    Clinical study on the melarsoprol-related encephalopathic syndrome: risk factors and HLA association

    Melarsoprol administration for the treatment of late-stage human African trypanosomiasis (HAT) is associated with the development of an unpredictable and badly characterized encephalopathic syndrome (ES), probably of immune origin, that kills approximately 50% of those affected. We investigated the characteristics and clinical risk factors for ES, as well as the association between the Human Leukocyte Antigen (HLA) complex and the risk for ES in a case-control study. Late-stage Gambiense HAT patients treated with melarsoprol and developing ES (69 cases) were compared to patients not suffering from the syndrome (207 controls). Patients were enrolled in six HAT treatment centres in Angola and in the Democratic Republic of Congo. Standardized clinical data was obtained from all participants before melarsoprol was initiated. Class I (HLA-A, HLA-B, HLA-Cw) and II (HLA-DR) alleles were determined by PCR-SSOP methods in 62 ES cases and 189 controls. The principal ES pattern consisted in convulsions followed by a coma, whereas ES with exclusively mental changes was not observed. Oedema, bone pain, apathy, and a depressed humour were associated with a higher risk of ES, while abdominal pain, coma, respiratory distress, and a Babinski sign were associated with higher ES-associated mortality. Haplotype C*14/B*15 was associated with an elevated risk for ES (OR: 6.64; p-value: 0.008). Haplotypes A*23/C*14, A*23/B*15 and DR*07/B*58 also showed a weaker association with ES. This result supports the hypothesis that a genetically determined peculiar type of immune response confers susceptibility for ES

As of 25 November, 1,891 cases have been detected on the island where the vector Aedes aegypti had been established in some areas since 2005.     Debates about the purpose and content of history education in schools have been prevalent in most Westernised democratic nations over the last thirty years. At expense are essential questions concerning national identity/ies, competing narratives and the aims of history education. The impact of “history wars” have been felt within both Australia and England, as conservative commentators – including politicians and historians – have raised concerns about the depth and effectiveness of history education and have sought to make significant changes to the history curriculum for schools. This analysis examines the history wars in Australia and England, exploring the view that history education has been in danger and/or crisis and examining the curricular implications of a move toward greater recognition of national narratives. It raises some essential tensions that remain regarding two aspects of history teaching in both nations – (i) historiography and (ii) chronological understanding

    What do general practitioners know about ADHD? Attitudes and knowledge among first-contact gatekeepers: systematic narrative review

    Background: Attention Deficit Hyperactivity Disorder (ADHD) is a common childhood disorder with international prevalence estimates of 5 % in childhood, yet significant evidence exists that far fewer children receive ADHD services. In many countries, ADHD is assessed and diagnosed in specialist mental health or neuro-developmental paediatric clinics, to which referral by General (Family) Practitioners (GPs) is required. In such ‘gatekeeper’ settings, where GPs act as a filter to diagnosis and treatment, GPs may either not recognise potential ADHD cases, or may be reluctant to refer. This study systematically reviews the literature regarding GPs’ views of ADHD in such settings. Methods: A search of nine major databases was conducted, with wide search parameters; 3776 records were initially retrieved. Studies were included if they were from settings where GPs are typically gatekeepers to ADHD services; if they addressed GPs’ ADHD attitudes and knowledge; if methods were clearly described; and if results for GPs were reported separately from those of other health professionals. Results: Few studies specifically addressed GP attitudes to ADHD. Only 11 papers (10 studies), spanning 2000–2010, met inclusion criteria, predominantly from the UK, Europe and Australia. As studies varied methodologically, findings are reported as a thematic narrative, under the following themes: Recognition rate; ADHD controversy (medicalisation, stigma, labelling); Causes of ADHD; GPs and ADHD diagnosis; GPs and ADHD treatment; GP ADHD training and sources of information; and Age, sex differences in knowledge and attitudes. Conclusions: Across times and settings, GPs practising in first-contact gatekeeper settings had mixed and often unhelpful attitudes regarding the validity of ADHD as a construct, the role of medication and how parenting contributed to presentation. A paucity of training was identified, alongside a reluctance of GPs to become involved in shared care practice. If access to services is to be improved for possible ADHD cases, there needs to be a focused and collaborative approach to training

