Caseload midwifery in a multi-ethnic community: The women's experiences
Objective: To evaluate caseload midwifery in a relatively deprived and ethnically diverse inner-city area.
Design and setting: Semi-structured interviews were undertaken with 24 women from diverse ethnic backgrounds, 12 of whom had received caseload care and 12 women from an adjacent area who had received conventional maternity care in a large inner-city maternity unit. Framework analysis was adopted drawing on links with the authors’ previous work on women’s views of caseload midwifery.
Findings: Key themes from previous work fitted well with the themes that emerged from this study. Themes included ‘knowing and being known’, ‘person-centred care’, ‘social support’, ‘gaining trust and confidence’, ‘quality and sensitivity of care’ and ‘communication’.
Key conclusions and implications: Women from this socially and ethnically diverse group of women had similar views and wanted similar care to those in previous studies of caseload midwifery. Many of the women receiving caseload care highlighted the close relationship they had with the midwives and as a result of thisfelt more able to discuss their concerns with them. This has the potential not only for improved quality of care but also improved safety
Present Limits on the Precision of SM Predictions for Jet Energies
We investigate the impact of theoretical uncertainties on the accuracy of
measurements involving hadronic jets. The analysis is performed using events
with a Z boson and a single jet observed in collisions at
= 1.96 TeV in 4.6 of data from the Collider Detector at
Fermilab (CDF). The transverse momenta (\pt) of the jet and the boson should
balance each other due to momentum conservation in the plane transverse to the
direction of the and beams. We evaluate the dependence of the
measured \pt-balance on theoretical uncertainties associated with initial and
final state radiation, choice of renormalization and factorization scales,
parton distribution functions, jet-parton matching, calculations of matrix
elements, and parton showering. We find that the uncertainty caused by parton
showering at large angles is the largest amongst the listed uncertainties. The
proposed method can be re-applied at the LHC experiments to investigate and
evaluate the uncertainties on the predicted jet energies. The distributions
produced at the CDF environment are intended for comparison to those from
modern event generators and new tunes of parton showering.Comment: Submitted to Nucl. Instr. and Meth.
A conformational RNA zipper promotes intron ejection during non-conventional XBP1 mRNA splicing.
The kinase/endonuclease IRE1 is the most conserved signal transducer of the unfolded protein response (UPR), an intracellular signaling network that monitors and regulates the protein folding capacity of the endoplasmic reticulum (ER). Upon sensing protein folding perturbations in the ER, IRE1 initiates the unconventional splicing of XBP1 mRNA culminating in the production of the transcription factor XBP1s, which expands the ER's protein folding capacity. We show that an RNA-intrinsic conformational change causes the intron of XBP1 mRNA to be ejected and the exons to zipper up into an extended stem, juxtaposing the RNA ends for ligation. These conformational rearrangements are important for XBP1 mRNA splicing in vivo. The features that point to such active participation of XBP1 mRNA in the splicing reaction are highly conserved throughout metazoan evolution, supporting their importance in orchestrating XBP1 mRNA processing with efficiency and fidelity

Remittances, exchange rate regimes, and the Dutch disease: a panel data analysis
Using disaggregated sectorial data, this study shows that rising levels of remittances have spending effects that lead to real exchange rate appreciation and resource movement effects that favor the nontradable sector at the expense of tradable goods production. These characteristics are two aspects of the phenomenon known as Dutch disease. The results further indicate that these effects operate more strongly under fixed nominal exchange rate regimes.Emigrant remittances ; Foreign exchange rates
Secondary pattern computation of an arbitrarily shaped main reflector
The secondary pattern of a perfectly conducting offset main reflector being illuminated by a point feed at an arbitrary location was studied. The method of analysis is based upon the application of the Fast Fourier Transform (FFT) to the aperture fields obtained using geometrical optics (GO) and geometrical theory of diffraction (GTD). Key features of the reflector surface is completely arbitrary, the incident field from the feed is most general with arbitrary polarization and location, and the edge diffraction is calculated by either UAT or by UTD. Comparison of this technique for an offset parabolic reflector with the Jacobi-Bessel and Fourier-Bessel techniques shows good agreement. Financial development, remittances, and real exchange rate appreciation
For developing countries, remittances are an important and expanding source of capital, equivalent to two-thirds of overall foreign direct investment and nearly 2 percent of gross domestic product. ; This article examines the relationship between remittance inflows, financial sector development, and the real exchange rate. The authors test whether financial sector development can prevent appreciation of the real exchange rate. In particular, they show that well-developed financial sectors can more effectively channel remittances into investment opportunities. ; Using panel data for 109 developing and transition countries for 1990–2003, the authors find that remittances by themselves tend to put upward pressure on the real exchange rate. But this effect is weaker in countries with deeper and more sophisticated financial markets, which seem to retain trade competitiveness.Emigrant remittances ; Foreign exchange rates
N<i>e</i>XOS – the design, development and evaluation of a rehabilitation system for the lower limbs
Recent years have seen the development of a number of automated and semi-automated systems to support for physiotherapy and rehabilitation. These deploy a range of technologies from highly complex purpose built systems to approaches based around the use of industrial robots operating either individually or in combination for applications ranging from stroke to mobility enhancement. The NeXOS project set out to investigate an approach to the rehabilitation of the lower limbs in a way which brought together expertise in engineering design and mechatronics with specilists in rehabilitation and physiotherapy. The resulting system has resulted in a prototype of a system which is capable in operating in a number of modes from fully independent to providing direct support to a physiotherapist during manipulation of the limb. Designed around a low cost approach for an implementation ultimately capable of use in a patients home using web-baased strategies for communication with their support team, the prototype NeXOS system has validated the adoption of an integrated approach to its development. The paper considers this design and development process and provides the results from the initial tests with physiotherapists to establish the operational basis for clinical implementation
