Data Mining techniques are used in the process of knowledge discovery for many domains’ problems. Feature Selection plays a vital role for a large number of datasets. In this research work, the model involves two main phases (i) Feature Selection and (ii) Classification. Since the length of feature vector tends to high, optimal feature selection technology is included, from which the most relevant features are selected by the Lion-based Firefly Algorithm which is referred as Optimization based Feature Selection Method (OFSM). The main objective of this paper is projected on minimizing the correlation between the selected features, which results in providing diverse information regarding the different classes of data. Once, the optimal features are selected, the classification algorithm called Neural Network (NN) is adopted, which can classify the data in an effective manner with the selected features

    A prospective observational study of adverse drug reactions to antiretroviral therapy: type and risk factors in a tertiary care teaching hospital

    Background: To collect demographic details of patients receiving antiretroviral therapy (ART) and study type of adverse drug reactions (ADRs) and risk factors for ADRs to ART and to assess causality, severity, and preventability assessment of the reported ADRs.Methods: A prospective observational study was conducted for 6 months from January 2012 until June 2012 at ART Center, KR Hospital of Mysore Medical College & Research Institute, Mysore. Data were evaluated for patient demography, risk factors for ADRs, type of ADRs. ADRs were also assessed for their causality, severity, and preventability as per the standard algorithm, using SPSS for windows (version 16.0).Results: Out of 158 patients evaluated, majority were of age group of 21-40 years (66.5%). More number of illiterate patients (55.7%) showed ADRs to ART. Most patients were of CD4 count <250 cells/μl (65.82%). Most common regimen which caused ADRs was zidovudine + lamivudine + nevirapine. Most common type of ADRs was anemia (55.06%) and rash (25.31%). On evaluation of the causality of ADRs, majority were found to be possible (89.24%). The severity assessment showed that most of the patients ADRs were of level 3 (93.05%). The preventability assessment showed that 30.38% patients ADRs were preventable.Conclusion: Identifying risk factors are of crucial importance to optimize the initial choice of ARVs regimen before initiating therapy and to prevent severity and complications caused by ART, thereby improving the quality of care to patients on ART

    Mature Cystic Teratoma with Squamous Cell Carcinoma-Rare Case Presentation

    Introduction: Mature cystic teratomas are part of a subclass of ovarian germ-cell tumour believed to arise from the primordial germ cells. Ovarian germ-cell tumours account for around 20–25% of ovarian neoplasms and 5% of ovarian cancers. A secondary malignant transformation of the various tissue components of mature cystic teratoma can occur, typically in postmenopausal women.&nbsp; More than 80% of malignant transformations are squamous-cell carcinomas arising from the ectoderm; the rest are carcinoid tumours or adenocarcinomas. Methods and Methodology, Case Report: A 40-year-old postmenopausal patient came with lower abdominal pain past 2 months. &nbsp;The patient was submitted to a gynecological examination and to transvaginal ultrasound, which confirmed the presence of right adnexal mass measuring 11×8x 2 cm; the mass proved to have cystic features in association with intracystic fat, raising the suspicion of an ovarian teratoma. In addition, areas of acoustic shadowing were discovered, raising the suspicion of a Rokitansky nodule exhibiting solid components such as hair and teeth. Pelvic CT scan demonstrated right adnexal dermoid cyst causing mild hydroureteronephrosis. A total hysterectomy with bilateral adnexectomy was performed, and the specimen was submitted for histopathological examination. Histopathological examination revealed Mature Cystic Teratoma with Squamous Cell Carcinoma. Discussion: Ovarian teratoma develops from germ cells and might present different cellular types originating from one or more of the germ layers, represented by endoderm, ectoderm and mesoderm.&nbsp; Of this malignant transformation is about 1- 2 %. Malignant transformation of ovarian teratoma can arise from any type of germ cell that is present at the level of these tumors; therefore, adenocarcinomas, squamous cell carcinomas, sarcomas, melanomas, adenosquamous carcinomas or even carcinoid tumors might occur. Of this squamous cell carcinoma is common. Conclusion: Although ovarian teratomas are frequently encountered, a small proportion of them will develop further complications, such as infection or malignancy.     Formulation of Sodium Alginate Nanospheres Containing Amphotericin B for the Treatment of Systemic Candidiasis