    The ATLAS level-1 Central Trigger

    The ATLAS Level-1 Central Trigger consists of the Muon-to-Central-Trigger-Processor Interface (MUCTPI), the Central Trigger Processor (CTP), and the Timing, Trigger and Control (TTC) partitions of the sub-detectors. The MUCTPI connects the output of the muon trigger system to the CTP. At every bunch crossing it receives information on muon candidates from each of the 208 muon trigger sectors and calculates the total multiplicity for each of six pT thresholds. The CTP combines information from the calorimeter trigger and the MUCTPI and makes the final Level-1 Accept (L1A) decision on the basis of lists of selection criteria (trigger menus). The MUCTPI and the CTP provide trigger summary information to the Level-2 trigger and to the data acquisition (DAQ) for every event selected at the Level-1. They further provide accumulated and, for the CTP, bunch-by-bunch counter data for monitoring of the trigger, detector and beam conditions. The TTC partitions send timing, trigger and control signals from the CTP to the sub-detectors and receive busy signals which can throttle the generation of L1As. The Local Trigger Processors (LTPs) normally receive the TTC signals from the CTP but can also generate them locally. The LTP interface (LTPIM) modules allow connecting of several LTPs for combined local running. The MUCTPI, the CTP and most of the TTC partitions of the ATLAS sub-detectors have been installed in the ATLAS experiment and are being used for commissioning tests with the trigger processors on the input and several sub-detectors as well as DAQ and Level-2 trigger on the output. Results of operating the Central Trigger in the experiment using trigger information from trigger processors connected to subdetectors observing cosmic rays will be shown

    Amplified Spontaneous Emission in Pentathienoacene Dioxides by Direct Optical Pump and by Energy Transfer: Correlation with Photophysical Parameters

    Amplified spontaneous emission (ASE) is observed, under optical pump, in polystyrene films doped with two pentathienoacene derivatives functionalised with thienyl-S,S-dioxide groups (compounds 2 and 3). The effect of the dioxide groups on the ASE properties is analysed by comparing the performance with that of its corresponding non-oxidized analogue (1). Films containing either 2 or 3 show ASE at 511 and 574 nm, respectively, when excited directly (at 435 nm) on their absorption bands, showing thresholds and linewidths larger than those obtained from films doped with 1, pumped at 355 nm. ASE is also observed under excitation at 355 nm, in samples containing 1 (host) and either 2 or 3 (guests), due to energy transfer from host to guest. For the blends with 3, the ASE threshold is lower than that obtained when the films are excited directly. Results are interpreted in terms of the photophysical parameters such as absorption capacity, fluorescence efficiency, singlet-to-triplet intersystem crossing leading to triplet-triplet re-absorptions, bimolecular energy-transfer efficiency, efficiency of internal conversion process, etc. State-of-the-art quantum chemical calculations are used in the interpretation of the experimental results.Authors from the University of Alicante acknowledge support from the Spanish Government (MINECO) and the European Community (FEDER) through grants MAT2008–06648-C02–01 and MAT2011–28167-C02–01.     ϕ\phi Meson Production in In-In Collisions and the ϕ\phi Puzzle

    The NA60 experiment measured dimuon production in In-In collisions at 158 AGeV. This paper presents a high statistics measurement of ϕ→ΌΌ\phi\to\mu\mu with the specific objective to provide insight on the ϕ\phi puzzle, i.e. the difference in the inverse TT slopes and absolute yields measured by NA49 and NA50 in the kaon and lepton channel, respectively. Transverse momentum distributions were studied as a function of centrality. The slope parameter TT shows a rapid increase with centrality, followed by a saturation. Variations of TT with the fit range of the order of 15 MeV were observed, possibly as a consequence of radial flow. The ϕ\phi meson yield normalized to the number of participants increases with centrality and is consistently higher than the yield measured by the NA49 experiment at any centrality.Comment: 4 Pages, 2 Figures.     Evidence for radial flow of thermal dileptons in high-energy nuclear collisions

    The NA60 experiment at the CERN SPS has studied low-mass dimuon production in 158 AGeV In-In collisions. An excess of pairs above the known meson decays has been reported before. We now present precision results on the associated transverse momentum spectra. The slope parameter Teff extracted from the spectra rises with dimuon mass up to the rho, followed by a sudden decline above. While the initial rise is consistent with the expectations for radial flow of a hadronic decay source, the decline signals a transition to an emission source with much smaller flow. This may well represent the first direct evidence for thermal radiation of partonic origin in nuclear collisions.Comment: Accepted for publication in Physical Review Letter

    Thermal dileptons at SPS energies

    Clear signs of excess dileptons above the known sources were found at the SPS since long. However, a real clarification of these observations was only recently achieved by NA60, measuring dimuons with unprecedented precision in 158A GeV, In-In collisions. The excess mass spectrum in the region M<1 GeV is consistent with a dominant contribution from pi+pi- -> rho -> mu+mu- annihilation. The associated rho spectral function shows a strong broadening, but essentially no shift in mass. In the region M>1 GeV, the excess is found to be prompt, not due to enhanced charm production. The inverse slope parameter Teff associated with the transverse momentum spectra rises with mass up to the rho, followed by a sudden decline above. While the initial rise, coupled to a hierarchy in hadron freeze-out, points to radial flow of a hadronic decay source, the decline above signals a transition to a low-flow source, presumably of partonic origin. The mass spectra show at low transverse momenta the steep rise towards low masses characteristic for Planck-like radiation. The polarization of the excess referred to the Collins Soper frame is found to be isotropic.
