    Histological evidence of successful internal fixation for traumatic chondral fracture of the femoral groove

    AbstractIsolated chondral fractures without cancellous bone have limited healing potential. We reattached a chondral fragment without cancellous bone using bone pegs for chondral fracture of the femoral groove in a 13-year-old boy. Five months after the surgery, we performed a biopsy to evaluate the histological findings for the healed osteochondral junction. Biopsy results showed that osteoblasts had proliferated in the subchondral layer, and some osteoblasts had invaded the calcified cartilage, suggesting active new bone formation. A tidemark was observed between the radial cartilage layer and the calcified cartilage, and it was evident that the osteochondral junction had been completely restored. Four years and 3 months after the surgery, magnetic resonance imaging revealed that the cartilage on the femoral groove was maintained and the patient can play basketball asymptomatically. This report shows that surgeons can attempt the reattachment of chondral fragments without cancellous bone in adolescent patients

    Differential responses of normal human coronary artery endothelial cells against multiple cytokines comparatively assessed by gene expression profiles

    AbstractEndothelial cells play an important role in terms of biological functions by responding to a variety of stimuli in the blood. However, little is known about the molecular mechanism involved in rendering the variety in the cellular response. To investigate the variety of the cellular responses against exogenous stimuli at the gene expression level, we attempted to describe the cellular responses with comprehensive gene expression profiles, dissect them into multiple response patterns, and characterize the response patterns according to the information accumulated so far on the genes included in the patterns. We comparatively analyzed in parallel the gene expression profiles obtained with DNA microarrays from normal human coronary artery endothelial cells (HCAECs) stimulated with multiple cytokines, interleukin-1β, tumor necrosis factor-α, interferon-β, interferon-γ, and oncostatin M, which are profoundly involved in various functional responses of endothelial cells. These analyses revealed that the cellular responses of HCAECs against these cytokines included at least 15 response patterns specific to a single cytokine or common to multiple cytokines. Moreover, we statistically extracted genes contained within the individual response patterns and characterized the response patterns with the genes referring to the previously accumulated findings including the biological process defined by the Gene Ontology Consortium (GO). Out of the 15 response patterns in which at least one gene was successfully extracted through the statistical approach, 11 response patterns were differentially characterized by representing the number of genes contained in individual criteria of the biological process in the GO only. The approach to dissect cellular responses into response patterns and to characterize the pattern at the gene expression level may contribute to the gaining of insight for untangling the diversity of cellular functions

    Gastrointestinal Endoscopy for Patients with High Levels of Serum CEA and CA19-9

    Serum levels of tumor markers, such as carcinoembryonic antigen (CEA) and carbohydrate antigen 19-9 (CA19-9), are often measured to detect potential malignancy. When these levels are high, the presence or absence of malignancy is confirmed via a more detailed examination using gastrointestinal (GI) endoscopy and computed tomography. The rate of confirmation of malignancy upon such a follow-up is unknown. This study aimed to investigate the malignancy detection rate via GI endoscopy for patients with high levels of serum CEA and CA19-9. All patients who underwent such GI endoscopy between January 2018 and February 2019 at Showa University Hospital were included in this study. The patients were divided into a follow-up group and a screening group, depending on the purpose of measuring their serum CEA/CA19-9 levels. There were 156 patients who underwent GI endoscopy because of high CEA/CA19-9 levels within the study period. Advanced malignant lesions were detected in 10 patients (6.4%), including seven cases of colorectal cancer and three cases of upper GI malignancies. In the screening group, six cases (5.7%) of GI malignancies were detected, none of which were found in asymptomatic patients without anemia. In the follow-up group, four cases (7.8%) of GI malignancies were detected; three patients were asymptomatic, and one patient had anemia. Our findings suggest that high serum CEA/CA19-9 levels in asymptomatic patients without anemia and without a history of malignancy do not indicate the presence of malignancy. However, high serum CEA/CA19-9 levels may indicate the potential presence of GI malignancies for patients with a history of malignant tumors, even if they are asymptomatic and do not have anemia

