    Back complaints in older adults

    With the aging of the Dutch population, the number of older aged back pain patients is also expected to increase. However, information on the course and prognosis of older patients with back pain in general practice is very scarce. Back pain is a common musculoskeletal disorder, also in older adults, information about back pain in older adults is therefore important. In addition, because there are several important reasons why back pain in older adults might differ from back pain in the younger adult population, information about younger back pain patients may not be generalizable to older adults. The overall objective of the studies presented in this thesis are therefore to acquire information about back pain in older adults and more specific to gain insight into: 1) the course of back pain in older adults in general practice, 2) the characteristics of these patients and their back pain, 3) prognostic factors for poor recovery of older adults with back pain, 4) the association between perceived lumbar stiffness and lumbar disc degeneration (LDD) as assessed on X-ray, and 5) the medical consumption of back pain patients

    Stratospheric age of air computed with trajectories based on various 3D-Var and 4D-Var data sets

    International audienceThe age of stratospheric air is computed with a trajectory model, using ECMWF ERA-40 3D-Var and operational 4D-Var winds. Analysis as well as forecast data are used. In the latter case successive forecast segments are put together to get a time series of the wind fields. This is done for different forecast segment lengths. The sensitivity of the computed age to the forecast segment length and assimilation method are studied, and the results are compared with observations and with results from a chemistry transport model that uses the same data sets. A large number of backward trajectories are started in the stratosphere, and from the fraction of these trajectories that has passed the tropopause the age of air is computed. First, for ten different data sets 50-day backward trajectories starting in the tropical lower stratosphere are computed. The results show that in this region the computed cross-tropopause transport decreases with increasing forecast segment length. Next, for three selected data sets (3D-Var 24-h and 4D-Var 72-h forecast segments, and 4D-Var analyses) 5-year backward trajectories are computed that start all over the globe at an altitude of 20km. For all data sets the computed ages of air in the extratropics are smaller than the observation-based age. For 4D-Var forecast series they are closest to the observation-based values, but still 0.5-1.5 year too small. Compared to the difference in age between the results for the different data sets, the difference in age between the trajectory and the chemistry transport model results is small

    Thermoelectric properties of lead chalcogenide core-shell nanostructures

    We present the full thermoelectric characterization of nanostructured bulk PbTe and PbTe-PbSe samples fabricated from colloidal core-shell nanoparticles followed by spark plasma sintering. An unusually large thermopower is found in both materials, and the possibility of energy filtering as opposed to grain boundary scattering as an explanation is discussed. A decreased Debye temperature and an increased molar specific heat are in accordance with recent predictions for nanostructured materials. On the basis of these results we propose suitable core-shell material combinations for future thermoelectric materials of large electric conductivities in combination with an increased thermopower by energy filtering.Comment: 12 pages, 8 figure

    Changes in aerosol properties during spring-summer period in the Arctic troposphere

    The change in aerosol properties during the transition from the more polluted spring to the clean summer in the Arctic troposphere was studied. A six-year data set of observations from Ny-Ã…lesund on Svalbard, covering the months April through June, serve as the basis for the characterisation of this time period. In addition four-day-back trajectories were used to describe air mass histories. The observed transition in aerosol properties from an accumulation-mode dominated distribution to an Aitken-mode dominated distribution is discussed with respect to long-range transport and influences from natural and anthropogenic sources of aerosols and pertinent trace gases. Our study shows that the air-mass transport is an important factor modulating the physical and chemical properties observed. However, the air-mass transport cannot alone explain the annually repeated systematic and rather rapid change in aerosol properties, occurring within a limited time window of approximately 10 days. With a simplified phenomenological model, which delivers the nucleation potential for new-particle formation, we suggest that the rapid shift in aerosol microphysical properties between the Arctic spring and summer is mainly driven by the incoming solar radiation in concert with transport of precursor gases and changes in condensational sink

    Integration of microRNA changes in vivo identifies novel molecular features of muscle insulin resistance in type 2 diabetes

