Multi-objective study on an innovative system for domestic hot water production: A pilot building in Southern Europe
The present study deals with a multi-objective analysis of an innovative decentralised system to produce and store domestic hot water (DHW), emphasising on the combined effects of the technological aspect, control strategy and user's behaviour. The proposed system, by relying on thermal energy storage, decouples energy production and demand while shaves peaks in the energy demand and, at the same time, provides more autonomy to users through local storages. To identify subtle interactions in components of DHW system, dynamic simulations are carried out by establishing a coupled TRNSYS-MATLAB code, calibrated and validated by experimental measurements. The energy analysis implies that the proposed system cuts the required annual electrical energy in half, of which up to 82% of needed primary energy is supplied from renewable sources, compared to previous electrical-decentralised system. The optimisation of the results through applying control strategies indicates that adopting a three-time charging scheme is advantageous in terms of providing a more stable temperature profile as well as a higher hot water temperature. Compared to an available-by-demand operation, this scheme reduces the required total annual electricity by 5.2 % and enhances total thermal loss from components up to 4.0%. Furthermore, a sensitivity analysis on the results emphasises the striking role of the user behaviour in electrical energy consumption either via draw-off temperature or adjusting the pre-defined temperature for activation of the built-in auxiliary heater

Comparison between measurement techniques to estimate flanking sound transmission
The flanking sound reduction index can be estimated using the intensity technique according to the method described in the standard EN ISO 15186-2 Annex C, this procedure can give us the contribution of each path and of each flanking wall.
The EN ISO 10848-1 gives us the guidelines to measure with the vibration velocity technique. This paper reports the results of the comparison between the Intensity measurement technique and the Vibration velocity based method, to estimate the flanking sound transmission.
The measurements take place on a laboratory with particular conditions (two suppressed junctions) and in field conditions
Air Quality and Comfort Characterisation within an Electric Vehicle Cabin in Heating and Cooling Operationsâ€
This work is aimed at the experimental characterisation of air quality and thermal profile within an electric vehicle cabin, measuring at the same time the HVAC system energy consumption. Pollutant concentrations in the vehicle cabin are measured by means of a low-cost system of sensors. The effects of the HVAC system configuration, such as fresh-air and recirculation mode, on cabin air quality, are discussed. It is shown that the PM concentrations observed in recirculation mode are lower than those in fresh-air mode, while VOC concentrations are generally higher in recirculation than in fresh-air mode. The energy consumption is compared in different configurations of the HVAC system. The novelty of this work is the combined measurement of important comfort parameters such as air temperature distribution and air quality within the vehicle, together with the real time energy consumption of the HVAC system. A wider concept of comfort is enabled, based on the use of low-cost sensors in the automotive field
Numerical Analysis on the Optimisation of Thermal Comfort Levels in an Office Located inside a Historical Building
The present study examines the possibility of thermal comfort optimisation inside an office room where, due to historical heritage, it is possible to modify neither the energetic characteristic of the envelope nor the position of the inlet air vents. The distribution of global and local thermal comfort indices is evaluated in both heating and cooling conditions by establishing a computational fluid dynamics (CFD) model validated against experimental data. The obtained results demonstrate a striking asymmetry of the air velocity and temperature distribution due to the low energy efficiency of the building. In heating mode, the predicted mean vote (PMV) values were improved if the discharged air from the fan coil was at its maximal velocity. However, at the same time, the vertical
air temperature gradient increased by around 0.5 C in each working station. In the cooling condition, in the absence of the solar radiation, the minimal air-flow rate satisfied the acceptable range of the draught rate (DR), whereas in the presence of a solar load, it could not meet the required cooling load in all positions, leading to higher floor temperature. The findings of this study allow for identifying and rearranging the optimal position of working stations in terms of thermal comfort
Virus-like Particle (VLP) Vaccines for Cancer Immunotherapy
Cancer vaccines are increasingly being studied as a possible strategy to prevent and treat cancers. While several prophylactic vaccines for virus-caused cancers are approved and efficiently used worldwide, the development of therapeutic cancer vaccines needs to be further implemented. Virus-like particles (VLPs) are self-assembled protein structures that mimic native viruses or bacteriophages but lack the replicative material. VLP platforms are designed to display single or multiple antigens with a high-density pattern, which can trigger both cellular and humoral responses. The aim of this review is to provide a comprehensive overview of preventive VLP-based vaccines currently approved worldwide against HBV and HPV infections or under evaluation to prevent virus-caused cancers. Furthermore, preclinical and early clinical data on prophylactic and therapeutic VLP-based cancer vaccines were summarized with a focus on HER-2-positive breast cancer
