    Utilidad de la escala qSOFA como predictor de mortalidad en pacientes politraumatizados del hospital regional docente de trujillo

    OBJETIVO: Determinar si la escala qSOFA tiene utilidad como predictor de mortalidad en pacientes politraumatizados del Hospital Regional Docente de Trujillo. MATERIAL Y MÉTODO: Estudio clínico observacional, analítico de corte transversal. Se incluyó 189 pacientes Politraumatizados atendidos en el Servicio de Trauma - Emergencia de Trauma del Hospital Regional Docente de Trujillo durante 3 años. Se registraron los datos generales de filiación, tiempo de enfermedad, hematocrito, lactato sérico, se registró la frecuencia respiratoria, presión arterial sistólica, puntaje en al escala de coma de Glasgow y se realizó el cálculo del score qSOFA; y la condición de alta del servicio. RESULTADOS: La escala qSOFA obtuvo una sensibilidad de 76%, una especificidad de 56%, un valor predictivo positivo de 50% y un valor predictivo negativo de 80%. Mostro un área bajo la curva ROC de 0.70 con mejor punto de corte un valor qSOFA ≥ 2. CONCLUSIONES: la escala qSOFA es de utilidad para predecir la mortalidad en pacientes politraumatizados.OBJECTIVE: To determine if the qSOFA scale has utility as a predictor of mortality in polytraumatized patients of the Hospital Regional Docente de Trujillo.     Clinical research in pediatric organ transplantation

    Solid organ transplantation has greatly improved survival in children with end-stage disease, becoming one of the main treatment options in this population. Nonetheless, there are significant challenges associated with validating and optimizing the effects of these interventions in clinical trials. Therefore, we reviewed the main issues related to conducting clinical transplantation research in children. We divided these challenges into three different categories: (i) challenges related to surgical techniques and anesthetic procedures, (ii) challenges related to post-transplant care and (iii) challenges specific to a particular population group and disease type. Some of the observed burdens for clinical research in this field are related to the limitations of conducting studies with a placebo or sham procedure, determining the standard of care for a control group, low prevalence of cases, ethical concerns related to use of a placebo control group and lack of generalizability from animal studies and clinical trials conducted in adult populations. To overcome some of these barriers, it is necessary to utilize alternative clinical trial designs, such as observational studies or non-inferiority trials, and to develop multicenter collaborations to increase the recruitment rate. In conclusion, the lack of robust data related to pediatric transplantation remains problematic, and further clinical trials are needed to develop more efficacious and safer treatments

    Personalidad e Intereses Vocacionales en estudiantes de secundaria de una institución educativa estatal – Guadalupe

    La presente investigación tiene como objetivo determinar la relación que existe entre personalidad e interés vocacional en estudiantes de secundaria de una institución estatal de Guadalupe. Es una investigación de tipo sustantiva con diseño descriptivo correlacional, participó una muestra de 193 estudiantes del quinto de secundaria de una institución educativa estatal de Guadalupe. Los instrumentos utilizados son: Cuestionario de los cinco grandes factores Big Five (BFQ) y el Inventario de Intereses Vocacionales y Ocupacionales CASM83-R10. Concluyendo que existe una correlación significativa (P<0.01) entre las dimensiones de la personalidad e intereses vocacionales de los estudiantes que cursan el quinto grado de secundaria de una institución. Evidenciando correlación muy significativa (p<01) entre las dimensiones energía y estabilidad emocional de la personalidad y las dimensiones Ciencias de la Comunicación, Artístico e Institutos Armados de los Intereses Vocacionales. A la vez existe una correlación muy significativa (p<01) entre la dimensión energía de la personalidad y las dimensiones Ciencias Sociales, Ciencias Naturales, Ciencias de la Comunicación, Ciencias Económico-Políticas e Institutos Armados de los Intereses Vocacionales. A la vez se evidencia una correlación muy significativa (p<01) entre la dimensión tesón de la personalidad y las dimensiones Ciencias Físico-Matemáticas, Burocracia, Finanzas y Jurisprudencia de los Intereses Vocacionales. Finalmente se evidencia una correlación muy significativa (p<01) entre la dimensión apertura mental de la Personalidad y la dimensión Lingüística de los Intereses Vocacionales.This research aims to determine the relationship between personality and vocational interest in high school students from a state institution in Guadeloupe. It is a substantive research with a descriptive correlational design, a sample of 193 students from the fifth year of secondary school from a state educational institution in Guadalupe participated. The instruments used are: The Big Five Factors Questionnaire (BFQ) and the CASM83-R10 Vocational and Occupational Interests Inventory. Concluding that there is a significant correlation (P <0.01) between the dimensions of the personality and vocational interests of the students who attend the fifth grade of secondary school of an institution. Showing a very significant correlation (p<01) between the dimensions of energy and emotional stability of the personality and the dimensions of Communication Sciences, Artistic and Armed Institutes of Vocational Interests. At the same time, there is a very significant correlation (p<01) between the energy dimension of personality and the Social Sciences, Natural Sciences, Communication Sciences, Economic-Political Sciences and Armed Institutes of Vocational Interests dimensions. At the same time, a very significant correlation (p<01) is evidenced between the tenacity dimension of personality and the Physical-Mathematical Sciences, Bureaucracy, Finance and Jurisprudence dimensions of Vocational Interests. Finally, a very significant correlation (p<01) is evidenced between the open-mindedness dimension of Personality and the Linguistic dimension of Vocational Interests.Tesi

