    Allogeneic stem cell transplantation in patients with atypical chronic myeloid leukaemia: a retrospective study from the Chronic Malignancies Working Party of the European Society for Blood and Marrow Transplantation

    Atypical chronic myeloid leukaemia (aCML) is an aggressive malignancy for which allogeneic haematopoietic stem cell transplantation (allo-HSCT) represents the only curative option. We describe transplant outcomes in 42 patients reported to the European Society for Blood and Marrow Transplantation (EBMT) registry who underwent allo-HSCT for aCML between 1997 and 2006. Median age was 46 years. Median time from diagnosis to transplant was 7 months. Disease status was first chronic phase in 69%. Donors were human leucocyte antigen (HLA)-identical siblings in 64% and matched unrelated (MUD) in 36%. A reduced intensity conditioning was employed in 24% of patients. T-cell depletion was applied in 87% and 26% of transplants from MUD and HLA-identical siblings, respectively. According to the EBMT risk-score, 45% of patients were ‘low-risk’, 31% ‘intermediate-risk’ and 24% ‘high-risk’. Following allo-HSCT, 87% of patients achieved complete remission. At 5 years, relapse-free survival was 36% and non-relapse mortality (NRM) was 24%, while relapse occurred in 40%. Patient age and the EBMT score had an impact on overall survival. Relapse-free survival was higher in MUD than in HLA-identical sibling HSCT, with no difference in NRM. In conclusion, this study confirmed that allo-HSCT represents a valid strategy to achieve cure in a reasonable proportion of patients with aCML, with young patients with low EBMT risk score being the best candidates

Dokumentation des Deutschen StĂ€dte- und Gemeindebundes und des Verbundforschungsprojekts KERNiG (Kommunale ErnĂ€hrungssysteme als SchlĂŒssel zu einer umfassend-integrativen Nachhaltigkeitsgovernance), gefördert durch das Bundesministerium fĂŒr Bildung und Forschung. FĂŒr Kommunen spielen Nachhaltigkeit und Klimaschutz eine immer wichtigere Rolle. Dabei wurden die Umweltauswirkungen von ErnĂ€hrung auf Ebene der Kommunen bisher nur wenig berĂŒcksichtigt. Im Themenfeld ErnĂ€hrung liegen jedoch bedeutende Reduktionspotentiale des ökologischen Fußabdrucks. Wenn Kommunen den Anspruch haben eine nachhaltige Stadtentwicklung zu fördern, kommen sie nicht umhin, sich auch mit ihren ErnĂ€hrungssystemen auseinanderzusetzen. Das Handbuch soll mit Hilfe von Praxisbeispielen aufzeigen, an welchen Stellen und in welchen Handlungsfeldern sich zentrale Gestaltungspotenziale fĂŒr ErnĂ€hrung auf kommunaler Ebene befinden. Kommunen sollen ermutigt werden, das Thema ErnĂ€hrung als Teil einer nachhaltigen Stadtentwicklung aufzugreifen

    Motor, cognitive and mobility deficits in 1000 geriatric patients : protocol of a quantitative observational study before and after routine clinical geriatric treatment – the ComOn-study

The currently available methods for diagnosis and for the evaluation of treatment in this vulnerable cohort are limited. The aims of the ComOn (COgnitive and Motor interactions in the Older populatioN) study are (i) to define quantitative markers with clinical relevance for motor and cognitive deficits, (ii) to investigate the interaction between both motor and cognitive deficits and (iii) to assess health status as well as treatment outcome of 1000 geriatric inpatients in hospitals of Kiel (Germany), Brescia (Italy), Porto (Portugal), Curitiba (Brazil) and Bochum (Germany). Methods: This is a prospective, explorative observational multi-center study. In addition to the comprehensive geriatric assessment, quantitative measures of reduced mobility and motor and cognitive deficits are performed before and after a two week's inpatient stay. Components of the assessment are mobile technology-based assessments of gait, balance and transfer performance, neuropsychological tests, frailty, sarcopenia, autonomic dysfunction and sensation, and questionnaires to assess behavioral deficits, activities of daily living, quality of life, fear of falling and dysphagia. Structural MRI and an unsupervised 24/7 home assessment of mobility are performed in a subgroup of participants. The study will also investigate the minimal clinically relevant change of the investigated parameters.
