    The status of school sanitation facilities in some selected primary and secondary schools in Dessie City Administration, South Wello Zone, Amhara Region

    Background: There is a growing demand on school hygiene and sanitation facilities given the growing number of school enrolment in Ethiopia. A safe school environment plays a key role in facilitating education and enduring pupils with improved life skills. Although there is much attention given for the expansion of schools to reach all school aged children in Ethiopia, the concern on focusing school hygiene and sanitation remains to be a challenge.Objectives of the study: A assessing the status of water and sanitary facilities in selected schools of Dessie Town with the intention of obtaining exploring baseline to serve for implementation of an Urban Health Extension Program in the region.Methods: This study was done in May 2010 in Dessie City Administration. Questionnaire-based interviews with school principals and record reviews supplemented by physical observation were used to collect the necessary data. Descriptive statistics was used to present the findings.Results: The overall latrine to school population ratio was (1:64), which was about twice less than the recommended one. The ratio for female students was significantly lower than that for males. The access to drinking water facilities (water taps) and hand washing facilities were very much limited to the extent not conforming to the standard. The presence of school-based water, sanitation, and hygiene committee and clubs was unheard.Conclusion: The inadequate sanitation facility in schools poses a health risk that would impact students’ potential to attend education. Hence, a coordinated effort among all concerned school to safeguard the school environment should be considered as one part of the path for reaching the MDG goals with respect to at least sanitation

    Aerosol chemical composition in Asian continental outflow during the TRACE-P campaign: Comparison with PEM-West B

    Aerosol associated soluble ions and the radionuclide tracers 7Be and 210Pb were quantified in 414 filter samples collected in spring 2001 from the DC-8 during the Transport and Chemical Evolution over the Pacific (TRACE-P) campaign. Binning the data into near Asia (flights from Hong Kong and Japan) and remote Pacific (all other flights) revealed large enhancements of NO3−, SO4=, C2O4=, NH4+, K+, Mg2+, and Ca2+ near Asia. The boundary layer and lower troposphere were most strongly influenced by continental outflow, and the largest enhancements were seen in Ca2+ (a dust tracer) and NO3− (reflecting uptake of HNO3 onto the dust). Comparing the TRACE P near Asia bin with earlier results from the same region during PEM-West B (in 1994) shows at least twofold enhancements during TRACE P in most of the ions listed above. Calcium and NO3− were most enhanced in this comparison as well (more than sevenfold higher in the boundary layer and threefold higher in the lower troposphere). Independent estimation of Asian emissions of gaseous precursors of the aerosol-associated ions suggest only small changes between the two missions, and precipitation fields do not suggest any significant difference in the efficiency of the primary sink, precipitation scavenging. It thus appears that with the possible exception of dust, the enhancements of aerosol-associated species during TRACE P cannot be explained by stronger sources or weaker sinks. We argue that the enhancements largely reflect the fact that TRACE P focused on characterizing Asian outflow, and thus the DC-8 was more frequently flown into regions that were influenced by well-organized flow off the continent

    Improving irrigation efficiency will be insufficient to meet future water demand in the Nile Basin

    The Nile River Basin covers an area of approximately 3.2 million km2 and is shared by 11 countries. Rapid population growth is expected in the region. The irrigation requirements of Nile riparian countries of existing (6.4 million ha) and additional planned (3.8 million ha, 2050) irrigation schemes were calculated, and the likely water savings through improved irrigation efficiency were evaluated. We applied SPARE:WATER to calculate irrigation demands on the basis of the well-known FAO56 Crop Irrigation Guidelines. Egypt (67 km3 yr-1) and Sudan (19 km3 yr-1) consume the highest share of the 84 km3 yr-1 total (2011). Assuming today’s poor irrigation infrastructure, the total consumption was predicted to increase to 123 km3 yr-1 (2050), an amount far exceeding the total annual yield of the Nile Basin. Therefore, a key challenge for water resources management in the Nile Basin is balancing the increasing irrigation water demand basin-wide with the available water supply. We found that water savings from improved irrigation technology will not be able to meet the additional needs of planned areas. Under a theoretical scenario of maximum possible efficiency, the deficit would still be 5 km3 yr-1. For more likely efficiency improvement scenarios, the deficit ranged between 23 and 29 km3 yr-1. Our results suggest that that improving irrigation efficiency may substantially contribute to decreasing water stress on the Nile system but would not completely meet the demand.     The phenomenon of calcium loss during bed rest was found to be analogous to the loss of bone material which occurs in the hypogravic environment of space flight. Ways of preventing this occurrence are investigated. A group of healthy adult males underwent 24-30 weeks of continuous bed rest. Some of them were given an exercise program designed to resemble normal ambulatory activity; another subgroup was fed supplemental potassium phosphate. The results from a 12-week period of treatment were compared with those untreated bed rest periods. The potassium phosphate supplements prevented the hypercalciuria of bed rest, but fecal calcium tended to increase. The exercise program did not diminish the negative calcium balance. Neither treatment affected the heavy loss of mineral from the calcaneus. Several additional studies are developed to examine the problem further

