    Development of risk-oriented technologies of the corporate financial controlling system

    Financial controlling is one of the functions of financial relations of management system whose main task is to monitor the correct functioning of these relations at the level of a certain managed object with the purpose to decide on sufficiency and effectiveness of managerial decisions. The article considers some peculiarities of creation of a corporate system of financial controlling on the basis of systematization of the existing theoretical and methodical approaches. Also the article presents some directions of improvement of financial controlling tools as well as the results of development of risk-oriented technologies which are aimed at providing financial stability of modern corporations.peer-reviewe

    Эффективность применения локальной инъекционной терапии при болевых синдромах пояснично-крестцовой локализации

    The objective of the study was to assess the effectiveness of local injection therapy in patients with lumbosacral pain during medical blockades.Methods and materials. The results of treatment of compressive lumbosacral radiculoischemia were studied in prospective trial with placebo control. One of the most valuable criteria of medical effectiveness was decrease of pain intensity by visual analog scale and McGill’s questionnaire. 58 patients were included in the study. Comparison between the groups was carried out depending on the treatment: first group was treated with combination of anesthetic with corticosteroids (diprospan), second group – only anesthetic. Rate of pain decrease was assessed during the first day and for 14 days.Results. In patients with lumbosacral pain treated with combination of anesthetic with corticosteroids, decrease of pain intensity was obtained by visual analog scale and McGill’s questionnaire point. Local corticosteroids use was the most effective with effect in a short time – during the first 24 hours.Conclusion. Short course of local injection therapy had high effectiveness in patients with lumbosacral pain syndrome.Цель – оценить эффективность применения локальной инъекционной терапии у больных пояснично-крестцовыми болями при проведении медикаментозных блокад.Методы и материалы. В проспективном исследовании с плацебо-контролем изучены результаты лечения при компрессионных пояснично-крестцовых радикулоишемиях. Одним из ключевых критериев достижения лечебного эффекта являлось снижение интенсивности боли по визуально-аналоговой шкале и опроснику Мак-Гилла. Выборка наблюдений соответствовала 58 наблюдениям. Сравнение между группами проводили в зависимости от проводимого лечения: в 1-й группе комбинирование анестетика и глюкокортикостероидного препарата (Дипроспана). Во 2-й – без основного вещества. Оценивалась быстрота снижения боли в течение 1-х суток наблюдения и на протяжении 14 дней.Результаты. При введении лекарственных препаратов (анестетик в комбинации с глюкокортикоидным препаратом) у пациентов с пояснично-крестцовой болью достигнуто значительное улучшение по интенсивности боли при использовании визуально-аналоговой шкалы и опросника Мак-Гилла. Более быстрый и выраженный эффект достигается при использовании глюкокортикостероидов локального действия. Улучшение наблюдается в короткие сроки – в течение 1-х суток.Заключение. Короткий курс локальной инъекционной терапии обеспечивает бóльшую эффективность у больных с пояснично-крестцовой дорсалгией

    Pharmacogenetics of Bleeding and Thromboembolic Events in Direct Oral Anticoagulant Users

This was a retrospective real-world study linking genotype data of three Finnish biobanks with national register data on drug dispensations and healthcare encounters. We investigated several single-nucleotide variants (SNVs) in the ABCG2, ABCB1, CES1, and CYP3A5 genes potentially associated with bleeding or thromboembolic events in direct oral anticoagulant (DOAC) users based on earlier research. We used Cox regression models to compare the incidence of clinical outcomes between carriers and noncarriers of the SNVs or haplotypes. In total, 1,806 patients on apixaban, dabigatran, or rivaroxaban were studied. The ABCB1 c.3435C>T (p.Ile1145=, rs1045642) SNV (hazard ratio (HR) 0.42, 95% confidence interval (CI), 0.18-0.98, P = 0.044) and 1236T-2677T-3435T (rs1128503-rs2032582-rs1045642) haplotype (HR 0.44, 95% CI, 0.20-0.95, P = 0.036) were associated with a reduced risk for thromboembolic outcomes, and the 1236C-2677G-3435C (HR 2.55, 95% CI, 1.03-6.36, P = 0.044) and 1236T-2677G-3435C (HR 5.88, 95% CI, 2.35-14.72, P A (rs4148738) SNV associated with a lower risk for bleeding events (HR 0.37, 95% CI, 0.16-0.89, P = 0.025) in apixaban users. ABCB1 variants are potential factors affecting thromboembolic events in rivaroxaban users and bleeding events in apixaban users. Studies with larger numbers of patients are warranted for comprehensive assessment of the pharmacogenetic associations of DOACs and their relevance for clinical practice.Peer reviewe

    We show that G\"odel's negative results concerning arithmetic, which date back to the 1930s, and the ancient "sand pile" paradox (known also as "sorites paradox") pose the questions of the use of fuzzy sets and of the effect of a measuring device on the experiment. The consideration of these facts led, in thermodynamics, to a new one-parameter family of ideal gases. In turn, this leads to a new approach to probability theory (including the new notion of independent events).     Features of drug-drug interactions rivaroxaban and calcium channel blockers depending on the ABCB1 genotype (rs1045642 and rs4148738) in patients 80 years of age and older with non-valvular atrial fibrillation

