    Projeto de implementação de serviço de adesão à terapêutica: Diabetes e hipertensão - Grupo de Farmácias Reis Barata

    A adesão à terapêutica é um problema mundial, sendo um dos principais fatores de sucesso no controlo e avaliação das doenças crónicas, em crescimento nas sociedades contemporâneas. A WHO estima que cerca de 50% dos doentes crónicos não adere corretamente à terapêutica prescrita, aumentando as consequências para a Saúde Pública, exponenciando problemas de ordem económica, financeira e social. O projeto apresentado visa a implementação de um serviço de adesão à terapêutica para os doentes crónicos com diabetes e/ou hipertensão nas farmácias comunitárias do Grupo Reis Barata. Os principais objetivos deste projeto são a implementação de um serviço diferenciador, no acompanhamento aos utentes, a avaliação da sua adesão terapêutica a antidiabéticos e anti hipertensores, através de aconselhamento farmacêutico e medição de indicadores de Saúde dos utentes do GRB. Para a realização do estudo foi elaborada uma revisão bibliográfica que evidencia os serviços das farmácias comunitárias, as doenças crónicas e a adesão à terapêutica. A análise de dados do projeto seguiu uma abordagem quantitativa, através da realização de questionários aos utentes diabéticos e/ou hipertensos do GRB, com a obtenção de 111 respostas. Foi também realizada a análise situacional e de diagnóstico, considerando as necessidades do projeto. Na realização deste trabalho é apresentado o respetivo plano de operacionalização do serviço de adesão à terapêutica, fluxograma a implementar, cronograma e indicadores de controlo do projeto. Através da análise realizada, conclui-se pela viabilidade de implementação do serviço, tendo em conta os potenciais ganhos em Saúde, acompanhamento dos utentes e fidelização às farmácias Reis Barata.Therapy adherence is a worldwide problem, being one of the main success factors in control and evaluation of chronic diseases, on the rise in contemporary societies. WHO estimates that around 50% of chronically ill patients do not correctly adhere to the prescribed medicines, increasing the consequences for Public Health, increasing economic, financial and social problems. The presented project aims at the implementation of a therapy adherence service for chronic patients with diabetes and/or hypertension in Reis Barata´s community pharmacies. The main objectives of this project are the implementation of a differentiating service in the monitoring of patients, the assessment of their therapeutic adherence to antidiabetics and antihypertensives through pharmaceutical advice and measurement of health indicators of GRB`s customers. In order to carry out the study, a bibliographical review was elaborated, highlighting the services of community pharmacies, chronic diseases and adherence to therapy. The analysis of the project's data followed a quantitative approach, by conducting questionnaires to diabetic and/or hypertensive customers of Reis Barata´s pharmacies, obtaining 111 responses. A situational and diagnostic analysis was also carried out, considering the project needs. In carrying out this work, the operationalization plan for the adherence to therapy service is presented, a flowchart to be implemented, schedule and project control indicators. Through the analysis carried out, it is concluded that the implementation of the service is possible, considering the potential gains in healthcare, monitoring of users and their loyalty to Reis Barata´s pharmacies

    The Effect of the Copigment on the pH-Dependent Reversible and Irreversible Processes

    no 219201Intermolecular copigmentation of malvidin-3-O-glucoside with caffeine was studied using a holistic procedure that includes the extension to basic pH values. In moderately basic solutions (7.5 < pH < 9.5) and independently of the copigment presence, there is a pH region where degradation of the quinoidal base and anionic quinoidal bases is faster than hydration and OH-nucleophilic addition, preventing the system from reaching the equilibrium. Intermolecular copigmentation with caffeine reduces significantly the degradation rate of quinoidal bases. In a more basic medium, the equilibrium is reached and degradation occurs from the anionic chalcones. In this case, the addition of caffeine also reduces the degradation rate in the interval 10 < pH < 11.5.publishersversionpublishe

