    Monitoring Training Load, Well-Being, Heart Rate Variability, and Competitive Performance of a Functional-Fitness Female Athlete: A Case Study

    The aim of this case study was to quantify the magnitude of internal load, acute/chronic workload ratio (ACWR), well-being perception, and heart rate variability (HRV) following 38 weeks of functional-fitness training in a female elite athlete. The internal load was obtained with session rating perceived exertion (session-RPE) while the ACWR was calculated by dividing the acute workload by the chronic workload (four-week average). Furthermore, HRV measurements were analyzed via a commercially available smartphone (HRV4training) each morning upon waking whilst in a supine position. The magnitude of internal load was: the weekly mean total during the 38 weeks was 2092 ± 861 arbitrary units (AU); during the preparation for the Open 2018 was 1973 ± 711 AU; during the Open 2018 it was 1686 ± 412 AU; and during the preparation for the Latin America Regional was 3174 ± 595 AU. The mean ACWR was 1.1 ± 0.5 and 50% of the weeks were outside of the ‘safe zone’. The well-being during the 38 weeks of training was 19.4 ± 2.3 points. There were no correlations between training load variables (weekly training load, monotony, ACWR, and HRV), and recuperation subjective variables (well-being, fatigue, sleep, pain, stress, and mood). This case study showed that the training load can be varied in accordance with preparation for a specific competition and ACWR revealed that 50% of the training weeks were outside of the ‘safe zone’, however, no injuries were reported by the athlete. The effectiveness and cost of these methods are very practical during real world functional-fitness

    OpenEHR Based Systems and the General Data Protection Regulation (GDPR)

    The concerns about privacy and personal data protection resulted in reforms of the existing legislation in European Union (EU). The General Data Protection Regulation (GDPR) aims to reform the existing measures on the topic of personal data protection of the European Union citizens, with a strong input on the rights and freedoms of people and in the establishment of rules for the processing of personal data. OpenEHR is a standard that embodies many principles of interoperable and secure software for electronic health records. This work aims to understand to what extent the openEHR standard can be considered a solution for the requirements needed by GDPR. A list of requirements for a Hospital Information Systems (HIS) compliant with GDPR and an identification of openEHR specifications was made. The requirements were categorized and compared with the specifications. The requirements identified for the systems were matched with the openEHR specifications, which result in 16 requirements matched with openEHR. All the specifications identified matched at least one requirement. OpenEHR is a solution for the development of HIS that reinforce privacy and personal data protection, ensuring that they are contemplated in the system development. The institutions can secure that their Eletronic Health Record are compliant with GDPR while safeguarding the medical data quality and, as a result, the healthcare delivery

    Different Cardiovascular Responses to a Resistance Training Session in Hypertensive Women Receiving Propanolol Compared with Normotensive Controls

    The aim of the present study was to compare the responses of blood pressure, heart rate, and rate-pressure product of hypertensive women using beta-blockers with healthy controls during resistance exercise (by the end of the concentric phase of the contractions) and in the postexercise period (5 and 30 minutes after). Ten untrained normotensive women (N) and 10 mildly hypertensive females receiving 40 mg/day of propanolol (H) were selected. Three sets of 10 repetitions at 80% of 10 repetitions maximum with 30 s rest interval were performed on the leg press exercise. The H group exhibited lower systolic blood pressure after the second set compared with N. Heart rate and rate-pressure product were lower in H in all analyzed periods compared with N. Propanolol attenuates the cardiovascular response to a leg press resistance exercise in mildly hypertensive women

    Exertional Rhabdomyolysis after an Extreme Conditioning Competition: A Case Report

    This case report describes an instance of exercise-induced rhabdomyolysis caused by an extreme conditioning program (ECP) competition. A 35-year-old female presented with abdominal pain and soreness, which began one day after she completed two days of ECPcompetition composed of five workouts. Three days after competition, creatine kinase (CK) was 77,590 U/L accompanied by myalgia and abnormal liver function tests, while renal function was normal and this resulted in a diagnosis of rhabdomyolysis. A follow-up examination revealed that her serum level of CK was still elevated to 3034 U/L on day 10 and 1257 U/L on day 25 following the ECP competition. The subject reported myalgia even up to 25 days after the ECP competition. Exertional rhabdomyolysis can be observed in ECP athletes following competition and highlights a dangerous condition, which may be increasing in recent years due to the massive expansion of ECP popularity and a growing number of competitions. Future research should investigate the causes of rhabdomyolysis that occur as a result of ECP, especially training methods and/or tasks developed specifically for these competitions

    Thigh muscles electromyographic activity during lunge exercise to fatigue

    O agachamento afundo possui um posicionamento dos membros inferiores diferencial em relação ao agachamento padrão, necessitando de maiores esclarecimentos acerca das participações dos músculos envolvidos. O objetivo foi analisar a atividade eletromiográfica dos músculos vastus lateralis (VL), vastus medialis (VM), bíceps femoris (BF) e semitendinosus (ST) durante a execução do agachamento afundo até à exaustão com o membro inferior posicionado frontalmente e posteriormente. Participaram do estudo nove mulheres ativas com média (DP) de idade de 22 (3,4) anos e massa corporal 60,3 (4,1) kg. O agachamento afundo foi dividido em duas etapas, diferindo apenas o posicionamento do membro inferior dominante (randomizado). Os sinais eletromiográficos foram captados utilizando um eletromiógrafo e analisados os valores "root mean square" (RMS) na fase concêntrica. Os resultados indicaram um aumento significativo do RMS em função do tempo para o membro inferior posicionado frontalmente e posteriormente (p< 0,001). No membro posicionado frontalmente, o aumento do RMS correspondeu a 50% para o VL, 54% para o VM e 48% para o BF. O membro posicionado posteriormente apresentou um aumento de 75% para o VL, 113% para o VM, 62% para o BF e 48% para o ST. O RMS também foi significativamente maior no músculo VM em relação ao ST no membro inferior posicionado anteriormente (p = 0,03) e em relação ao ST e BF no membro inferior posicionado posteriormente (p = 0,02). Não ocorreu interação significativa entre o efeito do tempo e músculo na atividade eletromiográfica. O RMS normalizado não apresentou diferenças estatisticamente significativas no que se refere ao posicionamento do membro inferior dominante. A atividade muscular foi semelhante em ambos os posicionamentos do membro inferior, apresentando maior aumento de ativação dos músculos VL e VM em relação ao BF e ST

