    Reduction of HbA1c levels following nonsurgical treatment of periodontal disease in type 2 diabetics

    Objectives: A study was made of the effect of nonsurgical periodontal treatment upon blood glucose control in type 2 diabetics with periodontal disease. Study design: A pre-post repeated measures analytical design was used, with a PubMed search of the related articles published up until December 2008. Results: The improvement in glycosylated hemoglobin (HbA1c) after nonsurgical periodontal treatment on comparing the baseline (8.0290±1.5539) and final values (7.7806±1.5636) was 0.2483±0.1171, and proved statistically significant (p=0.042). Conclusions: A decrease in HbA1c was observed after treatment, thus indicating improved blood glucose control in type 2 diabetics with periodontal disease

    [email protected] [email protected] literatura en marketing de servicios ha argumentado que el proceso de servicio puede ser el antecedente más importante de la evaluacióm del cliente sobre el resultado del servicio y que en ese juicio evaluativo último, el valor del servicio adquiere un papel esencial. Sin embargo, la naturaleza de este proceso de formación no ha sido extensamente estudiada, especialmente, en un contexto de relación interorganizativa. El presente estudio intenta contribuir al conocimiento de cómo el encuentro de servicio y el valor del servicio, contribuyen a aumentar la satisfacción del cliente. El trabajo explora la relación entre ambos, analizando el caso de una organización que provee servicios de intermediación financiera al sector bancario. Sobre la base de una revisión de la literatura, el análisis SEM es utilizado para contrastar las hipótesis de los vínculos entre los constructos analizados. El análisis de fiabilidad y validez de las escalas empleadas da resultados satisfactorios, y nuestras conclusiones establecen que el encuentro de servicio afecta directa y significativamente al valor del servicio, siendo éste el antecedente último de la satisfacción.In services marketing literature it has been said that service process can be the most important antecedent of customer’s service results assessment, and that service value has a key role in this last evaluative judgement. However, the nature of this service formation process has not extensively studied, specially, when talking about business to business relation. The present paper wishes to make a contribution in the sense of understanding how both service encounter and service value can increase customer satisfaction. In the work we explore the relationship between both elements in the particular case of an organization that provides financial mediation services to the banking sector. On the basis of previous literature review, a SEM analysis is proposed to validate several hypothesis on relationships between constructs. Scales reliability and validity were tested satisfactory. We conclude that service encounter has significant and direct effects on service value, the latter being the very last antecedent of satisfaction

    Optimal design of prestressed concrete hollow core slabs taking into account its fire resistance

    [EN] Prestressed hollow core slabs are a concrete element widely used as construction floor product, which manufacturing process has greatly been improved in recent years. Several research studies focused on hollow core slab performance, mainly related to its fire behavior, have provided new limit states to be assessed throughout its life cycle. Therefore, the hollow core slab design needs to be reviewed to allow for these improvements, a process which may involve changes to its geometry. In order to deal with this review, modern computational optimization techniques offer an alternative approach to traditional structural product design procedure, mainly based on the engineer's prior experience. This paper proposes a hollow core slab model (including variables and constraints) to develop heuristic search algorithms, such as simulated annealing, in order to find the most economical slab design including the fire resistant constraint and taking into account all available manufacturing technologies. The optimal designs obtained by this process save up to 20% in cross-section area compared with common circular void designs from market, which is taken as a comparison pattern. The results show that traditional designs are deficient when the fire resistant constraint is considered, so that precast manufacturers and machinery designers should use optimization techniques to modify their hollow core slab geometry.Albero Gabarda, V.; Saura Arnau, H.; Hospitaler Pérez, A.; Montalva Subirats, JM.; Romero, ML.     Eurasian otter Lutra lutra distribution and habitat use in a Mediterranean catchment managed for the control of invasive giant reed Arundo donax

    We assessed the distribution and habitat use of Eurasian otter Lutra lutra on the middle course of the River Segura and its tributaries (Murcia, SE Spain) within the framework of the project LIFE13BIO/ES/001407 RIPISILVANATURA (2014–2019). Between April 2016 and June 2018, we surveyed for otter spraints 42 sampling stretches (mean length ± SD = 0.5 ± 0.2 km), covering a 109.8 km long section of the river. The study time frame coincided with the main restoration activities of the LIFE project, consisting in the eradication of widespread, invasive alien giant reed Arundo donax and plantation of autochthonous riparian vegetation. Otter habitat preferences were assessed by measuring ten environmental parameters potentially affecting its occurrence. The relationship between the presence/absence of the otter and habitat variables was evaluated by both linear multiple regression and non-parametric random forest regression model. Otters preferred river stretches crossing forested areas, avoided polluted waters and their occurrence was inversely related to the width of the emerged aquatic vegetation, mostly consisting of Arundo reed beds. Moreover, in managed stretches, otter marking intensity increased after the removal of alien reeds and was higher in RIPISILVANATURA restored transects (1.18 spraints/100 m) than in the downstream, unmanaged half of the watercourse (0.76 spraints/100 m). The combined effects of water pollution and giant reed spread may alter fish distribution and abundance, reducing prey availability to otters. The improvement of chemical and physical habitat quality of streams through alien reeds removal seems thus pivotal to enhance the recovery of otter's populations in the south-eastern Iberian Peninsula

    Ajustando RFR por Preditores de Discordância, “The Adjusted RFR”: Uma Metodologia Alternativa para Melhorar a Capacidade Diagnóstica de Índices Coronarianos

