    Prevalência do aleitamento materno em recém-nascidos de baixo peso

    The present situation regarding the breast feeding of children born with a weight of 2,500 grammes or less (LWB) at the Hospital das Clínicas of the Medical School of the Universidade de S. Paulo in Ribeirão Preto (Brazil) was studied. The study was descriptive and the data were obtained through a longitudinal follow-up carried over the period from October 1985 to September 1986 inclusive. Of the 222 children included in the study, 30 (13.5%) infants were not breastfed. Thus the incidence of breast feeding among this population was of 86.5%. At 3 months 62.5% of these infants were still being breastfed. At 6 months this figure fell to 38.5% and only 25.0% were still being breast-fed at the end of 12 months. The average duration of breast feeding for the 192 children that had started it was of 4 months. An association was found between the level of literacy of the mothers and the duration of breast feeding. Birthweight was found to be associated with the incidence and duration of breast feeding in general, as well as with the incidence of exclusive breast feeding. These data are considerably lower than recent figures obtained from the populations of well-developed countries, especially than those of Finnish infants born with a weight of less than 1.500 g. Action programmes involving the community and/or the Institution must be implemented in order to improve these figures.Estudou-se de forma longitudinal e prospectiva a duração do aleitamento materno no primeira ano de vida de 222 crianças nascidas com peso menor ou igual a 2.500 gramas no Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto (Brasil), no período de outubro de 1985 a setembro de 1986. A incidência do aleitamento materno foi de 86,5%. A prevalência encontrada para as crianças que foram amamentadas foi de 62,5% aos 3 meses, 38,5% aos 6 meses, 28,7% aos 9 meses e 25,0% aos 12 meses de vida. A mediana da duração do aleitamento materno foi de 4 meses. O peso ao nascer mostrou associação positiva com a incidência e duração do aleitamento materno, O nível de escolaridade materno associou-se positivamente com sua duração. A idade materna, situação conjugal e o número de consultas pré-natais não apresentaram associação com o aleitamento materno. Estes dados mostram índices menores em relação aos de populações de países desenvolvidos, em especial os pré-termos finlandeses nascidos com peso menor do que 1.500 gramas. Programas de ações envolvendo a comunidade e/ou instituições devem resultar em melhoria de nossos índices, se bem implementados

