    Breaking from Tradition: Transforming Leadership Education in Nursing

    Leadership development is critical for nurses to be engaged in health system reform and transformation. Trends in nursing practice led to the development of an innovative senior consolidation course in our four year undergraduate nursing program. We hypothesized that providing opportunities for application of theoretical knowledge related to complex adaptive systems and leadership would enhance professional identity formation. We describe our experiences implementing a clinical course wherein nursing students are provided opportunities to explore and develop their leadership acumen within an undergraduate curriculum. We discuss how breaking from curricular tradition involved intentional use of pedagogy (transformational learning theory), innovative instructional design, and the formation of collaborative partnerships to provide a space for students to practice the cognitive, relational, and meaning making skills required for leadership development. We highlight the impact of this journey on students, faculty, and community partners as we forge ahead in planning next steps to determine the level of engagement of our graduates in health system design, advocacy, and stewardship. Résumé Le développement du leadership par les infirmières est essentiel si elles veulent participer à la réforme et à la transformation du système de santé. Les tendances observées dans la pratique infirmière ont mené à l’élaboration d’un cours d’intégration en fin de programme de premier cycle de quatre années en sciences infirmières. Nous avons fait l’hypothèse qu’en offrant des occasions d’appliquer les connaissances théoriques liées aux systèmes adaptatifs complexes et au leadership, la formation de l’identité professionnelle serait renforcée. Nous décrivons ici nos expériences de la mise en œuvre d’un cours clinique où les étudiantes en sciences infirmières ont l’occasion d’explorer et de développer leur compétence de leadership dans le cadre d’un programme de premier cycle. Nous abordons en quoi le fait de s’être éloignés du programme traditionnel a pavé la voie à l’utilisation intentionnelle de la pédagogie (théorie de l’apprentissage transformationnel), à une conception pédagogique novatrice et à la formation de partenariats de collaboration afin d’offrir aux étudiantes un milieu où elles peuvent mettre en pratique les habiletés cognitives, relationnelles et de réflexion nécessaires au développement du leadership. Nous mettons en valeur l’impact de ce cheminement sur les étudiantes, le corps enseignant et les partenaires de la communauté alors que nous nous préparons à planifier les prochaines étapes pour déterminer le degré de participation de nos diplômées à la conception, à la promotion et à la gestion dans le système de santé

    Tissue-specific inactivation of p53 tumor suppression in the mouse.

    The p53 gene is the most frequent target of structural and functional genetic mutations in human cancer. Thus, considerable effort has been devoted to mapping the functional domains of p53 with regard to their impact on tumorigenesis in vivo. Studies have shown that the carboxy-terminal domain of p53 is sufficient for transformation in vitro. To determine whether a transdominant-negative p53 protein could be used to elicit a tissue-specific p53-null effect in vivo, we tested whether a carboxy-terminal p53 fragment (amino acids 302-390) could abolish p53-dependent apoptosis in an established tumor progression model. We showed previously that loss of p53-dependent apoptosis accelerates brain tumorigenesis in a transgenic mouse model. Here, we show that the same effect can be elicited by expressing a dominant-negative p53 protein tissue specifically in the presence of wild-type p53. Transgenic mice in which pRb function has been disrupted and that coexpress a p53 carboxy-terminal dominant-negative fragment (p53DD) develop aggressive brain tumors mimicking genetic loss of p53 in this model. Inactivation of endogenous p53, which we show to be complexed with p53DD, results in a reduction in apoptosis and acceleration of tumorigenesis. These studies establish a mechanism for tissue-specific knock out of p53 function in vivo

    Background Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage.     Large-Scale Meta-GWAS Reveals Common Genetic Factors Linked to Radiation-Induced Acute Toxicities across Cancers