    Background: Cutoff thresholds for the "resting full-cycle ratio" (RFR) oscillate in different series, suggesting that population characteristics may influence them. Likewise, predictors of discordance between the RFR and fractional flow reserve (FFR) have been documented. The RECOPA Study showed that diagnostic capacity is reduced in the RFR "grey zone", requiring the performance of FFR to rule out or confirm ischemia. Objectives: To determine predictors of discordance, integrate the information they provide in a clinical-physiological index, the "Adjusted RFR", and compare its agreement with the FFR. Methods: Using data from the RECOPA Study, predictors of discordance with respect to FFR were determined in the RFR "grey zone" (0.86 to 0.92) to construct an index ("Adjusted RFR") that would weigh RFR together with predictors of discordance and evaluate its agreement with FFR. Results: A total of 156 lesions were evaluated in 141 patients. Predictors of discordance were: chronic kidney disease, previous ischemic heart disease, lesions not involving the anterior descending artery, and acute coronary syndrome. Though limited, the "Adjusted RFR" improved the diagnostic capacity compared to the RFR in the "grey zone" (AUC-RFR = 0.651 versus AUC-"Adjusted RFR" = 0.749), also showing an improvement in all diagnostic indices when optimal cutoff thresholds were established (sensitivity: 59% to 68%; specificity: 62% to 75%; diagnostic accuracy: 60% to 71%; positive likelihood ratio: 1.51 to 2.34; negative likelihood ratio: 0.64 to 0.37). Conclusions: Adjusting the RFR by integrating the information provided by predictors of discordance to obtain the "Adjusted RFR" improved the diagnostic capacity in our population. Further studies are required to evaluate whether clinical-physiological indices improve the diagnostic capacity of RFR or other coronary indices.Fundamento: Os limiares de corte para a “relação do ciclo completo de repouso” (RFR) oscilam em diferentes séries, sugerindo que as características da população podem influenciá-los. Da mesma forma, foram documentados preditores de discordância entre a RFR e a reserva de fluxo fracionado (FFR). O Estudo RECOPA, mostrou que a capacidade diagnóstica está reduzida na “zona cinzenta” da RFR, tornando necessária a realização de FFR para descartar ou confirmar isquemia. Objetivos: Determinar os preditores de discordância, integrar as informações que eles fornecem em um índice clínico-fisiológico: a “RFR Ajustada”, e comparar sua concordância com o FFR. Métodos: Usando dados do Estudo RECOPA, os preditores de discordância em relação à FFR foram determinados na “zona cinzenta” da RFR (0,86 a 0,92) para construir um índice (“RFR Ajustada”) que pesaria a RFR juntamente com os preditores de discordância e avaliar sua concordância com a FFR. Resultados: Foram avaliadas 156 lesões em 141 pacientes. Os preditores de discordância foram: doença renal crônica, cardiopatia isquêmica prévia, lesões não envolvendo a artéria descendente anterior esquerda e síndrome coronariana aguda. Embora limitada, a “RFR Ajustada” melhorou a capacidade diagnóstica em comparação com a RFR na “zona cinzenta” (AUC-RFR = 0,651 versus AUC-“RFR Ajustada” = 0,749), mostrando também uma melhora em todos os índices diagnósticos quando foram estabelecidos limiares de corte otimizados (sensibilidade: 59% a 68%; especificidade: 62% a 75%; acurácia diagnóstica: 60% a 71%; razão de verossimilhança positiva: 1,51 a 2,34; razão de verossimilhança negativa: 0,64 a 0,37). Conclusões: Ajustar a RFR integrando as informações fornecidas pelos preditores de discordância para obter a “RFR Ajustada” melhorou a capacidade diagnóstica em nossa população. Mais estudos são necessários para avaliar se os índices clínico-fisiológicos melhoram a capacidade diagnóstica da RFR ou de outros índices coronarianos

    The COVID-19 pandemic represents a valuable opportunity to carry out cohort studies that allow us to advance our knowledge on pathophysiological mechanisms of neuropsychiatric diseases. One of these opportunities is the study of the relationships between inflammation, brain development and an increased risk of suffering neuropsychiatric disorders. Based on the hypothesis that neuroinflammation during early stages of life is associated with neurodevelopmental disorders and confers a greater risk of developing neuropsychiatric disorders, we propose a cohort study of SARS-CoV-2-infected pregnant women and their newborns. The main objective of SIGNATURE project is to explore how the presence of prenatal SARS-CoV-2 infection and other non-infectious stressors generates an abnormal inflammatory activity in the newborn. The cohort of women during the COVID-19 pandemic will be psychological and biological monitored during their pregnancy, delivery, childbirth and postpartum. The biological information of the umbilical cord (foetus blood) and peripheral blood from the mother will be obtained after childbirth. These samples and the clinical characterisation of the cohort of mothers and newborns, are tremendously valuable at this time. This is a protocol report and no analyses have been conducted yet, being currently at, our study is in the recruitment process step. At the time of this publication, we have identified 1,060 SARS-CoV-2 infected mothers and all have already given birth. From the total of identified mothers, we have recruited 537 SARS-COV-2 infected women and all of them have completed the mental health assessment during pregnancy. We have collected biological samples from 119 mothers and babies. Additionally, we have recruited 390 non-infected pregnant women