    The Galabat-Metema cross-border onchocerciasis focus: The first coordinated interruption of onchocerciasis transmission in Africa.

    BACKGROUND: Onchocerciasis transmission across international borders is not uncommon, yet a coordinated cross border stops mass drug administration (MDA) decision has not been documented. METHODS/PRINCIPLE FINDINGS: The Galabat-Metema focus involves neighboring districts on the border between Sudan and Ethiopia. Mass drug administration (MDA) was provided once and subsequently twice per year in this focus, with twice-per-year beginning in Ethiopia's Metema subfocus in 2016 and in the Sudan's Galabat subfocus in 2008. Ov16 ELISA-based serosurveys were conducted in 6072 children under 10 years of age in the Metema subfocus in 2014, and 3931 in the Galabat in 2015. Between 2014 and 2016, a total of 27,583 vector Simulium damnosum flies from Metema and 9,148 flies from Galabat were tested by pool screen PCR for Onchocerca volvulus O-150 DNA. Only 8 children were Ov16 seropositive (all in the Metema subfocus); all were negative by skin snip PCR. The upper limit of the 95% confidence interval (UCL) for Ov16 seropositive was <0.1% for the overall focus and 0.14 positive fly heads per 2000 (UCL = 0.39/2000). However, an entomological 'hotspot' was detected on the Wudi Gemzu river in Metema district. The hotspot was confirmed when 4 more positive fly pools were found on repeat testing in 2017 (1.04 L3/2000 flies (UCL = 2.26/2000). Information exchange between the two countries led to stopping MDA in a coordinated fashion in 2018, with the exception of the hotspot at Wudi Gemzu, where MDA with ivermectin was increased to every three months to hasten interruption of transmission. CONCLUSION: Coordinated stop MDA decisions were made by Sudan and Ethiopia based on data satisfying the World Health Organization's criteria for interruption of onchocerciasis transmission.     Knowledge, Practice, and Attitudes of Physicians in Low- and Middle-Income Countries on Fertility and Pregnancy-Related Issues in Young Women With Breast Cancer

    PURPOSE: Fertility and pregnancy-related issues are highly relevant for young ( 64 40 years) patients with breast cancer. Limited evidence exists on knowledge, practice, and attitudes of physicians from low- and middle-income countries (LMICs) regarding these issues. METHODS: A 19-item questionnaire adapted from an international survey exploring issues about fertility preservation and pregnancy after breast cancer was sent by e-mail between November 2019 and January 2020 to physicians from LMICs involved in breast cancer care. Descriptive analyses were performed. RESULTS: A total of 288 physicians from Asia, Africa, America, and Europe completed the survey. Median age was 38 years. Responders were mainly medical oncologists (44.4%) working in an academic setting (46.9%). Among responders, 40.2% and 53.8% reported having never consulted the available international guidelines on fertility preservation and pregnancy after breast cancer, respectively. 25.0%, 19.1%, and 24.3% of responders answered to be not at all knowledgeable about embryo, oocyte, or ovarian tissue cryopreservation, respectively; 29.2%, 23.6%, and 31.3% declared that embryo, oocyte, and ovarian tissue cryopreservation were not available in their countries, respectively. 57.6% of responders disagreed or were neutral on the statement that controlled ovarian stimulation can be considered safe in patients with breast cancer. 49.7% and 58.6% of responders agreed or were neutral on the statement that pregnancy in breast cancer survivors may increase the risk of recurrence overall or only in those with hormone receptor-positive disease, respectively. CONCLUSION: This survey showed suboptimal knowledge, practice, and attitudes of physicians from LMICs on fertility preservation and pregnancy after treatment completion in young women with breast cancer. Increasing awareness and education on these aspects are needed to improve adherence to available guidelines and to promote patients' oncofertility counseling
