    First-Trimester Sequential Screening for Preeclampsia Using Angiogenic Factors : Study Protocol for a Prospective, Multicenter, Real Clinical Setting Study

    The incidence of preeclampsia (PE) is about 2-8%, making it one of the leading causes of perinatal morbidity and maternal mortality in the world. Early prophylactic low dose administration (150 mg) of acetylsalicylic acid is associated with a significant reduction in the incidence of early-onset PE, intrauterine growth restriction (IUGR), and neonatal mean stay in the intensive care unit (ICU). Universal implementation of a first-trimester screening system including angiogenic and antiangiogenic markers [the Placental Growth Factor (PlGF) and/or soluble fms-like Tyrosine Kinase-1 (sFlt-1)] has shown a prediction rate of 90% for early-onset PE but entails a high financial cost. The aim of this study is to determine the predictive and preventive capacity of a universal PE first-trimester two-step sequential screening model, determining the PlGF only in patients previously classified as intermediate risk by means of a multivariate model based on resources already used in the standard pregnancy control, in a real clinical setting. We hypothesize that this screening model will achieve similar diagnostic performance as the universal determination of PlGF but at a lower economic cost. This is a prospective, multicentric, cohort study in a real-world clinical setting. Every singleton pregnancy will be recruited at the routine first pregnancy visit. In a first step, the first-trimester risk of PE will be calculated using a multivariate Gaussian distribution model, based on medical history, mean blood pressure, Pregnancy-Associated Plasma Protein A (PAPP-A), and Uterine Artery Doppler Pulsatility Index (UTPI). Patients will be classified into three risk groups for PE: (1) risk ≥ 1/50, high-risk with no further testing (blinded PlGF); (2) risk between 1/51 and 1/500, medium-risk requiring further testing; and (3) risk ≤ 1/501, low-risk with no further testing. In a second step, the PlGF will only be determined in those patients classified as intermediate risk after this first step, and then reclassified into high- or low-risk groups. Prophylactic administration of aspirin (150 mg/day) will be prescribed only in high risk patients. As a secondary objective, sFlt-1 values will be blindly determined in patients with high and intermediate risk to assess its potential performance in the screening for PE. The study will be conducted in accordance with the principles of Good Clinical Practice. La PEPC está ampliamente distribuida en plantas, algas verdes y microorganismos pero ausente en levaduras y animales (Chollet et al., 1996). En plantas vasculares su papel estelar está relacionado con la fotosíntesis C4 y CAM («Crassulacean acid metabolism»), sin embargo desempeña otras funciones como la anaplerótica, en relación a la síntesis de proteínas, homeostasis del pH citosólico, electroneutralidad y osmolaridad. Está formada por una pequeña familia multigénica algunos de cuyos representantes están regulados a nivel transcripcional por factores como luz, hormonas y metabolitos (Chollet et al., 1996; Vidal y Chollet, 1997). La naturaleza alostérica de la enzima permite una regulación fina en relación a diferentes ambientes metabólicos. La PEPC está regulada por fosforilación reversible, proceso ligado a una cascada de transducción de señales de alta complejidad. En la actualidad es uno de los mejores modelos de señalización descritos en plantas. Este capítulo se centra en los eventos relacionados con este proceso en plantas C4 y CAM, los dos sistemas mejor estudiados en la actualidad (Chollet et al., 1996; Echevarría y Vidal, 2003; Izui et al., 2004; Nimmo, 2000; Vidal y Chollet, 1997).Phosphoenolpyruvate carboxylase (EC 4.1.1.31, PEPC) catalyzes the b-carboxylation of phosphoenolpyruvate (PEP) by HCO3 - in the presence of Mg2+ to yiel oxaloacetate and Pi (Chollet et al., 1996). PEPC is a widely distributed enzyme in plants, green algae and micro-organisms but absent in yeast and animals (Chollet et al., 1996). In higher plants, it catalyses a pivotal reaction related to such important processes as C4 and Crassulacean acid metabolism (CAM) photosynthesis, the anaplerotic pathway linked to amino acid synthesis, homeostasis of cytosolic pH, electroneutrality and osmolarity. PEPC belongs to a small multigenic family (Chollet et al., 1996; Vidal y Chollet, 1997). At the transcriptional level, some PEPC genes respond to external and internal factors (light, hormones and metabolites), while at the protein level, the allosteric nature of the enzyme allows its activity to be fine-tuned in relation to a varying metabolic environment. PEPC undergoes a posttranslational control by a phosphorylation process linked to a highly complex signal transduction cascade. Today, it is one of the best-described models of plant signaling. This chapter will focus on what is known about these processes in leaves of C4 and CAM plants, the two systems that have been studied in detail so far (Chollet et al., 1996; Echevarría y Vidal, 2003; Izui et al., 2004; Nimmo, 2000; Vidal y Chollet, 1997)