    BACKGROUND: This study was designed to identify common genetic susceptibility and shared genetic variants associated with acute radiation-induced toxicity (RIT) across four cancer types (prostate, head and neck, breast, and lung).METHODS: A GWAS meta-analysis was performed using 19 cohorts including 12,042 patients. Acute standardized total average toxicity (rSTATacute) was modelled using a generalized linear regression model for additive effect of genetic variants adjusted for demographic and clinical covariates. LD score regression estimated shared SNP-based heritability of rSTATacute in all patients and for each cancer type.RESULTS: Shared SNP-based heritability of STATacute among all cancer types was estimated at 10% (se = 0.02), and was higher for prostate (17%, se = 0.07), head and neck (27%, se = 0.09), and breast (16%, se = 0.09) cancers. We identified 130 suggestive associated SNPs with rSTATacute (5.0x10-8&lt;P-value&lt;1.0x10-5) across 25 genomic regions. rs142667902 showed the strongest association (effect allele A; effect size -0.17; P-value=1.7x10-7), which is located near DPPA4, encoding a protein involved in pluripotency in stem cells, which are essential for repair of radiation-induced tissue injury. Gene-set enrichment analysis identified 'RNA splicing via endonucleolytic cleavage and ligation' (P = 5.1 x10-6, Pcorrected =0.079) as the top gene set associated with rSTATacute among all patients. In-silico gene expression analysis showed the genes associated with rSTATacute were statistically significantly up-regulated in skin (not sun exposed Pcorrected=0.004; sun exposed Pcorrected=0.026).CONCLUSIONS: There is shared SNP-based heritability for acute RIT across and within individual cancer sites. Future meta-GWAS among large radiotherapy patient cohorts are worthwhile to identify the common causal variants for acute radiotoxicity across cancer types.</p

    Background: Antiplatelet therapy reduces the risk of major vascular events for people with occlusive vascular disease, although it might increase the risk of intracranial haemorrhage. Patients surviving the commonest subtype of intracranial haemorrhage, intracerebral haemorrhage, are at risk of both haemorrhagic and occlusive vascular events, but whether antiplatelet therapy can be used safely is unclear. We aimed to estimate the relative and absolute effects of antiplatelet therapy on recurrent intracerebral haemorrhage and whether this risk might exceed any reduction of occlusive vascular events. Methods: The REstart or STop Antithrombotics Randomised Trial (RESTART) was a prospective, randomised, open-label, blinded endpoint, parallel-group trial at 122 hospitals in the UK. We recruited adults (≥18 years) who were taking antithrombotic (antiplatelet or anticoagulant) therapy for the prevention of occlusive vascular disease when they developed intracerebral haemorrhage, discontinued antithrombotic therapy, and survived for 24 h. Computerised randomisation incorporating minimisation allocated participants (1:1) to start or avoid antiplatelet therapy. We followed participants for the primary outcome (recurrent symptomatic intracerebral haemorrhage) for up to 5 years. We analysed data from all randomised participants using Cox proportional hazards regression, adjusted for minimisation covariates. This trial is registered with ISRCTN (number ISRCTN71907627). Findings: Between May 22, 2013, and May 31, 2018, 537 participants were recruited a median of 76 days (IQR 29–146) after intracerebral haemorrhage onset: 268 were assigned to start and 269 (one withdrew) to avoid antiplatelet therapy. Participants were followed for a median of 2·0 years (IQR [1·0– 3·0]; completeness 99·3%). 12 (4%) of 268 participants allocated to antiplatelet therapy had recurrence of intracerebral haemorrhage compared with 23 (9%) of 268 participants allocated to avoid antiplatelet therapy (adjusted hazard ratio 0·51 [95% CI 0·25–1·03]; p=0·060). 18 (7%) participants allocated to antiplatelet therapy experienced major haemorrhagic events compared with 25 (9%) participants allocated to avoid antiplatelet therapy (0·71 [0·39–1·30]; p=0·27), and 39 [15%] participants allocated to antiplatelet therapy had major occlusive vascular events compared with 38 [14%] allocated to avoid antiplatelet therapy (1·02 [0·65–1·60]; p=0·92). Interpretation: These results exclude all but a very modest increase in the risk of recurrent intracerebral haemorrhage with antiplatelet therapy for patients on antithrombotic therapy for the prevention of occlusive vascular disease when they developed intracerebral haemorrhage. The risk of recurrent intracerebral haemorrhage is probably too small to exceed the established benefits of antiplatelet therapy for secondary prevention