    Time-Course of Changes in Physiological, Psychological and Performance markers Following a Functional-Fitness Competition

Functional Fitness Training (FFT) programs are characterized by utilizing a high volume of training and using a variety of high intensity exercises. While FFT are growing in the number of practitioners and popularity, the relationship between physiological biomarkers and subjective scales in the specific context of FFT has not yet been evaluated in the literature. The purpose of the present study was to monitor the time-course response of cytokines (IL-10 and 1L-1b), immune variables (C-reactive protein -CRP and immunoglobulin A-IgA), hormonal milieu (cortisol-C, total testosterone-TT, free testosterone-FT and testosterone/cortisol-T/C ratio), creatine kinase-CK, muscle performance (countermovement jump height) and perceived well-being (WB) following a functional fitness competition. Nine amateur male athletes (age 27.1 ± 4.1 years; training experience 2.2 ± 1.3 years) completed five workouts over three consecutive days of FFT-competition. All variables were measured before, 24 h, 48 h, and 72 h following the last day of competition. The FFT-competition induced a decrease in IL10/IL1bratio approximately 5% after 24h, 21% after 48h and 31% after 72h. Delta T/C ratio remained unchanged during the post-competition period. IgA displayed a significant increase 24h and 72h post FFT-competition. The WB status score was higher 72h after the FFT-competition as compared with pre-competition. The present findings suggest that FFT-competition induces transient changes in some inflammatory and hormonal biomarkers, and perceived well-being seems to be efficient to detect changes in muscle performance

    Enhancing of women functional status with metabolic syndrome by cardioprotective and anti-inflammatory effects of combined aerobic and resistance training

    These data describe the effects of combined aerobic plus resistance training (CT) with regards to risk factors of metabolic syndrome (MetS), quality of life, functional capacity, and pro- and anti-inflammatory cytokines in women with MetS. In this context, thirteen women (35.466.2 yr) completed 10 weeks of CT consisting of three weekly sessions of ,60 min aerobic training (treadmill at 65–70% of reserve heart rate, 30 min) and resistance training (3 sets of 8–12 repetitions maximum for main muscle groups). Dependent variables were maximum chest press strength; isometric hand-grip strength; 30 s chair stand test; six minute walk test; body mass; body mass index; body adiposity index; waist circumference; systolic (SBP), diastolic and mean blood pressure (MBP); blood glucose; HDL-C; triglycerides; interleukins (IL) 6, 10 and 12, osteoprotegerin (OPG) and serum nitric oxide metabolite (NOx); quality of life (SF-36) and Z-Score of MetS. There was an improvement in muscle strength on chest press (p = 0.009), isometric hand-grip strength (p = 0.03) and 30 s chair stand (p = 0.007). There was a decrease in SBP (p = 0.049), MBP (p = 0.041), Z-Score of MetS (p = 0.046), OPG (0.4260.26 to 0.3860.19 ng/mL, p,0.05) and NOx (13.362.3 mmol/L to 9.162.3 mmol/L; p,0.0005). IL-10 displayed an increase (13.667.5 to 17.2612.3 pg/mL, p,0.05) after 10 weeks of training. Combined training also increased the perception of physical capacity (p = 0.011). This study endorses CT as an efficient tool to improve blood pressure, functional capacity, quality of life and reduce blood markers of inflammation, which has a clinical relevance in the prevention and treatment of MetS

    Resistance exercise leading to failure versus not to failure : effects on cardiovascular control

    Background: The aim of the present study was to evaluate the acute effects of resistance exercise (RE) leading to failure and RE that was not to failure on 24 h blood pressure (BP) and heart rate variability (HRV) in sedentary normotensive adult women. Methods: Ten women (33.2 ± 5.8 years; 159.3 ± 9.4 cm; 58.0 ±6.4 kg; body fat 28.4 ± 2.8%) randomly underwent three experimental sessions: control (40 minutes of seated rest), RE leading to failure with 3 sets of 10 repetitions maximum (10-RM), and RE not to failure at 60% of 10-RM with 3 sets of 10 repetitions. Immediately post session BP and HRV were measured for 24 h. Results: Ratings of perceived exertion and heart rate were higher during the 10-RM session when compared with 60% of 10-RM (6.4 ± 0.5 vs 3.5 ± 0.8 and 123.7 ± 13.9 vs 104.5 ± 7.3 bpm, respectively). The systolic, diastolic and mean BP decreased at 07:00 a.m. after the 10-RM session when compared with the control session (−9.0 ± 7.8 mmHg, -16.0 ± 12.9 mmHg and −14.3 ± 11.2 mmHg, respectively). The root mean square of the squared differences between R-R intervals decreased after both the 60% of 10-RM and 10-RM sessions compared with the control session. Conclusions: An acute RE session leading to failure induced a higher drop of BP upon awakening, while both RE sessions reduced cardiac parasympathetic modulation. RE may be an interesting training strategy to acutely decrease BP in adult women