    Skeletal muscle insulin resistance (IR) is considered a critical component of type II diabetes, yet to date IR has evaded characterization at the global gene expression level in humans. MicroRNAs (miRNAs) are considered fine-scale rheostats of protein-coding gene product abundance. The relative importance and mode of action of miRNAs in human complex diseases remains to be fully elucidated. We produce a global map of coding and non-coding RNAs in human muscle IR with the aim of identifying novel disease biomarkers. We profiled >47,000 mRNA sequences and >500 human miRNAs using gene-chips and 118 subjects (n = 71 patients versus n = 47 controls). A tissue-specific gene-ranking system was developed to stratify thousands of miRNA target-genes, removing false positives, yielding a weighted inhibitor score, which integrated the net impact of both up- and down-regulated miRNAs. Both informatic and protein detection validation was used to verify the predictions of in vivo changes. The muscle mRNA transcriptome is invariant with respect to insulin or glucose homeostasis. In contrast, a third of miRNAs detected in muscle were altered in disease (n = 62), many changing prior to the onset of clinical diabetes. The novel ranking metric identified six canonical pathways with proven links to metabolic disease while the control data demonstrated no enrichment. The Benjamini-Hochberg adjusted Gene Ontology profile of the highest ranked targets was metabolic (P < 7.4 × 10-8), post-translational modification (P < 9.7 × 10-5) and developmental (P < 1.3 × 10-6) processes. Protein profiling of six development-related genes validated the predictions. Brain-derived neurotrophic factor protein was detectable only in muscle satellite cells and was increased in diabetes patients compared with controls, consistent with the observation that global miRNA changes were opposite from those found during myogenic differentiation. We provide evidence that IR in humans may be related to coordinated changes in multiple microRNAs, which act to target relevant signaling pathways. It would appear that miRNAs can produce marked changes in target protein abundance in vivo by working in a combinatorial manner. Thus, miRNA detection represents a new molecular biomarker strategy for insulin resistance, where micrograms of patient material is needed to monitor efficacy during drug or life-style interventions

    The influence of biogenic emissions from Africa on tropical tropospheric ozone during 2006: a global modeling study

    We have performed simulations using a 3-D global chemistry-transport model to investigate the influence that biogenic emissions from the African continent exert on the composition of the troposphere in the tropical region. For this purpose we have applied two recently developed biogenic emission inventories provided for use in large-scale global models (Granier et al., 2005; LathiSre et al., 2006) whose seasonality and temporal distribution for biogenic emissions of isoprene, other volatile organic compounds and NO is markedly different. The use of the 12 year average values for biogenic emissions provided by LathiSre et al. (2006) results in an increase in the amount of nitrogen sequestrated into longer lived reservoir compounds which contributes to the reduction in the tropospheric ozone burden in the tropics. The associated re-partitioning of nitrogen between PAN, HNO3 and organic nitrates also results in a similar to 5% increase in the loss of nitrogen by wet deposition. At a global scale there is a reduction in the oxidizing capacity of the model atmosphere which increases the atmospheric lifetimes of CH4 and CO by similar to 1.5% and similar to 4%, respectively. Comparisons against a range of different measurements indicate that applying the 12 year average of LathiSre et al. (2006) improves the performance of TM4_AMMA for 2006 in the tropics. By the use of sensitivity studies we show that the release of NO from soils in Africa accounts for between similar to 2-45% of tropospheric ozone in the African troposphere, similar to 10% in the upper troposphere and between similar to 5-20% of the tropical tropospheric ozone column over the tropical Atlantic Ocean. The subsequent reduction in OH over the source regions allows enhanced transport of CO out of the region. For biogenic volatile organic C1 to C3 species released from Africa, the effects on tropical tropospheric ozone are rather limited, although this source contributes to the global burden of VOC by between similar to 2-4% and has a large influence on the organic composition of the troposphere over the tropical Atlantic Ocean