    Mestrado em Desenvolvimento e Cooperação InternacionalO surgimento de novos actores, para além dos Estados, contribuiu para uma adaptação e consequente evolução no panorama da cooperação para o desenvolvimento. Neste âmbito, a cooperação intermunicipal, uma tipologia de cooperação descentralizada, corresponde ao estabelecimento de relações entre duas ou mais comunidades, tendo como principais actores os Municípios ou seus equivalentes, de acordo com o sistema de organização administrativo dos países, podendo assumir várias formas, a saber: geminações, protocolos, acordos de cooperação e redes. Num país como Moçambique, um dos mais pobres do mundo, o Município da Matola, onde está concentrado o maior parque industrial do país, é considerado um laboratório para aferir o grau de sucesso, quer da própria experiência de descentralização em Moçambique, quer das possibilidades que os projectos de cooperação intermunicipal encerram, nomeadamente com o Município de Loures (Portugal). À procura de soluções locais, integradas e sustentáveis, de desenvolvimento, fruto da emergência da própria sociedade civil moçambicana e da consequente proliferação de ONG nacionais e estrangeiras, e de uma dinâmica crescente que cruza diferentes sectores da população, pretende-se associar o reforço dos laços entre os municípios como um factor de sucesso, em termos de sustentabilidade dos projectos de desenvolvimento, numa abordagem bottom-up, em virtude de uma maior participação das comunidades e da integração de diferentes entidades na concepção, implementação e avaliação dos mesmos.The emergence of new actors, besides states, has contributed to an adaptation, and consequent evolution on development cooperation. In this sense, inter-municipal cooperation, a form of decentralised cooperation, entails the establishment of relations between two or more communities where the main actors are the Municipal Councils or their equivalent, in accordance with the organisational system of the involved countries. This cooperation can take on the forms of twinning, protocols, cooperation agreements, and networks. In a country like Mozambique, one of the poorest in the world, the Community of Matola, where it is concentrated the most important industrial centre in the country, is considered a laboratory to assess the success level, on the decentralisation process experience in Mozambique, and on the possibilities of inter-municipal cooperation projects, above all with the Community of Loures (Portugal). Joining the look for local, integrated and sustainable solutions of development, as a result of the emergence of the Mozambican civil society, and consequently of the proliferation of national and foreigner NGOs, and of a dynamic which cuts different sectors of the population; there's the enforcement of the links between the local communities as a factor of success, in terms of sustainability of the development projects, in a bottom-up approach, because of the larger participation of communities and the integration of different entities in mutually conception, implementation, and evaluation of the projects

    Evolution of Flavylium-Based Color Systems in Plants: What Physical Chemistry Can Tell Us

    Anthocyanins are the basis of the color of angiosperms, 3-deoxyanthocyanins and sphagnorubin play the same role in mosses and ferns, and auronidins are responsible for the color in liverworts. In this study, the color system of cyanidin-3-O-glucoside (kuromanin) as a representative compound of simpler anthocyanins was fully characterized by stopped flow. This type of anthocyanin cannot confer significant color to plants without intra- or intermolecular interactions, complexation with metals or supramolecular structures as in Commelina communis. The anthocyanin's color system was compared with those of 3-deoxyanthocyanins and riccionidin A, the aglycone of auronidins.     Toward Light-Controlled Supramolecular Peptide Dimerization

    The selective photodeprotection of the NVoc-modified FGG tripeptide yields the transformation of its 1:1 receptor−ligand complex with cucurbit[8]uril into a homoternary FGG2@CB8 assembly.     Adaptive Tracking Techniques in Non-Stationary Environments

    The continuously changing environments have been the main challenge for classical GNSS receiver implementations, as they can have a great impact on signal tracking performance and positioning. For this reason techniques capable of mitigating the impact of time-varying phenomena by adapting to changing conditions, thus improving performance are of great interest. This study reports the benefits of using adaptive techniques for standalone GNSS receivers in three different scenarios. The first scenario compares an adaptive Kalman filter against a classical DLL/PLL architecture in interference environments with user dynamics. The second scenario uses a multipath propagation channel to test alternative carrier tracking architectures which provide better results in terms of robustness. The third scenario uses the same multipath propagation channel to test the adaptive switching technique, 2-Step, in order to evaluate its capacity of guaranteeing and maintaining unambiguous tracking for BOC-type modulations. In order to assess the performance of these techniques a semi-analytical platform has been used. The results presented here show the benefits and trade-offs of different techniques taking into account different propagation channels and scenarios