    Canagliflozin and renal outcomes in type 2 diabetes and nephropathy

    BACKGROUND Type 2 diabetes mellitus is the leading cause of kidney failure worldwide, but few effective long-term treatments are available. In cardiovascular trials of inhibitors of sodium–glucose cotransporter 2 (SGLT2), exploratory results have suggested that such drugs may improve renal outcomes in patients with type 2 diabetes. METHODS In this double-blind, randomized trial, we assigned patients with type 2 diabetes and albuminuric chronic kidney disease to receive canagliflozin, an oral SGLT2 inhibitor, at a dose of 100 mg daily or placebo. All the patients had an estimated glomerular filtration rate (GFR) of 30 to &lt;90 ml per minute per 1.73 m2 of body-surface area and albuminuria (ratio of albumin [mg] to creatinine [g], &gt;300 to 5000) and were treated with renin–angiotensin system blockade. The primary outcome was a composite of end-stage kidney disease (dialysis, transplantation, or a sustained estimated GFR of &lt;15 ml per minute per 1.73 m2), a doubling of the serum creatinine level, or death from renal or cardiovascular causes. Prespecified secondary outcomes were tested hierarchically. RESULTS The trial was stopped early after a planned interim analysis on the recommendation of the data and safety monitoring committee. At that time, 4401 patients had undergone randomization, with a median follow-up of 2.62 years. The relative risk of the primary outcome was 30% lower in the canagliflozin group than in the placebo group, with event rates of 43.2 and 61.2 per 1000 patient-years, respectively (hazard ratio, 0.70; 95% confidence interval [CI], 0.59 to 0.82; P=0.00001). The relative risk of the renal-specific composite of end-stage kidney disease, a doubling of the creatinine level, or death from renal causes was lower by 34% (hazard ratio, 0.66; 95% CI, 0.53 to 0.81; P&lt;0.001), and the relative risk of end-stage kidney disease was lower by 32% (hazard ratio, 0.68; 95% CI, 0.54 to 0.86; P=0.002). The canagliflozin group also had a lower risk of cardiovascular death, myocardial infarction, or stroke (hazard ratio, 0.80; 95% CI, 0.67 to 0.95; P=0.01) and hospitalization for heart failure (hazard ratio, 0.61; 95% CI, 0.47 to 0.80; P&lt;0.001). There were no significant differences in rates of amputation or fracture. CONCLUSIONS In patients with type 2 diabetes and kidney disease, the risk of kidney failure and cardiovascular events was lower in the canagliflozin group than in the placebo group at a median follow-up of 2.62 years

    The impact of boredom on creativity in the classroom

    How does engagement impact creativity in the classroom? In general, engagement with lessons is widely regarded as the goal of an educational set-up. Engagement is generally associated with positive emotions, which some work has suggested promotes creativity (e.g., de Rooij &amp; Vromans, 2018). Thus, one might expect engagement to lead to greater creativity. Boredom, however, induced better performance in divergent-thinking tasks in adults (Mann &amp; Caldman, 2013): adults who had been asked to copy numbers from a phone book came up with more creative uses for two plastic cups. Boredom puts learners in a state in which they require stimulation, and thus the deprivation triggers them to generate that needed stimulation through thought and imagination (e.g., Brisset &amp; Snow, 1993; Hebb, 1996; Fisher, 1993; Kierkegaard, 1843). Despite the popularity of both notions, little work has sought to uncover a unified theory of how exactly engagement or the lack of it might work to contribute to creativity and imagination. Further, no work has examined how engagement in the classroom impacts creativity in school-aged children in the classroom. Could this boredom-induced boost to creativity observed in adults be strategically employed to induce heightened creativity in grade-schooled children? While it is possible, it remains untested. Bored adults expressed more divergent thinking, but we do not yet know if bored children would behave similarly. Adolescents emotional volatility is one factor that could change the outcome, for example. Though bored adults were creative, they also reliably associate boredom with a range of negative feelings, including stress, misery, frustration, and lethargy (Mann, 2006; Martin et al, 2006). These negative feelings have previously been associated with negative performance outcomes in adults, including poor work performance (Drory, 1982; Vodanovich, 2003) and absenteeism (Brisset &amp; Snow, 1993). Further, arousal and emotional valence are known to interact in their impact on creativity in adults (Tidikis, Ash, &amp; Collier, 2017). Since emotions are heightened throughout adolescents, the emotional experience of boredom may interact with the state itself in impacting changes in creativity. Boredom is a common experience throughout school. Its ubiquity makes it vital to understand in the interest of maximizing children's intellectual potential. We aim to conduct the first study of boredom on creativity in grade-school children. Previous studies of classroom disengagement have focused on harm to specific learning outcomes. Ours, by contrast, may show how disengagement could be leveraged towards a different end, in the interest of promoting innovation in the classroom. PI Celeste Kidd's group (UC Berkeley, www.kiddlab.com) has studied the cognitive mechanisms (including engagement) that govern learning throughout development, starting in infancy, using a combination of computational and behavioral methods. Co-PI Jennifer Symonds' group (University College Dublin) has previously studied the impact of boredom on children's learning, recently using a mundane English grammar exercise from the 1980's to induce boredom. Their combined skill set is ideal for this quantitative experimental proposal on creativity induction in the classroom. may be able to increase creative thinking. Any outcome, however, will be useful towards understanding how engagement impacts creativity
