    Caracterización clínica e histopatológica de niños con hepatitis autoinmune en un centro de referencia de bogotá, colombia

    Autoimmune hepatitis (AIH) is a progressive inflammatory liver disease. It is uncommon in children and adolescents, and is a diagnostic challenge for clinicians and pathologists. We describe the clinical, biochemical and histopathological characteristics of 21 pediatric patients with AIH diagnosed in the last 14 years. Liver biopsies were reassessed to analyze histopathological findings in detail. Of the 21 cases evaluated, 12 (57.1%) were girls and young women, the median age was 14 years old, and 17 (80.9%) had type 1 AIH. The most frequent clinical signs were jaundice (66.7%), choluria (44.4%), evidence of portal hypertension with esophageal varices (47.1%), and splenomegaly (41.2%). Histories of other autoimmune diseases were found in 11.8% of these patients. Elevated levels of aminotransferases were found in 89.5% of the patients, hyperbilirubinemia was found in 88.9%, and 60.0% of the cases had low levels of serum albumin. Reassessed biopsies showed portal lymphoplasmocytic infiltrate (94.4%), interface hepatitis (77.8%) and rosette formation (50.0%). Hyaline inclusions were found in Kupffer cells in 42.9% of the biopsies. About 33.5% of the cases showed cirrhosis at the initial biopsy. Despite immunosuppressive treatment, four patients required liver transplantation and two are on the waiting list. AIH in children can manifest with jaundice, choluria, signs of portal hypertension, elevated aminotransferases, hyperbilirubinemia and circulating antibodies. Hyaline inclusions in Kupffer cells may be a useful finding in the histopathological diagnosis of AIH in children. © 2019 Asociaciones Colombianas de Gastroenterología, Endoscopia digestiva, Coloproctología y Hepatología

    Este artículo analiza Los materiales (Colectivo Los Hijos, 2009) desde una perspectiva formal e ideológica. Sitúa a la película dentro de la producción del colectivo Los Hijos, así como dentro de la evolución del cine material-estructural —como fue caracterizado en los análisis pioneros de P. Adams Sitney y Peter Gidal— al que alude el mismo título. En particular, conecta el film con la deriva política de mutaciones recientes del «cine de estructura», especialmente las que utilizan procedimientos estructurales para explorar la relación entre paisaje e historia. El film enfatiza el poso material de la historia, la forma en que la acción colectiva deviene, por un lado, sedimento y resto fósil y, por otro, memoria de lugar, transmitida a través de narraciones, a menudo orales, que integran la peripecia personal en el devenir colectivo. A la vez, la película ficcionaliza el proceso de filmación y la relación entre los cineastas, y reflexiona sobre la política de la visión, tanto mediante los diálogos como a partir de recursos formales que inciden en la desconexión, la opacidad, y la imposibilidad de visualizar de forma totalizadora y objetiva. Por ello, resulta un ejemplo de una tradición documental que utiliza las limitaciones del dispositivo y la ambigüedad de la imagen para reflexionar sobre la fiabilidad de la imagen audiovisual como vehículo de verdad y objetividad y para abordar al otro desde perspectivas deliberadamente abiertas y antiautoritarias. This article analyses Los materiales (Los Hijos, 2009) from formal, ideological, and historical perspectives. It places the film within the collective''s relatively brief trajectory, as well as within the longer history of structural-material film, directly alluded to in the film''s title. It underlines its structural-material features - as these were pioneeringly described by critics P. Adams Sitney and Peter Gidal - and situates the film in relation to recent political mutations of the "cinema of structure", especially in relation to works that use structural procedures to explore the relationship between landscape and history from a critical-political perspective. The film shows that collective action ends up evolving into geological strata and fossilized remains as well as into a memory of place - narratives, often oral, that weave together personal experience and collective becoming. In addition to reflecting on history, the film fictionalizes its own process of production and the filmmakers'' mutual rapport. Finally, Los materiales also meditates on the politics of vision and visuality both through dialogue and through the deployment of formal resources that insist on disconnection, opaqueness, and the impossibility of seeing in an objective, totalizing fashion. In this regard, the film may be seen as part of a documentary tradition that highlights the limitations of the apparatus and the ambiguity of the moving image, and that seeks to engage the Other - the object of the documentary - in an open-ended, dialogic, and anti-authoritarian manner

    Psychological Stress Triggers a Hyperammonemia Episode in Patient with Ornithine Transcarbamylase Deficiency.