    精神科においては精神障害者の生活の質の向上のために退院促進が求められている.そのためにはチームアプローチが不可欠であり,それを実践するためには,各専門職者の持っている能力を統合した活動を展開しなければならない.学際的連携チームケアモデルInterdisciplinary collaborative teamcare model (ICTCM)とは,チームケアを実践するための具体的な方法(教育,理論,実践,研究)を示すものである.そこで,ICTCMを使用して,チームケアサービスの質の改善活動の一環として,看護サービスをレベルアップすることを目的とした多職種参加による看護診断学習会を行ってきた.その結果,チームケア運営体制の整備,職種間の役割理解及び職種間で積極的に情報を共有しようとする姿勢が改善したと考えられた.This article describes workshop on nursing diagnosis that aimed improving an interdisciplinary collaboration in the psychiatric hospital. Psychiatric hospitals are required the promotion of discharge a patients with chronic mental disorders in order to enhance the quality of life of the people with mental disorders. For that purpose, team approach is absolutely imperative. In order to practice team approach, we have to unify the competency of each professional. Interdisciplinary Collaborative Team Care Model (ICTCM)shows the concrete method(education, theory, practice, research)for practicing team care. Our hospitals’ staffs are working based on this model in order to assure continuous improvement of providing efficient and high-quality of team care services. Nursing diagnosis workshop based on this model has been performed which aims to improve nursing services involve the participation of various health care professional. The overall conclusion of the report was that the management system of the team care is well-organized, also understanding of the role of other professions, the active and positive attitude for sharing information between inter-health care providers have improved

    Relation among the knee, sagittal spinal alignment, and the spinal range of motion: Investigation in local medical check-ups using the SpinalMouse

    Background: This study was designed as an investigation in a local population to assess the relation between the knee joint and spinal alignment in a population-based study using the SpinalMouse. Methods: Medical check-ups were conducted for residents of a mountain village in Japan. The study population included 107 men and 157 women (528 knees) with a mean age of 71.1 ± 6.8 years (range, 60–87 years). A questionnaire dealing with any current symptoms involving the knees was administered, and physical examinations dealing with the range of motion (ROM) of knee were conducted. The SpinalMouse was used to measure sagittal spinal alignment and spinal ROM. The parameters considered were thoracic kyphosis angle, lumbar lordosis angle (LLA), sacral inclination angle (SIA), and trunk angle of inclination (INC). The patients were divided into a group with knee flexion contracture (FC group) and a group without knee FC (non-FC group) to conduct a comparative study of both groups. Results: With regard to static spinal alignment, LLA and SIA decreased significantly in the FC group (p < 0.05). With regard to spinal ROM, LLA and INC decreased significantly in the FC group (p < 0.05). Conclusion: Results suggested that the knee and the spine affect each other and that the spinal ROM is also involved. The current study may explain the development of knee-spine syndrome

    Risk Factors for the Development of Osteoarthritis after Anterior Cruciate Ligament Reconstruction

    Background: Risk factors have been reported for osteoarthritis (OA) after anterior cruciate ligament (ACL) reconstruction, but previous studies did not include a control group, and may have included OA that would have naturally developed even in the absence of ACL injury. This study investigated risk factors compared with the patient’s own contralateral knee. M ethods:Three hundred forty-nine patients who had undergone ACL reconstruction at least 15 years previously were invited to visit the hospital. After exclusion criteria were applied, 40 patients were included in the study, including 16 with progressive OA and 24 without OA. Progressive OA was defined as OA that was more advanced on the affected side than on the contralateral side. The variables evaluated included age at the time of surgery,time from injury to surgery, sex, graft material, cartilage damage, and meniscectomy. Results:No significant differences between groups were seen in terms of age,time from injury to surgery,sex,graft material, or cartilage damage. Meniscectomy was ultimately performed significantly more frequently in the OA group (88%) than in the non-OA group (38%;p<0.01). Conclusions:Meniscectomy was found to constitute a risk factor for the progression of OA after ACL reconstruction
