    Study of craniofacial alterations and of the importance of the rapid maxillary expansion after tonsillectomy

    Obstructive hypertrophy of the tonsils and/or adenoids is associated with mouth breathing and can lead to facial imbalances. Adenotonsillectomy is not enough to treat the anatomic changes. Facial orthopedic techniques aid in morphological and functional recovery. This prospective longitudinal clinical study aimed to observe craniofacial changes after adenotonsillectomy and to verify the importance of linking rapid maxillary expansion to treatment. METHOD: Fifty-three children of both genders, aged 6 to 12 years, were allocated to: Group 1, 20 children with nasal breathing; and group 2, 33 children with obstructive hypertrophy of pharyngeal and/or palate undergoing adenotonsillectomy. After surgery, this group was subdivided into Group 2A, 16 patients not treated with rapid maxillary expansion; and Group 2B, 17 patients treated with maxillary rapid expansion. Frontal and lateral cephalometric measurements were made prior to surgery and after 14 months. Statistical analysis used the Kruskal-Wallis and Wilcoxon tests - significance level of 5%. RESULTS: Adenotonsillectomy balanced transversal, sagittal and vertical growth in both groups, and was more effective in the group undergoing combined treatment. CONCLUSIONS: Adenotonsillectomy improved the facial growth of children with obstructive hypertrophy, which was more evident when associated with rapid maxillary expansion.A hipertrofia obstrutiva das tonsilas palatinas e farĂ­ngeas estĂĄ associada Ă  respiração oral e pode levar a desequilĂ­brios faciais. A adenotonsilectomia parece ser insuficiente para o tratamento quando ocorreram alteraçÔes anatĂŽmicas. TĂ©cnicas ortopĂ©dicas faciais auxiliam no restabelecimento morfofuncional. Estudo clĂ­nico longitudinal prospectivo objetivou observar alteraçÔes craniofaciais apĂłs adenotonsilectomia e verificar a importĂąncia de associar ortopedia ao tratamento. MATERIAL E MÉTODO: Cinquenta e trĂȘs crianças de ambos os gĂȘneros, entre 6 e 12 anos, divididas em: Grupo 1, 20 crianças com respiração nasal; Grupo 2, 33 crianças com hipertrofia obstrutiva das tonsilas farĂ­ngeas e/ou palatinas, submetidas Ă  adenotonsilectomia. ApĂłs a cirurgia, este grupo foi subdividido: Grupo 2A, 16 pacientes nĂŁo tratados com expansĂŁo rĂĄpida da maxila; Grupo 2B, 17 pacientes tratados com disjunção maxilar. Foram realizadas medidas cefalomĂ©tricas em norma frontal e lateral anteriores Ă  cirurgia e apĂłs 14 meses. Foram utilizados os testes Kruskal-Wallis e Wilcoxon, com nĂ­vel de significĂąncia de 5%. RESULTADOS: A adenotonsilectomia equilibrou o crescimento transversal, sagital e vertical em ambos os grupos, sendo mais efetiva no grupo submetido ao tratamento combinado.     Quality of life in post-menopausal osteoporosis

    BACKGROUND: To evaluate the impact of osteoporosis on the patients' quality of life, particularly in the absence of fractures. METHODS: 100 post-menopausal women (age 50-85) - 62 with uncomplicated primary osteoporosis and 38 with primary osteoporosis complicated by vertebral fractures; all already treated - were studied using two validated questionnaires: Qualeffo-41 for quality of life in osteoporosis, and Zung for depression. Data were compared to those of 35 controls of comparable age, affected by a different chronic disease (hypothyroidism). RESULTS: Family history of osteoporosis and T-score of spine were similar in the two subgroups of osteoporotic women. Body mass index, age at menopause and education level were similar in the two subgroups of osteoporotic women and in the control group. The patients affected by osteoporosis perceived it as a disease affecting their personal life with undesirable consequences: chronic pain (66% of women with fractures and 40% of women without fractures), impaired physical ability, reduced social activity, poor well-being (21% of women without fractures) and depressed mood (42% of women irrespective of fractures). Overall, 41% of the women showed a reduced quality of life. On the contrary, in the control group only 11% reported a reduced quality of life. CONCLUSION: The quality of life of osteoporotic patients should be investigated even before fractures, in order to develop appropriate counselling, support and care interventions to help patients develop efficient strategies for accepting the disease and coping with it

