    Mass Azithromycin Distribution and Community Microbiome: A Cluster-Randomized Trial.

    BackgroundMass distributions of oral azithromycin have long been used to eliminate trachoma, and they are now being proposed to reduce childhood mortality. The observed benefit appears to be augmented with each additional treatment, suggesting a possible community-level effect. Here, we assess whether 2 biannual mass treatments of preschool children affect the community's gut microbiome at 6 months after the last distribution.MethodsIn this cluster-randomized controlled trial, children aged 1-60 months in the Dossa region of Niger were randomized at the village level to receive a single dose of azithromycin or placebo every 6 months. Fecal samples were collected 6 months after the second treatment for metagenomic deep sequencing. The prespecified primary outcome was the Euclidean PERMANOVA of the gut microbiome, or effectively the distance between the genus-level centroid at the community level, with the secondary outcome being the Simpson's α diversity.ResultsIn the azithromycin arm, the gut microbial structures were significantly different than in the placebo arm (Euclidean PERMANOVA, P < .001). Further, the diversity of the gut microbiome in the azithromycin arm was significantly lower than in the placebo arm (inverse Simpson's index, P = .005).ConclusionsTwo mass azithromycin administrations, 6 months apart, in preschool children led to long-term alterations of the gut microbiome structure and community diversity. Here, long-term microbial alterations in the community did not imply disease but were associated with an improvement in childhood mortality.Clinical trials registrationNCT02048007

    Recognizing thyrotoxicosis in a patient with bipolar mania: a case report

    <p>Abstract</p> <p>Background</p> <p>A thyroid stimulating hormone level is commonly measured in patients presenting with symptoms of mania in order to rule out an underlying general medical condition such as hyperthyroidism or thyrotoxicosis. Indeed, many cases have been reported in which a patient is initially treated for bipolar mania, but is later found to have a thyroid condition. Several case reports have noted the development of a thyroid condition in bipolar patients either on lithium maintenance treatment or recently on lithium treatment.</p> <p>Case presentation</p> <p>We review a case in which a patient with a long history of bipolar disorder presents with comorbid hyperthyroidism and bipolar mania after recent discontinuation of lithium treatment.</p> <p>Conclusion</p> <p>Physicians should consider a comorbid hyperthyroidism in bipolar manic patients only partially responsive to standard care treatment with a mood stabilizer and antipsychotic.</p

    The scale of VGI in map production: a perspective of European National Mapping Agencies

    The perspective of European National Mapping Agencies (NMA) on the role of citizen sensing in map production was explored. The NMAs varied greatly in their engagement with the community generating volunteered geographic information (VGI) and in their future plans. From an assessment of NMA standard practices, it was evident that much VGI was acquired with a positional accuracy that, while less than that typically acquired by NMAs, actually exceeded the requirements of the nominal data capture scale used by most NMAs. Opportunities for VGI use in map revision and updating were evident, especially for agencies that use a continuous rather than cyclical updating policy. Some NMAs had also developed systems to engage with citizen sensors and examples are discussed. Only rarely was VGI used to collect data on features beyond the standard set used by the NMAs. The potential role of citizen sensing and so its current scale of use by NMAs is limited by a series of concerns, notably relating to issues of data quality, the nature and motivation of the contributors, legal issues, the sustainability of data source, and employment fears of NMA staff. Possible priorities for future research and development are identified to help ensure that the potential of VGI in mapping is realized

Citizen science is an emerging example of a non-traditional data source that is already making a contribution. In this Perspective, we present a roadmap that outlines how citizen science can be integrated into the formal Sustainable Development Goals reporting mechanisms. Success will require leadership from the United Nations, innovation from National Statistical Offices and focus from the citizen-science community to identify the indicators for which citizen science can make a real contribution
