    Frecuencia y perfil clínico de cáncer de vesícula biliar en pacientes colecistectomizados en 3 hospitales referenciales de chiclayo entre 2011 y 2016

    Objetivos: describir la frecuencia y perfil clínico de cáncer de vesícula biliar en pacientes colecistectomizados en 3 hospitales referenciales en Chiclayo entre 2011-2016. Materiales y métodos: estudio descriptivo transversal retrospectivo. El estudio está dividido en 2 fases, la primera consistió en contabilizar el número total de informes de patología de vesícula biliar y seleccionar los casos positivos para cáncer; la segunda consistió en recolectar datos de las historias clínicas de los pacientes positivos para cáncer. Resultados: de 5720 reportes de anatomía patológica de vesícula biliar, 58 (1,01%) fueron positivas para neoplasia vesicular. 42 casos (72,4%) fueron mujeres y solo 16 (27,6%) fueron hombres. El síntoma más frecuente fue dolor abdominal con 41 casos (70,7%). 18 pacientes (31%) refirieron otra sintomatología, la hiporexia fue la más frecuente con 8 casos (13,8%). 41 casos (70,7%) presentaron cálculos vesiculares. El tipo de cáncer que predomino fue el adenocarcinoma con 48 casos (82,8%) seguido por carcinoma mucinoso con 3 casos (5,2%). Conclusiones: en Lambayeque la frecuencia de cáncer de vesícula biliar fue de 1,01% con predominancia en el sexo femenino y sexta década de vida, siendo el adenocarcinoma, el tipo más común. Así mismo los pacientes presentaron con mayor frecuencia dolor abdominal e ictericia

    Ovulatory and embryonic response to recombinant bovine somatotropin in goats super ovulated with FSHp

    Artículo científico publicado en revista indizada.El objetivo del estudio fue evaluar el efecto de la somatotropina bovina recombinante (rBST) en la actividad ovárica, cantidad y calidad de embriones transferibles en cabras durante el anestro estacional. Se utilizaron 30 cabras multíparas sincronizadas con protocolo corto de CIDR y superovuladas con FSHp, asignadas a tres tratamientos (n = 10): T1 = control, sin rBST, T2 = 100 mg rBST y T3 = 200 mg rBST. Las variables cuantitativas se analizaron mediante prueba de Kruskal-Wallis; el porcentaje de estro y de embriones recuperados, mediante prueba de Chi - cuadrada. Las cabras del T3 tuvieron mayor número de CL, seguidas del T2 (p = 0.002). El número de embriones recuperados y de blastocitos fue mayor en T3 (p = 0.001). El número de mórulas fue mayor en T1 (p = 0.001). La aplicación de rBST al inicio del estro incrementa la actividad ovárica y la cantidad de embriones transferibles en cabras

    In vitro gas production kinetics and degradability of a diet for growing lambs: effect of fibrolytic enzyme products at different dose levels

    The objective of this study was to evaluate the effects of three fibrolytic enzyme products (cellulase (CEL), xylanase (XYL) and a 1:1 mixture of CEL and XYL (MIX)) at three dose levels (0, 1 and 3 lL/0.5 g DM) on the in vitro fermentation of a diet for growing lambs. Bottles were incubated for 96 h at 39 C. A mathematical model was used to estimate the parameters describing the gas production (GP) curve (b, c and L). Dry matter degradability (DMD) and fibre (NDFD and ADFD) degradability were determined at the end of the incubation period. Metabolisable energy (ME) and short chain fatty acids (SCFA) were calculated at 24 h of incubation. The asymptotic GP (parameter b) was affected (p<0.02) by enzyme product and dose level, with a significant linear response (p<0.05). Dose level affected ME and SCFA with a significant linear (p<0.05) and quadratic (p<0.01) response. The interaction between enzyme product and dose level was significant (p<0.05) for cumulative GP up to 72 and 96 h of incubation, pH, ADFD and DMD. The results suggest that application of exogenous cellulases has the potential to alter asymptotic GP and degradability of ADF and DM of a diet for growing lambs, but most of the results depend on the interaction between enzyme product and dose level. Future studies are required to determine the ideal combination between enzyme product and dose level for optimal degradation of ruminant feeds

    Effects of exogenous enzymes and application method on nutrient intake, digestibility and growth performance of Pelibuey lambs

