    Sensitization to isothiazolinones in the Spanish Contact Dermatitis Registry (REIDAC): 2019–2021 epidemiological situation

    Background: Current frequency and risk factors for sensitization to methylisothiazolinone (MI), methylchloroisothiazolinone/methylisothiazolinone (MCI/MI), benzisothiazolinone (BIT) and octylisothiazolinone (OIT) in Spain are not well known. Objectives: To study the frequency of sensitization, risk factors and simultaneous sensitization between the four isothiazolinones. Materials and Methods: We analysed all 2019-2021 consecutive patients patch-tested with MI (0.2% aq.), MCI/MI (0.02% aq.), BIT (0.1% pet.) and OIT (0.1% pet) within the Spanish Contact Dermatitis Registry (REIDAC). Results: A total of 2511 patients were analysed. Frequencies of sensitization were: any isothiazolinone 15.7%, MI 6.8%, MCI/MI 4.8%, BIT 3.5% and OIT 0.5%. MI and MCI/MI sensitization was associated with being occupationally active, hand dermatitis, detergents and age over 40. BIT sensitization was associated with leg dermatitis and age over 40. About one in nine MI-positive patients were positive to BIT, whereas one in five BIT-positive patients were positive to MI. Conclusions: Sensitization to MI, MCI/MI and BIT is still common in Spain, while sensitization to OIT is rare. Currently, sensitization to MI and MCI/MI seems to be occupationally related. Although its origin is unknown, sensitization to BIT is more frequent in patients aged over 40 years.     Con un enfoque para la docencia que plantea una universidad basada en el entusiasmo y en la participación a través de un modelo de aprendizaje que motive y sepa entusiasmar a los estudiantes, planteamos como proyecto de innovación para mejora de la calidad docente la creación de un programa de televisión sobre la cultura urbana vinculado a las asignaturas de: - "Producción y realización publicitaria" (Grado en Publicidad y Relaciones Públicas) - "Realización publicitaria" (Grado en Comunicación Audiovisual) - "Dirección cinematográfica" (Grado en Comunicación Audiovisual) - "Realización televisiva" (Grado en Comunicación Audiovisual) Y a los TFG relacionados con las áreas de conocimiento de realización y producción.Se crearían tres categorías en las que los alumnos podrían participar con sus trabajos: rodajes,reportajes audiovisuales y reportajes fotográficos y spots publicitarios. Los alumnos, tras haber pasado por alguna de las asignaturas o TFG propuestos, con sus respectivas prácticas, ya tienen un training que les capacita para sacar adelante un trabajo colaborativo, un proyecto y además les forma en las competencias profesionales necesarias para el trabajo en equipo. Además, el proyecto incluye la creación de una web y un laboratorio audiovisual (que ya está en marcha) para la difusión de los trabajos presentados y como herramienta para ampliar la proyección profesional y académica de los alumnos. El reto es poner Internet al servicio de la capacidad creativa y técnica de los estudiantes como fórmula para descubrir el nuevo talento, para poder experimentar con un nuevo medio al alcance de todos, y para conseguir una muy amplia difusión. Internet, lo interactivo, lo virtual y lo on-line serían las grandes herramientas para la realización de este proyecto. Es una experiencia de trabajo en el que varias asignaturas se unen y en el que la planificación y la nota final de cada una de las asignaturas se consolidan un proyecto real para dar a conocer el funcionamiento del mundo laboral.     [SPA] Este trabajo presenta el proceso seguido por el Equipo de Innovación Docente de Prácticas Externas del Centro Universitario de la Defensa de San Javier para el desarrollo de las prácticas curriculares de los alumnos del Grado de Ingeniería y futuros oficiales del Ejército del Aire, que se impartirán dentro del nuevo modelo de enseñanza militar. La metodología propuesta se presenta como un ejemplo de adaptación a un nuevo modelo de enseñanza dentro del EEES que busca solventar deficiencias de modelos universitarios previos como eran la falta de especificación de las tareas en las prácticas o la realización de otras no vinculadas a la titulación (Ballesteros-Velázquez et al. 2001). Se propone una adaptación de los contenidos de las prácticas a partir de un análisis in situ, de los centros donde se realizarán y utilizando como criterio para la selección de puestos y tareas su vinculación con asignaturas del grado y a profesionales con distinto rango de responsabilidad y cualificación. Así, el alumno puede aprender a utilizar las competencias concretas y necesarias para ejercer su profesión, porque las recibe de profesionales que están aplicando capacidades distintas en función de la etapa de la vida laboral en la que se encuentran. [ENG] This paper presents the process followed by the teaching innovation team at the Defence University Centre in San Javier for the design of a traineeship for Industrial Organization Engineering students, future officers of the Air Force, according to the new military education programme. The traineeship design steps are described as a specific example of a methodology adapted to a new specialized education programme within the European Space for Higher Education (EEES) in order to solve the limitations of educational models previous to EEES (Ballesteros-Velázquez et al. 2001). The authors propose in situ monitoring on the part of lecturers in each of the centres where students will carry out their traineeship to properly identify tasks and adapt the traineeship contents linking them to the degree courses and to reference professionals at different stages of their career.     Tobacco control policies in hospitals before and after the implementation of a national smoking ban in Catalonia, Spain

