Seizure in later life: an ode to the elderly
Background: Seizure disorder and epilepsy are one of the common presenting symptoms to an emergency department for geriatric population. Cerebrovascular accident, metabolic derangement, trauma, neurodegenerative diseases, tumor, infections and psychiatric illnesses add up to the frequent etiological spectrum of seizure in elderly. Objective of current study was to describe clinical and etiological spectrum of seizures in later life.Methods: This retrospective study was conducted from medical records of 227 cases of elderly subjects presenting with seizure to emergency department, during a period of 3 years (2010-2013). Results: Most common age group involved was 65-75 years. Mean age of subjects included in the study was 68.13 ± 23.38 years. The most common etiology was cerebrovascular disorders 132 (58.14%), followed by CNS tumor in 16 (07.04%), hypoglycemia in 14 (06.16%), infections 14 (06.16%), substance abuse 12 (05.28%) and trauma 10 (04.40%). The most common presentation was with focal seizures, with 119 (52.42%) subjects whereas 82 (36.12%) presented with generalized tonic clonic seizure, 20 (08.81%) with generalized tonic seizure and 05 (02.20%) with absence seizures.Conclusion: The present study aims at pointing out the different clinical features and etiologies of geriatric seizures, thereby lets the reader have a head start with the management of an elderly patient presenting with seizure.
Imidacloprid poisoning: a modern foe
Imidacloprid is a relatively new insecticide in the chloronicotinyl nitroguanidine class. Imidacloprid has a wide variety of uses; it is used on cotton and vegetable crops, turf grass and ornamental plant products, in indoor and outdoor cockroach control products and in termite control products. Imidacloprid acts as a competitive inhibitor at nicotinic acetylcholine receptors in the nervous system resulting in impairment of normal nerve function. Student Usage of Q&A Forums: Signs of Discomfort?
Q&A forums are widely used in large classes to provide scalable support. In
addition to offering students a space to ask questions, these forums aim to
create a community and promote engagement. Prior literature suggests that the
way students participate in Q&A forums varies and that most students do not
actively post questions or engage in discussions. Students may display
different participation behaviours depending on their comfort levels in the
class. This paper investigates students' use of a Q&A forum in a CS1 course. We
also analyze student opinions about the forum to explain the observed
behaviour, focusing on students' lack of visible participation (lurking,
anonymity, private posting). We analyzed forum data collected in a CS1 course
across two consecutive years and invited students to complete a survey about
perspectives on their forum usage. Despite a small cohort of highly engaged
students, we confirmed that most students do not actively read or post on the
forum. We discuss students' reasons for the low level of engagement and
barriers to participating visibly. Common reasons include fearing a lack of
knowledge and repercussions from being visible to the student community.Comment: To be published at ITiCSE 202

RECUPERACIÓN DEL PATRIMONIO CULTURAL INMATERIAL DE GRAMALOTE A PARTIR DE LA CONSTRUCCIÓN DE RELATOS CON ADOLESCENTES DEL MUNICIPIO
Este artículo tiene como fin dar a conocer como se realizó el proyecto que tenía como objetivo recuperar el patrimonio cultural de Gramalote a partir de los relatos construidos por los adolescentes del Municipio. Se realizó una revisión documental de artículos teóricos con referencias a la comunicación y educación, que llevaron a definir cuál metodología se utilizaría, entre las referencias tenemos a Martín Barbero quien habla del nuevo imaginario y relaciona “Para ser reconocidos necesitamos contar nuestro relato, pues no existe identidad sin narración ya que ésta no es sólo expresiva sino constitutiva de lo que somos”, es así que el proyecto se enmarco una metodología cualitativa con enfoque fenomenológico, para que los adolescentes escribieran relatos y permitieron visibilizar las tradiciones culturales del territorio gramalotero el cual había pasado por la pérdida de su casco urbano a causa del fenómeno natural del 2010 en Colombia

5C le otorgaron a la literatura en que hace presencia el erotismo, a partir de la Práctica Profesional
I y II de Licenciatura en Educación Básica con Énfasis en Humanidades y Lengua Castellana de la
Universidad de Antioquia, llevada a cabo en la Institución Educativa Liceo Caucasia de Caucasia,
Antioquia.