    Pelibuey sheep is the main breed in the tropical and subtropical regions of Mexico, and high demand of sheep meat has favored the finishing of lambs in feedlots with diets containing high levels of grains. The objective of this study was to evaluate the effects of exogenous enzymes (EE) and application method on nutrient intake and digestibility and performance of growing Pelibuey lambs. Treatments were based on comparison of two different methods of adding an enzyme product (sprayed on the total mixed ration or applied orally to the lambs) versus control treatment (no added enzyme). Twenty-one Pelibuey lambs, weighing 15.7 kg (SD = 1.8 kg) initial body weight, were individually housed in shaded pens and assigned randomly to one of the three enzyme treatments. At the end of study (lasting for 45 days), three lambs from each treatment were randomly selected and adapted to a pants and harness designed for fecal collection to measure nutrient digestibilities. Total body gain and average daily gain were affected (P < 0.05) by supplemental EE. The application method of EE had significant (P < 0.05) effect on FCE and FCR, but no effects were observed on nutrient intake. Supplemental EE did improve (P < 0.05) the digestibilities of dry matter, organic matter, neutral and acid detergent fiber, but no differences were observed in crude protein digestibility. The application method of EE had significant (P < 0.05) effect on the digestibility of acid detergent fiber. Supplemental EE can improve body weight gain and nutrient digestibilities without affecting nutrient intake in Pelibuey lambs, but the results of feed conversion efficiency and acid detergent fiber digestibility depend on the application method used of the EE

    Investigation of the thermal and physicochemical behavior of two types of gutta-percha cones for back-filling the root canal

    Gutta-percha (Gp) is an inert thermoplastic polymer used as a filling to replace the dental pulp space, which has been reformulated to improve its three-dimensional sealing properties. Therefore, this study aimed to analyze the physical, chemical and thermal properties of two types of gutta-percha filling. As well as measuring the temperature distribution along the cone at the time of cutting through an in-situ test. MATERIAL AND METHODS: Two commercially available brands of gutta-percha point were investigated: Conform Fit TM Gutta-Percha for ProTaper Gold(R) (PTG) (Dentsply Sirona), and Hygenic Gutta-Percha (Coltene whaledent). Differential scanning calorimetry (DSC) and thermogravimetric analysis (TGA) were selected for the thermal characterization of materials, and Fourier Transform Infrared Spectroscopy (FT-IR) for the chemical analysis of Gp cones. Regarding temperature distribution, it was evaluated using a thermographic camera (FLIR ONE(R) PRO by MicroUSB P/N 435-0011-01) at 0 to 20 s after the cutting process (n=11/group). RESULTS: Both materials have three fusion endotherms associated with the three crystalline phases of Gp, with similar temperatures but enthalpies that differ by 60%, the fusion enthalpy being higher for Conform Fit. In the chemical characterization, elements such as Zn, C, O, Ba, S and Si were found in both materials but in different proportions. Regarding the content of fillers, the Conform Fit presented around 30% of Gp polymer and 25% for the Hygenic. The morphological characterization shows a microtexturized coating in the form of bars on a micrometric scale for the Conform Fit, which could favor a better three-dimensional seal. In addition to that, in heat transfer studies they showed greater temperature control. CONCLUSIONS: The characterization of the materials allowed us to see the variation in terms of their composition and configuration to the Gp cones of two commercial brands.     Trabajo de investigación cuya temática es la sincronización del estro en ovejas de pelo.This study aimed to evaluate the effect of reinserting controlled internal drug release (CIDR) devices after fixed-time artificial insemination (FTAI) on pregnancy rates in goats. Oestrus was synchronized with a short (5 days) CIDR protocol, and FTAI was conducted 54 hours after CIDR removal. According to the reinsertion of CIDR after FTAI, goats were assigned to three treatments: G0 control group (n = 29), no CIDR reinsertion; G7 group (n = 27), CIDR reinserted 7 days; G14 group (n = 29), CIDR reinserted 14 days. Blood samples were collected to determine progesterone levels. Oestrus appearance (92.9%), interval of CIDR removal to oestrus (IRE = 34.1 ± 1.1 hours), interval of oestrus onset to artificial insemination (IEAI = 20.2 ± 1.0 hours), mean duration of oestrus (38.4 ± 1.4 hours), and pregnancy rates (61.0%) were similar (P > .05) among groups. Progesterone concentrations were higher (P  .05) for all groups. In conclusion, reinsertion of CIDR for 7 or 14 days after a short oestrus synchronization protocol and FTAI did not increase the overall pregnancy rate of goats