    En esta memoria se describe el trabajo de la red docente para el seguimiento y control de calidad de las asignaturas del segundo curso del Grado en Ingeniería Informática impartido en la Escuela Politécnica Superior de la Universidad de Alicante. En esta edición, el trabajo de la red se ha centrado en el estudio de las necesidades formativas y los contenidos impartidos en las asignaturas. El resultado ha sido la creación de un grafo de dependencias entre asignaturas de segundo y primer curso (y de segundo curso entre sí), un mapa de necesidades formativas para acceder a las asignaturas de segundo curso y un mapa de los contenidos impartidos en éstas. Asimismo, se ha elaborado un calendario on-line de evaluaciones para el curso 2014-2015

    Growth patterns and breast milk/infant formula energetic efficiency in healthy infants up to 18 months of life: the COGNIS study

Modifications in infant formula composition have led to evaluate their effects on growth and energetic efficiency (EE) compared with breast-feeding. Main goal was to analyse type of feeding potential effects during first months of life, plus its EE, on growth patterns in healthy formula fed (standard infant formula (SF) vs. experimental infant formula enriched with bioactive nutrients (EF)) and breastfed (BF) infants participating in the COGNIS RCT (http://www.ClinicalTrials.gov, Identifier: NCT02094547) up to 18 months of age. Infants follow-up to 18 months of age (n 141) fed with a SF (n 48), EF(n 56), or BF (n 37), were assessed for growth parameters using WHO standards. Growth velocity (GV) and catch-up were calculated to identify growth patterns. EE of breast milk/infant formula was also estimated. Infants' growth at 6 months showed higher length and lower head circumference gains in SF and EF infants than BF infants. Both weight-for-length and weight-for-age catch-up growth showed significant differences in formula fed groups compared with the BF. No significant differences in GV or catch-up were found at 6-12 and 12-18 months. Regarding EE, infant formula groups showed significantly lower weight and length gains/g of milk protein, and higher weight and length gains/g of milk lipids, than the BF infants. GV during first 6 months, which may be influenced by feeding, seems to be the main predictor of subsequent growth trajectory.     Cortical Visual Evoked Potentials and Growth in Infants Fed with Bioactive Compounds-Enriched Infant Formula: Results from COGNIS Randomized Clinical Trial

    Postnatal nutrition is essential for growth and neurodevelopment. We analyzed the influence of a new enriched-infant formula with bioactive compounds on growth, neurodevelopment, and visual function (VF) in healthy infants during their first 18 months of life. A total of 170 infants were randomized in the COGNIS randomized clinical trial (RCT) to receive a standard infant formula (SF = 85) or a new experimental infant formula supplemented with functional nutrients (EF = 85). As a control, 50 breastfed infants (BF) were enrolled. Growth patterns were evaluated up to 18 months of life; neurodevelopment was assessed by general movements at 2, 3, and 4 months; VF was measured by cortical visual evoked potentials at 3 and 12 months. No differences in growth and neurodevelopment were found between groups. Regarding VF, SF and EF infants presented prolonged latencies and lower amplitudes in the P100 wave than BF infants. In the EF group, a higher percentage of infants presented response at 7 1/2' of arc at 12 months compared to 3 months of age; a similar proportion of BF and EF infants presented responses at 7 1/2' of arc at 12 months of age. Early nutritional intervention with bioactive compounds could narrow the gap in growth and neurodevelopment between breastfed and formula-fed infants.This project has been funded by Ordesa Laboratories, S.L.     Impact of HLA Mismatching on Early Subclinical Inflammation in Low-Immunological-Risk Kidney Transplant Recipients

    The impact of human leukocyte antigen (HLA)-mismatching on the early appearance of subclinical inflammation (SCI) in low-immunological-risk kidney transplant (KT) recipients is undetermined. We aimed to assess whether HLA-mismatching (A-B-C-DR-DQ) is a risk factor for early SCI. As part of a clinical trial (Clinicaltrials.gov, number NCT02284464), a total of 105 low-immunological-risk KT patients underwent a protocol biopsy on the third month post-KT. As a result, 54 presented SCI, showing a greater number of total HLA-mismatches (p = 0.008) and worse allograft function compared with the no inflammation group (48.5 ± 13.6 vs. 60 ± 23.4 mL/min; p = 0.003). Multiple logistic regression showed that the only risk factor associated with SCI was the total HLA-mismatch score (OR 1.32, 95%CI 1.06-1.64, p = 0.013) or class II HLA mismatching (OR 1.51; 95%CI 1.04-2.19, p = 0.032) after adjusting for confounder variables (recipient age, delayed graft function, transfusion prior KT, and tacrolimus levels). The ROC curve illustrated that the HLA mismatching of six antigens was the optimal value in terms of sensitivity and specificity for predicting the SCI. Finally, a significantly higher proportion of SCI was seen in patients with >6 vs. ≤6 HLA-mismatches (62.3 vs. 37.7%; p = 0.008). HLA compatibility is an independent risk factor associated with early SCI. Thus, transplant physicians should perhaps be more aware of HLA mismatching to reduce these early harmful lesions