    Background. The use of P-glycoprotein (P-gp) inhibitors and carriage of certain ABCB1 polymorphisms can lead to increased concentrations of rivaroxaban and the development of bleeding.The aim of the study. To study the features of drug-drug interactions (DDI) of rivaroxaban in patients over 80 years of age with non-valvular atrial fibrillation depending on the ABCB1 genotype (rs1045642 and rs4148738) using the example of verapamil (P-gp inhibitor) and amlodipine.Materials and methods. One hundred and twenty-eight patients were examined (median age – 87.5 [83–90] years). Genotyping, determination of the minimum equilibrium concentration of rivaroxaban (Cmin, ss), with standardization for the daily dose (Cmin, ss/D), coagulogram and analysis of medical documentation for the presence of clinically relevant non-major bleeding (CRNM) were carried out. Analysis of CRNM was performed depending on the ABCB1 genotype.Results. The use of rivaroxaban with verapamil in comparison with patients not taking calcium channel blockers (CCBs) leads to high Cmin, ss values in the CC genotype (rs1045642, rs4148738); Сmin, ss and Сmin, ss/D in the CT genotype (rs1045642); prothrombin time in the CC genotype (rs1045642), more frequent occurrence of CRNM in the TT  genotype (rs1045642, rs4148738). In  comparison with patients taking amlodipine, it leads to high Cmin, ss values in the CT genotype (rs1045642), a more frequent occurrence of CRNM in the TT genotype (rs1045642, rs4148738). The use of rivaroxaban with amlodipine in comparison with patients not taking CCBs leads to high Cmin, ss and Cmin, ss/D values in the CC genotype (rs1045642) (p < 0.017).Conclusion. The use of verapamil with rivaroxaban in ABCB1 TT carriers (rs4148738 and rs4148738) leads to the development of CRNM in 75 and 78 % of cases, respectively. In  patients taking rivaroxaban, it is advisable to test the ABCB1 genotype (rs4148738 and rs4148738) before adding a P-gp inhibitor to therapy

    This paper presents the experience of practical use of the technology of computer stabilography with biological feedback in rehabilitation of patients suffering from lumbar-sacral dorsopathies and diagnosed large lumbar hernias. The data of high diagnostic and rehabilitation effectiveness of stabilometry systems was obtained during the 14-days observation period of 16 patients with clinical manifestations of L5-S1 radiculopathy and dorsalgia. The method was used in assessment of neurology deficit and secondary myotonic disturbances. The decrease of intensity of pain and recovery of axial balance and coordination were achieved. As a statistically significant improvement of the equilibrium, the Romberg’s coefficient was decreased from 670 to 295% (p<0.05) during the drug therapy and 10 trainings on stabiloplatform. The achieved data show the improvement of segmental motor and sensory functions, decrease of disturbances in statics and dynamics of vertebral column in patients with radicular syndrome and large hernias after the rehabilitation course with the application of the technology of computer stabilometry.Представлен опыт практического использования технологии компьютерной стабилометрии с биологической обратной связью в реабилитации пациентов с пояснично-крестцовыми дорсопатиями при выявлении грыж межпозвонковых дисков больших размеров. При наблюдении в течение 14 дней у 16 пациентов с клиническими проявлениями радикулопатии L5-S1 и дорсалгией получены данные, указывающие на высокую диагностическую и реабилитационную эффективность стабилометрических систем. Методика применялась для оценки неврологического дефицита, вторичных мышечно-тонических нарушений. Отмечено уменьшение интенсивности болевого синдрома, нормализация осевого баланса и координации движений. Установлено статистически значимое улучшение баланса равновесия в виде снижения коэффициента Ромберга с 670 до 295 % (p<0,05) на фоне медикаментозной терапии и 10 процедур тренировки на стабилоплатформе. Полученные данные свидетельствуют об улучшении сегментарной двигательной и чувствительной функции, регрессировании нарушений статики и динамики позвоночника у пациентов с корешковым синдромом при грыже диска больших размеров по окончании курса реабилитации с применением технологии компьютерной стабилометрии.

    Search for Kaluza-Klein Graviton Emission in ppˉp\bar{p} Collisions at s=1.8\sqrt{s}=1.8 TeV using the Missing Energy Signature

    We report on a search for direct Kaluza-Klein graviton production in a data sample of 84 pb1{pb}^{-1} of \ppb collisions at s\sqrt{s} = 1.8 TeV, recorded by the Collider Detector at Fermilab. We investigate the final state of large missing transverse energy and one or two high energy jets. We compare the data with the predictions from a 3+1+n3+1+n-dimensional Kaluza-Klein scenario in which gravity becomes strong at the TeV scale. At 95% confidence level (C.L.) for nn=2, 4, and 6 we exclude an effective Planck scale below 1.0, 0.77, and 0.71 TeV, respectively.Comment: Submitted to PRL, 7 pages 4 figures/Revision includes 5 figure

    Measurement of the average time-integrated mixing probability of b-flavored hadrons produced at the Tevatron

    We have measured the number of like-sign (LS) and opposite-sign (OS) lepton pairs arising from double semileptonic decays of bb and bˉ\bar{b}-hadrons, pair-produced at the Fermilab Tevatron collider. The data samples were collected with the Collider Detector at Fermilab (CDF) during the 1992-1995 collider run by triggering on the existence of μμ\mu \mu and eμe \mu candidates in an event. The observed ratio of LS to OS dileptons leads to a measurement of the average time-integrated mixing probability of all produced bb-flavored hadrons which decay weakly, χˉ=0.152±0.007\bar{\chi} = 0.152 \pm 0.007 (stat.) ±0.011\pm 0.011 (syst.), that is significantly larger than the world average χˉ=0.118±0.005\bar{\chi} = 0.118 \pm 0.005.Comment: 47 pages, 10 figures, 15 tables Submitted to Phys.