    Background: Diverse projects and guidelines to assist hospitals towards the attainment of comprehensive smoke-free policies have been developed. In 2006, Spain government passed a new smoking ban that reinforce tobacco control policies and banned completely smoking in hospitals. This study assesses the progression of tobacco control policies in the Catalan Network of Smokefree Hospitals before and after a comprehensive national smoking ban. Methods: We used the Self-Audit Questionnaire of the European Network for Smoke-free Hospitals to score the compliance of 9 policy standards (global score = 102). We used two crosssectional surveys to evaluate tobacco control policies before (2005) and after the implementation of a national smoking ban (2007) in 32 hospitals of Catalonia, Spain. We compared the means of the overall score in 2005 and 2007 according to the type of hospital, the number of beds, the prevalence of tobacco consumption, and the number of years as a smoke-free hospital. Results: The mean of the implementation score of tobacco control policies was 52.4 (95% CI:45.4-59.5) in 2005 and 71.6 (95% CI: 67.0-76.2) in 2007 with an increase of 36.7% (p 300 beds (41.1% increase; p < 0.01), hospitals with employees' tobacco consumption prevalence 35-39% (72.2% increase; p < 0.05) and hospitals that had recently implemented smoke-free policies (74.2% increase; p < 0.01). Conclusion: The national smoking ban appears to increase tobacco control activities in hospitals combined with other non-bylaw initiatives such as the Smoke-free Hospital Network

En este proyecto se ha grabado más escenas antes del confinamiento y preparados la historioteca con una de las píldoras ya definitivas

    IL-6 serum levels predict severity and response to tocilizumab in COVID-19: An observational study

    Background: Patients with coronavirus disaese 2019 (COVID-19) can develop a cytokine release syndrome that eventually leads to acute respiratory distress syndrome requiring invasive mechanical ventilation (IMV). Because IL-6 is a relevant cytokine in acute respiratory distress syndrome, the blockade of its receptor with tocilizumab (TCZ) could reduce mortality and/or morbidity in severe COVID-19. Objective: We sought to determine whether baseline IL-6 serum levels can predict the need for IMV and the response to TCZ. Methods: A retrospective observational study was performed in hospitalized patients diagnosed with COVID-19. Clinical information and laboratory findings, including IL-6 levels, were collected approximately 3 and 9 days after admission to be matched with preadministration and postadministration of TCZ. Multivariable logistic and linear regressions and survival analysis were performed depending on outcomes: need for IMV, evolution of arterial oxygen tension/fraction of inspired oxygen ratio, or mortality. Results: One hundred forty-six patients were studied, predominantly males (66%); median age was 63 years. Forty-four patients (30%) required IMV, and 58 patients (40%) received treatment with TCZ. IL-6 levels greater than 30 pg/mL was the best predictor for IMV (odds ratio, 7.1; P < .001). Early administration of TCZ was associated with improvement in oxygenation (arterial oxygen tension/fraction of inspired oxygen ratio) in patients with high IL-6 (P = .048). Patients with high IL-6 not treated with TCZ showed high mortality (hazard ratio, 4.6; P = .003), as well as those with low IL-6 treated with TCZ (hazard ratio, 3.6; P = .016). No relevant serious adverse events were observed in TCZ-treated patients. Conclusions: Baseline IL-6 greater than 30 pg/mL predicts IMV requirement in patients with COVID-19 and contributes to establish an adequate indication for TCZ administrationThis study was funded by Spanish Ministry of Economy, Industry and Competitiveness (MINECO) and Instituto de Salud Carlos III (grant nos.