    Las rickettsiosis transmitidas por garrapatas son afecciones de distribución mundial, que por diferentes motivos se pueden considerar emergentes y reemergentes. Hasta hace escasos años la única rickettsiosis transmitida por garrapatas en Latinoamérica era la infección por Rickettsia rickettsii, pero en la actualidad y fundamentalmente, gracias a la incorporación de nuevas herramientas para el diagnóstico microbiológico como la reacción en cadena de la polimerasa y secuenciación o el cultivo celular rápido en tubo cerrado, se han descrito e involucrado otras especies de Rickettsia en la producción de patología humana. En estas guías se detallan y describen las diferentes técnicas utilizadas para el diagnóstico microbiológico de las rickettsiosis. Además, se incluye una sección en la que se detallan las especies más importantes de garrapatas duras relacionadas con las rickettsiosis en Latinoamérica, con claves para su clasificación taxonómica.Tick-borne rickettsioses are worldwide infectious diseases that are considered emerging and re-emerging. Until recently the only tick-borne rickettsiosis present in Latin America was Rickettsia rickettsii infection, but to date, with the incorporation of new tools as PCR and sequencing and the quick cellular close tube cultures (Shell-vial), new species has been involved as human pathogens. In these guidelines, we offer an update of the microbiological assays for diagnosing rickettsioses.     Data on genus and infectious by Rickettsia were retrospectively compiled from the critical review literature regarding all countries in Latin America, Caribbean islands, Portugal and Spain. We considered all Rickettsia records reported for human and/or animal hosts, and/or invertebrate hosts considered being the vector. In a few cases, when no direct detection of a given Rickettsia group or species was available for a given country, the serologic method was considered. A total of 13 Rickettsia species have been recorded in Latin America and the Caribbean. The species with the largest number of country confirmed records were Rickettsia felis (9 countries), R. prowazekii (7 countries), R. typhi (6 countries), R. rickettsii (6 countries), R. amblyommii (5 countries), and R. parkeri (4 countries). The rickettsial records for the Caribbean islands (West Indies) were grouped in only one geographical area. Both R. bellii, R. akari, and Candidatus ‘R. andeane’ have been recorded in only 2 countries each, whereas R. massiliae, R. rhipicephali, R.monteiroi, and R. africae have each been recorded in a single country (in this case, R. africae has been recorded in nine Caribbean Islands). For El Salvador, Honduras, and Nicaragua, no specific Rickettsia has been reported so far, but there have been serological evidence of human or/and animal infection. The following countries remain without any rickettsial records: Belize, Venezuela, Guyana, Surinam, and Paraguay. In addition, except for a few islands, many Caribbean islands remain without records. A total of 12 Rickettsia species have been reported in Spain and Portugal: R. conorii, R. helvetica, R. monacensis, R. felis, R. slovaca, R. raoultii, R. sibirica, R. aeschlimannii, R. rioja, R. massiliae, R. typhi, and R. prowazekii. Amongst these Rickettsia species reported in Spain and Portugal, only R. prowazekii, R. typhi, R. felis, and R. massiliae have also been reported in Latin America. This study summarizes the current state of art on the rickettsial distribution in Latin America, Caribbean, Spain and Portugal. The data obtained allow a better understanding on rickettsial epidemiology and distribution of vector ecology.Reportes del genero Rickettsia y sus asociadas infecciones fueron compilados en una revisión crítica retrospectiva de la literatura científica de los países de Latinoamérica, el Caribe, Portugal y España. Se consideraron todos los reportes para huéspedes humanos y/o animales y también para huéspedes invertebrados los cuales fueron considerados como vectores asociados con Rickettsia. En algunos casos, cuando no existió detección directa a un determinado grupo de rickettsias o especies no disponible en un país, se tuvo en cuenta la detección indirecta por serología. Un total de 13 especies de Rickettsia han sido reportadas en Latinoamérica y el Caribe. Las especies más encontradas en los países fueron: Rickettsia felis (9 países), R. prowazekii (7 países), R. typhi (6 países), R. rickettsii (6 países), R. amblyommii (5 países) y R. parkeri (4 países). Los datos de las islas del Caribe (antillas menores o Indias occidentales), fueron agrupados en una sola área geográfica como un solo país. Ambas R. bellii, R. akari y Candidatus ‘R. andeane’ fueron reportadas en solo 2 países, mientras que R. massiliae, R. rhipicephali, R.monteiroi, y R. africae fueron informadas en un solo país. En este caso R. africae fue reportada en 9 islas de las Antillas menores. Para El Salvador, Honduras y Nicaragua, hasta ahora no se han reportado especies de Rickettsia, pero si evidencia serológica de infección humana y/o animal. Sin reportes de infección por Rickettsia permanecen: Belice, Venezuela, Guayana, Surinam y Paraguay. Además, a excepción de algunas islas del Caribe, muchas de ellas permanecen sin reportes. Un total de 12 especies de Rickettsia han sido documentadas en España y Portugal: R. conorii, R. helvetica, R. monacensis, R. felis, R. slovaca, R. raoultii, R. sibirica, R. aeschlimannii, R. rioja, R. massiliae, R. typhi y R. prowazekii. Entre estas, solamente R. prowazekii, R. typhi, R. felis y R. massiliae han sido documentados en Latinoamérica, España y Portugal.     La importancia de la producción de leña en la región chaqueña pone particular interés en fomentar el uso sustentable de este recurso, como fuente de trabajo e ingresos para las familias productoras. Para ello el presente trabajo se propone analizar la demanda doméstica de leña y carbón, en Santiago del Estero, Catamarca, Tucumán y Córdoba. Se realizaron 923 encuestas a consumidores de leña y/o carbón durante el año 2013, con el fin de conocer cuál es el consumo anual de leña y carbón, cuáles son sus principales usos, las fuentes de abastecimiento, el precio al cual adquieren estos productos, cuál es la valoración relativa de diferentes atributos de los combustibles como la leña, el carbón y el gas y con qué otros productos se asocia la leña. Los resultados obtenidos indican que el principal uso doméstico de la leña y el carbón es la cocción de alimentos, el carbón se consume en más hogares que la leña, pero ésta presenta mayor consumo en kg per cápita que el carbón y presenta mayor diversidad de usos. Se visualiza además la ausencia de un sistema de logística en provisión y venta a un precio de referencia para la leña. La leña y el gas se presentan como productos más atractivos que el carbón para la cocción de alimentos, pero la dificultad para conseguir leña puede ser una limitante si se promociona como un producto de consumo masivo.The importance of producing firewood in the Chaco region sets particular interest in promoting the sustainable use of this resource, as a source of family producers labor and income. The present work intends to analyze the domestic demand of firewood and coal, in Santiago del Estero, Catamarca, Tucumán and Córdoba. To know what the annual consumption of firewood and charcoal is as well as its main uses, the sources of supply, the price they pay for these products, their relative valuation of the different features of the fuels (firewood, coal and gas) and to what other products firewood is associated, 923 surveys were conducted on consumers of firewood and / or coal in 2013. The results obtained indicate that the main domestic use of firewood and charcoal is cooking; most homes consumed coal instead firewood though its consumption per capita is higher than coal and presents greater possibilities of uses. The absence of a logistic system for the supply of frewood and of a reference price for its sale is observed. Firewood and gas appear as more attractive products than coal for cooking, but the difficulty in obtaining firewood can be a drawback in case it is promoted as a product of mass consumption.EEA Santiago del EsteroFil: De Bedia, Gonzalo Rafael.     Consensus Statement on Bone Conduction Devices and Active Middle Ear Implants in Conductive and Mixed Hearing Loss