    An 18-year-old male motorcycle racer, who was a participant in the FIM Road RacingWorld Championship and had a history of Ornithine Transcarbamylase deficiency, developed nausea and dizziness while driving his motorcycle and became unconscious right after he stopped at the box. He was rapidly attended to by the medical personnel of the circuit, and once he recovered consciousness, he was taken to the local hospital where the blood analysis showed hyperammonemia (307 _g/dL) and excess alkalosis. The patient was properly following the prescribed treatment, and there were no environmental stressors. Hence, psychological stress and its somatization due to the risky task that the patient was performing could have triggered the episode. Stress must be considered as a potential cause, triggering strenuous metabolic stress that leads to hyperammonemia

    The behavior of Baserca and Llauset dams in the new energetic scenarios

    The growing energetic demands are promoting new business strategies in the operating of hydroelectric power plants. These are materialized by increasing the role of the reversible plants to produce high quality energy. This fact becomes in an abrupt and quick variation of the water level of the reservoirs. This influences at the temperature levels in the dams. Furthermore, since the dams have been working for years, temperature registers from them are available. This enlarges the initial hypotheses considered in the project calculation. This paper summarizes the results obtained in the enlargement of the operating system involving the Baserca and Llauset dams. The initial experimental data come from the measurements which have been taken in those dams after 30 years of operation.Postprint (published version

    Las crecientes demandas energéticas están promoviendo nuevas estrategias comerciales en la explotación de las infraestructuras hidráulicas que se materializan aumentando el protagonismo de los saltos reversibles en la producción de energía. Estos nuevos escenarios dan lugar a operaciones, de embalse y desembalse, más bruscas y frecuentes que pueden alterar, entre otros, los efectos que ocasionan en las estructuras las acciones térmicas durante la explotación. Adicional mente al actuar sobre estructuras con un largo recorrido funcional, se dispone de una información muy completa sobre su comportamiento que complementa y enriquece las hipótesis consideradas en el cálculo durante la fase de proyecto. En esta comunicación se presentan los resultados obtenidos en los análisis realizados en este contexto en las presas bóveda de Baserca y Llauset, el tratamiento de los datos experimentales disponibles después de más de 25 años de funcionamiento satisfactorio y como se han incorporado a los cálculos realizados buscando un equilibrio y una coherencia con las hipótesis utilizadas durante su diseño y construcción.Postprint (published version

    In this note we introduce the notion of t-analytic sets. Using this concept, we construct a class of closed prime ideals in Banach function algebras and discuss some problems related to Alling’s conjecture in H infinity. A description of all closed t-analytic sets for the disk-algebra is given. Moreover, we show that some of the assertions in [8] concerning the O-analyticity and S-regularity of certain Banach function algebras are not correct.     Long-term effect of 2 intensive statin regimens on treatment and incidence of cardiovascular events in familial hypercholesterolemia : The SAFEHEART study

Little is known about the use of different statin regimens in FH. Objectives: The objectives of the study were to describe the treatment changes and low-density lipoprotein cholesterol (LDL-C) goal achievement with atorvastatin (ATV) and rosuvastatin (RV) in the SAFEHEART cohort, as well as to analyze the incidence of atherosclerotic cardiovascular events (ACVEs) and changes in the cardiovascular risk. Methods: SAFEHEART is a prospective follow-up nationwide cohort study in a molecularly defined FH population. The patients were contacted on a yearly basis to obtain relevant changes in life habits, medication, and ACVEs. Results: A total of 1939 patients were analyzed. Median follow-up was 6.6 years (5-10). The estimated 10-year risk according the SAFEHEART risk equation was 1.61 (0.67-3.39) and 1.22 (0.54-2.93) at enrollment for ATV and RV, respectively (P <.001). There were no significant differences at the follow-up: 1.29 (0.54-2.82) and 1.22 (0.54-2.76) in the ATV and RV groups, respectively (P =.51). Sixteen percent of patients in primary prevention with ATV and 18% with RV achieved an LDL-C <100 mg/dL and 4% in secondary prevention with ATV and 5% with RV achieved an LDL-C <70 mg/dL. The use of ezetimibe was marginally greater in the RV group. One hundred sixty ACVEs occurred during follow-up, being its incidence rate 1.1 events/100 patient-years in the ATV group and 1.2 in the RV group (P =.58). Conclusion: ATV and RV are 2 high-potency statins widely used in FH. Although the reduction in LDL-C levels was greater with RV than with ATV, the superiority of RV for reducing ACVEs was not demonstrated
