locus, including the less understood IGHD gene. CSRnc transcription was restricted to B cells and is widely distributed in normal adult tissues, but predominant in blood, spleen, MALT-containing tissues, visceral adipose tissue and some so-called “immune privileged” tissues. However, significant Iγ4 expression was found even in non-lymphoid fetal tissues. CSRnc expression in cancer tissues mimicked the expression of their normal counterparts, with notable pattern changes in some common cancer subsets. CSRnc transcription in tumors appears to result from tumor infiltration by B cells, since CSRnc transcription was not detected in corresponding tumor-derived immortal cell lines. Additionally, significantly increased Iδ transcription in ileal mucosa in Crohn's disease with ulceration was found. In conclusion, CSRnc transcription occurs in multiple anatomical locations beyond classical secondary lymphoid organs, representing a potentially useful marker of effector B cell responses in normal and pathological immune responses. The pattern of IH exon expression may reveal clues of the local immune response (i.e., cytokine milieu) in health and disease. This is a great example of how the public recount2 data can be used to further our understanding of transcription, including regions outside the known transcriptome

    Society of Dermatology Hospitalists supportive care guidelines for the management of Stevens-Johnson syndrome/toxic epidermal necrolysis in adults

    Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are life-threatening conditions with high morbidity and mortality. Supportive care management of SJS/TEN is highly variable. A systematic review of the literature was performed by dermatologists, ophthalmologists, intensivists, and gynecologists with expertise in SJS/TEN to generate statements for supportive care guideline development. Members of the Society of Dermatology Hospitalists with expertise in SJS/TEN were invited to participate in a modified, online Delphi-consensus. Participants were administered 9-point Likert scale questionnaires regarding 135 statements. The RAND/UCLA Appropriateness Method was used to evaluate and select proposed statements for guideline inclusion; statements with median ratings of 6.5 to 9 and a disagreement index of ≤1 were included in the guideline. For the final round, the guidelines were appraised by all of the participants. Included are an evidence-based discussion and recommendations for hospital setting and care team, wound care, ocular care, oral care, urogenital care, pain management, infection surveillance, fluid and electrolyte management, nutrition and stress ulcer prophylaxis, airway management, and anticoagulation in adult patients with SJS/TEN