    Um dos desafios da era pós-genética é a compreensão da estrutura e o comportamento das redes complexas de interações moleculares que controlam ou comportam como células. Isso impõe, uma busca por métodos inovadores para tratar esses dados com o aprimoramento da compreensão dos processos biológicos que atuam no interior da célula. Neste contexto, é possível interligar como vias e melhorar a compreensão das interações biológicas na ausência ou presença de uma doença específica, usando ferramentas como a linguagem R e seus pacotes e scripts disponíveis. Com uma nanotecnologia, surgem obstáculos na terapia, como uma barreira no cérebro, que impedem a difusão livre da maioria das moléculas estranhas,incluindo agentes terapêuticos e com isso o estudo de redes de interação que podem contribuir para o desenvolvimento de novos nanoencapsulados, disponibilizar subsídio para o entendimento de como as vias se comportam quando induzidas em diferentes tipos de uso e métodos. Os resultados preliminares consistiram na construção da rede basal com 104 vias selecionadas desde o uso pré-especificado, sendo que as análises de expressão de glioblastoma foram tratadas com o uso de receptores de quimiocinas diretamente após a presença de fármaco no glioblastoma

    New insights into the pathogenesis and transmission of Brucella pinnipedialis: systemic infection in two bottlenose dolphins ( Tursiops truncatus)

    The emergence of Brucella infections in marine mammals is a growing concern. The present study reports two cases of systemic Brucella pinnipedialis infection detected in bottlenose dolphins (Tursiops truncatus) pair stranded together in the Cantabrian coast of Spain. Both animals showed systemic lesions associated with the Brucella infection, more severe in the younger dolphin, considered the likely offspring of the other individual. Real-time PCR, bacterial culture, and whole-genome sequencing were used to detect and characterize the Brucella strains involved in both dolphins. The phylogenetic analysis performed on the Brucella genomes retrieved revealed that the species involved was B. pinnipedialis (ST25). Both animals resulted seropositive in a commercial multispecies blocking ELISA but tested negative in the standard Rose Bengal test. To the best of our knowledge, this is the first report of a systemic infection resulting in various lesions associated with Brucella pinnipedialis (ST25) in two bottlenose dolphins. It is also the initial isolation of Brucella in the milk of a non-pregnant or non-aborting female cetacean likely stranded with its offspring. These findings provide new insights into the epidemiology and clinical impact of B. pinnipedialis infection in cetaceans and underscore the importance of continued diagnostic surveillance to gain better understanding of brucellosis effects and transmission in marine mammal populations.Este trabajo ha contado con el apoyo de un convenio de colaboración entre la Fundación Oceanogràfic (Valencia), la Consejería de Medio Rural, Ganadería, Pesca, Alimentación y Medio Ambiente del Gobierno de Cantabria y el Centro de Vigilancia Sanitaria Veterinaria (VISAVET) de la Universidad Complutense de Madrid.     Measurement of the (eta c)(1S) production cross-section in proton-proton collisions via the decay (eta c)(1S) -&gt; p(p)over-bar

    The production of the ηc(1S)\eta_c (1S) state in proton-proton collisions is probed via its decay to the ppˉp \bar{p} final state with the LHCb detector, in the rapidity range 2.06.52.0 6.5 GeV/c. The cross-section for prompt production of ηc(1S)\eta_c (1S) mesons relative to the prompt J/ψJ/\psi cross-section is measured, for the first time, to be σηc(1S)/σJ/ψ=1.74±0.29±0.28±0.18B\sigma_{\eta_c (1S)}/\sigma_{J/\psi} = 1.74 \pm 0.29 \pm 0.28 \pm 0.18 _{B} at a centre-of-mass energy s=7\sqrt{s} = 7 TeV using data corresponding to an integrated luminosity of 0.7 fb1^{-1}, and σηc(1S)/σJ/ψ=1.60±0.29±0.25±0.17B\sigma_{\eta_c (1S)}/\sigma_{J/\psi} = 1.60 \pm 0.29 \pm 0.25 \pm 0.17 _{B} at s=8\sqrt{s} = 8 TeV using 2.0 fb1^{-1}. The uncertainties quoted are, in order, statistical, systematic, and that on the ratio of branching fractions of the ηc(1S)\eta_c (1S) and J/ψJ/\psi decays to the ppˉp \bar{p} final state. In addition, the inclusive branching fraction of bb-hadron decays into ηc(1S)\eta_c (1S) mesons is measured, for the first time, to be B(bηcX)=(4.88±0.64±0.25±0.67B)×103B ( b \rightarrow \eta_c X ) = (4.88 \pm 0.64 \pm 0.25 \pm 0.67 _{B}) \times 10^{-3}, where the third uncertainty includes also the uncertainty on the J/ψJ/\psi inclusive branching fraction from bb-hadron decays.
