    Content analysis of promotional material for asthma-related products and therapies on Instagram

    BACKGROUND: Increasingly, social media is a source for information about health and disease self-management. We conducted a content analysis of promotional asthma-related posts on Instagram to understand whether promoted products and services are consistent with the recommendations found in the Global Initiative for Asthma (GINA) 2019 guidelines. METHODS: We collected every Instagram post incorporating a common, asthma-related hashtag between September 29, 2019 and October 5, 2019. Of these 2936 collected posts, we analyzed a random sample of 266, of which, 211 met our inclusion criteria. Using an inductive, qualitative approach, we categorized the promotional posts and compared each post\u27s content with the recommendations contained in the 2019 GINA guidelines. Posts were categorized as consistent with GINA if the content was supported by the GINA guidelines. Posts that promoted content that was not recommended by or was unrelated to the guidelines were categorized as not supported by GINA . RESULTS: Of 211 posts, 89 (42.2%) were promotional in nature. Of these, a total of 29 (32.6%) were categorized as being consistent with GINA guidelines. The majority of posts were not supported by the guidelines. Forty-one (46.1%) posts promoted content that was not recommended by the current guidelines. Nineteen (21.3%) posts promoted content that was unrelated to the guidelines. The majority of unsupported content promoted non-pharmacological therapies (n = 39, 65%) to manage asthma, such as black seed oil, salt-room therapy, or cupping. CONCLUSIONS: The majority of Instagram posts in our sample promoted products or services that were not supported by GINA guidelines. These findings suggest a need for providers to discuss online health information with patients and highlight an opportunity for providers and social media companies to promote evidence-based asthma treatments and self-management advice online

    2.5-D/3-D resistivity modelling in anisotropic media using Gaussian quadrature grids

    We present a new numerical scheme for 2.5-D/3-D direct current resistivity modelling in heterogeneous, anisotropic media. This method, named the 'Gaussian quadrature grid' (GQG) method, cooperatively combines the solution of the Variational Principle of the partial differential equation, Gaussian quadrature abscissae and local cardinal functions so that it has the main advantages of the spectral element method. The formulation shows that the GQG method is a modification of the spectral element method but does not employ the constant elements or require the mesh generator to match the Earth's surface. This makes it much easier to deal with geological models having a 2-D/3-D complex topography than using traditional numerical methods. The GQG technique can achieve a similar convergence rate to the spectral element method. We show it transforms the 2.5-D/3-D resistivity modelling problem into a sparse and symmetric linear equation system that can be solved by an iterative or matrix inversion method. Comparison with analytic solutions for homogeneous isotropic and anisotropic models shows that the error depends on the Gaussian quadrature order (abscissa number) and the subdomain size. The higher the order or the smaller the subdomain size that is employed, the more accurate are the results obtained. Several other synthetic examples, both homogeneous and inhomogeneous, incorporating sloping, undulating and severe topography, are presented and found to yield results comparable to finite element solutions involving a dense mesh.Bing Zhou, Mark Greenhalgh and S.     The Bipolar Interactive Psychoeducation (BIPED) study: trial design and protocol

    <b>Background</b> Bipolar disorders affect between 3–5% of the population and are associated with considerable lifelong impairment. Since much of the morbidity associated with bipolar disorder is caused by recurrent depressive symptoms, which are often only poorly responsive to antidepressants, there is a need to develop alternative, non-pharmacological interventions. Psychoeducational interventions have emerged as promising long-term therapeutic options for bipolar disorder.<p></p> <b>Methods/design</b> The study is an exploratory, individually randomised controlled trial. The intervention known as 'Beating Bipolar' is a psychoeducational programme which is delivered via a novel web-based system. We will recruit 100 patients with a diagnosis of DSM-IV bipolar disorder (including type I and type II) currently in clinical remission. The primary outcome is quality of life. This will be compared for those patients who have participated in the psychoeducational programme with those who received treatment as usual. Quality of life will be assessed immediately following the intervention as well as 10 months after randomisation. Secondary outcomes include current depressive and manic symptoms, number of episodes of depression and mania/hypomania experienced during the follow-up period, global functioning, functional impairment and insight. An assessment of costs and a process evaluation will also be conducted which will explore the feasibility and acceptability of the intervention as well as potential barriers to effectiveness.<p></p> <b>Discussion</b> Bipolar disorder is common, under-recognised and often poorly managed. It is a chronic, life-long, relapsing condition which has an enormous impact on the individual and the economy. This trial will be the first to explore the effectiveness of a novel web-based psychoeducational intervention for patients with bipolar disorder which has potential to be easily rolled out to patients.<p></p> <b>Trial registration</b> Current Controlled Trials ISRCTN8137544