    Introduction: Clostridium difficile is the main pathogen related to healthcare-associated diarrhea and it is the cause of 20 to 30% of diarrhea cases caused by antibiotics. In Colombia and Latin America, the knowledge about the epidemiological behavior of this infection is limited.Objective: To describe the characteristics of a series of patients with C. difficile infection.Materials and methods: We performed a descriptive case series study of patients with C. difficile infection hospitalized in the Fundación Clínica Shaio from January, 2012, to November, 2015.Results: We analyzed 36 patients. The average age was 65 years. The risk factors associated with the infection were: previous use of antibiotics (94.4%), prior hospitalization in the last three months (66.7%) and use of proton pump inhibitors (50%). The most common comorbidities were chronic kidney disease (41.7%) and diabetes mellitus (30.6%). The most frequent symptoms were more than three loose stools per day (97.1%) and abdominal pain (42.9%). According to the severity of the disease, 44.4% of cases were classified as mild to moderate, 38.9% as severe, and 11.1% as complicated or severe. The detection of the toxin by PCR (GeneXpert) was the most common diagnostic procedure (63.8%). Global mortality during hospitalization was 8%. We identified four strains with serotype NAP1/027 and nine samples positive for binary toxin.Conclusion: Clostridium difficile infection should be suspected in patients with diarrhea and traditional risk factors associated with this disease. We report the circulation of the hypervirulent strain serotype NAP1/027 in Colombia, which should be countered with epidemiological surveillance and a prompt diagnosis.Introducción. Clostridium difficile es el principal responsable de la diarrea asociada al uso de antibióticos. En Colombia y en Latinoamérica, el conocimiento sobre el comportamiento epidemiológico de la infección por C. difficile todavía es limitado.Objetivo. Describir las características de una serie de pacientes con infección por C. difficile.Materiales y métodos. Se hizo un estudio descriptivo de una serie de casos de pacientes con infección por C. difficile atendidos en la Fundación Clínica Shaio, entre enero de 2012 y noviembre de 2015.Resultados. Se estudiaron 36 pacientes con una edad promedio de 65 años. Se determinaron los siguientes factores relacionados con la infección por C. difficile: uso previo de antimicrobianos (94,4 %), hospitalización en los últimos tres meses (66,7 %) y uso de inhibidores de la bomba de protones (50 %). Las comorbilidades más comunes fueron la enfermedad renal crónica (41,7 %) y la diabetes mellitus (30,6 %). Los síntomas más frecuentes fueron más de tres deposiciones diarreicas (97,1 %) y dolor abdominal (42,9 %). En cuanto a la gravedad de los casos, 44,4 % se clasificó como leve a moderado, 38,9 % como grave, y 11,1 % como complicado o grave. El método de diagnóstico más utilizado (63,8% de los pacientes) fue la identificación de la toxina mediante reacción en cadena de la polimerasa (PCR). La mortalidad global durante la hospitalización fue de 8 %. Se identificaron cuatro cepas del serotipo NAP1/027 y nueve muestras fueron positivas para la toxina binaria.Conclusión. La infección por C. difficile debe sospecharse en pacientes con deposiciones diarreicas y factores asociados tradicionalmente a esta enfermedad. Se reportó la circulación de cepas hipervirulentas del serotipo NAP1/027 en Colombia, lo cual debe enfrentarse con la vigilancia epidemiológica y el diagnóstico tempran

    Non-productive angiogenesis disassembles Aß plaque-associated blood vessels

    The human Alzheimer’s disease (AD) brain accumulates angiogenic markers but paradoxically, the cerebral microvasculature is reduced around Aß plaques. Here we demonstrate that angiogenesis is started near Aß plaques in both AD mouse models and human AD samples. However, endothelial cells express the molecular signature of non-productive angiogenesis (NPA) and accumulate, around Aß plaques, a tip cell marker and IB4 reactive vascular anomalies with reduced NOTCH activity. Notably, NPA induction by endothelial loss of presenilin, whose mutations cause familial AD and which activity has been shown to decrease with age, produced a similar vascular phenotype in the absence of Aß pathology. We also show that Aß plaque-associated NPA locally disassembles blood vessels, leaving behind vascular scars, and that microglial phagocytosis contributes to the local loss of endothelial cells. These results define the role of NPA and microglia in local blood vessel disassembly and highlight the vascular component of presenilin loss of function in AD