    Purpose: The aim of this work was to formulate sodium alginate nanospheres of amphotericin B by controlled gellification method and to evaluate the role of the nanospheres as a “passive carrier” in targeted antifungal therapy. Methods: Sodium alginate nanospheres of amphotericin B were prepared by controlled gellification method, and the particle size analysis was carried out by scanning electron microscopy. The carrier capacity of sodium alginate was evaluated in terms of drug to polymer ratio. In vitro release study was carried out on all drug loaded nanospheres by the dialysis method. Release kinetics of drug from different drug loaded nanospheres was also determined. The in vivo antifungal efficacy of nanospheres bound drug vis-à-vis the free drug was evaluated in candidiasis- induced mice models. Results: Preparation of nanospheres through controlled gellification method yielded particles with a size range of 419.6 ± 0.28 nm. Studies on drug to polymer ratio showed a linear relationship between concentration of drug and drug loading capacity. In vitro release kinetic study revealed that the release of drug from the nanospheres followed Fickian diffusion. In vivo studies showed that the nanosphere-bound drug produced a higher antifungal efficacy than the free drug. Conclusion: The formulated sodium alginate nanospheres containing amphotericin B was found to have better antifungal activity when compared to the free drug and also yielded sustained in vitro release.     CHARACTERIZATION OF Pr6O11 DOPED ZINC FLUOROBORATE GLASS

    Pr3+ doped zinc fluoroborate glasses with the chemical composition [(mol %) 30ZnF2 – 20 TeO2 – (50-x) B2O3 – xPr6O11] (where x = 0.0, 0.1, 0.5, 1.0 and 1.5 mol %) of different concentration of rare earth element praseodymium (Pr3+) have been prepared by conventional melt quench technique. The physical parameters like density, refractive index, number density, molar refraction, molar electronic polarizability, electronic polarizability, dielectric constant, polaron radius, inter–ionic separation, molar volume and oscillator strength of these glasses were calculated as a function of dopant concentration. The densities and refractive indices of these glasses were found to be in the range 2.0 g/cm3 - 2.67 g/cm3 and 1.644 – 1.73 respectively.     Effective One-Dimensional Coupling in the Highly-Frustrated Square-Lattice Itinerant Magnet CaCo2y_{\mathrm{2}-y}As2_{2}

    Inelastic neutron scattering measurements on the itinerant antiferromagnet (AFM) CaCo2y_{\mathrm{2}-y}As2_{2} at a temperature of 8 K reveal two orthogonal planes of scattering perpendicular to the Co square lattice in reciprocal space, demonstrating the presence of effective one-dimensional spin interactions. These results are shown to arise from near-perfect bond frustration within the J1J_1-J2J_2 Heisenberg model on a square lattice with ferromagnetic J1J_1, and hence indicate that the extensive previous experimental and theoretical study of the J1J_1-J2J_2 Heisenberg model on local-moment square spin lattices should be expanded to include itinerant spin systems

    Pemphigus Vegetans -Masquerading as malignancy of lip A Case Report

    Pemphigus vegetans is a rare variant of pemphigus vulgaris characterized by pustules or papillomatous vegetations, preferentially affecting intertriginous and periorificial areas. Exceptional manifestations may be misdiagnosed resulting in delayed diagnosis and treatment.&nbsp; We herein report an unusual manifestation of Pemphigus Vegetans in a 58-year-old female presented with history of swelling and pain in the lower lip for 4 months. Suspecting malignancy of lip-edge wedge biopsy of the lesion was done. On examination it was a proliferative lesion of size 2 x 1 cm in the lower lip, irregular in shape with no discharge. On cut section a grey white to grey brown soft tissue fragment measuring 1.5 x 1 x 0.7 cm lesion was received. Microscopic examination of multiple sections showed a polypoidal ulcerative swelling with marked pseudoepitheliomatous hyperplasia, hyperkeratosis, marked irregular acanthosis and a focus showing suprabasal clefting. The base of the supra basal cleft showed villi formation covered by stratum basalis cells giving “tomb stone appearance”. Dermis showed marked inflammatory infiltrates consisting of intraepidermal eosinophils, numerous plasma cells and lymphocytes, along with inflammatory infiltrates in the minor salivary glands. There is no evidence of malignancy. On further evaluation with immunofluorescence intercellular deposition of IgG and C3 in a fishnet pattern was seen confirming the diagnosis of Pemphigus vegetans
