    Risk factors and outcome of COVID-19 in patients with hematological malignancies

    Background: Prognostic factors of poor outcome in patients with hematological malignancies and COVID-19 are poorly defned. Patients and methods: This was a Spanish transplant group and cell therapy (GETH) multicenter retrospective observational study, which included a large cohort of blood cancer patients with laboratory-confrmed SARS-CoV-2 infection through PCR assays from March 1st 2020 to May 15th 2020. Results: We included 367 pediatric and adult patients with hematological malignancies, including recipients of autologous (ASCT) (n=58) or allogeneic stem cell transplantation (allo-SCT) (n=65) from 41 hospitals in Spain. Median age of patients was 64 years (range 1-93.8). Recipients of ASCT and allo-SCT showed lower mortality rates (17% and 18%, respectively) compared to non-SCT patients (31%) (p=0.02). Prognostic factors identifed for day 45 overall mortality (OM) by logistic regression multivariate analysis included age>70 years [odds ratio (OR) 2.1, 95% con‑ fdence interval (CI) 1.2-3.8, p=0.011]; uncontrolled hematological malignancy (OR 2.9, 95% CI 1.6-5.2, p20 mg/dL (OR 3.3, 95% CI 1.7-6.4, p<0.0001). In multivariate analysis of 216 patients with very severe COVID-19, treatment with azithromycin or low dose corticosteroids was associated with lower OM (OR 0.42, 95% CI 0.2-0.89 and OR 0.31, 95% CI 0.11-0.87, respectively, p=0.02) whereas the use of hidroxycloroquine did not show signifcant improvement in OM (OR 0.64, 95% CI 0.37-1.1, P=0.1). Conclusions: In most patients with hematological malignancies COVID-19 mortality was directly driven by older age, disease status, performance status, as well as by immune (neutropenia) parameters and level of infammation (high CRP). Use of azithromycin and low dose corticosteroids may be of value in very severe COVID-19

    Role of age and comorbidities in mortality of patients with infective endocarditis

    [Purpose]: The aim of this study was to analyse the characteristics of patients with IE in three groups of age and to assess the ability of age and the Charlson Comorbidity Index (CCI) to predict mortality. [Methods]: Prospective cohort study of all patients with IE included in the GAMES Spanish database between 2008 and 2015.Patients were stratified into three age groups:<65 years,65 to 80 years,and ≥ 80 years.The area under the receiver-operating characteristic (AUROC) curve was calculated to quantify the diagnostic accuracy of the CCI to predict mortality risk. [Results]: A total of 3120 patients with IE (1327 < 65 years;1291 65-80 years;502 ≥ 80 years) were enrolled.Fever and heart failure were the most common presentations of IE, with no differences among age groups.Patients ≥80 years who underwent surgery were significantly lower compared with other age groups (14.3%,65 years; 20.5%,65-79 years; 31.3%,≥80 years). In-hospital mortality was lower in the <65-year group (20.3%,<65 years;30.1%,65-79 years;34.7%,≥80 years;p < 0.001) as well as 1-year mortality (3.2%, <65 years; 5.5%, 65-80 years;7.6%,≥80 years; p = 0.003).Independent predictors of mortality were age ≥ 80 years (hazard ratio [HR]:2.78;95% confidence interval [CI]:2.32–3.34), CCI ≥ 3 (HR:1.62; 95% CI:1.39–1.88),and non-performed surgery (HR:1.64;95% CI:11.16–1.58).When the three age groups were compared,the AUROC curve for CCI was significantly larger for patients aged <65 years(p < 0.001) for both in-hospital and 1-year mortality. [Conclusion]: There were no differences in the clinical presentation of IE between the groups. Age ≥ 80 years, high comorbidity (measured by CCI),and non-performance of surgery were independent predictors of mortality in patients with IE.CCI could help to identify those patients with IE and surgical indication who present a lower risk of in-hospital and 1-year mortality after surgery, especially in the <65-year group

    In Mexico, the National Forestry Commission implements the payment for hydrological environmental services, whose objective is to reduce the rate of deforestation in critical areas for water recharging. Although the approved projects are followed up, there is no analysis of the benefits generated by this mechanism. Generating information for these evaluations plays a key role. The main objective of this research was to perform an ecological analysis of payment for hydrological environmental services beneficiary area in the common land La Ciudad, seven years after the allocation of support. The forest cover of the area, the state of the vegetation, the diversity of species and the quality of the water were analyzed. The results showed that the area has a considerable percentage of forest cover and acceptable biodiversity. In the analysis of water quality, contamination can occur, which is why more frequent monitoring is recommended.En México la Comisión Nacional Forestal implementa el pago por servicios ambientales hidrológicos, cuyo objetivo es reducir la tasa de deforestación en zonas críticas para recarga de agua. Aunque se da seguimiento a los proyectos aprobados, no se tiene un análisis de los beneficios que genera este mecanismo. Generar información para evaluar estos beneficios juega un papel clave. El objetivo principal de esta investigación fue realizar un análisis ecoló- gico de un área beneficiaria de pago por servicios ambientales hidrológicos del ejido La Ciudad, a siete años de la asignación del apoyo. Se analizó la cobertura forestal del área, el estado de la vegetación, la diversidad de especies y la calidad del agua. Los resultados mostraron que el área cuenta con un porcentaje considerable de cobertura forestal y niveles aceptables de biodiversidad. En análisis de calidad del agua mostró una posible contaminación por lo que se recomienda un monitoreo más recurrente