En el marco de este proceso, se desarrolló un encuentro con la literatura que desde el paradigma
de investigación cualitativa con enfoque hermenéutico a partir de la teoría de la recepción de
Jauss y de diferentes propuestas metodológicas como la matriz didáctica, entrevistas, escritos
propuestos en los encuentros con los estudiantes, y en consonancia con el propósito de esta
investigación le apostó a la desinstrumentalización de la literatura desde el presupuesto de una
igualdad existente, al postular un algo común: el erotismo en la literatura como un contenido
formativo en sentido político. Y al realizar este ofrecimiento a los estudiantes y analizar sus
significaciones, se podría afirmar que este trabajo alcanzó gran parte de los objetivos trazados,
pues posibilitó otra lectura, otra mirada y otro acercamiento a la enseñanza de la literatura puesto
que se ha enseñado en algunos momentos como un medio para alcanzar un fin específico y no
como un fin en sí mismo, el cual posibilita desanclar destinos fijados, como una lectura de sí, como
transgresión social, como un acto erótico por excelencia, pero aún más importante como aquella
que en tanto actividad política dio voces a seres que no habían sido escuchados y que lograron así
alzar sus voces para ser reconocidos y de esta manera, expresar sus propias visiones y formas de
ver el mundo
Do shorter lengths of stay increase readmissions after total joint replacements?
BackgroundEnhanced recovery after surgery protocols for total joint replacements (TJRs) emphasize early discharge, yet the impact on readmissions is not well documented. We evaluate the impact of a one-day length of stay (LOS) discharge protocol on readmissions.MethodsWe conducted a retrospective review of all primary TJRs (hip and knee) from April 2014 to March 2015. Patients who had adequate support to be discharged home were categorized into 2 groups, 1-day (n = 174) vs 2-day (n = 285) LOS groups. Patients discharged to rehabilitation were excluded (n = 196).ResultsPatients in the 1 day group were more likely to be younger (61.7 vs 64.8 years, P < .001), be male (56.3% vs 40.4%, P = .001), and have a lower body mass index (30.0 vs 31.4 kg/m2, P = .012). One-day LOS patients had shorter surgical times (79.7 vs 85.6 minutes, P = .001) and more likely had spinal anesthesia (46.0% vs 31.2%, P = .001). The overall 30-day all-cause (2.3% vs 2.5%, P = .591) and 90-day wound-related (1.1% vs 1.1%, P = .617) readmission rates were equivalent between groups.ConclusionsEarly discharge does not increase readmissions and may help attenuate costs associated with TJRs. Further refinement of protocols may allow for more patients to be safely discharged on postoperative day 1
Evaluation of the use of blood in surgeries as a tool to change patterns for requesting blood product reserves
OBJECTIVES: Thirty to sixty percent of prepared blood products are not transfused. Blood reserves for surgeries lead to many unused blood products, which increases hospital costs. The aim of this study is to identify the request and use profiles of blood products for elective surgeries in different surgical specialties, the influence of surgery time and demographic, clinical, and laboratory variables on the number of red blood cells (RBCs) used and to calculate the rate of transfused patients (RTP) and cross-matched and transfused (C/T) RBCs. METHODS: Observational and prospective studies. Sociodemographic, clinical and quantitative data on the request and use of blood products were collected. The influence of the data on the use of RBCs was examined by binary logistic regression. Chi-square, one-way ANOVA and Kruskal-Wallis tests were utilized to compare the data among the specialties. RESULTS: In total, 822 procedures were included. Most of the requested blood products were not used, even 24 hours postoperatively. Of the 2,483 RBC units, 314 were transfused, leaving 87.6% unused; however, cardiac, digestive tract, vascular, gynecologic, urologic and thoracic surgery procedures transfused 50%, 25%, 16.5%, 11%, 9.5% and 8.1% of requested RBCs, respectively. The factors that influenced the transfusions were age, time of surgery and cardiac surgeries. The RTP was 410% in 22 surgical types and o1% in 24 surgical types, and 88% of samples presented a C/T ratio 42.5. CONCLUSION: The RTP and C/T ratios can guide RBC requests in the preoperative period. Knowing the standard of use of blood products and developing protocols enables the optimization of reserves, reduction of costs and improvement of care
Modeling Women's Elective Choices in Computing
Evidence-based strategies suggest ways to reduce the gender gap in computing. For example, elective classes are valuable in enabling students to choose in which directions to expand their computing knowledge in areas aligned with their interests. The availability of electives of interest may also make computing programs of study more meaningful to women. However, research on which elective computing topics are more appealing to women is often class or institution specific. In this study, we investigate differences in enrollment within undergraduate-level elective classes in computing to study differences between women and men. The study combined data from nine institutions from both Western Europe and North America and included 272 different classes with 49,710 student enrollments. These classes were encoded using ACM curriculum guidelines and combined with the enrollment data to build a hierarchical statistical model of factors affecting student choice. Our model shows which elective topics are less popular with all students (including fundamentals of programming languages and parallel and distributed computing), and which elective topics are more popular with women students (including mathematical and statistical foundations, human computer interaction and society, ethics, and professionalism). Understanding which classes appeal to different students can help departments gain insight of student choices and develop programs accordingly. Additionally, these choices can also help departments explore whether some students are less likely to choose certain classes than others, indicating potential barriers to participation in computing
