    Retrospective longitudinal analysis of phonetic and phonological cleft palate speech characteristics

    In this study, we analysed phonetic and phonological consonant characteristics of cleft palate speech (CPS) at ages 5 and 10 in cleft palate with/without cleft lip (CP±CL) based on Cleft Audit Protocol for Speech – Augmented (CAPS-A) data collected in the Dental Hospital in Glasgow. The nature and extent of CPS characteristics at different ages were investigated. Video-recordings of 42 cleft palate (CP), unilateral cleft lip and palate (UCLP) and bilateral cleft lip and palate (BCLP) boys and girls were analysed based on narrow transcription and automatic PROPH (profile of phonology) of types of articulation features. Results of this study showed that CP±CL children produce a significantly higher number of phonetic CPS characteristics than phonological processes at both ages

    BERT WEAVER: Using WEight AVERaging to enable lifelong learning for transformer-based models in biomedical semantic search engines

    Recent developments in transfer learning have boosted the advancements in natural language processing tasks. The performance is, however, dependent on high-quality, manually annotated training data. Especially in the biomedical domain, it has been shown that one training corpus is not enough to learn generic models that are able to efficiently predict on new data. Therefore, in order to be used in real world applications state-of-the-art models need the ability of lifelong learning to improve performance as soon as new data are available - without the need of re-training the whole model from scratch. We present WEAVER, a simple, yet efficient post-processing method that infuses old knowledge into the new model, thereby reducing catastrophic forgetting. We show that applying WEAVER in a sequential manner results in similar word embedding distributions as doing a combined training on all data at once, while being computationally more efficient. Because there is no need of data sharing, the presented method is also easily applicable to federated learning settings and can for example be beneficial for the mining of electronic health records from different clinics

    Serum proteome profiling identifies novel and powerful markers of cystic fibrosis liver disease.

    Cystic Fibrosis associated liver disease (CFLD) develops in approximately 30% of CF patients. However, routine sensitive diagnostic tools for CFLD are lacking. Within this study, we aimed to identify new experimental biomarkers for the detection of CFLD. 45 CF patients were included in the study and received transient elastography. Differential regulation of 220 different serum proteins was assessed in a subgroup of patients with and without CFLD. Most interesting candidate proteins were further quantified and validated by ELISA in the whole patient cohort. To assess a potential relation of biomarker expression to the degree of hepatic fibrosis, serum biomarkers were further determined in 18 HCV patients where liver histology was available. 43 serum proteins differed at least 2-fold in patients with CFLD compared to those without liver disease as identified in proteome profiling. In ELISA quantifications, TIMP-4 and Endoglin were significantly up-regulated in patients with CFLD as diagnosed by clinical guidelines or increased liver stiffness. Pentraxin-3 was significantly decreased in patients with CFLD. Serum TIMP-4 and Endoglin showed highest values in HCV patients with liver cirrhosis compared to those with fibrosis but without cirrhosis. At a cut-off value of 6.3 kPa, transient elastography compassed a very high diagnostic accuracy and specificity for the detection of CFLD. Among the biomarkers, TIMP-4 and Endoglin exhibited a high diagnostic accuracy for CFLD. Diagnostic sensitivities and negative predictive values were increased when elastography and TIMP-4 and Endoglin were combined for the detection of CFLD. Serum TIMP-4 and Endoglin are increased in CFLD and their expression correlates with hepatic staging. Determination of TIMP-4 and Endoglin together with transient elastography can increase the sensitivity for the non-invasive diagnosis of CFLD