    Two-spotted spider mite (Tetranychus urticae) is one of the most problematic species of mites for agriculture around the world. Its control is mainly based on synthetic pesticides which may gradually reduce their efficacy due to development of pesticide resistance. In this research, samples of two-spotted spider mite were collected in vegetable commercial greenhouses of the state of Guanajuato, Mexico in order to evaluate the degree of two-spotted mite tolerance to synthetic acaricides belonging to different toxicologic groups (Avermectin, Dicofol, Fenbutatin-Oxide and Naled). The mite samples were taken to the Acarology Lab at the Universidad Antonio Narro. Mite samples were bioassayed to determine their degree of tolerance to each acaricide. The LC50 detected for Avermectin, Dicofol, Fenbutatin-Oxide and Naled were 1.8, 1009.5, 52.9 and 1014.7 ppm, respectively. Our results suggest that T. urticae has developed some tolerance against two of the four evaluated acaricides (Naled and Dicofol).La araña de dos manchas (Tetranychus urticae) está catalogada como una de las especies que le ocasiona más problemas a la agricultura en todo el mundo. Su control se basa principalmente en agroquímicos que con el paso del tiempo resultan menos efectivos debido al surgimiento de resistencia. En la presente investigación se recolectaron muestras de T. urticae en invernaderos comerciales de hortalizas del Estado de Guanajuato, México con el objetivo de evaluar su grado de tolerancia a acaricidas de diferente grupo toxicológico (Avermectina, Dicofol, Óxido de Fenbutatin y Naled). Las muestras de ácaros se trasladaron al Laboratorio de Acarología de la Universidad Autónoma Agraria Antonio Narro, donde se sometieron a bioensayos para determinar el grado de tolerancia a los acaricidas mencionados. La CL50 obtenida para Avermectina, Dicofol, Óxido de Fenbutatin y Naled fue de 1.8, 1109.5, 52.9 y 1014.7 ppm, respectivamente. Los resultados sugieren que T. urticae ha desarrollado cierto grado de tolerancia a dos de los cuatroacaricidas evaluados (Naled y Dicofol)

    Acanthamoeba castellani and Acanthamoeba polyphaga are free-living amebae that cause keratitis and granulomatous encephalitis in humans. We have analyzed the early morphological and electrophysiological changes occurring during the in vitro interaction of cultured amebae with intact or physically damaged corneas obtained from hamsters. Both species of Acanthamoeba produced similar cytopathic changes, as seen by light microscopy and scanning electron microscopy. After adhesion to the epithelial surface, trophozoites formed clumps and migrated toward the cell borders, causing the separation of adjacent cells at 1 h of coculture. At later stages (2 to 4 h), some amebae were found under desquamating epithelial cells whereas others were seen associated with damaged cells or forming amebostome-like structures to ingest detached epithelial cells. Control corneas incubated in culture medium conditioned with amebae showed a cytoplasmic vacuolization and blurring of the epithelial-stromal junction. The early stages of corneal epithelial damage caused by amebae were also analyzed by measuring the transepithelial resistance changes in corneas mounted in Ussing chambers. Both species of Acanthamoeba caused a rapid decrease in electrical resistance. The present observations demonstrate that under in vitro conditions, Acanthamoeba trophozoites rapidly cause significant damage to the corneal epithelium. Furthermore, in our experimental model, previous physical damage to the corneas was not a prerequisite for the development of amebic corneal ulcerations

    Down syndrome as risk factor for respiratory syncytial virus hospitalization : A prospective multicenter epidemiological study

    Respiratory syncytial virus (RSV) infection in childhood, particularly in premature infants, is associated with significant morbidity and mortality. To compare the hospitalization rates due to RSV infection and severity of disease between infants with and without Down syndrome (DS) born at term and without other associated risk factors for severe RSV infection. In a prospective multicentre epidemiological study, 93 infants were included in the DS cohort and 68 matched by sex and data of birth (±1 week) and were followed up to 1 year of age and during a complete RSV season. The hospitalization rate for all acute respiratory infection was significantly higher in the DS cohort than in the non-DS cohort (44.1% vs 7.7%, P<.0001). Hospitalizations due to RSV were significantly more frequent in the DH cohort than in the non-DS cohort (9.7% vs 1.5%, P=.03). RSV prophylaxis was recorded in 33 (35.5%) infants with DS. The rate of hospitalization according to presence or absence of RSV immunoprophylaxis was 3.0% vs 15%, respectively. Infants with DS showed a higher rate of hospitalization due to acute lower respiratory tract infection and RSV infection compared to non-DS infants. Including DS infants in recommendations for immunoprophylaxis of RSV disease should be considered