Aim: To develop an instrument to investigate knowledge and predictive factors of needlestick and sharps injuries (NSIs) in nursing students during clinical placements. Design: Instrument development and cross-sectional study for psychometric testing. Methods: A self-administered instrument including demographic data, injury epidemiology and predictive factors of NSIs was developed between October 2018–January 2019. Content validity was assessed by a panel of experts. The instrument's factor structure and discriminant validity were explored using principal components analysis. The STROBE guidelines were followed. Results: Evidence of content validity was found (S-CVI 0.75; I-CVI 0.50–1.00). A three-factor structure was shown by exploratory factor analysis. Of the 238 participants, 39% had been injured at least once, of which 67.3% in the second year. Higher perceptions of “personal exposure” (4.06, SD 3.78) were reported by third-year students. Higher scores for “perceived benefits” of preventive behaviours (13.6, SD 1.46) were reported by second-year students

    Magnetic Forces And Magnetized Biomaterials Provide Dynamic Flux Information During Bone Regeneration

    The fascinating prospect to direct tissue regeneration by magnetic activation has been recently explored. In this study we investigate the possibility to boost bone regeneration in an experimental defect in rabbit femoral condyle by combining static magnetic fields and magnetic biomaterials. NdFeB permanent magnets are implanted close to biomimetic collagen/hydroxyapatite resorbable scaffolds magnetized according to two different protocols. Permanent magnet only or non-magnetic scaffolds are used as controls. Bone tissue regeneration is evaluated at 12 weeks from surgery from a histological, histomorphometric and biomechanical point of view. The reorganization of the magnetized collagen fibers under the effect of the static magnetic field generated by the permanent magnet produces a highly-peculiar bone pattern, with highly-interconnected trabeculae orthogonally oriented with respect to the magnetic field lines. In contrast, only partial defect healing is achieved within the control groups. We ascribe the peculiar bone regeneration to the transfer of micro-environmental information, mediated by collagen fibrils magnetized by magnetic nanoparticles, under the effect of the static magnetic field. These results open new perspectives on the possibility to improve implant fixation and control the morphology and maturity of regenerated bone providing “in site” forces by synergically combining static magnetic fields and biomaterials

    Avaliação das características do líquido ruminal, hemogasometria, atividade pedométrica e diagnóstico de laminite subclínica em vacas leiteiras

    Este estudo objetivou avaliar as caracterĂ­sticas do lĂ­quido ruminal, hemogasometria, atividade pedomĂ©trica e ocorrĂȘncia laminite subclĂ­nica, por meio da presença de enfermidades podais secundĂĄrias, em vacas leiteiras de alta produção, provenientes de um rebanho comercial. Foram avaliadas 200 vacas holandesas, oriundas da mesma propriedade, localizada na regiĂŁo de Araçatuba, SP, divididas em quatro grupos, sendo estes estabelecidos a partir da produtividade diĂĄria. Inicialmente procedeu-se o exame clĂ­nico dos animais, seguido da colheita de amostras do lĂ­quido ruminal, por meio de sondagem esofĂĄgica, sendo este avaliado quanto ao pH, cor, odor, consistĂȘncia, sedimentação, flutuação e prova de redução pelo azul de metileno. TambĂ©m foram colhidas amostras de sangue venoso para hemogasometria, alĂ©m da coleta dos dados da pedometria (nĂșmero de passos) e produção de leite diĂĄria das vacas. Os dados obtidos foram tabulados e submetidos Ă  anĂĄlise de correlação. Nenhum animal avaliado apresentou alteraçÔes no pH ruminal, bem como nĂŁo foram encontrados distĂșrbios do desequilĂ­brio ĂĄcido bĂĄsico, pois os valores de pH sanguĂ­neo, PCO2, TCO2, HCO3- e EB estavam dentro da normalidade, durante a anĂĄlise hemogasomĂ©trica. A pedometria foi efetiva como mĂ©todo de triagem para as vacas acometidas de afecçÔes podais, pois se observou a redução no nĂșmero de passos devido Ă  dor, correlacionada a menor produção leiteira. Contudo, a identificação destas afecçÔes, somente foi possĂ­vel mediante exame clĂ­nico especĂ­fico dos dĂ­gitos. A ocorrĂȘncia das afecçÔes podais em 49,5% do rebanho deveu-se aos fatores de riscos presentes na propriedade, como o concreto abrasivo e instalaçÔes inadequadas, associados tambĂ©m a possĂ­vel ocorrĂȘncia de acidose ruminal subaguda, nĂŁo diagnosticada pela metodologia utilizada. A correlação entre os valores do pH ruminal, pedometria e hemogasometria se mostrou eficiente para o diagnĂłstico precoce das afecçÔes podais e tambĂ©m no estabelecimento da etiologia destas enfermidades. A laminite subclĂ­nica acometeu primariamente as vacas do rebanho, considerando a etiologia multifatorial desta afecção, ocorrĂȘncia e distribuição das enfermidades podais diagnosticadas
