    Patient participation in electronic nursing documentation:an interview study among community nurses

    Background Patient participation in nursing documentation has several benefits like including patients' personal wishes in tailor-made care plans and facilitating shared decision-making. However, the rise of electronic health records may not automatically lead to greater patient participation in nursing documentation. This study aims to gain insight into community nurses' experiences regarding patient participation in electronic nursing documentation, and to explore the challenges nurses face and the strategies they use for dealing with challenges regarding patient participation in electronic nursing documentation. Methods A qualitative descriptive design was used, based on the principles of reflexive thematic analysis. Nineteen community nurses working in home care and using electronic health records were recruited using purposive sampling. Interviews guided by an interview guide were conducted face-to-face or by phone in 2019. The interviews were inductively analysed in an iterative process of data collection-data analysis-more data collection until data saturation was achieved. The steps of thematic analysis were followed, namely familiarization with data, generating initial codes, searching for themes, reviewing themes, defining and naming themes, and reporting. Results Community nurses believed patient participation in nursing documentation has to be tailored to each patient. Actual participation depended on the phase of the nursing process that was being documented and was facilitated by patients' trust in the accuracy of the documentation. Nurses came across challenges in three domains: those related to electronic health records (i.e. technical problems), to work (e.g. time pressure) and to the patients (e.g. the medical condition). Because of these challenges, nurses frequently did the documentation outside the patient's home. Nurses still tried to achieve patient participation by verbally discussing patients' views on the nursing care provided and then documenting those views at a later moment. Conclusions Although community nurses consider patient participation in electronic nursing documentation important, they perceive various challenges relating to electronic health records, work and the patients to realize patient participation. In dealing with these challenges, nurses often fall back on verbal communication about the documentation. These insights can help nurses and policy makers improve electronic health records and develop efficient strategies for improving patient participation in electronic nursing documentation

    Direct and Regioselective Di-alpha-fucosylation on the Secondary Rim of beta-Cyclodextrin

    A straightforward glycosylation method is described to regio- and stereoselectively introduce two alpha-L-fucose moieties directly to the secondary rim of beta-cyclodextrin. Using NMR and MS fragmentation studies, the nonasaccharide structure was determined, which was also visualized using molecular dynamics simulations. The reported glycosylation method proved to be robust on gram-scale, and may be generally applied to directly glycosylate beta-cyclodextrins to make well-defined multivalent glycoclusters.</p

    Reviewing ecopsychology research: exploring five databases and considering the future

    This brief article aims to provide an overview of the current dispersal of the term ecopsychology within the academic literature. It summarizes the results found when entering the term ecopsychology into five academic databases. The numbers of hits are discussed and comparisons are made with other related terms. The results are further broken down to provide a snapshot of the type of material referred to in these searches. A more detailed inspection of the material referenced in journal articles is undertaken. Here, popular outlets and dates of publication are highlighted. A final search for articles that report original research data directly associated with ecopsychology is also reported. Possible reasons for the low yield and the implications of this are highlighted along with suggested pathways forward

    Transmission spectrum of Venus as a transiting exoplanet

    On 5-6 June 2012, Venus will be transiting the Sun for the last time before 2117. This event is an unique opportunity to assess the feasibility of the atmospheric characterisation of Earth-size exoplanets near the habitable zone with the transmission spectroscopy technique and provide an invaluable proxy for the atmosphere of such a planet. In this letter, we provide a theoretical transmission spectrum of the atmosphere of Venus that could be tested with spectroscopic observations during the 2012 transit. This is done using radiative transfer across Venus' atmosphere, with inputs from in-situ missions such as Venus Express and theoretical models. The transmission spectrum covers a range of 0.1-5 {\mu}m and probes the limb between 70 and 150 km in altitude. It is dominated in UV by carbon dioxide absorption producing a broad transit signal of ~20 ppm as seen from Earth, and from 0.2 to 2.7 {\mu}m by Mie extinction (~5 ppm at 0.8 {\mu}m) caused by droplets of sulfuric acid composing an upper haze layer above the main deck of clouds. These features are not expected for a terrestrial exoplanet and could help discriminating an Earth-like habitable world from a cytherean planet.Comment: 4 pages, 3 figures, 1 table.     A new class of glycomimetic drugs to prevent free fatty acid-induced endothelial dysfunction

    Background: Carbohydrates play a major role in cell signaling in many biological processes. We have developed a set of glycomimetic drugs that mimic the structure of carbohydrates and represent a novel source of therapeutics for endothelial dysfunction, a key initiating factor in cardiovascular complications. Purpose: Our objective was to determine the protective effects of small molecule glycomimetics against free fatty acid­induced endothelial dysfunction, focusing on nitric oxide (NO) and oxidative stress pathways. Methods: Four glycomimetics were synthesized by the stepwise transformation of 2,5­dihydroxybenzoic acid to a range of 2,5­substituted benzoic acid derivatives, incorporating the key sulfate groups to mimic the interactions of heparan sulfate. Endothelial function was assessed using acetylcholine­induced, endotheliumdependent relaxation in mouse thoracic aortic rings using wire myography. Human umbilical vein endothelial cell (HUVEC) behavior was evaluated in the presence or absence of the free fatty acid, palmitate, with or without glycomimetics (1µM). DAF­2 and H2DCF­DA assays were used to determine nitric oxide (NO) and reactive oxygen species (ROS) production, respectively. Lipid peroxidation colorimetric and antioxidant enzyme activity assays were also carried out. RT­PCR and western blotting were utilized to measure Akt, eNOS, Nrf­2, NQO­1 and HO­1 expression. Results: Ex vivo endothelium­dependent relaxation was significantly improved by the glycomimetics under palmitate­induced oxidative stress. In vitro studies showed that the glycomimetics protected HUVECs against the palmitate­induced oxidative stress and enhanced NO production. We demonstrate that the protective effects of pre­incubation with glycomimetics occurred via upregulation of Akt/eNOS signaling, activation of the Nrf2/ARE pathway, and suppression of ROS­induced lipid peroxidation. Conclusion: We have developed a novel set of small molecule glycomimetics that protect against free fatty acidinduced endothelial dysfunction and thus, represent a new category of therapeutic drugs to target endothelial damage, the first line of defense against cardiovascular disease