    Insertion and functionalization of gallasilylenes [LPh^{Ph}Si–Ga(Cl)LBDI^{BDI}] (LPh^{Ph} = PhC(NtBu)2_{2}; LBDI^{BDI} = [{2,6-iPr2_{2}C6_{6}H3_{3}NCMe}2_{2}CH]) into the cyclo-E5_{5} rings of [Cp*Fe(η5^{5}-E5_{5})] (Cp* = η5^{5}-C5_{5}Me5_{5}; E = P, As) are reported. Reactions of [Cp*Fe(η5^{5}-E5_{5})] with gallasilylene result in E–E/Si–Ga bond cleavage and the insertion of the silylene in the cyclo-E5_{5} rings. [(LPh^{Ph}Si-Ga(Cl)LBDI^{BDI}){(η4^{4}-P5_{5})FeCp*}], in which the Si atom binds to the bent cyclo-P5_{5} ring, was identified as a reaction intermediate. The ring-expansion products are stable at room temperature, while isomerization occurred at higher temperature, and the silylene moiety further migrates to the Fe atom, forming the corresponding ring-construction isomers. Furthermore, reaction of [Cp*Fe(η5^{5}-As5_{5})] with the heavier gallagermylene [LLPh^{Ph}Ge–Ga(Cl)LBDI^{BDI}B] was also investigated. All the isolated complexes represent rare examples of mixed group 13/14 iron polypnictogenides, which could only be synthesized by taking advantage of the cooperativity of the gallatetrylenes featuring low-valent Si(II) or Ge(II) and Lewis acidic Ga(III) units/entities

    Extensive harmonization of survey and especially biographical data has not been common in the social sciences. HaSpaD provides a tool for harmonizing and accumulating, and thus comprehensively analyzing, survey-based longitudinal data sets on partnership biographies. The following studies were harmonized and merged for a joint analysis for the third-party funded project "Harmonizing and synthesizing partnership histories from different research data infrastructures" (HaSpaD): the panel studies pairfam, SOEP and SHARE; the cross-section studies General German Social Survey, Mannheim Divorce Study and the Fertility and Family Survey; as well as the cross-section studies combined with partially repeated surveys Family Surveys, German Life History Studies and the Generations and Gender Surveys. The project provides syntax-based harmonization processes available through the HaSpaD Harmonization Wizard. The HaSpaD Harmonization Wizard enables a customized selection of survey programs and variables. After downloading the source data sets from their repositories, the HaSpaD syntax package enables to generate an individually customized and harmonized data set of the source datasets. In addition to biographical data on partnerships, the HaSpaD target dataset may include other variables such as age, gender, citizenship, and education level. If all surveys are selected, the target data set will contain approximately 182,000 partnership biographies

    Development and initial validation of the Seated Posture Scale

    —Literature shows that some health outcomes (e.g., eating, breathing, and speaking) are directly related to posture. Evidence of outcomes mediated by wheelchair seated posture is limited to interface pressure, physical function, and wheelchair skills and safety. This study’s purpose was to develop and validate a rapid, low-burden, paper-pencil assessment of wheelchair seated posture for research use and to test feasibility of its use with a sample of older adults. We used a prospective design and a convenience sample of older adults who were receiving rehabilitation services in a community living center. Forty-nine older wheelchair users participated. Main measures were the Seated Posture Scale (SPS), Modified Ashworth Scale, Barthel Index, Visual Descriptor Scale, scale-content validity index (S-CVI), Cronbach alpha, and test-retest reliability. Rating by six experts yielded the overall content validity score (S-CVI) of 0.744. Total SPS score correlated positively with physical function (Barthel Index, r = 0.46, p \u3c 0.001) and negatively with muscle tone (Modified Ashworth Scale, r = –0.44, p = 0.001), supporting SPS construct validity. Internal consistency was 0.66 (Cronbach alpha). Test-retest reliability yielded Pearson product-moment correlations of 0.89 to 0.99. We conclude that the SPS has sufficient preliminary validity and reliability to support its use as an evaluation of wheelchair seated posture in outcomes research

    The unified protocol as an internet-based intervention for emotional disorders: Randomized controlled trial

    The Unified Protocol (UP) as a transdiagnostic intervention has primarily been applied in the treatment of anxiety disorders and in face-to-face-settings. The current study investigated the efficacy of a 10-week Internet-based adaptation of the UP for anxiety, depressive, and somatic symptom disorders. The trial was registered under DRKS00014820 at the German Clinical Trial Registry, DRKS. Participants (n = 129) were randomized to treatment or waitlist control. Significant treatment effects were found for symptom distress, satisfaction with life, positive/negative affect and markers of anxiety, depression, and somatic symptom burden (within-group Hedges’ g = 0.32–1.38 and between-group g = 0.20–1.11). Treatment gains were maintained at 1- and 6-month-follow-up. Subgroup analyses showed comparable effects in participants with anxiety and depressive disorders. 26.6% dropped out of treatment and 35.38% did not provide post-treatment assessments. The results strengthen the application of the UP as an Internet-based treatment for alleviating symptom distress across emotional disorders. More research on the applicability for single disorders is needed and avenues to improve adherence and attrition rates should be explored