    Nowadays, several options are available to treat patients with conductive or mixed hearing loss. Whenever surgical intervention is not possible or contra-indicated, and amplification by a conventional hearing device (e.g., behind-the-ear device) is not feasible, then implantable hearing devices are an indispensable next option. Implantable bone-conduction devices and middle-ear implants have advantages but also limitations concerning complexity/invasiveness of the surgery, medical complications, and effectiveness. To counsel the patient, the clinician should have a good overview of the options with regard to safety and reliability as well as unequivocal technical performance data. The present consensus document is the outcome of an extensive iterative process including ENT specialists, audiologists, health-policy scientists, and representatives/technicians of the main companies in this field. This document should provide a first framework for procedures and technical characterization to enhance effective communication between these stakeholders, improving health care

    Distinct patterns of somatic alterations in a lymphoblastoid and a tumor genome derived from the same individual

    Although patterns of somatic alterations have been reported for tumor genomes, little is known on how they compare with alterations present in non-tumor genomes. A comparison of the two would be crucial to better characterize the genetic alterations driving tumorigenesis. We sequenced the genomes of a lymphoblastoid (HCC1954BL) and a breast tumor (HCC1954) cell line derived from the same patient and compared the somatic alterations present in both. The lymphoblastoid genome presents a comparable number and similar spectrum of nucleotide substitutions to that found in the tumor genome. However, a significant difference in the ratio of non-synonymous to synonymous substitutions was observed between both genomes (P = 0.031). Protein–protein interaction analysis revealed that mutations in the tumor genome preferentially affect hub-genes (P = 0.0017) and are co-selected to present synergistic functions (P < 0.0001). KEGG analysis showed that in the tumor genome most mutated genes were organized into signaling pathways related to tumorigenesis. No such organization or synergy was observed in the lymphoblastoid genome. Our results indicate that endogenous mutagens and replication errors can generate the overall number of mutations required to drive tumorigenesis and that it is the combination rather than the frequency of mutations that is crucial to complete tumorigenic transformation