Prognostic value of high-sensitivity cardiac troponin I early after coronary artery bypass graft surgery
Background: The diagnosis of periprocedural myocardial infarction (PMI) after coronary artery bypass graft (CABG) is based on biochemical markers along with clinical and instrumental findings. However, there is not a clear cutoff value of high-sensitivity cardiac troponin (hs-cTn) to identify PMI. We hypothesized that isolated hs-cTn concentrations in the first 24 h following CABG could predict cardiac adverse events (in-hospital death and PMI) and/or left ventricular ejection fraction (LVEF) decrease. Methods: We retrospectively enrolled all consecutive adult patients undergoing CABG, alone or in association with other cardiac surgery procedures, over 1 year. Hs-cTn I concentrations (Access, Beckman Coulter) were serially measured in the post-operative period and analyzed according to post-operative outcomes. Results: 300 patients were enrolled; 71.3% underwent CABG alone, 33.7% for acute coronary syndrome. Most patients showed hs-cTn I values superior to the limit required by the latest guidelines for the diagnosis of PMI. Five patients (1.7%) died, 8% developed a PMI, 10.6% showed a LVEF decrease ≥ 10%. Hs-cTn I concentrations did not significantly differ with respect to death and/or PMI whereas they were associated with LVEF decrease ≥ 10% (p value < 0.005 at any time interval), in particular hs-cTn I values at 9–12 h post-operatively. A hs-cTn I cutoff of 5556 ng/L, a value 281 (for males) and 479 (for females) times higher than the URL, at 9–12 h post-operatively was identified, representing the best balance between sensitivity (55%) and specificity (79%) in predicting LVEF decrease ≥ 10%. Conclusions: Hs-cTn I at 9–12 h post-CABG may be useful to early identify patients at risk for LVEF decrease and to guide early investigation and management of possible post-operative complications
Persistence of antibody responses to the sars-cov-2 in dialysis patients and renal transplant recipients recovered from covid-19
Nephropathic subjects with impaired immune responses show dramatically high infection rates of coronavirus disease 2019 (COVID-19). This work evaluated the ability to acquire and maintain protective antibodies over time in 26 hemodialysis patients and 21 kidney transplant recipients. The subjects were followed-up through quantitative determination of circulating SARS-CoV-2 S1/S2 IgG and neutralizing antibodies in the 6-month period after clinical and laboratory recovery. A group of 143 healthcare workers with no underlying chronic pathologies or renal diseases recovered from COVID was also evaluated. In both dialysis and transplanted patients, antibody titers reached a zenith around the 3rd month, and then a decline occurred on average between the 270th and 300th day. Immunocompromised patients who lost antibodies around the 6th month were more common than non-renal subjects, although the difference was not significant (38.5% vs. 26.6%). Considering the decay of antibody levels below the positivity threshold (15 AU/mL) as “failure”, a progressive loss of immunisation was found in the overall population starting 6 months after recovery. A longer overall antibody persistence was observed in severe forms of COVID-19 (p = 0.0183), but within each group, given the small number of patients, the difference was not significant (dialysis: p = 0.0702; transplant: p = 0.1899). These data suggest that immunocompromised renal patients recovered from COVID-19 have weakened and heterogeneous humoral responses that tend to decay over time. Despite interindividual variability, an association emerged between antibody persistence and clinical severity, similar to the subjects with preserved immune function
The Anti-Inflammatory Reliever (AIR) Algorithm Study: a protocol for a single-group study of an AIR stepwise approach to the treatment of adult asthma
Background The stepwise approach to long-term asthma management, which traditionally incorporates short-acting β2-agonist reliever therapy, has been a core feature of asthma guidelines for over 30 years. There have been no studies, however, directly investigating the use of an entire guideline-recommended track. Recently, inhaled corticosteroid–formoterol has been recommended as the preferred reliever therapy in adult asthma, in accordance with a stepwise “Anti-Inflammatory Reliever” (AIR) treatment track. Objective The aim of this study was to evaluate the AIR stepwise approach recommended by the New Zealand adolescent and adult asthma guidelines, in combination with a novel algorithm for transitioning between treatment steps. Methods This 52-week, open-label, single-group study will recruit 100 adults aged 18 to 75 years with mild, moderate and moderate–severe asthma (ACTRN12620001010987). Participants will be allocated to budesonide–formoterol 200/6 µg, one actuation as needed (Step 1), one actuation twice daily and as needed (Step 2), or two actuations twice daily and one as needed (Step 3). Treatment steps will be adjusted throughout the study, in response to reliever use and asthma attacks, according to a stepwise AIR algorithm. Following a 26-week period of investigator-led transitions, participants will adjust their own treatment step. The primary outcome is participant satisfaction as measured by the Global Satisfaction score of the Treatment Satisfaction Questionnaire for Medication. Secondary outcomes will assess efficacy and safety, and describe patterns of medication use and participant flow through the treatment steps. Conclusion This is the first trial to assess the AIR treatment track and algorithm. The results will provide knowledge to guide the clinical use of this approach