    Safety and efficacy of nonacog alfa for the treatment of haemophilia B in children younger than 6 years of age in a routine clinical care setting: the EUREKIX registry study.

    INTRODUCTION European regulatory authorities request postmarketing safety and efficacy data for factor IX (FIX) products. AIM Collect additional clinical data from routine nonacog alfa use in children aged <6 years with haemophilia B. METHODS The EUREKIX registry included retrospective and prospective data collection phases. Safety was assessed via adverse drug reactions (ADRs)/adverse events (AEs) and events of special interest (ESIs) as the primary objective; efficacy was evaluated via annualised bleeding rates (ABRs). RESULTS The retrospective phase comprised 37 subjects. Of these, 25 had severe haemophilia B. One subject experienced 2 ADRs; another experienced 4 ESIs of hypersensitivity. Median ABR in subjects receiving a predominantly on-demand regimen (prophylaxis <50% of time; n = 11) was 2.0; median ABR was 3.8 in those receiving predominantly prophylactic treatment (prophylaxis ≥50% of time; n = 24). Joint bleeding was infrequent (median ABR, 0.4; n = 35). The prospective phase included 26 subjects, with 17 continuing from the retrospective phase. A total of 20 subjects had severe haemophilia B. Three subjects experienced 7 treatment-related AEs; 3 experienced 4 ESIs. Median ABR was 4.5 and 1.1 in subjects who received predominantly on-demand (n = 5) or prophylactic treatment (n = 19), respectively; the overall median ABR for joint bleeding events was 0.0. CONCLUSIONS Overall, nonacog alfa treatment effectively controlled bleeding events, with no new safety signals identified. These data support the safety and efficacy of nonacog alfa in routine clinical settings in children aged <6 years

    Reduction of Pesticide Toxicity Under Field-Relevant Conditions? The Interaction of Titanium Dioxide Nanoparticles, Ultraviolet, and Natural Organic Matter

    In surface waters, the illumination of photoactive engineered nanomaterials (ENMs) with ultraviolet (UV) light triggers the formation of reactive intermediates, consequently altering the ecotoxicological potential of co-occurring organic micropollutants including pesticides due to catalytic degradation. Simultaneously, omnipresent natural organic matter (NOM) adsorbs onto ENM surfaces, altering the ENM surface properties. Also, NOM absorbs light, reducing the photo(cata)lytic transformation of pesticides. Interactions between these environmental factors impact 1) directly the ecotoxicity of photoactive ENMs, and 2) indirectly the degradation of pesticides. We assessed the impact of field-relevant UV radiation (up to 2.6 W UVA/m(2)), NOM (4 mg TOC/L), and photoactive ENM (nTiO(2), 50 mu g/L) on the acute toxicity of 6 pesticides inDaphnia magna. We selected azoxystrobin, dimethoate, malathion, parathion, permethrin, and pirimicarb because of their varying photo- and hydrolytic stabilities. Increasing UVA alone partially reduced pesticide toxicity, seemingly due to enhanced degradation. Even at 50 mu g/L, nano-sized titanium dioxide (nTiO(2)) reduced but also increased pesticide toxicity (depending on the applied pesticide), which is attributable to 1) more efficient degradation and potentially 2) photocatalytically induced formation of toxic by-products. Natural organic matter 1) partially reduced pesticide toxicity, not evidently accompanied by enhanced pesticide degradation, but also 2) inhibited pesticide degradation, effectively increasing the pesticide toxicity. Predicting the ecotoxicological potential of pesticides based on their interaction with UV light or interaction with NOM was hardly possible, which was even more difficult in the presence of nTiO(2).