    Responses of tomato (Solanum lycopersicum L.) plants to iron deficiency in the root zone

    Iron deficiency induces a yellowing in the aerial part of plants, known as iron chlorosis, and reduces the growth, yield, and quality of the fruits. Understanding plant response to iron deficiency is essential for agronomic management. This study decoded the temporal response of tomato plants (Solanum lycopersicum L.) to iron deficiency by quantifying different vegetative parameters. Subapical root swelling in the first 2.0 mm and several shoot and root growth parameters were measured in plants grown in a nutrient solution with and without Fe, on different dates designated as days after transplantation (DAT). Correlations between the total chlorophyll concentration in young leaves and 22 morphological and physiological parameters were also calculated. The plants grown in the absence of Fe had a higher number of secondary roots at 3 DAT, compared to control plants. On the same date, subapical root swelling was also observed, particularly at 1.5 and 2.0 mm from the root tip. Those plants also had a lower chlorophyll content in young leaves and a higher ferric-chelate reductase activity (FCR; EC 1.16.1.17) in the roots. At 9 DAT, the overall vegetative performance (plant height, fresh weight of stems and leaves) was negatively affected. At the end of the experiment (14 DAT), significant correlations were found between chlorophyll and the studied parameters. In conclusion, tomato plants experienced a cascade of responses to Fe deficiency throughout nine days: firstly, root lateralization increased; later, root swelling was observed, and a decrease in leaf chlorophyll content was registered associated with an increase in root FCR.     Modelización matemática y resolución numérica de problemas de combustión de carbón pulverizado

    El punto de partida de este trabajo es un código diseñado especialmente para la simulación de calderas en tres dimensiones, llamado SC3D, realizado a finales de los años 90 en la tesis doctoral de J.L. Ferrín (J. L. Ferrín. Algunas contribuciones a la modelización matemática de procesos de combustión de carbón. Tesis doctoral, Universidad de Santiago de Compostela, Departamento de Matemática Aplicada, 1999). Este programa fue realizado en el marco de la colaboración entre la empresa ENDESA Generación S.A. (en concreto, con la Central Térmica situada en As Pontes (Galicia)) y con el Departamento de Matemática Aplicada de la USC. El objetivo inicial de esta tesis era realizar la revisión del código SC3D para obtener un software con nuevos modelos, métodos numéricos más precisos, más rápido y sin algunas de las limitaciones que tenía el citado código, como el uso de mallas estructuradas. Esta actualización del código nos ha llevado a las tres contribuciones fundamentales de esta tesis: el estudio y ampliación de un nuevo modelo de combustión, así como su interpretación para poder ser implementado en un código de simulación, el desarrollo y análisis numérico de nuevos métodos numéricos precisos y estables y, por último, la validación de los modelos y métodos implementados con diferentes problemas

    Efficient distributed matrix-free multigrid methods on locally refined meshes for FEM computations

    This work studies three multigrid variants for matrix-free finite-element computations on locally refined meshes: geometric local smoothing, geometric global coarsening, and polynomial global coarsening. We have integrated the algorithms into the same framework-the open-source finite-element library deal.II-, which allows us to make fair comparisons regarding their implementation complexity, computational efficiency, and parallel scalability as well as to compare the measurements with theoretically derived performance models. Serial simulations and parallel weak and strong scaling on up to 147,456 CPU cores on 3,072 compute nodes are presented. The results obtained indicate that global coarsening algorithms show a better parallel behavior for comparable smoothers due to the better load balance particularly on the expensive fine levels. In the serial case, the costs of applying hanging-node constraints might be significant, leading to advantages of local smoothing, even though the number of solver iterations needed is slightly higher.Comment: 34 pages, 17 figure

    The perception of the afterlife in late medieval Castile through pastoral sources

    RESUMEN Este trabajo tiene como objetivo explorar la percepción bajomedieval del más allá a través del análisis de algunas fuentes doctrinales de carácter pastoral, de entre los siglos XIV y XV en Castilla, que muestran la gran presencia que estos elementos escatológicos tenían en la mentalidad medieval. Se pretende así dar una visión de la construcción mental de las ideas y los espacios asociados al más allá, de cómo se pensaba que se desarrollaba la existencia en ellos y a quién estaban destinados. Para ello, además, es necesario contar con las concepciones de muerte y pecado y su relación con el acceso a los espacios del infierno, purgatorio y paraíso; así como comprender las diferencias entre juicio particular y Juicio final. Después de presentar una síntesis del tema y su evolución a lo largo de la Edad Media, se tratará de buscar en algunas fuentes concretas si prevaleció la uniformidad o la pluralidad de visiones.ABSTRACT The objective of this paper is to explore the late medieval perception on the spaces of the Afterlife, through some doctrinal sources written between the XIVth and XVth centuries in Castile, that show the great presence these elements had within the medieval mentality. We intend to give a general perspective about the mental construction of the ideas and spaces associated to the Afterlife, how people thought their existence would carry on in them and to whom they where oriented. To do so it is necessary to understand the concept of death and sin so we comprehend how those Afterlife spaces were accessed and to understand the differences between particular judgement and Final Judgment.     We introduce a second order in time modified Lagrange--Galerkin (MLG) method for the time dependent incompressible Navier--Stokes equations. The main ingredient of the new method is the scheme proposed to calculate in a more efficient manner the Galerkin projection of the functions transported along the characteristic curves of the transport operator. We present error estimates for velocity and pressure in the framework of mixed finite elements when either the mini-element or the P2/P1P2/P1 Taylor--Hood element are used
