    Literatur Review : Hubungan Tingkat Pendidikan Ibu Dengan Praktik Pemberian Makanan Prelakteal Pada Bayi Usia Kurang Dari 24 Bulan Di Indonesia.

    Background: Prelacteal feeding is a barrier to optimal breastfeeding practice, and increases the risk of neonatal illness and death. Factors that influence prelacteal feeding in children are maternal education and mother’s knowledge. Mother’s knowledge can be seen based on the level of education that has been taken previously. The higher education level of the mother, the more knowledge the mother has. Objective: This study aimed to find the correlation between maternal education level and the practice of giving prelacteal feeding of children aged less than 24 months in Indonesia. Methodology: The method used was systematic review. Articles were searched using google scholar whit the provisions of national indexed by Sinta S1-S4 and International indexed by Schimagojr Q1-Q4. Key words used prelacteal feeding” and “maternal education” and ”Indonesian” or “ determinant/ factor associated “ and “prelacteal feeding”. The inclusion criteria were relevant topic, subject were mothers aged 19-49 years who had children or babies aged less than 24 months, the study design included completely or independently was cross sectional and cohort, and full text articles published in English and Indonesian within the last 10 years. The exclusion criteria were articles that were not full text, did not report results specifically for factors influencing the practice of prelacteal feeding in infants. Results: The results of the study on the average prevalence of prelacteal feeding of children aged less than 25 months in Indonesia was 34,1%-51,9%. While mothers who successfully completed secondary education was 50%-60%, high level education was 17%-65%, and mothers who completed the lowest level of education was 20%-48%. There was only one article which showed correlation between the mother’s level of education and the practice of giving prelacteal feeding to infants. Conclusion: The conclusion obtained is that there is no significant correlation between maternal education level and the practice of giving prelacteal feeding of children aged less than 24 months in Indonesia.     Elevated plasma adiponectin levels in patients with chronic obstructive pulmonary disease

    BACKGROUND: Adiponectin is an anti-inflammatory adipokine that may play a role in chronic obstructive pulmonary disease (COPD) pathogenesis. OBJECTIVE: To investigate the relationship between adiponectin, interleukin (IL) 6, IL-8 and C-reactive protein (CRP) and COPD by evaluating these biomarkers in ever-smokers with or without the disease. METHOD: Plasma levels of adiponectin, IL-6, IL-8 and CRP were measured using commercially available kits in COPD patients (n = 71), healthy ever-smokers (n = 62) and non-smokers (n = 51). RESULTS: There were significant increases in plasma adiponectin, IL-6 and CRP in COPD patients (median [IQR] 4.39 μg/ml [2.68-6.98], 4.19 pg/ml [<2.40-6.40], 8.75 mg/l [4.26-40.63], respectively) compared to healthy ever-smokers (1.90 μg/ml [0.86-2.86], <2.40 pg/ml [<2.40-2.77], 3.71 mg/l [1.97-10.37 mg/l], respectively, P < 0.001) and non-smokers (1.76 μg/ml [1.34-2.52], <2.40 pg/ml [<2.40-2.78], 3.12 mg/l [2.11-5.71], respectively, P < 0.001). COPD patients had lower plasma IL-8 levels than healthy ever-smokers. Among ever-smokers with or without COPD, plasma adiponectin, IL-6 and CRP levels were inversely correlated with forced expiratory volume in 1 second (% predicted) after adjustment for age, body mass index, smoking status and pack-years.     Interpretation of uniocular and binocular trials of glaucoma medications: an observational case series

    <p>Abstract</p> <p>Background</p> <p>To predict the effectiveness of topical glaucoma medications based on initial uniocular and binocular treatment. To test a traditional hypothesis that effectiveness following a uniocular trial is associated with the change in IOP in the initially treated eye minus the change in the initially untreated eye. To determine whether uniocular or binocular treatment trials are superior.</p> <p>Methods</p> <p>Based on a review of medical records, we identified 168 instances in 154 patients with bilateral primary open angle glaucoma of initial uniocular use of a topical glaucoma medication with well-documented intraocular pressure (IOP) readings at baseline (IOP<sub>A</sub>), during the trial (IOP<sub>B</sub>), and at follow-up (IOP<sub>C</sub>). Abstracted data included demographic data, IOP, and medication use. Predictors of the IOP following the trial (IOP<sub>C</sub>) in each eye were identified by multivariable linear regression. In 70 cases, the predictive ability of initial uniocular and binocular treatment could be directly compared.</p> <p>Results</p> <p>In a multivariable analysis, the follow-up pressure in the initially treated eye (IOP<sub>1C</sub>) was directly correlated with treated eye IOP during initial uniocular use (IOP<sub>1B</sub>, p < 0.001). In a multivariable analysis, the follow-up pressure in the initially untreated eye (IOP<sub>2C</sub>) was directly correlated with its baseline IOP<sub>2A </sub>(p < 0.001), and also tended to be associated with treated IOP<sub>1B </sub>(p = 0.07). The multivariable regression coefficient (b) for the IOP change in the initially untreated eye was generally not close to the value of -1 expected by the classic teaching (for eye 1, b = 0.04, p = 0.35; for eye 2, b = 0.07, p = 0.50). In 70 cases, the uniocular and binocular trials predicted a similar fraction of the variance in follow-up IOP<sub>1C </sub>(r<sup>2 </sup>= 0.56 and 0.57, respectively) and IOP<sub>2C </sub>(r<sup>2 </sup>= 0.39 and 0.38, respectively).</p> <p>Conclusion</p> <p>1) For uniocular trials, the IOP change in the untreated eye should not be subtracted from that in the treated eye. 2) Uniocular and binocular trials have similar predictive value when interpreted correctly. Either may be selected based on clinical circumstances.</p

    Molecular mechanisms of severe acute respiratory syndrome (SARS)

    Severe acute respiratory syndrome (SARS) is a new infectious disease caused by a novel coronavirus that leads to deleterious pulmonary pathological features. Due to its high morbidity and mortality and widespread occurrence, SARS has evolved as an important respiratory disease which may be encountered everywhere in the world. The virus was identified as the causative agent of SARS due to the efforts of a WHO-led laboratory network. The potential mutability of the SARS-CoV genome may lead to new SARS outbreaks and several regions of the viral genomes open reading frames have been identified which may contribute to the severe virulence of the virus. With regard to the pathogenesis of SARS, several mechanisms involving both direct effects on target cells and indirect effects via the immune system may exist. Vaccination would offer the most attractive approach to prevent new epidemics of SARS, but the development of vaccines is difficult due to missing data on the role of immune system-virus interactions and the potential mutability of the virus. Even in a situation of no new infections, SARS remains a major health hazard, as new epidemics may arise. Therefore, further experimental and clinical research is required to control the disease
